Objective: To observe the effects of autotransplantatio n of costochondral graft in the defects in temporomandibular joint. Methods: Twenty New Zealand white rabbits aged 3 months were div ided into 4 groups: the animals in group A received autotransplantation of 4 mm costochondral graft and in group B received that of 1 mm after excision of the disk, condyle and upper part of the ramus. The rabbits in group C received the same excision operation without graft transplantation as surgical controls and those in group D were used as healthy controls. Samples of temporomandibula r joint were obtained and examined by X-ray cephalometry and histopathology. Objective:To review the clinical efficacy of botulinum toxin type A(BTX-A)in the treatment of trigeminal neuralgia (TN).Methods:The main bibliographic databases were searched from English and Chinese literatures on the topic and the refer-ences of the identified articles were also searched for additional studies.2 reviewers assessed the quality of the included studies and extracted data duplicate.Results:3 Chinese and 1 English reports with 203 participants were included.However,substantial hetero-geneity(I2 =77%)precluded meta-analysis and the results were individually described.3 trials reported that BTX-A was more effec-tive than the control and 1 showed no difference between groups.Adverse effects of BTX-A were reported in 4 reports and were mild to moderate.Conclusion:BTX-A is effective in treating TN with few adverse effects.However,the evidence is weak because of a few studies included and most of them with low methodological quality.Further studies with high quality are needed to testify the evidence.

OBJECTIVEThe aim of this study was to set up an animal model of osteoarthritis (OA) in the rabbit temporomandibular joint (TMJ).

METHODS0.3 ml of 1.6% papain solution was injected into the upper compartment of each left TMJ of six Japanese big ear white rabbits, and the same volume of saline was injected into the upper compartment of left TMJ as control. The rabbits were sacrificed 2, 4, 6 weeks respectively after injections and the TMJ specimens were pathologically examined.

RESULTSThe degenerative changes were demonstrated in the TMJs injected with papain, such as thinner articular cartilage, fibrillation and destroyed cartilage matrix, and inflammation, proliferation, and degeneration of the synovial tissue. All these changes were much worse with prolonged observation time. Some kinds of reparative changes appeared in the sixth week after papain injection.

CONCLUSIONPathological similarities between papain induced rabbit TMJ osteoarthritis and human TMJ osteoarthritis were observed. This animal model has good reproducibility and hence can be used for exploring pathogenesis of this disease and selection of treatments for osteoarthritis of TMJ.

Objective:To determine the effectiveness of the interve ntion of burning mouth syndrome(BMS) reported by Chinese articles and to assess the quality of the studies by evidence-based medicine. Methods: relevant literatures were searched and identified in Chinese Medical Bio logical Database (CMB) and Chinese Technological Periodical Database (VIP) . Stu dies were selected with following criteria: study design- randomized controlle d trials; participants-patients with BMS. An evidence-based analysis was perfo rmed to the literatures. Results: There were totally 47 articles examined and only 11 of them met the criteria . Meta analysis was not possible due to the low quantity and poor quality, However, as far as the p ublished articles concerned, Lusunjing capsule showed good short-term effect in the treatment and nerve block with vitamin B 1 and B 12 was more effec tive than oral admenistering of vitamin. Conclusion: Th e quantity and quality of the reviewed studies are far from satisfaction and ca n not meet the clinical needs. Further trials of high methodological quality ne ed to be undertaken in order to develop effective treatment of BMS.

OBJECTIVETo determine the clinical effects of preventing dental fear (DF) by pre-operation-education or local anesthesia method during the process of tooth filling.

METHODS150 school children, aged 7 to 12, participated present study. All of them suffered from occlusal caries on their mandibular first permanent molars. They were divided into 3 equal groups, and each had 50, 25 boys and 25 girls. Group1 (pre-operation-education): taking about 1 hour to show them science and educational video tape on caries, then, clinic environment, including machine and instruments, and answering their questions; Group2 (local anesthesia): about 15 minutes before treatment, injecting 1.8 ml of 2% lidocaine for local anesthesia; Group3 (blank): without any measurements for DF. After that, all subjects accepted same filling treatment as usual. DF of each case was evaluated by 3 evaluators blindly based on venham's clinical ratings of anxiety and cooperative.

RESULTSSignificant difference was found between groups (0.57 +/- 0.59, 0.83 +/- 0.66, 1.05 +/- 0.68, H = 18.646, P = 0.0001), also in DF rate (10%, 18% and 42%, chi(2) = 15.5031, P = 0.0004). But not between groups 1 and 2.

CONCLUSIONDuring decayed tooth filling treatment, pre-operation-education is better than that of local anesthesia method, in DF prevention.

OBJECTIVETo measure the agreement, reliability and validity of assessment of multiple systematic reviews (AMSTAR) to assess Chinese systematic reviews on stomatology.

METHODSA comprehensive electronic search was made through Chinese BioMedical Literature Database, VIP Database for Chinese Technical Periodicals and China National Knowledge Infrastructure electronically on March 1st 2011 together with handsearch through 19 stomatological journals to identify published Chinese systematic reviews on stomatology. Each systematic review was assessed by two reviewers with overview quality assessment questionnaire (OQAQ) and AMSTAR. And reliability (interobserver Kappa of the 11 AMSTAR items), interclass correlation coefficient (ICC) of the sum scores and construct validity (ICC of the sum scores of AMSTAR compared with those of the OQAQ) were reported.

RESULTSA total of 52 systematic reviews on stomatology were eligible. The reviewers agreement of the individual items of AMSTAR was with a mean Kappa of 0.81 [95% CI(0.73, 0.89)] while the OQAQ was 0.74 [95% CI (0.66, 0.83)]. The ICC of the total score for AMSTAR was 0.98 [95% CI (0.97, 0.99), P = 0.000]. Cronbach' alpha was 0.69 [95% CI (0.56, 0.80), P = 0.000]. And ICC of the sum scores of AMSTAR compared with those of the OQAQ was 0.94 [95% CI (0.90, 0.97), P = 0.000].

CONCLUSIONAMSTAR has good agreement, reliability and validity. AMSTAR can be well used in Chinese stomatology and can bring dentists much convenience when assess the methodological quality of systematical reviews on stomatology.

OBJECTIVETo evaluate the accuracy of positron emission tomography (PET)/computed tomography (CT) for detecting cervical nodule metastases of oral and maxillofacial cancer.

METHODSDiagnostic test accuracy studies assessing the accuracy of PET/CT in diagnosis of cervical metastases of oral and maxillofacial cancer were retrieved from the databases of CBM, Medline (OVID) and EMBASE (searched on Dec. 13th, 2010) and manual searching of 19 dental journals in Chinese. Two researchers were employed in the articles investigation independently and evaluated the quality with quality assessment of diagnostic accuracy studies (QUADAS). Meta-analysis was done using MetaDisc 1.4.

RESULTSNine studies met the inclusion criteria and demonstrated high methodological quality. The results of Meta-analysis showed that, when nodal level was used as unit, PET/CT had good diagnostic value in detecting the cervical nodule metastases of oral and maxillofacial cancer. Under such a condition, the pooled sensitivity was 0.84, pooled specificity 0.93, pooled diagnostic odds ratio 47.89 and area under the curve 0.973 8.

CONCLUSIONThe accuracy of PET/CT for detecting cervical nodule metastases of oral and maxillofacial cancer is relatively high and it could be used as a good diagnostic tool. And special attention should be payed to its diagnostic expense and safety when considering PET/CT.

OBJECTIVETo investigate effectiveness and safety of combined treatment with glucosamine hydrochloride orally and hyaluronate intraarticularly in patients with temporomandibular joint osteoarthritis.

METHODSTwenty eight participants who were diagnosed as temporomandibular joint osteoarthritis were randomly allocated to intervention and control groups (n = 14). The former received oral glucosamine hydrochloride (each day 1.44 g) and the latter placebo for four weeks. Both groups received intraarticular injection of hyaluronate for upper and inferior compartment, once a week for four weeks. Maximal mouth opening, pain on opening, deviation at the end of opening were measured at baseline and each week of follow up, quality of life was measured by using the instrument of oral health impact profile-14 (OHIP-14) at baseline and the fourth week. Intention-to-treat analysis was adopted for data analysis.

RESULTSAll participants except two in control group adhered to the follow-up. Parameters on baseline were balanced between the two groups. Maximal mouth opening was significantly increased in intervention group compared with control group (P = 0.03) and decrease was found for pain on opening (P = 0.02). No statistical difference was detected between the two groups on deviation (P = 0.67). The intervention group didn't show advantages compared with placebo on quality of life assessed by OHIP-14 (P = 0.77). No serious adverse drug events were detected in both groups.

CONCLUSIONThe glucosamine hydrochloride plus double joint cavity injection of hyaluronate is safe and may reduce pain scores and improve opening in patients with temporomandibular joint osteoarthritis.

OBJECTIVETo evaluate the report quality of randomized controlled trials (RCT) of oral and maxillofacial surgery in China during 2000-2009.

METHODSA comprehensive electronic search was carried out through Chinese Biomedical Literature Database (CBM), VIP Database for Chinese Technical Periodicals (VIP) and China National Knowledge Infrastructure (CNKI), and 19 kinds of journals of stomatology in China were also hand-searched. We identified RCT published between 2000 and 2009, and classified into oral and maxillofacial surgery and labeled "random" and assessed the quality of these reports using the consolidated standards of reporting trials (CONSORT) statement.

RESULTS53 RCT articles were included. Reporting quality of the 53 articles was not high and the CONSORT score was 8.2 +/- 2.5.

CONCLUSIONThe reporting quality of RCT of oral and maxillofacial surgery in China is poor. The CONSORT statement should be used to standardize the reporting of RCT.

OBJECTIVETo assess the reporting quality of randomized controlled trials (RCT) related to dentine hypersensitivity which were published between 2000 and 2009 in nineteen Chinese stomatological journals, learn the current status and the influence factors of reporting quality of the dentine hypersensitivity RCT, and determine whether they can provide high quality evidence clinically.

METHODSA handsearching of nineteen Chinese stomatological journals to identify dentine hypersensitivity RCT which were published between 2000 and 2009 and labeled "random". Assess the reporting quality of these RCT by Consolidated Standards of Reporting Trials (CONSORT) statement and then analyze the influence factors of reporting quality by univariate and multivariate analyses.

RESULTSWe identified 61 dentine hypersensitivity RCT, among which 45 belonged to journals of Chinese scientific study statistical source and 16 belonged to journals of non-statistical source. The reporting quality of dentine hypersensitivity RCT was not high at large and the CONSORT score was 8.1 +/- 2.4 on average. Multiple linear regression showed that the score of improved Jadad scale (t=4.656, P=0.000) was the main influence factor of reporting quality of dentine hypersensitivity RCT. The better the intrinsic authenticity was, the higher the reporting quality would be.

CONCLUSIONCurrently, reporting quality of Chinese dentine hypersensitivity RCT needs to be improved. In order to improve the reporting quality of dentine hypersensitivity RCT, experts in stomatology should not only improve experimental design before experiments, but also write papers in accordance with CONSORT statement.
