1.Analysis on Medication Experiences of Professor Sun Zixue in the Treatment of Asthenospermia

This study was aimed to analyze medication experiences of Professor Sun Zixue in the treatment of asthenospermia. A total of 286 asthenospermia cases treated by Prof. Sun Zixue were collected. Frequency and index clustering analysis method were used in the processing of patient data. The results showed that the using frequency of 37 types of drugs is more than 10%. And the clustering analysis classified drugs into seven cat-egories , which were the kidney essence tonification , kidney-yang tonification , qi tonification , blood nourish-ment , heat-toxin reducing , heat-dampness reducing , blood-activation and collateral-regulation. It was conclud-ed that based on Prof. Sunˊs syndrome differentiation , the treatment of asthenospermia can be from the combi-nation of kidney tonification , qi tonification and blood nourishment , or the combination of heat-dampness re-ducing and blood-activation and stasis-removing , or the combination of kidney tonification and collateral-regu-lation. The commonly used drugs are 38 types from seven categories .
2.Clinical research of chronic pelvic cavity pain syndrome treated with acupoint catgut embedding therapy.

OBJECTIVETo explore the clinical efficacy of acupoint catgut embedding therapy on chronic pelvic cavity pain syndrome differentiated as kidney deficiency and stagnation of damp heat, and explore the impacts on plasma P substance (SP), plasma beta-endorphin (β-EP).
METHODSOne hundred and eighty cases were randomly divided into a catgut embedding group (90 cases) and a western medication group (90 cases). In the western medication group, tamsulosin capsules 0. 2 mg were prescribed for oral administration, once a day; indometacin sustained release tablets, 25 mg, three times a day. Totally, the oral administration for 8 weeks was required. In the catgut embedding group, the acupoint catgut embedding therapy was applied to Qugu (CV 2), Shenshu (BL 23), Zhibian (BL 54), Huiyin (CV 1) and Sanyinjiao (SP 6), once every two weeks; the treatment of 4 weeks made one session, and two sessions were required. Before and after treatment, TCM symptom score, NIH-CPSI (the National Institute of Health Chronic Prostatitis Symptom Index) score, lecithin body numbers in prostatic fluid, score in SAS (self-rating anxiety scale), score in SDS (self-rating depression scale), the levels of SP and β-EP, etc. were observed in the two groups, and the clinical efficacy was assessed in the two groups.
RESULTS(1) Ten cases were dropped in either group. The total effective rate was 91. 25% (73/80) in the catgut embedding group, higher than 78. 75% (63/80) in the western medication group (P<0. 05). (2) After treatment, TCM symptom score, total score in NIH-CPSI, pain score and the scores in SAS and SDS were all reduced as compared with those before treatment in the two groups (all P<0. 05). After treatment, TCM symptom score, total score and pain score in NIH-CPSI, and the scores in SAS and SDS in the catgut embedding group were both lower than those in the western medication group (all P<0. 05). (3) After treatment, the lecithin body numbers were both increased as compared with those before treatment in the two groups (both P<0. 05), and the result in the catgut embedding group was higher than that in the western medication group (P<0. 05). (4)After treatment, the SP level was lower than that before treatment in the two groups (both P<0. 05); the level of p-EP was increased as compared with that before treatment (both P<0. 05). The SP level in the catgut embedding group was lower than that in the western medication group (P<0. 05); the level of β-EP was higher than that in the western medication group (P<0. 05).
CONCLUSIONThe acupoint catgut embedding therapy apparently relieves the clinical symptoms of chronic pelvic cavity pain syndrome differentiated as kidney deficiency and stagnation of damp heat as well as the condition of anxiety and depression, increases lecithin body numbers in prostatic fluid and β-EP level and reduces SP level.

3.Analysis on Effect of Free-frying Granules of Traditional Chinese Medicine on Treatment of Chronic Prostatitis

This study was aimed to evaluate the efficacy and safety of free-frying granules of traditional Chinese medicine(TCM) on treatment of chronic prostatitis. A total of 120 chronic prostatitis patients that were diagnosed with the dampness-heat and blood-stasis syndrome and kidney-deficiency syndrome were divided into the TCM group and western medicine group by random digit table. Category II and ⅢA chronic prostatitis were treated with oral administration of Levofloxacin Mesylate Tablets (0.2 g, bid) and the category ⅢB chronic prostatitis was treated with oral administration of Terazosin Hydrochloride Tablets (2 mg, qd). In the TCM group, the dampness-heat and blood-stasis syndrome was treated with Tongluo Qingjie formula ( which was self-made formula containing Rhizoma Dioscoreae Hypoglaucae, Herba Patriniae, Semen Plantaginis, and etc.) and the kidney-deficiency syndrome was treated with Jiawei Wuzhi Yanzong pills (which containing Semen Cuscutae, Chinese Wolfberry, Radix Rehmanniae Preparata, Fructus Rubi and Schisandra chinensis, and etc.). Four weeks was one course of treatment. The results showed that the total effective rate of TCM group was better than that of western medicine group (P< 0.05). Effect of TCM group was obviously better than that of western medicine group in the improvement of prostatitis NIH-CPSI score, the quality of life and symptoms of the patients. TCM may increase lecithin corpuscle in prostatic fluid of ⅢA chronic prostatitis (P< 0.05). Meanwhile, no obvious toxiferous and side reactions were found. It was concluded that the free-frying granules of TCM have good efficacy and safety in the treatment of chronic prostatitis.
4.Spatial heterogeneity of left ventricular myocardium attenuation during 64 multi-slice CT coronary artery angiography

Objeetive To analyze the attenuation differences of left yentricular myocardium on 64-slice CT coronary artery angiography in adults.Methods of 1130 consecutive patients,119 patients (65 males,54 females,ranged from 27 to 83 years with mean age of 52 years)with no stenosis or stenosis less than 25% in the left anterior descending arteries were enrolled.The standard axial image with 3mm slice thickness just below the aorta valve was selected for measttrement.The regions of interest(0.1 cm× 0.1 cm)in posterior interventricular septum(septal P),anterior interventricular septum(septal A),apex,anterior free wall and posterior free wall of the left ventricle were selected within the myocardium and the mean CT value wag measured.A paired Student t test was used for the comparison of CT values in different areas,P value under 0.05 was considered statistically significant.Results The megn attenuafion of the apical region(69.9±15.5)HU was lower than that of septal A(91.9±15.1)HU(t=11.060,P<0.01)and anterior free wall(79.7±16.9)HU(t=4.639,P<0.01),the mean attenuation in septal(93.8±14.8)HU and posterior free wall(88.0±14.3)HU showed relatively higher values.The myocardial intensity attenuation presented as a U-shape in the left ventricular myoeardium. Conclusions The mean attenuation in the left ventrieular myoeardium demonstrates as U-shape during the coronary artery angiography examination in normal adults.This can be helpful for iudging the extent of myocardial infarctiotr.
5.Imaging features of primary mediastinal cysts with clinical and histopathologic correlation

Objective To characterize the imaging features of mediastinal cysts and gain a better understanding of atypical manifestations of various mediastinal cysts and improve the level of diagnosis. Methods The CT and/or MR images(CT n=28, MRI n=26, CT and MRI n=10), surgical information, and pathologic material in 44 histopathologically proved cases of mediastinal cyst were retrospectively reviewed. Results The mediastinal cysts were located in the anterior mediastinum(n=13), middle mediastinum(n=18), anterior-middle mediastinum (n=2) and posterior mediastinum(n=11). Bronchogenic cysts and pericardial cysts were atypically located in 7/20(35%)and 5/6 respectively. The density of 42.9% cysts on CT was close to that of water. Each of the cysts had a high signal intensity that was equal to or greater than cerebrospinal fluid on T_2-weighted MR images. One cyst showed marked loss of signal on MR hydrography. The signal intensities of the cysts were variable on T_1-weighted images. The signal intensity of MRI was not homogeneous in 5 and the reasons were different. Fourteen cases were misdiagnosed. Conclusion 1.The cysts with soft-tissue attenuation on CT in the anterior and posterior mediastinum may be easily misdiagnosed as neoplasm. Pericardial cysts located in the paratrachea may be easily misdiagnosed as bronchogenic cysts .2. MRI can be used to diagnose the cysts with high density on CT.3. Heterogeneous signal on MRI and loss of signal on MR hydrography might be the reasons for misdiagnosis.
6.Effects of serum cystatin C level on the occurrence and its long-term prognosis of contrast agent-induced acute kidney injury after coronary intervention in elderly patients

Objective:To investigate the effect of serum cystatin C level on the occurrence and its long-term prognosis of contrast agent-induced acute kidney injury(CI-AKI)after percutaneous coronary intervention(PCI)in elderly patients.Methods:A total of 848 elderly patients(≥60 years)undergoing PCI in our department between Mar 2015 and Dec 2017 were enrolled in a prospective cohort.The CI-AKI was defined as the increase of serum creatinine ≥44.2 μmol/L within 48-72 h after using iodine contrast agent or more than 25 % higher than base level within 48-72 h after PCI.A receiver operating characteristic curve was used to analyze the optimal cut-off value of Cystatin C for predicting CI-AKI after PCI.Patients were divided into 2 groups based on the optimal cut-off value of Cystatin C: the high Cystatin C group(Cystatin C ≥1.3 mg/L, n=178)and the control group(Cystatin C<1.3 mg/L, n=670). The differences in the incidence of CI-AKI after PCI and major adverse cardiac events(MACE)at 1 year follow-up were compared between the two groups.The Cox regression model was further used to analyze the predictors of the long-term prognosis after PCI.Results:Of 848 patients receiving PCI, the incidence of CI-AKI was 9.4%.The incidence of MACE at 1 year after PCI was higher in the high Cystatin C group than in the control group(15.7% vs.9.3%, χ2=6.524, P=0.011). Cox regression analysis confirmed that the high baseline level of Cystatin C was the most independent predictive factor for MACE at 1 year of follow-up( HR=16.244, P<0.001). Conclusions:The high baseline level of Cystatin C(≥1.3 mg/L)is an independent risk factor for CI-AKI and is also the most important predictor for the occurrence of long-term MACE in elderly patients undergoing PCI.
7.Clinical Efficacy of Longbeisan Application at Shenque (CV 8) Combined with Oral Administration of Chinese Medicine in Treatment of Premature Ejaculation

ObjectiveTo investigate the clinical value of Longbeisan application at Shenque (CV 8) combined with oral administration of Chinese medicine in the treatment of premature ejaculation (PE). MethodA total of 98 PE patients treated in the andrology department of the First Affiliated Hospital of Henan University of Chinese Medicine at the same time period were randomly assigned into an observation group and a control group, with 49 patients in each group. The observation group received Longbeisan application at Shenque (CV 8) combined with oral treatment of Chinese medicine according to syndrome differentiation, and the control group was treated with dapoxetine hydrochloride tablets. The treatment in both groups lasted for 8 weeks. The intravaginal ejaculatory latency time (IELT), Chinese index of premature ejaculation-5 (CIPE-5) score, patient's sexual life satisfaction, spouse's sexual life satisfaction, effective rate, and adverse reaction incidence were compared between the two groups. ResultAfter treatment, the observation group had higher total effective rate than the control group [(85.71% (42/49) vs. 67.35% (33/49), χ2=6.262, P<0.05]. The IELT, CIPE-5 score, and patient's and spouse's satisfaction scores after treatment increased compared with those before treatment (P<0.01), and the increases were more significant in the observation group (P<0.05, P<0.01). The clinical effect of the observation group was better than that of the control group. During the treatment, 7 (7/49,14.29%) patients in the control group and 2 ,2/49,4.08%) patients in the observation group showed adverse reactions, which indicated the safety of the observation group was better than that of the control group (χ2=9.000, P<0.05). In the follow-up period, 11 (11/49,22.45%) patients in the control group and 3 (3/49,6.12%) patients in the observation group showed aggravation of symptoms, which meant that the observation group had better lasting effect (χ2=0.317, P<0.05). ConclusionLongbeisan application at Shenque (CV 8) combined with oral administration of Chinese medicine has better clinical effect, stronger safety, and longer effect than dapoxetine hydrochloride in the treatment of PE.
8.Retrospective Study in Applying Fuzheng Huaji Yingjie Prescription to Treat 132 Cases of Peyronie's Disease

ObjectiveTo retrospectively analyze 132 cases of Peyronie's disease in Prof. WANG Zulong's outpatient clinic,evaluate the clinical efficacy of Fuzheng Huaji Yingjie prescription in the treatment of Peyronie's disease,explore the main causes of Peyronie's disease,and optimize the color ultrasound diagnosis method of Peyronie's disease. MethodsThe case data of patients diagnosed with penyronie's disease in the Department of Andrology of the First Affiliated Hospital of Henan University of Chinese Medicine from November 2019 to October 2024 were selected,and SPSS 29.0 was used to statistically analyze the general data,concomitant diseases,location of induration,size of induration before and after treatment,pain severity,elasticity of penile induration,angle of penile curvature,and erectile function,and the adverse reactions and treatment methods during treatment were recorded. ResultsDiabetes mellitus accounted for about 40.15% of the diseases associated with Peyronie's disease,and the size,erection pain,and elasticity and bending angle of penile induration were significantly improved after 12 weeks of treatment with Fuzheng Huaji Yingjie prescription after treatment (P<0.05). There was no significant difference in erectile function compared with that before treatment,and there were nine cases of adverse reactions during the treatment,all of which were gastrointestinal reactions caused by improper medication,and the symptoms disappeared after adjustment. ConclusionDiabetes mellitus may be the primary risk factor of Peyronie's disease,and two-dimensional ultrasound combined with elastography may be the current optimal scheme for the diagnosis of Peyronie's disease. Fuzheng Huaji Yingjie prescription is safe and effective in the treatment of Peyronie's disease.
9.Effect of Wuwei Fuzheng Yijing Decoction on Sperm DNA Integrity in Male Infertility Patients

ObjectiveTo observe the effect of Wuwei Fuzheng Yijing decoction on sperm DNA integrity in male infertility patients. MethodOne hundred male infertility patients with sperm DNA damage (spleen and kidney Qi deficiency and blood stasis) that met the inclusion criteria were randomly divided into an observation group (Wuwei Fuzheng Yijing decoction) and a control group (L-carnitine oral solution), with 50 cases in each group. After 12 weeks of treatment, the DNA fragmentation index (DFI) and traditional Chinese medicine (TCM) syndrome score, progressive sperm motility (PR), sperm survival rate, and sperm concentration of the two groups before and after treatment were observed and compared. ResultThe total effective rate of the observation group was 89.80% (44/49) higher than 62.50% (30/48) of the control group (P<0.05). Compared with the conditions before treatment, the DFI and TCM syndrome score of the two groups were reduced after treatment, while the PR, sperm survival rate, and sperm concentration were increased (P<0.05), and the observation group was superior to the control group in terms of the above indexes (P<0.05). ConclusionWuwei Fuzheng Yijing decoction obviously repairs sperm DNA of male infertility patients and improves sperm quality, with high safety.
10.Efficacy and safety of phosphodiesterase type 5 inhibitors and selective serotonin reuptake inhibitors in the treatment of comorbiditiy of erectile dysfunction and premature ejaculation: a meta analysis

【Objective】 To evaluate the efficacy and safety of phosphodiesterase type 5 (PDE5) inhibitors alone or in combination with selective serotonin reuptake inhibitors (SSRIs) compared with SSRIs alone in the treatment of comorbidity of erectile dysfunction (ED) and premature ejaculation (PE). 【Methods】 The clinical randomized controlled trials of ED and PE comorbidity treated with PDE5 inhibitors alone or in combination with SSRIs were searched from database inception to Sep.2022, in CNKI, PubMed, Web of Science, Embase, Wanfang Database, cqVIP Database, SinoMed and Yiigle. The intravaginal ejaculatory latency time(IELT), score of International Index of Erectile Function 5 (IIEF-5) and adverse reaction rate were analyzed with RevMan 5.4.1 software. 【Results】 A total of9 studies involving 793 patients were included. Meta analysis showed that compared with SSRIs alone, PDE5 inhibitors alone or in combination with SSRIs yielded better results in IELT [MD=1.99, 95%CI(1.51-2.46), P<0.001] and higher IIEF-5 score [MD=4.61, 95%CI(3.68-5.55), P<0.001] , but no increase in adverse events [RR=0.99, 95%CI(0.74-1.31), P=0.92] . 【Conclusion】 In the treatment of ED and PE comorbidity, priority should be given to ED or both ED and PE, which can produce better efficacy without increasing the adverse effects.
