1.Prevention and treatment for complications of cardiac myxoma excision in 215 cases

Objective To summarize the prevention and treatment experience of complications of cardiac myxoma excision.Methods Cardiac myxoma excision were performed in 215 cases under general anesthesia,cardiopulmonary bypass and cardiac arrest condition.Results Two cases died from severe low cardiac output syndrome which eventually caused multiple organ failure.The remaining 213 cases were cured and discharged.Among 213 cases,low cardiac output syndrome occurred in 35 cases,re-open chest for bleeding was performed in 8 cases,30 cases in arrhythmia (including paroxysmal supraventricular tachycardia in 20 cases and ventricular premature in 10 cases),no vital organs embolism occurred.Follow-up was done to 200 patients for 1-8 years with a follow-up rate of 93.9% (200/213).In 200 cases,1 case died from lung cancer,1 case recurred and was cured after re-operation.The remaining patients had no obvious abnormalities.Conclusion As the particularity of surgical treatment of cardiac myxoma,it is essential to take prevention measures to reduce complications of cardiac myxoma excision in perioperative periods.
2.Purification,renaturation and antiviral effects of recombinant cyanovirin-N on herpes simplex virus type 1

Objective To purify,renature and explore the antiviral effects of recombinant cyanovirin-N(CV-N)on herpes simplex virus type 1(HSV-1).Methods The recombinant CV-N was purified with Ni Sepharose column and renatured by dilution method.The antiviral activities of CV-N were carried out in Vero cells by observing cytopathic effect(CPE)and by using MTT colorimetric assay for vital cell rate.Results SDS-PAGE showed that the purified protein was in the position of 11KDa with only one clear band.The renatured CV-N had little cytotoxic effect on Vero cells,it could not directly inactivate HSV-1 infectivity.CV-N not only interfered in adsorption of HSV-1 to Vero cells but also inhibited HSV-1 biosynthesis in the cells,which were more effective than the positive control Acyclovir.Conclusion CV-N exhibited pronounced antiviral activities agaist HSV-1,further development of CV-N might yield novel candidates of antiviral drugs.
3.Effects of compound Stichopus variegatus mucopolysaccharide on the levels of blood nitric oxide and endothelin after angioplasty in rabbits

Objective To investigate the effects of Stichopus variegatus mucopolysaccharide on plasma nitric oxide (NO) and serum endothelin(ET) after angioplasty in rabbits. Methods 50 New Zealand rabbits were randomly divided into four groups: Stichopus group, simvastatin group, model group and normal group. Endothelium of iliac arteries in Stichopus group, simvastatin group and model group were denuded by balloon catheter and the rabbits were fed with 2%cholesterol, 3%lard and 3%yolk mixed forage for six weeks, and then, atherosclerotic stenosis was showed by iliac angiography.When transluminal balloon angioplasty was done, from then on the feeds was changed from high cholesterol to ordinary forage. Stichopus (0.5g?kg -1?d -1) and simvastatin (0.5mg?kg -1?d -1) were given by gastric canal. While NaCl solution (0.9%) was given in model group. All rabbits were fed in separate cages and free drinking. After four weeks since transluminal balloon angioplasty, blood was collected from carotid artery to measure the changes of concentrations of endothelin(ET) and nitric oxide(NO). Examination of pathology by optical and electron microscope were done at same time. Results After four weeks since transluminal angioplasty, the concentration of plasm ET in model group increased significantly compared with the normal group(114.86?24.89 ng?L -1 vs 75.62?9.68 ng?L -1) and serum NO decreased significantly(37.50? 12.79?mol?L -1 vs 106.37?18.01 ?mol?L -1).The concentration of plasm ET in model group was higher than that in the simvastatin groups and Stichopus groups(114.86?24.89 ng?L -1 , 81.08?14.71 ng?L -1 and 83.34?11.41 ng?L -1 ,respectively),and the concentration of serum NO decreased obviously(95.63?8.62 ?mol?L -1,98.80?9.06 ?mol?L -1 and 37.50?12.79 ?mol?L -1,respectively). Conclusion After iliac arterial TA the concentrations of plasm ET were increased and the serum NO were decreased, the endothelial function fell into disorder. The Stichopus variegatus mucopolysaccharide had the effects of improving the endothelial function after angioplasty in rabbits by adjustment of plasm ET and serum NO.
4.Extension Force Deficiency of the Lower Extremity in Squat Contributing to Increased Fall Risk in the Female Elderly

Objectives To explore the relationship between the extension force deficiency of the lower extremity and fall risks among the senior female,so as to reveal the potential factors contributing to fall.Methods Forty community-dwelling senior females aged 65 and older were assigned to the group of fallers or non-fallers according to their reported fall history and measured fall risk index (FRI).Extension force of the lower extremity was measured through 3-consecutive fast squats on a force platform.The time for 3m-time up and going (TUG),as well as static and dynamic balance were also measured.Results Compared to the non-fallers,the fallers showed lower peak extension force per body weight (PEF/BW) of the left leg and larger asymmetry of the peak extension force per body weight between the two legs in squats.Correlation analysis showed that FRI had a strong negative relation with PEF/BW of the left leg and a strong positive relation with extension force asymmetry/body weight.Also time for 3m-TUG was positively related to all the force variables standardized by the body weight,especially for the left leg.However,the analysis failed to find a relationship between the velocity of the center of pressure (COP) sway on static balance and any force variables.In addition,PEF/BW of the left leg decreased and force asymmetry/body weight between the two legs increased with aging.Conclusion Extension force measurement in squat is an effective way to assess the muscle strength deficiency related to the increased fall risk.The extension force of the lower extremity to support the body weight in squat had a strong relation to the function decline in the dynamic balance,which contributes greatly to fall risk.The extensive force asymmetry in squat between the two legs is another important fall risk factor for the senior females.
6.The influence of atrial fibrillation on prognosis after mitral valve surgery in rheumatic mitral valve stenosis and mitral valve prolapse patients

Objective Atrial structure remodeling is the important pathologic basis of generate and development in chronic atrial fibrillation(AF) of valvular heart disease.To analyze the changed feature of AF in rheumatic mitral valve stenosis (MS) and mitral valve prolapse(MVP) after mitral valve surgery,along with fundamental change of hemodynamics in left atrial.Methods Firstly,divided into sinus rhythm (SR) group and AF group according to cardiac rhythm postoperative 6 months,and then divided into MS and MVP two subgroups with age matched,namely rheumatic sinus rhythm group (RS group),MVP sinus rhythm group(PS group),rheumatic AF group(RAF group) and MVP AF group(PAF group),30 patients in each group.Independent sample t test andx2 test were used in comparison among groups,and matched t test in preoperative and postoperative comparison of each group.Results There are 15 (50%) AF patients before surgery and 10 (33.3%) AF patients postoperative 1 month in RS group.But cardiac rhythm of MVP patients has no significant change.Left atrial diameter(LAD) in AF group was larger than in SR group significantly preoperative and postoperative 1 month and 6 months(P < 0.05),and LAD have no significant difference between RAF and PAF group,P > 0.05 ; LAD in RS group preoperative and postoperative 1 month was larger than in PS group(P =0.008 and 0.018,respectively),but there is no significant difference between RS and PS groups postoperative 6 months(P =0.558).Systolic peak velocity(Smm) at valve ring with PWTDI were(6.0 ± 1.4) cm/s,(6.7 ± 1.8) cm/s and (6.2 ± 1.6) cm/s preoperative and postoperative 1 month and 6 months,lower than normal range obviously; Smm before surgery in PAF group was(9.3 ± 3.7)cm/s,but reduced obviously after surgery 1 month and 6 months and near the level of rheumatic patients.Conclusion Generate and development mechanism of AF in MS and MVP patients exist some extent difference,the rhythm of partial MS patients with chronic AF will turn to and maintain sinus rhythm along with LAD decreased,there is no this characteristic in MVP patients.
7.Development of dynamic EMG scanning signal generator.

Development of a dynamic EMG scanning signal general based on microcontroller is introduced to be used in such fields as research of dynamic magnetic resonance scanning and moving muscle surface EMG. Optical fiber and photoelectrical switch are applied to work in strong electromagnetic environments to produce trigger signal which is similar to ECG. The occurrence of signals and the activity of the scanned bodies can be delayed at will. The results show that it not only has complete functions, simple operation, but also has superior capability of stabilization and credibility than the same product.

8.Clinical analysis for 241 cases of subpulmonic ventricular septal defect

Objective To summarize the clinical features and treatment experience of subpulmonic ventricular septal defect (SPVSD). Method The clinical data of 241 cases of SPVSD were analyzed retrospectively. Results Two hundred and fifteen cases were confirmed by echocardiography before operation, while other 26 cases were misdiagnosed (10.79%, 26/241), 42 cases with aortic valve prolapsed,ECG showed left ventricular hypertrophy. All patients underwent surgical repair of VSD. Other procedures had been done in the same stage including aortic valve replacement (AVR) in 8 cases, aortic valvuloplasty (AVP) in 6 cases and other operations. The size of VSD was larger than the value echocardiography measured before operation. There were no perioperative death and no complete atrioventricular block. Two hundred and thirty cases (95.44%, 230/241) were followed up for 3 months to 5 years. The cardiac function (NYHA) in 189 cases were grade Ⅰ and 41 cases were grade Ⅱ. There was no residual shunt of VSD. Among patients who underwent AVP or AVR, 1 case developed mild insufficiency, others developed well. Others underwent another operations all developed well. Conclusion To achieve satisfactory results, it should enhance the overall understanding of SPVSD, improve the diagnostic accuracy of SPVSD, take positive surgical repair of VSD and reasonable treatment with aortic valve disease, and other malformation or disease.
9.Relaxing effect on radial arterial spasm in coronary artery bypass graft: A comparison among nitroglycenn,verapamil and paraverine

BACKGROUND; As the bridge vessel, radial artery is easy to occur spasm and injury of endometrium, and the abnormal proliferation of endometrium, re-vasospasm, etc. appear at early period postoperatively, which can affect the operative outcomes.OBJECTIVE: To compare the effects of nitroglycenn (NTG), verapamil (VP), paraverine (PA), and the mixture of NTG and VP on relaxing spasm of radial artery in human being.DESIGN: A randomized controlled trial.SETTING: Department of Cardiosurgery, General Hospital of Shenyang Military Area Command of Chinese PLA.PARTICIPANTS: Thirty patients who underwent coronary artery bypass graft (CABG) with autologous radial artery, were selected from the Department of Cardiosurgery, General Hospital of Shenyang Military Area Command of Chinese PLA from September to November in 2006, including 18 males and 12 females, 48-74 years of age. Informed consents were obtained from all the patients or their relatives preoperatively.METHODS:① The samples of human radial artery in CABG were collected, and totally 95 vascular rings were used in this study. The rings were divided into five groups: NTG group, VP group, PA group, VP+NTG group and control group.There two parts of the experiment, vasospasm relaxation test and vasospasm prevention test. The vascular rings in the control group were only immersed in the Krebs- Ringer solution without any drug. ② Drugs: NTG was purchased from Solo Pak Laboratories (Franklin Park, IL), PA from Eli Lilly & Co., (Indianapolis, IN), VP and others from Sigma (St.Louis, MO). ③ Thirty vascular rings were used in the vasospasm relaxation test, and the method of bath chamber was applied. Deoxyepinephrine (terminal concentration of 1 ×10-3 mol/L) was added to stimulate the contraction and spasm of the vascular rings, then four vasodilatators were added, the concentrations of VP and NTG were 30 μmol/L, and that of PA was 0.1%. Relaxation rate=(resting tension after stimulation-initial resting tension)/initial resting tension× 100%. ④ The other 60 vascular rings were used in the vasospasm prevention test. Firstly, the rings were immersed in the four solutions of vasodilatators of corresponding concentrations and Krebs- Ringer solution respectively for 30 minutes. After pretreatment, the vascular rings were randomly divided intc normothermia group (n =30) and cryopreservation group (n =30). In the cryopreservation group, the vascular rings were put into the Krebs-Ringer buffer solution at 4 ℃, and kept in refrigerator at 4 ℃ for 24 hours. The spasms of radial artery were observed.MAIN OUTCOME MEASURES: Changes of resting tension (degree of contraction and relaxation rate of dilatation) of vascular rings before and after administration.RESULTS: ① Effect on the dilatation of spastic radial artery: The radial artery was completely within 11 minutes in all the VP+NTG group, VP group, NTG group and PA group. But in the first 3 minutes, the dilating effects of VP+NTG and NTG were obviously better than the other two groups. The dilatation curves showed that the dilating ability in order was VP+NTG > NTG > VP > PA. ② Effect of pretreatment of radial artery on antispasm: In the normothermia group, the contractility of the vascular ring produced by 1 ×10-3 mol/L deoxyepinephrine was close between the VP+NTG group and VP group [(0.47±0.06), (0.49±0.08) g, P > 0.05], which were obviously different from those in the NTG group and PA group [(0.81±0.22), (0.87±0.26) g, P < 0.05]. After cryopreservation for 24 hours, the contractility in the VP+NTG group was not obviously different from that in the VP group [(0.86±0.11), (0.90±0.13) g, P > 0.05], and obviously lower than those in the NTG group, PA group and control group [(4.82±0.87), (5.00±0.53), (5.10±0.67) g, P < 0.01], whereas those in the NTG group and PA group were close to that in the control group (P > 0.05).CONCLUSION: The drugs can prevent and relax the spasm of radial artery to different extents, whereas considering from preventing spasm and the time-effect of treatment, the mixture of VP and NTG seems to be more proper as the preparation solution for the treatment of radial artery in CABG.
10.Construction of Wip1 gene recombinant lentiviral expression vector and its effects on breast cancer MCF-7 cell biological behavior

Objective To construct the Wip1 gene recombinant lentiviral expression vector and to investigate its effects on breast cancer cell biological behaviors .Methods Wip1 gene short hairpin RNAs (shRNA) was transfected into breast cancer MCF-7 cells through lentiviral infection method .Wip1 mRNA and protein expressions before and after transfection were detected by u-sing qRT-PCR and Western blotting .The effects of Wip1-shRNA on the proliferation ,apoptosis ,cell cycle ,invasion and metastasis in MCF-7 cells were determined by using the MTT assay ,flow cytometry and transwell invasion test .Interfering RNA molecule p53dsRNA inhibiting p53 gene expression (p53 dsRNA) was screened ,and the effect of p53 inhibition on MCF-7 cells invasion and metastasis was analyzed .Results After transfection for 48 h ,the cellular fluorescence in the Wip1-shRNA group was stronger , while which in the NC-shRNA group was weaker .Cellular Wip1 mRNA and protein expressions in the Wip1 shRNA group were 0 .291 ± 0 .025 and 0 .203 ± 0 .021 respectively ,which in the NC-shRNA group were 0 .954 ± 0 .090 and 0 .963 ± 0 .092 respectively , the difference between the two groups was statistically significant (P<0 .05) .The cellular survival rate at various time points had statistical difference between the two groups (P<0 .05) .The early and late cell apoptosis number in the NC-shRNA group was less than that in the Wip1-shRNA group ,the difference was statistically significant (P<0 .05) .The cells numbers at phase G0 +G1 and phase S in the NC-shRNA group were 53 .5 ± 3 .6 and 27 .3 ± 1 .5 respectively ,which in the Wip-shRNA group were 72 .3 ± 5 .2 and 14 .6 ± 0 .8 respectively ,the difference was statistically significant(P<0 .05) .The Transwell invasion and metastasis results showed that the cell transmembrane number in the NC-shRNA group was more than that in the Wip1-shRNA group(P<0 .05) .The cellu-lar p53 mRNA and protein expression had statistical difference between the control group and p53dsRNA group(P<0 .05) .Conclu-sion RNA interference can effectively suppress Wip1 expression in MCF-7 cells .Wip1 may affect the proliferation ,apoptosis ,cell cycle ,invasion and metastasis of breast cancer cells by modulating protein expression .p53dsRNA increases the invasion and metas-tasis ability of breast cancer MCF-7 cells by interfering p53 gene down-regulation .
