2.Comparison of the effect between two methods of digestive tract reconstruction after gastrectomy

ObjectiveTo compare the advantages,disadvantages and clinical value of the improved loop the jejunum behalf of the stomach surgery and the P - type jejunum on behalf of stomach surgery.Methods56 patients with gastric cancer were divided into two groups by different ways of gastrectomy alimentary tract after gartrectomy.Patients in observation group( n =35 ) were given the modified loop jejunum on behalf of gastric surgery and patients in control group( n =31 ) received P-type jejunum on behalf of the stomach surgery.The clinical effects were compared between two groups.ResultsThere were no significant differences in surgical time and bleeding volume in 2 groups ( P > 0.05 ).Emptying time [(61 ± 3 ) min] of observation group was longer than that of control group (37 ± 19) min]( t =3.03,P < 0.05 ) ; Each food intake [( 308 ± 44 ) ml] in observation group was significiantly improved compared with control group [(262 ± 34) ml (t =2.55,P < 0.05) ; The times of daily diet [(4.2 ± 1.2) times] in observation group was lower than that of control group [( 5.7 ± 2.3 )] ( t =2.46,P < 0.05 ).The incidence of postoperative complications( 14.3% ) of the observation group was significantly lower than the control group (42.9%) ( x2 =5.71,P < 0.05).ConclusionOn the terms of 2 ways of digestive tract reconstruction,the improved loop the jejunum behalf of the stomach surgery is superior to the P-type jejunum on behalf of the stomach surgery,which can effectively improve the quality of life of patients and reduce the incidence of complications,but have no complex surgical procedures.
3.Working connotation development and establishment of working model for outpatient triage nurses

Objective To explore the work connotation development and establishment of working model for outpatient triage nurses.Methods 1000 outpatients were selected and randomly divided into the control group and the experimental group with 500 patients in each group.The control group selected doctors freely,while the experimental group let patients choose doctors after work connotation of triage nurses was developed.The application effect of two working models was compared.Results The rates of power satisfaction,anxiety,knowledge of re-visit and total satisfaction degree in the experimental group were higher than those in the control group.Conclusions The implementation of work connotation development for triage nurses can facilitate consultation outside the hospital and reservation for return visit,which is worthy of reference.
4.Research advances on cancer stem cells

Certain specific cancer stem cell subpopulation has been isolated from many kinds of tumors by some specified cell surface maker, including myeloma, lung carcinoma, colon cancer, prostate cancer and pancreatic carcinoma. However, poor individual specificity of cell surface maker molecules and heterograft-related problems provoke more researchers to find more specified cell surface makers and establish humanized mice receptor model in the future. From a clinical perspective, tumor stem cells are considered to be the potential target for curing cancer. Research advances in tumor stem cells can help us to elucidate the close relationship between tumor stem cells and tumorigenesis and provide new ideas and alternative pathways for us to conquer the tumor and avoid tumor recurrence and metastasis.
5.Anti-tumor mechanisms of fucoidan

8.The application research of endoscopic equipment with power through detection of electrical prop-erties in sterilization and supply center

Objective To investigate the effects of the failure rate of laparoscopic apparatus with power through detecting the electrical properties before packaging it. Methods The laparoscopic apparatus with power used in the operation in January to June 2014 were randomly selected and divided into the control group and the observation group according to whether they underwent electricity performance detection. The control group did not receive testing of electrical properties before packing. The observation group tested the electrical conductivity of the single or double pole conductor, unipolar electric coagulation hook, bipolar electric coagulation forceps, the handle of electric knife by digital multimeter and the performance of the uterus rotary cutting machine by electric hysterectomy controller before packing. The difference of equipment in the number of failures, the type of failure, the number of cases and the satisfaction of medical personnel were observed between the two groups. Results In the observation group, 12 cases with unipolar and bipolar coagulation failure, 10 cases with electrocision handle failure, 2 cases with uterine rotary- cutting failure, 18 cases with adverse conductive or non-conductive, 6 cases with short circuit, which were significantly lower than those in the control group (36,25, 11, 48, and 26 cases), the differences were statistically significant (χ2=5.627-12.063, P<0.05). The satisfaction degree in the observation group was 94.74% (36/38), which was better than 76.32%(29/38) of the control group, the difference was statistically significant (Z=-2.267, P<0.05). Conclusions Testing the electric performance of laparoscopic apparatus with power before packing could reduce the incidence of intraoperative equipment failure rate, This method can provide safe and high quality surgical instruments.
9.Neuroprotective effects of salvianolic acid B against Schwann cell injury induced by lipopolysaccharide

BACKGROUND:Salvianolic acid B is an effective monomer component of Salvia miltiorrhiza, which has been shown in recent years to have neuroprotective role and to promote nerve recovery, but its mechanism is not clear. OBJECTIVE: To explore the protective mechanism of salvianolic acid B on Schwann cels in rats with spinal cord injury. METHODS: Schwann cels of Sprague-Dawley rats were cultured and divided into normal control group, model group, 10 μmol/L methylprednisolone group and salvianolic acid B group (0.1, 1, 10, 100 μmol/L). Models of Schwann cel injury induced by lipopolysaccharide were established in al the groups except the normal control group. After intervention, growth curve and proliferative activity of Schwann cels were detected, and protein and gene expressions of β-catenin and nuclear factor-κB were observed. RESULTS AND CONCLUSION: At 48, 72, 96 hours after intervention, the cel viability of the model group was significantly lower than that of the normal control group (P < 0.01). Compared with the model group, the cel viability of salvianolic acid B group (10 μmol/L) was significantly increased at 72 and 96 hours (P < 0.05); the expressions of nuclear factor-κB protein and mRNA in the methylprednisolone group and salvianolic acid B group were declined significantly (P < 0.01), but the expressions of β-catenin mRNA and protein in the salvianolic acid B group significantly increased (P < 0.05). These results suggest that salvianolic acid B improves the viability of Schwann cels which are stimulated with lipopolysaccharide, suppresses expression of nuclear factor-κB mRNA and protein, and promotes the expression of β-catenin mRNA and protein. Above may be one of the mechanisms which salvianolic acid B protects Schwann cels.
10.The application value of laparoscopic surgery in type 2 diabetes patients with acute cholecystitis

Objective To explore the application value of laparoscopic surgery in type 2 diabetes patients with acute cholecystitis.Methods 50 type 2 diabetic patients with acute cholecystitis were chosen as the research objects,the patients were divided into the observation group and control group according to the method of treatment, 25 cases in each group.Strict control of blood glucose levels in patients before operation,the observation group treated by laparoscopy,the control group were treated with conventional open surgery.Recorded the operation time and hemor-rhage,postoperative abdominal drainage,anal exhaust time,patient ambulation time and hospitalization time and other indicators.The patients were followed up to record the postoperative infection,occurrence of complications such as postoperative inflammation.Results There was no statistically significant differences between the two groups in oper-ation time(t =1.02,P >0.05).In the observation group,the intraoperative blood loss was (41.2 ±8.3)mL,postop-erative drainage volume was (107.5 ±16.4)mL,which were significantly lower than those in the control group,and the anal exhaust time was (1.7 ±0.3)d,total hospitalization time was (7.1 ±1.7)d,which were significantly shorter than those of the control group,the differences were statistically significant(t =3.43,3.78,3.43,4.21,all P <0.05).The observation group the complications included wound infection 1 cases,the complication rate was 4.0%, while in the control group,the complications included bile leakage,reflux esophagitis,incision infection,fat liquefac-tion,pulmonary infection and incisional hernia,the overall complication rate was 24.0%,there was statistically signifi-cant difference in the complication rate between the two groups(χ2 =4.54,P <0.05).Conclusion The laparoscopic surgery in the treatment of type 2 diabetic patients with acute cholecystitis has exact effect,and the patients recovery time is short,the complication rate is low.
