Interleukin 31 (IL-31)is one of the cytokines which appears to be an important regulator of Th2 responses. Previous study has been done to determine IL-31 serums levels in atopic dermatitis (AD). However, the serum levels of IL-31 in allergic rhinitis (AR) and atopic asthma (AA) is not many reported and still unclear. The objective of this cross sectional study is to determine an association between IL-31 and other predisposing factors with allergic diseases in HRPZ II (Hospital Raja PerempuanZainab II) and HUSM (Hospital UniversitiSains), Kelantan, Malaysia. This study involved 70 patients of AD, 70 patients of AR, 70 patients of AA and 70 healthy controls from staffs and people in HUSM.Five milliliters of blood were withdrawn and centrifuged for 5 minutes at 2000 rpm to obtain the serum and analyzed for IL-31 levels by using enzymelinked immunosorbent (ELISA) kits (Human IL 31 Duoset, R&D System). Simple and multiple logistic regressions were used to analyze the association between IL-31 levels and predisposing factors among allergic diseases. The levels of IL-31 and other predisposing factors showed significant associations in smoking status, occupational exposure and area of living for AD and AR, however in AA, the significant association only found in smoking status and occupational exposure. In conclusion, we found that there were associations between IL-31 serum levels and other predisposing factors with AD, AR and AA. The findings can be the pilot study to determine IL-31 levels in allergic diseases in Malaysia.

Gold therapy of rheumatoid arthritis is effective, but adverse reactions are also common. There are some cutaneous, hematologic, renal, and pulmonary side effects which have been associated with immunologic abnormalities. Also, hypersensitivity pneumonitis induced by gold therapy was rarely reported in literature. But, the mechanism for the induction of the pulmonary disease is not blown. We report a case of seropositive rheumatoid arthritis presented as gold induced hypersensitivity pneumonitis which was confirmed by the peripheral blood lymphocyte stimulation test with gold.

7.Allergic Bronchopulmonary Aspergillosis Coupled with Broncholithiasis in a Non-asthmatic Patient.

Allergic bronchopulmonary aspergillosis (ABPA), an asthmatic disease, is caused primarily by hypersensitivity to Aspergillus species. ABPA is rarely observed in the absence of asthma, which is, in fact, the principle criterion for its diagnosis. Here, we report the case of a 36-yr-old woman without a history of bronchial asthma, who manifested a localized pneumonic consolidation, coupled with broncholithiasis. Pathologic examinations of bronchoscopic biopsy specimens and resected surgical specimens revealed features typical of ABPA. This is a very rare case of ABPA coupled with broncholithiasis in a non-asthmatic individual.

8.In Vivo Delayed Hypersensitivity to the Recall Antigens in Different Dermatoses.

Authors have performed patch test to evaluate the differences between the allergic contact dermatitis to topical medicaments and to the other substances including cosmetics. One hundred eighteen patients were evaluated according to age, sex, and causative agents of the disease. Furthermore, these patients were analysed as to the causative allergic contact factors by clinical inspection and history. The most common causative allergic contact factors by clinical, history and patch test were topical medicarnents(45.8%), cosmetics(24. 6%), rubbery(10.2%), and metals(8.59%.). The common etiogloic medicaments of contact dermatitis were Madecassol, Betadin, Tiger balm, Canesten, Mercurochrom, and Tego solution. The causative agents were confirmed by the patch test with Hollister-Stiers standard allergens in 57 patients from Jan. 1983 to Dec. 1984, 1 hirty eight among 57 patients were positive to patch test, Back ground possible explanation of the patch test results were discussed and a comparison was made between this report and others.

9.Usefulness of the MAST in Allergic Skin Diseases.

BACKGROUND: Allergic skin diseases such as atopic dermatitis and urticaria are known to be mediated by IgE. It is important to confirm a causative allergen for diagnosis and treatment. The multiple allergosorbent test (MAST) is a simple method for simultaneously measuring total and allergen-specific IgE. OBJECTIVE: The purpose of this study was to analyze results from the MAST, which measures total and specific IgE, and to compare the different results. METHODS: We reviewed the MAST results of 270 allergic disease patients tested between June 2007 and May 2012. RESULTS: There were statistical differences in total IgE production and in positive sensitization to specific allergens between the disease groups. The level of total IgE and positive rates of specific IgE were highest in atopic dermatitis patients, followed by urticaria, allergic contact dermatitis, and pruritus patients. Atopic dermatitis patients had significantly more allergens than those with other diseases. There were no statistical differences in total IgE level, rates of positive sensitization to specific allergens, and the number of causative allergens between the patients with acute and chronic urticaria. CONCLUSION: Each disease showed a different IgE pattern. Atopic dermatitis showed the highest level of serum IgE. There were no differences in IgE levels between acute and chronic urticaria patients. We identified an increase in IgE level in allergic contact dermatitis patients. Further study is needed to determine whether these patterns could be useful in diagnosis and choice of treatment methods.

Hypersensitivity pneumonitis (HP) is an immunologically-mediated disease resulting from repeated exposure to sensitizing agents, such as organic dusts or chemicals. Isocyanate is a volatile and highly reactive chemical that is extensively used in the manufacturing of automobiles, upholstery, and polyurethane foam. Occupational respiratory diseases associated with isocyanate, such as bronchial asthma, are well-known. It is thought that HP is one of the rare diseases induced by isocyanate with a very low frequency worldwide. We report a case of HP in an automobile painting sprayer which appeared to be associated with isocyanate.
