1.Effects of short-term insulin pump intensive treatment on tumor necrosis factor-α and interleukin-6 in new type 2 diabetic patients

Sixty three new type 2 diabetes(T2DM)mellitus were treated with insulin pump for 2 weeks.Free insulins,fasting plasma slucose,tumor necrosis factor-α(TNF-α)and interleukin(IL)-6 level and other parameters were measured before and after insulin treatment.HOMA-insulin resistance (HOMA-IR)and β-cell function(HOMA-β)were compared.After 2 weeks insulin intensive treatment, TNF-α and IL-6 level were significant lower(P<0.01).HOMA-IR and HOMA.B were signifiant improved (P<0.01).TNF-α and IL-6 levels were positively associated with tlle body mess index and HOMA.IR (P<0.01).TNF-α,IL-6 and HOMA-IR could be significanly improved by short-term insulin pump intensive treatment in new T2DM patients with severe hyperglycemia.
2.Clinical Observation of 3 Metronidazol Therapeutic Regimens for Trichomonas Vaginitis

OBJECTIVE:To investigate observation of three metronidazol combined with drug for vaginal local application on trichomnas vaginitis. METHODS:195 patients with trichomonas vaginitis were randomly divided into three groups. They were treated with drug for vaginal local application plus single dose or multiple dose of metronidazol(p.o) ,respectively. Then their curative effects were compared. RESULTS:In single large dose of metronidazol group remission rate,cure rate and total effective rate were higher than in multiple low dose group in the first and the second day. Significant difference was noted between the two groups(P0.05) . CONCLUSION:The curative effect of single large dose of metronidazol was better than the multiple low dose. The therapeutic effect of single dose of metronidazol(1.6 g) is similar to single dose(2.0 g) .
3.Multiple schwannomas

Objective To study the clinical features and treatment of multiple schwannomas (neurilemmomas). Methods 9 patients with multiple schwannomas treated with surgery from 1975 to 1999 were reviewed retrospectively. Results In 7 of the 9 cases of schwannomas, the number of tumor lesions was seen more than that diagnosed preoperatively. All the patients had surgical resection; 5 had enucleation and 4 had tumors excised with the invovled nerve. Recurrence occured in 4 patients. Conclusion Because of the difficulty in predetermination of the exact lesional number, thoroughly preoperative examination including MRI is nessesary. Enucleation is recommended to those tumors involving important nerves. There is a high rate of recurrence.
4.The curative effects of intramedullary interlocking nailand small incision for the treatment of tibial shaft fracture

Objective To evaluate the clinical applied effects of tibial shaft fracture with intramedullary interlocking nail and small incision. Methods From November 2004 to October 2005,47 cases of tibial shaft fracture were treated with intramedullary interlocking nail, all the cases were adopted with small incision and intramedullary interlocking nail, and analysed the curative effects Results 47 cases were followed-up ranged 8 ~ 27 months. All cases resumed normal knee function 6 to 12 weeks after operation,without nonunion and malunion. Conclusion Treatment of tibial shaft fracture with intramedullary interlocking nail and small incision had the advantages of little trauma,good stability, high rate of fracture union and rapid functions recovery. It is worthy deserveed to be put into application and populatization in clinics.
5.Study on optimal method for local compression following removal of needle after intravenous injection

Objective We aimed to study the optimal method for local compression following removal of needle after intravenous injection and protect patients' vein to the highest degree. Methods 620 patients were divided into the observation group and the control group with 310 cases in each group randomly. Comparison study was carried out by adopting interchange compression (routine method) and noninterchange compression (direct pressing with hypothenar). Results The incidence of pain, bleeding and subcutaneous stasis after removal of needle in the observation group was lower than those in the control group (P<0.01). Conclusions Local compression following removal of needle with hypothenar was superior to routine compression method and it was worth applying in clinics widely.
6.Antimicrobial resistance of gram-positive pathogenic bacteria isolated from children's respiratory tract infection

Objective To investigate antirnierobial resistance of comlnon pathogenic bacteria isolated from children's respiratory tract in Beijng friendship Hospital during the period between January 2004 and December 2006.Methods Forty-nine strains of Streptococcus pneumoniae,fifty-five strains of Staphylococcus aureus and fifteen strains of Group A β-hemolytic streptococcus were isolated from children with community-acquired respiratory tract infeetion in pediatric department of Beijng Friendship Hospital.Kirby-Bauer agar dilution method Was used to determine the susceptibility to antimicrobial agents.Results Ninety percent of Streptococcus pneumoniae strains were resistant to erythromycin.Penicillin-non-susceptible streptococcus pneumoniae accounted for 10%,while 90% of which were multi-drug resistant.For Staphylococcus aureu strains,the resistant rates of ampicillin,penicillin and oxacfllin were 86%,82%,and 9% respectively,and serere multi-drug resistance was found.Eighty percent of the group A β-hemolytic streptococcus strains were resistant to erythromycin,however all remained sensitive to penicillin and ampicillin.Conclusion The antimicrobials resistant pathogenic bacteria isolated from children with respiratory tract infection have become a severe prdblem.The resistant rates of macrolides are high for Streptococcus and Group A β-hemolytic streptococcus.MeticiUin resistant staphylococcus aureus(MRSA)strains were found in the study.Severe multi-drug resistance was found.Non β-lactam antibiotics should be avoided when respiratory tract bacterial infection occurred in children.

8.Changes of Serum Myocardial Zymogram in Children with Mycoplasma Pneumoniae Pneumonia and Its Clinical Significance

180 U/L.CK-MB raised in only 5 cases(15.6%),and the highest was 28 U/L.Of the 218 MPP patients,only the 5 patients(2.3%) with raised CK-MB levels could be diagnosed as myocardial damage complicated with MPP.Conclusions The abnormal ECG is more important than myocardial zymogram in the diagnosis of myocardial damage complicated with MPP.Only higher level of certain myocardial zymogram without abnormal ECG can not be diagnosed as myocardial damage if lack of clinic and other objective evidences for myocardial damage.In watching out for myocardial damage,excessive examinations and treatments shall be avoided if possible.
9.Surgical effect of traumatic lens dislocation with secondary glaucoma

AIM: To retrospectively evaluate the effect of lens extraction combined with vitrectomy to treat traumatic lens dislocation with secondary glaucoma.
METHODS: Thirty - one eyes ( 31 cases ) of lens dislocation caused by blunt trauma with secondary glaucoma were treated respectively with cataract extraction combined with anterior vitrectomy, trabeculectomy and intraocular lens implantation. The visual acuity and pressure were observed 1wk, 1 and 3mo after operative.
RESULTS: Thirty - one eyes were all complete the operation successfully, and 6 eyes were given combined trabeculectomy, 9 eyes were implanted anterior chamber intraocular lens implantation ( IOL ) and 15 eyes were given posterior chamber suture fixation. Sixteen eyes were implanted in one-stage operation, while 8 eyes were implanted in two-stage operation. All intraocular pressure ( IOP ) were controlled to the normal level after operation and 23 eyes had visual acuity of more than 0. 3. CONCLUSION: Lens extraction combined with vitrectomy is an effective method for treatment of lens dislocation with secondary glaucoma. In order to control the IOP and get well visual function, we should choose IOL implantation or trabeculectomy according to the patient's condition.
10.Antifungal Susceptibility of Candida spp.to Lisea Cubeba Oil:a Broth Microdil ution Tests and Electron Microscopy

Objective To determine the susceptibility of yeasts and to explore the antifungal m echanismof Lisea cubeba oil,so as to provide evidence for the development of antifungal medicinal herbs.Methods The minimuminhibitory concentrations(MIC)of Lisea cubeba oil and fluconazole were determined by broth microdilution method according to the National Com mittee on Clinical Laboratory Standards(NCCLS)reference method for broth dilution antifungal susceptibility testing of yeasts.Ultra-structur al changes of C.krusei were observed by electron microscopy before and after Lisea cubeba oil treatment.Results Five previously identified quality control(QC)strains(Candida albicans,Candida tropical is,Candida glabrata,Candida parapsilosis,Candida krusei)were susceptible to Lisea cubeba oil,their MICs were 14.14?3.64?g /mL,23.22?2.85?g /mL,31.24?2.88?g /mL,76.19?4.40?g /mL,28.30?2.54?g /mL,respectively.After treatmen t with Lisea cubeba oil the mor-phology and ultrastructure of Candida krusei showed obvious changes:their cell wall and cytoplasmic membrane ruptured;intracellar components dissolved;organellae swollen and di ssolved.There were no changes observed in the morphology and ultrastructure of C.krusei treated with fluconazole.Conclusions The data indicate that Lisea cubeba oil has antifungal effect not only on C.albican and other medically important Candida spp.,but also on fluconazole resistant isolates such as C.krusei and thus has great clinical importan ce.The antifungal mechanismof Lisea cubeba oil may be that the structure of cell w all and cell membrane of C.krusei being inhibited.
