1.CHANGES OF THE FUNCTION OF SPONTANEOUS T SUPPRESSOR (ELL AND THE SUBSET OF T CELLS IN PATIENTS WITH VARIOUS STAGES OF SCHISTOSOMIASIS JAPONICA

The function of spontaneous T suppressor cell (STs) of peripheral blood was detected in 17 patients with acute schistosomiasis japonica, 14 cases with chronic schistosomiasis and 25 patients with advanced schistosomiasis. The subset of T cell were simultaneously phenotyped in all cases except 10 patients with advanced schistosomiasis. Among the patients with acute schistosomiasis the rates of OKT3 + (pan T cell), OKT4+ (helper T cell) and OKT8 + (suppressor/cytotoxic T cell) were all significantly increased, and the function of STs was normal. In chronic and advanced schistosomiasis, the rate of OKT8 + T cell was raised, the rate of OKT3+ T cell and ratio of OKT4+/OKT8+ were statistically reduced, and the function of STs was significantly increased. The rate of OKT8+ T cell was negatively correlated with the function of STs in acute schistosomiasis, whereas in chronic and advanced rchistosomiasis, the rate of OKT8 + T cell had significant positive correlation with STs function while OKT4 +/OKT8 + had significant negative correlation with this function. The results indicate that the cellular immune response is increased in patients with acute schistosomiasis and down regulated in patients with chronic and advanced schistosomiasis. The function of T suppressor cell and the subset of T cell may play an important role in the immunoregulation of patients with schistosomiasis japonica.
2.Analysis of 44 Cases of Iatrogenic Preterm Labor

Objective To analyse the affecting factor of iatrogenic preterm labor and the prognosis of premature infants. Methods 44 cases of iatrogenic preterm labor and 64 cases of preterm labor of caesarean section because of/in our hospital other reasons from 1998 to 2001 were enrolled in this study. The risk factors of the two groups of pregnant woman and the prognosis of their premature infants were analyzed. Results Iatrogenic preterm labor was related with multiple factors. Regular antenatal care and proper treatment may get a better pregnant result in the inevitable iatrogenic preterm delivery. Conclusion Selective caesarean section and intensive care after birth will reduce the complications and the mortality of the premature infants.
3.Effect of cognitive-behavioral intervention on mental state and quality of life in elderly patients with coronary heart disease

Ninety elderly patients with coronary heart disease were randomly divided into 2 groups:in study group (n = 45) the cognitive-behavioral intervention as well as conventional medical treatment were performed and in control group ( n = 45 ) only conventional treatment was given. Short Form-36 ( SF-36),symptom checklist 90(SCL-90), Hamilton anxiety scale( HAMA), Hamilton depression scale( HAMD)were evaluated before and after intervention, the serum total cholesterol (TC), triglyceride (TG)and body mass index (BMI)were measured, the smoking and drinking behaviors were recorded. After 6-month of intervention the smoking [(0. 6 ± 0. 2)cigarettes/d] and drinking [(10. 1 ± 2. 3)g/d] behaviors, the scores of SF-36 (85 ± 16), SCL-90 (104 ±4), HAMA (10 ±4) and HAMD (12 ±3), the systolic blood pressure [( 123 ± 11 ) mm Hg ( 1 mm Hg = 0. 133 kPa)], diastolic blood pressure [(77 ± 5 ) mm Hg], the fasting blood glucose [(4. 2 ± 2. 6 ) mmol/L] and BMI ( 22.5 ± 0. 5 ) in study group were significantly improved compared to control group ( P < 0. 01 ). The results indicate that cognitive-behavioral intervention can improve the mental state and quality of life in elderly patients with coronary heart disease.
4.The correlation between myocardial microcirculation and inflammation in acute myocardial infarction

Objective To investigate the correlation between myocardial microcirculation and inflammation in acute myocardial infarction(AMI)by myocardial contrast echocardiography(MCE).Methods Eighty-one AMI patients(AMI group)and 30 healthy adults(normal control group)were enrolled.White blood cell(WBC),hypersensitive C reactive protein(hsCRP)and neutrophilic gtanulocyte percentage(G%)were measured in patients and healthy adults.According to different count of WBC,AMI group was devided into A group(n=40,WBC≥10×109/L)and B group(n=41,WBC<10×109/L).Myocardial contrast intensity(A),blood flow rate(β)and myocardial blood volume(MBF=A×β)of each segment were measured by MCE during 5-7 days of hospitalization.Results Compared with control group.WBC,hsCRP in A group and hsCRP in B group were significantly elevated(P<0.05).A,β and MBF in A group were significantly lower than those in B group(P<0.05).WBC and hsCRP in A group had significant negative correlations with β respectively(BWBC=-0.67,P<0.05;BhsCRP=-0.5 1,P<0.05).Conclusions The level of plasma inflammatory factors were remarkably increased after AMI,which had a positive correlation with low microcirculation perfusion.
5.CHANGES OF LEVEL OF IL-1 PRODUCED BY PERIPHERAL BLOOD MONOCYTES IN PATIENTS WITH SCHISTOSOMIASIS JAPONICA IN VITRO

Activity of IL-1 produced by peripheral blood monocytes stimulated with lipopolysaccharide in vitro was detected from 17 acute cases, 14 chronic cases and 19 advanced cases of schistosomiasis japonica. It was found that the level of IL-1 was significantly increased and positively related to the body tempereture in the group with acute schistosomiasis. The activity of IL-1 was statistically reduced in the chronic and advanced groups, especially in the latter. After inhibiting the synthesis of prostaglandin with indomethacin, the level of IL-1 was significantly increased in three groups of patients, but no apparent change in the normal contro group. The results indicate that IL-1 may play an important role in inducing the inflammatory reaction in patients with acute schistosomiasis japonica and in the immunoregulation in the chronic stage. The changes of IL-1 activity in patients with schistosomiasis japonica may be closely related to prostaglandin.
6.Therapeutic choice of axillary treatment after neoadjuvant chemotherapy in patients with node-positive breast cancer

Objective: To analyze the feasibility of axillary lymph node staging through sentinel lymph node biopsy (SLNB) after neoad-juvant chemotherapy (NAC) in patients with node-positive breast cancer and to explore the follow-up treatment of these patients. Methods: Clinical data of 82 patients with node-positive breast cancer before NAC in Tianjin Medical University Cancer Institute and Hospital from January 2016 to January 2018 were analyzed retrospectively. All these patients accepted SLNB after NAC. The detection rate, accuracy, false negative rate (FNR), and influencing factors were analyzed. Results: A nodal pathological complete response (PCR) was achieved in 43 of 82 patients. The PCR rate was 52.4%. The detection rate, accuracy, and FNR were 97.56% (80/82), 88.75% (71/80), and 23.08% (9/39), respectively. The accuracy of 1, 2, and≥3 SLNs detected were 90.9% (20/22), 66.7% (10/15), and 95.3% (41/43), respectively. The FNRs were 20.0% (2/10), 71.4% (5/7), and 9.1% (2/22), respectively (both P<0.05). Conclusions: Due to its overall high FNR, without clinically acceptable limits, post-NAC SLNB cannot completely replace axillary lymph node dissection (ALND) in node-positive patients. However, with no less than 3 SLNs detected, SLNB can accurately evaluate the status of axillary lymph nodes.
7. Effect of the primary tumor location on the prognosis of breast invasive ductal carcinoma patients treated with radical mastectomy

Objective:
To analyze the clinicopathological features and prognosis of breast invasive ductal carcinoma patients receiving radical mastectomy according to the primary tumor location.
Methods:
From January 2008 to December 2008, 993 patients with breast invasive ductal carcinoma received radical mastectomy in Tianjin Medical University Cancer Institute and Hospital. Patients were grouped according to the primary tumor location when breast cancer was diagnosed. The clinicopathological characteristics and follow-up information of them was collected and analyzed retrospectively.
Results:
Of the 993 patients, primary tumor located in the upper-outer quadrant (UOQ) in 556 patients (56.0%), the lower-outer quadrant (LOQ) in 97 (9.8%), the central portion in 99 (10.0%), the upper-inner quadrant (UIQ) in 186 (18.7%), and the lower-inner quadrant (LIQ) in 55 (5.5%). Patients in the central portion tended to have larger tumors, and more patients in the upper-inner quadrant received endocrine therapy. The estimated 5-year disease-free survival (DFS) rates of patients with primary lesion in the UOQ, LOQ, central portion, UIQ and LIQ were 90.3%, 88.7%, 79.8%, 86.0% and 72.7%, respectively, with significant differences (
