1.Therapeutic Effect of Recombinant Human Growth Hormone on the Stature of Children with Turner Syndrome

Objective To explore the therapeutic effect of recombinant human growth hormone (rhGH) on the stature of children with Turner syndrome(TS) and analyze its influencing factors in treatment.Methods Twelve children with TS were selected in the outpatient department of West China second hospital of sichuan university between Jan.2004 to Jun.2007.They were diagnosed as TS with karyotypically.The average age was 4.9-16.9(11.28?3.64)years old.Every patient must be made sure to have normal liver function,kidney funtion and MRI for hypothalamus and pituitary before enrollment.All cases were never treated with growth hormone or estrin.Not all the patients had the same manifestations:12 cases with short stature,9 cases with cubitus valgue,8 cases with broad chest,4 cases with low posterior hairline,2 cases with webbed neck,11 cases without pubes and armpit hair,none with menarche.Every patient must be made sure to have normal liver function,kidney funtion and MRI for hypothalamus and pituitary before enrollment.rhGH was used subcutaneously 3-5 cm around navel eve-ry night before sleep.Average rhGH dose was 0.16 IU/(kg?d)[0.15-0.18 IU/(kg?d)],and height standard deviation score (HtSDS) was calculated and compared before and after rhGH treatment,and adverse reactions were observed.Results The duration of treatment was different:9 cases for 12 cases months,2 cases for 15 cases months and 1 case for 18 cases months.The average height increase rat was (0.55?0.15) cm / month.Difference of HtSDS existed before and after treatment by t test(t=2.87 P
2.Clinical Analysis of Hashimoto′s Thyroiditis Combined with Hyperthyroidism in 56 Children

Objective To explore the clinical feature,diagnosis,treatment and prognosis of Hashimoto′s thyroiditis(HT) combined with hyperthyroidism in children.Methods The clinical features of 56 children with HT combined with hyperthyroidism,including clinical features,complications,thyroid hormone,thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb),the detection of ultrasonic imaging and fine needle aspiration biopsies,diagnosis,treatment and prognosis were analyzed.Results The proportion of men to women was 16 in all 56 children,and the mean age at diagnosis was(9.95?2.09) years.The percentage of positive TGAb and TPOAb were 93% and 98%,respectively in all the 56 children.One child was diagnosed as HT combined with hyperthyroidism coexistent and myasthenia gravis type Ⅰ.The duration of hyperthyroidism ranged from 3 to 24 months.There were 14 children who had hypothyrodism and 8 children were euthyroid in all the 56 children.Conclusions Children with HT hyperthyroidism are more frequent in young females.TGAb and TPOAb are important markers for the diagnosis of HT.Pharmacotherapy is the first choice to treat HT hyperthyroidism.Hyperthyroidism is a clinical process of HT,and the percentage of children with hypothyrodism is increasing along with the development of disease.
3.Effect of adenovirus-mediated gene transfection of vascular endothelial growth factor on survival of random flaps in rats.

OBJECTIVETo evaluate the effect of local application of vascular endothelial growth factor (VEGF) via adenovirus-mediated gene transfer on survival of full thickness flaps selected randomly in rats.
METHODSThirty Sprague-Dawley rats weighing 480-520 g were used in this study. A dorsal flap (8 cm x 2 cm) in full thickness with the pedicle located at the level of the iliac crest was designed. Then the rats received 1,012 pfu replication-deficient recombinant adenovirus carrying VEGF (AdCMV-VEGF group, n=10), 1,012 pfu recombinant beta-galactosidase adenovirus (AdCMV-Gal group, n=10) and 1 ml saline (saline group, n=10), respectively, in the distal two thirds of the proposed flap by means of subdermal injection at 8 different locations. Three days after treatment, the flaps were elevated as originally designed and sutured back in situ. The survival rate of the flaps was evaluated on day 7 after operation.
RESULTSThe survival rate of the flaps in the AdCMV-VEGF group increased significantly as compared with those of the AdCMV-Gal group (P<0.01) and the saline group (P<0.01). Immunohistochemical staining showed that VEGF was expressed in the survival flaps injected with AdCMV-VEGF. Histological analysis showed that more granulation tissues and angiogenesis were observed in the AdCMV-VEGF group than those in the AdCMV-Gal and the saline groups.
CONCLUSIONSLocal application of adenovirus-mediated VEGF165 cDNA may efficiently improve the survival of ischemic skin flaps.

5.Roles of CD8+ CD28- T regulatory cells in acute infectious mononucleosis in children.

OBJECTIVETo explore the role of CD(8)(+)CD(28)(-) T regulatory cells (Tr) in the immunological pathogenesis of acute infection with Epstein-Barr virus in children.
METHODSThe present study enrolled 25 children with infectious mononucleosis (IM) and 25 age-matched healthy children. Flow cytometric analysis was performed to detect the percentage of CD(3)(+), CD(3)(+)CD(4)(+), CD(3)(+)CD(8)(+), CD(8)(+)CD(28)(+) by determining the ratio of positive cells in lymphocytes. Reverse transcriptase-polymerase chain reaction (RT-PCR) and real-time PCR were used to analyze IL-6, IL-10, IFN-gamma expression in CD(8)(+)CD(28)(-) Tr cells and ILT-3, ILT-4 expression in monocytes/macrophages.
RESULTSThe proportions of CD(8)(+)CD(28)(-)T cells in children with acute-phase IM was significantly higher than those in the controls (P < 0.01). The expression level of IL-6, IL-10, IFN-gamma, ILT-3, ILT-4 mRNA significantly increased compared to those of the controls (P < 0.01).
CONCLUSIONThe CD(28) expressed on CD(8)(+) T cells in vivo is gradually lost with age and CD(8)(+)CD(28)(-) cells increase up 50% to adult. EBV can directly infect B cells, trigger CD(8)(+) CTL response and destroy the target cells to cause serious immunopathological lesion. Therefore we speculate that the expansion of CD(8)(+)CD(28)(-) Tr cells in children with IM may be an adaptive immune response to avoid serious inflammation and autoimmune reactions.

6.An experimental study on angiogenesis of non-vascularized autogenous bone graft with vascular bundle implantation.

OBJECTIVETo investigate the effect of vascular bundle implantation in autogenous bone graft on angiogenesis.
METHODSThirty-six New Zealand white rabbits were evaluated in this study. A portion of bilateral radial bones of a rabbit were removed as free bone grafts, whose periostea were peeled off. In test group, the external maxillary artery bundle was passed through the marrow cavity of the bone. In control group, there was no vascular bundle implantation. Each bone was placed in masseter muscle separately. The rabbits were sacrificed and the specimens were procured at 3 days, 1, 2, 3, 4 and 6 weeks after surgery for histological observation, Chinese ink perfusion and CD34 immunohistochemistry. Microvessel density (MVD) was assessed in order to evaluate angiogenesis of autogenous bone grafts.
RESULTSThe bone grafts were found revascularization in 3 days after surgery in the test group, whereas at 2 weeks in the control group. In 3 days, 1 week, 2 weeks, 3 weeks and 4 weeks after surgery, the MVD of test group was significantly higher than that of control group. In 4 weeks after surgery, angiogenesis of test group reached to peak.
CONCLUSIONVascular bundle implantation improved angiogenesis in non-vascularized autogenous bone graft in this study.

7.Association of blood and cerebrospinal fluid IgG contents and severity of craniocerebral injury.

OBJECTIVETo explore the association of blood and cerebrospinal fluid (CSF) IgG contents and the severity of craniocerebral injury.
METHODSTotalling 143 patients with craniocerebral injury were divided into 3 groups according Glasgow Coma Scale (GCS) scores, namely the mild injury group with GCS score of 12-15 (n=41), moderate injury group with GCS score of 9-11 (n=71) and severe injury group (GCS score 3-8, n=32). Another 9 patients with congenital hydrocephalus were also recruited as the control group. The CSF and blood samples were collected from these patients to measure the IgG contents 4 and 14 days and 1, 2, and 6 months after the injury, respectively. Physical disabilities of the patients were estimated with Rappaport's disability rating scale (DRS), whose correlations with CSF and blood IgG contents were analyzed.
RESULTSIn the early stage of moderate to severe brain injury, the IgG content was lowered significantly in the blood but increased in CSF as compared with the control patients (P<0.05), and the changes in CSF and blood IgG displayed a significant correlation with the severity of the injury (r=0.950, P<0.01). During the recovery of severe brain injury, DRS score was in inverse correlation with blood IgG content but in positive correlation with CSF IgG content (Spearman's correlation coefficient of 0.800, P<0.05).
CONCLUSIONIn the early stage of brain injury, detection of blood IgG content may help with the assessment of the injury severity. During the recovery of the injury, dynamic monitoring of blood and CSF IgG contents provides clues of the outcome of the patients and benefit the modification of the treatment plan.

9.Effects of hVEGF cDNA on random skin flap via a replication-deficient adenovirus vector.

OBJECTIVEThe purpose of this study was to determine the effects of local delivery of vascular endothelial growth factor( VEGF) transferred with adenovirus-mediated gene on the survival of ischemic random skin flap in rats.
METHODSThe animals were divided into three groups randomly (n = 10) . A 2 cm x 8 cm dorsal skin flap was designed with the pedicle at the level of the iliac crest. In group A (AdCMV-VEGF), each animal received 10(12) PR replication-deficient recombinant adenovirus (AdCMV-VEGF) in the distal two-thirds of the proposed flap by means of the subdermal injection at ten different locations. In group B (AdCMV-GaI), each received 1012 PR AdCMV-Gal. In Group C (Saline), each received 1 ml saline. Three days after the treatment, the flap was elevated as planed way and re-sutured back to its donor site. All the animals were evaluated 7 days after the operation.
RESULTSThe mean percentage of surviving flap area was (85.91 +/- 2.54)% in group A, (59.56 +/- l.18)% in group B, and (61.48 +/- l.09)% in group C. There was a significant increase in the percentage of the survival area in the flaps of the group A, compared with the group B and group C (Group B vs. Group A, P < 0.01; Group C vs.Group A, P < 0.01, Group B vs. Group C, P >0.05). Hybridization in the situ, the immunohistochemical stain showed that the VEGF was expressed in the survival tissue of the flap treated with the AdCMV-VEGF, but it was not found in the control groups. Histological analysis demonstrated qualitatively greater amount of granulation tissue and angiogenesis was found in the group treated with the AdCMV-VEGF than the controls.
CONCLUSIONSThe results may indicate that Ad vector carrying VEGF cDNA could be useful in enhancing the survival of the skin flap due to the effect of the local delivery of the VEGF.

10.Effects of components isolated from Astragalus mongholicus on expression of p-selectin in shock wave induced kidney injury in rabbit model.

OBJECTIVETo investigate the effective components of Astragalus mongholicus and their mechanisms in alleviating extracorporeal shock wave lithotripsy (ESWL) induced kidney injury.
METHOD69 male rabbits were randomly assigned into control group, sham treatment group, Total Saponins of Astragalus (TSA) group, Total Flavonoids of Astragalus (TFA) group, and Total Polysaccharide of Astragalus group (TPA). The shock wave treated kidneys were observed for the expression of p-selectin and the change of cells ultrastructure pre and post ESWL. The concentration of Maleic Dialdehyde (MDA) as well as activity of superoxide dismutase (SOD) in the kidney tissues was also analyzed.
RESULTAfter application of shock wave treatment, p-selectin was expressed extensively in renal glomerulus, renal tubules and renal interstitium of the treated kidneys. It showed a significant increase of MDA levels and decrease of SOD activity in control group (P < 0.05) after ESWL. The comparison between controls and TSA group demonstrated that TSA could significantly reduce the positive rate of p-selectin P < 0.05) and alleviate the injuries of cells ultrastructure. The results also showed a significant decrease of MDA levels and increase of SOD activity in TSA group compared to controls (P < 0.05). The protective effects of TFA and TPA in alleviating kidney injury induced by shock wave were lower than those of TSA; they had no effects of the expression of P-selectin.
CONCLUSIONTSA is the main components of A. mongholicus in alleviating shock wave induced kidney injury not only by scavenging oxygen free radicals but also inhibiting the expression of p-selectin.
