1.Clinical analysis of nine patients with presumed tubercular retinal vasculitis

Objective To observe the clinical features and treatment outcomes of presumed tubercular retinal vasculitis.Methods This is a retrospective non-comparative interventional clinical research.A total of nine patients (11 eyes) with major presentation of retinal vasculitis were included in this study.Patients first consulted the eye clinic and were diagnosed presumed tubercular retinal vasculitis.The patients,seven males and two females,aged from 19 to 66 years,with an average of 43.89 years.The time interval from symptoms to diagnosis ranged from two weeks to six months with an average of 76.27 days.Visual acuity,slit lamp ophthalmoscopy,fundus fluorescein angiography (FFA),optical coherence tomography (OCT),hematological and tuberculosis related investigations were examined and analyzed.All patients had standard anti-tuberculosis treatment.Treatment outcomes were followed for six to 37 months with an average of 14.11 months.Results Baseline visual acuity ranged from hand movement to 0.8 with an average of 0.28.Among 11 eyes,six presented mild to moderate vitritis,five presented as retinal vein occlusion with no obvious vitirits.Fundus examination showed six cases with retinal hemorrhage,four cases with macular edema,two with macular epiretinal membrane,and two with vitreous hemorrhage.FFA revealed 11 cases with leakage of vessels,11 with nonperfusion area,four with macular edema,three with retinal neovascularization,and two with choroidal lesions.OCT of nine eyes suggested six eyes with retinal edema,three with macular edema,three with macular epiretinal membrane.TST of seven patients were all strong positive.T-SPOT.TB of four patients were all positive.Three of eight patients who had chest X-ray or chest CT were suggested tuberculosis infection.Four to six weeks after the start of anti-tuberculosis treatment,vitritis,exudates,retinal and macular edema subsided.During follow up,inflammation was stable with no recurrence observed.The visual acuity of last follow-up ranged from 0.15 to 0.8 with an average of 0.51.Conclusions The main presentations of presumed tubercular retinal vasculitis are vitritis,retinal vein occlusion,and retinal hemorrhage.Standard anti-tuberculosis treatment can improve inflammation and retinal hemorrhage.
2.An investigation of risk factors of wound complications after operative fixation for calcaneal fractures

Objective To explore the risk factors contributing to wound complications following operative fixation for fractured calcanea.Methods Fitly-six patients with 61 fractured calcanea who had been treated with operative fixation from March 2000 to March 2003 were enrolled in investigation.General information of the pa- tients,ages,injury causes,classification of fracture,fall heights,smoking,delayed operation time,operation time and tourniquet time were documented preoperatively and postoperatively to be analyzed statistically.Results We report an overall wound complication rate of 14.8%(9/61).Wound infection,haematoma,dehiscence,heel necrosis and osteomyelitis were noted in our series.We identified the following as risk factors of postoperative wound compli- cations:1)fall from more than 3.4 m high(P<0.05);2)delayed operation time less than seven days(P<0.05); 3)operating time beyond two hours(P<0.05);4)tourniquet time beyond 1.5 hours(P<0.05).Conclusion In order to decrease wound complications following operative fixation tor fractured calcanea,surgeons should pertorm the operation 7 to 10 days after the injury,and reduce the operation time and tourniquet time.
3.A multiple correlation factor analysis of femoral head necrosis in patients over 45 years old with femoral neck fractures after closed reduction and cannulated screws fixation

Objective To investigate the multiple factors correlated to femoral head necrosis in patients over 45 years old with femoral neck fractures after closed reduction and cannulated screws fixation.Methods Three hundred patients with femoral neck fracture were admitted from May 1999 to May 2004 and treated by closed reduction and internal fixation with cannulated screws.We analyzed the multiple factors:age,gender,Garden classification,follow-up time,procedure delay,quality of reduction,time of full-weight-loading,fixation removal, and preoperative traction.Multiple correlation factors were analyzed by SPSS10.0 statistic system.Results Complete case records were documented in 99 cases who were followed up for an average of 24.5 months(range,8 to 60 months).Femoral head necrosis occurred in 15 cases(15.2%).Time of femoral head necrosis was 8 to 50 months after operation.The combination of quality of reduction,preoperative traction,age,and age?fixation re- moval was found to have the most significant influence on femoral head necrosis after internal fixation for femoral neck fractures.Conclusions Good reduction is the most important factor correlated to the outcome of a femoral neck fracture.Age is linked to the risk of necrosis but it is relatively insignificant.Preoperative traction in wrong positions can increase the incidence of necrosis.Garden classification is important in predicting femoral head necrosis and is associated with quality of reduction,but it is not a risk one in the combination of multiple correlation factors that lead to the necrosis.
4.Ultrastructure of Vascularization During Osteogenesis by Tissue Engineering Technique

Objective To observe the changes of osteoblasts and vascularization during osteogenesis by tissue engineering technique under the electron microscope and study the feasibility of improving vascularization of the tissue engineering bone by using the small intestine submucosa(SIS) as the scaffold. Methods The bone mesenchymal stem cells(BMSCs) were isolated by using the gradient centrifuge method.BMSCs were seeded in the SIS.The scaffold-cell constructs were cultured in vitro for two weeks.There were no cells on the SIS as control.They were implanted subcutaneously in the dorsa of the athymic mice.The implants were harvested after(in vivo) incubation for 4,8 and 12 weeks.The changes of osteoblasts and vascularization were observed under the transmission electron microscope and the scanning electron microscope. Results The BMSCs grew quite well.BMSCs differentiated on the surface of the SIS and secreted a great deal of extracellular matrices.The scaffold-cell constructs formed a lot of bone and vessels in vivo.The scaffold degraded after 12 weeks.No osteoblasts but vascularization and fibroblasts were observed as control. Conclusion SIS can be used as the scaffold for constructing tissue engineering bone as it can improve the formation of bone and vessels in vivo.
5.Effect of a novel isoflavone compound on plasma lipid and cholesterol of ovarectomied rats

Objective To study the differential expressions of labour-associated genes,including interleukin-8 (IL-8),oxytocin receptor (OTR) and prostaglandin H synthase type-2 (PGHS-2)) between preterm labour and term labour myometrium and their clinical significance.Methods Seventeen cases of preterm labour (PTL) and 31 cases of preterm no labour (PTNL) of pregnant women were selected as study group,while 6 cases of term labour (TL) and 6 cases term no labour (TNL) of pregnant women were chosen as control.Lower myometrial tissue biopsies were collected to detect the expressions of IL-8,OTR and PGHS-2 by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR) as well as Western blot analysis.The expression data were then statistically analyzed in combination with clinical data.Results In the lower myometrial tissues,the mRNA levels of IL-8 and PGHS-2 were significantly lower in PTL group than in TL samples (P0.05),but the PGHS-2 mRNA level was significantly higher in TL group than in TNL group (P0.05).Correlation analysis suggested that the mRNA level of IL-8 was significantly correlated with gestational age in PTNL group (r=0.294,P=0.042).Multiple linear regression analysis showed a mutual control effect of the expressions of IL-8,OTR and PGHS-2 in preterm but not in term labour group.Conclusion Our results show that there are significant differences of the expression of labour-associated genes in lower myometrum between preterm labour and term labour,which might becorrelated with the progression of preterm labour.
10.Graft repair of peripheral nerve defects using small intestinal submucosa with crude Schwann cells

Objective To investigate the results of graft repair of peripheral nerve defects using small intestinal submucosa(SIS) with crude Schwann cells. Methods Thirty-six male SD rats were randomly divided into three groups,with 12 in each group.Firstly,12-mm gaps of sciatic nerve were made in rats and then treated respectively with porcine SIS(group A),SIS compounded with crude Schwann cells(group B) and auto-nerve grafting(group C) in the three groups.The grafts were harvested 16 weeks postoperatively to evaluate the results of nerve regeneration through histological examination, triceps surae weight,computerized imaging analysis,Trueblue retrograde tracing and transmission electron microscopy. Results Nerve regeneration was confirmed to have extended through the gaps according to the results of histological examination,Trueblue retrograde tracing and transmission electron microscopy.Furthermore,in the respect of triceps surae weight,quantity of axis cylinder per area and percentage of neural tissue,the results of group B and group C were superior to those of group A with a significantly statistical difference(P0.05). Conclusion SIS compounded crude Schwann cells was able to achieve the outcome of auto-nerve grafting and was a promising replacement graft.
