1.Bone mesenchymal stem cell transplantation for treatment of cerebral infarction in rats

Objective To observe the protective and repairing effects of bone mesenchymal stem cells(BMSCs) transplantation on cerebral infarction in rats and to study the different effect of transplantation at different time points. Methods Middle cerebral artery occlusion(MCAO) models were set up,and the rats were divided into a control group,a group with PBS transplantation and five groups with BMSCs transplantation 3,6,12,24 and 72 h after MCAO,respectively.The volume of infarction area and the neurological severity score(NSS) in all the groups were compared. 2.Transoral Orvil EEA stapler (OrVil) in laparoscopy-assisted total gastrectomy for cardiac carcinoma

Objective To evaluate transoral Orvil EEA stapler (OrVil) procedure in laparoscopic total gastrectomy for cardiac carcinoma compared with conventional anvil head method (purse-string suture).Methods From May 2014 to December 2014 20 cases were included into OrVil group,and 25 cases into purse-string suture group.Results The two groups had similar mean numbers of dissected lymph nodes [(25 ± 3) vs.(24 ± 4),t =1.067,P =0.292],the mean time of operation,intraoperative blood loss,and postoperative complications (5 vs.6,P =0.938).The length of incision was significantly shorter [(5 ±1) cm vs.(11 ± 2) cm,t =-10.724,P < 0.0l] and the esophagojejunostomy time was significantly less [(28 ± 4) min vs.(39 ± 5) min,t =-7.996,P < 0.01] with the use of OrVil.The time to first flatus and postoperative hospital stay were (3.7 ± 0.9) d vs.(4.4 ± 1.0) d,t =-2.485,P =0.017 and (13 ± 5) d vs.(16 ±4) d,t =-2.184,P =0.035.Conclusions OrVil is a technically safe and feasible surgical procedure for esophagojejunostomy in laparoscopy assisted total gastrectomy in the treatment of cardiac carcinoma.
3.Application of free anterior serratus musculo-fascial flap in bridge style for the soft tissue defect at leg.

OBJECTIVETo investigate the application of free anterior serratus musculo-fascial flap in bridge style for the soft tissue defect at leg.
METHODSFrom Sept. 2006 to Jan. 2009, the free anterior serratus musculo-fascial flaps were used in bridge style in 7 cases with soft tissue defects at legs. The anterior serratus musculo-fascial flaps were elevated with subscapular and circumflex scapular vessels forming a T-shaped vascular pedicles. The T-shaped pedicle was end-to-end anastomosed with the two ends of the posterior tibial artery at the healthy leg. The musculo-fascial flap and its pedicle were covered with skin graft.
RESULTSAll the 7 flaps survived completely with satisfactory result. The patients were followed up for 9-42 months with good functional and esthetic result both in donor site and recipient site. The patency of posterior tibial artery was demonstrated by clinical and Doppler examination.
CONCLUSIONSThis technique is particularly useful in leg reconstructive surgery when only one vessel remains. The patency of the posterior tibial artery at the healthy leg is preserved and the morbidity in donor site is minimal.

4. Clinicopathologic features of primary hepatic marginal zone lymphoma of mucosa-associated lymphoid tissue and hepatic pseudolymphoma

Objective:
To study the clinicopathological features of primary hepatic extranodal marginal zone lymphoma of mucosa associated lymphoid tissue (MALT lymphoma) and hepatic pseudolymphoma, and to discuss their differential diagnosis, treatment and prognosis.
Methods:
Three primary hepatic MALT lymphomas and two hepatic pseudolymphomas collected from January 2012 to March 2017 in the First Affiliated Hospital of Nanjing Medical University were evaluated by HE and immunohistochemistry(IHC), in-situ hybridization and immunoglobulin (Ig) gene rearrangement detection, and the relevant literature reviewed.
Results:
In the three MALT lymphomas, tumor cells infiltrated the portal areas with nodular pattern, and invaded the surrounding normal liver with serpiginous configuration and formation of confluent sheets. A number of bile ducts were entrapped within the lesions, and showed lymphoepithelial lesion. Reactive lymphoid follicles were present and surrounded by tumor cells, consisting of predominantly centrocyte-like cells and monocytoid B cells. There were clusters of epithelioid histiocytes in one case. The tumor cells were positive for CD20, PAX5 and negative for CD5, CD23, CD10, bcl-6, and cyclin D1. In the two hepatic pseudolymphomas, the lesions presented as solitary nodules well-demarcated from the surrounding liver tissue; one case was partially encapsulated with fibrous tissue. Entrapped bile ducts were only found at the edge of the lesions without lymphoepithelial lesion. The lesions comprised of massive lymphoid proliferation consisting predominantly of reactive lymphoid follicles, but not monocytoid B-cells or atypical cells. By IHC, a mixture of B- and T-cell population was identified. A monoclonal rearrangement of the Ig gene was detected in all three MALT lymphomas but not in two pseudolymphomas. Interphase fluorescence in situ hybridiazation test for MALT1 break-apart gene was positive in two cases of MALT lymphomas and EBER was negative in all studied cases.
Conclusions
Primary heptic MALT lymphoma and pseudolymphoma are both rare lymphoid proliferative lesions of liver. These two lesions have overlapping histological and IHC features and are top differential diagnosis to each other. A combination analysis of morphology, immunophenotype and Ig gene rearrangement is helpful to distinguish between them.
6.Non-alkaloid constituents from a Tibetan medicine Meconopsis quintuplinervia.

OBJECTIVETo study the chemical constituents of a Tibetan medicine Meconopsis quintuplinervia.
METHODColumn chromatographic techniques were applied to isolate constituents. A combination of IR, MS and NMR spectroscopy was used to identify structures of constituents.
RESULTTwelve compounds were isolated from the ethanolic extract and their structures were elucidated as quercetin 3-O-beta-D-glucopyranoside (I), quercetin 3-O-beta-D-galactopyranosyl-(1-->6)-glucopyranoside (II), kaempferol 3-O-beta-D-glucopyranosyl-(1-->2)-beta-D-glucopyranoside (III), isorhamnetin 3-0-beta-D-galactopyranosyl-(1-->6)-beta-D-glucopyranoside (IV), caffeic acid (V), protocatechuic acid (VI), p-hydroxycinnamic (VII), 2-(3,4-dihydroxyphenyl )-ethyl-O-beta-D-glucopyranoside (VIII), p-hydroxybenzoyl-beta-D-glucopyranoside (IX), 4-O-beta-D-glucopyranosyl-(Z)-p-coumaric acid (X), 5, 7-dihydroxy-4H-4-chromenone (XI), daucosterol (XII).
CONCLUSIONTen compounds were isolated from this genus for the first time except for XI and XII.

7.Mechanism underlying the inhibitory effects of peroxisome proliferator-activated receptor γ agonists on transforming growth factor β1 in adult skin fibroblasts.

OBJECTIVETo study the mechanism underlying the inhibitory effects of peroxisome proliferator-activated receptor γ (PPARγ) agonists on transforming growth factor β1 (TGF-β(1))-induced scarring of skin.
METHODSFibroblasts isolated from healthy adult skin were cultured in vitro and divided into blank control group (serum-free DMEM culture), TGF-β(1) group (with stimulation of 10 ng/mL TGF-β(1) for 48 hours), troglitazone group (with the same treatment as in TGF-β(1) group after stimulation of 10 µmol/L troglitazone for 2 hours), and 15-dioxygen prostaglandin J2 (15d-PGJ2) group (with the same treatment as in TGF-β(1) group after stimulation of 10 µmol/L 15d-PGJ2 for 2 hours) according to the stimulation added into DMEM. The expression of connective tissue growth factor (CTGF) was determined with Western blot. The mRNA levels of CTGF, matrix metalloproteinase-1 (MMP-1) and platelet-derived growth factor (PDGF) were determined with real-time fluorescence RT-PCR. Data were processed with one-way analysis of variance.
RESULTSThe expression of CTGF at mRNA and protein levels in skin fibroblasts were significantly increased in TGF-β(1) group as compared with control group; while expression of CTGF at mRNA and protein levels in 15d-PGJ2 and troglitazone groups were significantly decreased as compared with that in TGF-β(1) group. The mRNA level of MMP-1 in TGF-β(1) group (0.193 ± 0.051) was obviously lower than that in blank control group (1.281 ± 0.195, F = 12.811, P < 0.01), while the mRNA levels of MMP-1 in troglitazone group (0.417 ± 0.043) and 15d-PGJ2 group (0.485 ± 0.027) were significantly increased as compared with that in TGF-β(1) group (F = 12.811, P values all below 0.01). The mRNA level of PDGF in TGF-β(1) group (1.044 ± 0.237) was obviously higher than that in control group (0.349 ± 0.057, F = 16.848, P < 0.01), while the levels in troglitazone group (0.677 ± 0.055) and 15d-PGJ2 group (0.511 ± 0.017) were significantly decreased as compared with that in TGF-β(1) group (F = 16.848, P values all below 0.01).
CONCLUSIONSThe inhibitory effect of activated PPARγ on the expression of CTGF induced by TGF-β(1) may be the main mechanism of its inhibitory effect on TGF-β(1)-induced scarring on skin, and its influence on MMP-1 and PDGF may also be one of the underlying mechanisms.

8.The ultrasonic diagnosis of abnormal fetal genitalia

Objective To explore the clinical significance of abnormal fetal genitalia detected by routine prenatal ultrasound. Methods In Peking University Third Hospital from January 2014 to January 2016, the ultrasonic diagnosis and sonographic features in 14 suspected cases of abnormal fetal genitalia were analyzed retrospectively and compared with postpartum morphological and pathological characteristics. Results The fetal ultrasound findings and clinical outcomes in 14 cases were: (1) There were 4 cases of partial penoscrotal transposition (28.6%, 4/14), in which all fetus hadshort penis and hypospadias. In 2 termination cases, unilateral hydronephrosis and anal atresia was found in one case, while ventricular septal defect was found in the other case. In 2 term-birth cases, there was one case of recessive spina bifida. In the 4 cases, prenatal ultrasonographic manifestation showed that the short penis was located between the division of the scrotum, named as ″tulip sign″. (2) There were 2 cases (14.3%, 2/14) of complete penoscrotal transposition, in which all the pregnancies were terminated. On prenatal ultrasound of external genitalia, the location of the penis and scrotum was reversed. In one case, anal atresia and small eye malformation were found. In one case, the tetralogy of Fallot, duodenal atresia and anal atresia were found. (3) There were 4 cases of the micropenis (28.6%, 4/14). On prenatal ultrasound, the penis was difficult to demonstrated and was shown merely as an echoic dot. Among 2 cases of FGR, one infant dies for unexplained reasons after birth, and one infant has hypospadias after birth. The pregnancy was terminated in two cases of 18-trisomia and microdeletion in chromosome 9. (4) There were 2 cases of penis absence (14.3%, 2/14), in which all pregnancies were terminated. The sonographic manifestations show the absence of penis and bladder eversion. (5) In 2 cases, sex couldn`t be identified (14.3%, 2/14). In one case, the 46 xy infant showed a female external genitalia appearance after birth. In one case, the clitorism and congenital adrenal cortical hyperplasia were found in a 46 xx infant after birth. Conclusions Prenatal ultrasonography is effective and feasible in detecting genitalia abnormalities, especially for penis scrotal transposition and penis absence. When the fetal gender cannot be identified on ultrasound, chromosome examination is warranted. For indeterminable cases, ultrasound follow-up in 2nd and 3rd trimester is necessary.
9.Construction and expression of recombinant plasmid to initiate polarization and activation of regulatory T cells.

In order to construct a recombinant plasmid initiating the polarization and activation of the regulatory T cells (Treg), the fragments of hTGF-β1 and C2-C4 of gp120 amplified by RT-PCR and cloned into pCR2.1 vector respectively. hTGF-β1 and C2-C4 DNA fragments were obtained, then sub-cloned to generate the prokaryotic expression vector named pET-28a/C2-C4-Linker- hTGF-β1. The expression of recombinant protein was induced by IPTG (0.1 mmol/L) for 6 hours. The results showed that the fragments of hTGF-β1 and C2-C4 were amplified and cloned into pCR2.1, the prokaryotic expression vector pET-28a/C2-C4-Linker- hTGF-β1 was constructed successfully. The recombinant protein was expressed as inclusion body after being induced by IPTG. It is concluded that this recombinant protein can initiate the polarization and activation of Treg cells, indicating the engineering E.coli strain is successfully obtained.

10.Cross-sectional survey on smoking and smoking cessation behaviors among Chinese adults in 2010.

OBJECTIVETo describe the prevalence of smoking and smoking cessation in Chinese adults in 2010.
METHODSA face-to-face questionnaire survey was carried out in 162 surveillance points to collect information on non-communicable diseases related risk factors. Multi-stage stratified cluster random sampling method was used to select 98 712 individuals aged 18 and over to be interviewed and 98 526 records were included in the analysis of smoking and smoking cessation. Sample was weighted to represent the population of Chinese adults. Indicators such as current smoking and smoking cessation among different population were calculated.
RESULTSCurrent smoking rate of our sample was 26.4% (26 047/98 526). With complex weighting, current smoking rate in Chinese adults aged 18 and above was 28.3% (95%CI: 27.2% - 29.4%), which is much higher among men (53.3%, 95%CI: 51.4% - 55.2%) than in women (2.5%, 95%CI: 1.9% - 3.0%) (P < 0.05). Most male current smokers (88.3%, 95%CI: 87.3% - 89.3%) smoked every day and average daily manufacture cigarettes consumption of male adults was (17.8 ± 9.3) cigarettes. Only 14.8% (95%CI: 13.8% - 15.8%) of male ever smokers quitted smoking and 10.7% (95%CI: 9.9% - 11.5%) quitted smoking. Only 38.8% (95%CI: 36.9% - 40.8%) of male current smokers intended to quit smoking. For current smokers aged from 18 to 24, proportion of those who intended to quit smoking was highest (50.5%, 95%CI: 46.1% - 54.8%), but proportion of those who quitted smoking (7.1%, 95%CI: 5.2% - 8.9%) was lowest comparing with other age groups (P < 0.05).
CONCLUSIONPrevalence of smoking in Chinese adults was high and only a few smokers quit smoking. Prevalence of smoking in Chinese male adults was still high. Fairly low proportion of male current smokers intend to quit smoking and even lower proportion of them quit smoking successfully.
