1.Expression of a new qnr gene subtype and mechanism of multidrug resistance

Objective To study the biologic characteristics of the new qnr gene subtype and multi-drug resistant mechanism in a clinical isolate of Klebsiella oxytoca.Methods We cloned and expressed the qnr gene,?-lactamase and integrase genes were detected by PCR.Results Susceptibility of transformant containing qnr gene against common fluoroquinolones was 3~25 times lower than recipient stain,but drug-resistance was 4~256 times lower than the clinical isolate.KLUC-1,TEM-1 and OXA-30 genes were also found in the isolate.Conclusions qnr gene can raise the drug-resistance to fluoroquinolones slightly. There are multiple drug-resistant genes in the strain containing the qnr gene.
2.Genotypes of Meticillin-resistant Staphylococcus aureus

OBJECTIVE To investigate the genotypes of meticillin-resistant Staphylococcus aureus(MRSA)and its resistance.METHODS A total of 73 MRSA clinical isolates were collected in Anhui Province,MIC of sixteen different antibacterial agents against the isolates were determined by agar dilution method.PCR amplified the mec associated hypervariable region(HVR)of MRSA,and the genotypes were classified based on the fragments of amplified products.The correlation of genotypes and antimicrobial resistance was analyzed.RESULTS Seventy three strains of MRSA from Anhui Province were grouped into A,B and C genotypes based on HVR polymorphism,and respectively 17.8%,23.3%,and 58.8%.All strains were sensitive to vancomycin.CONCLUSIONS MRSA is multi-drug resistant and can be divided into 3 genotypes based on the HVR PCR amplified products.HVR PCR method is a rapid and convenient method for molecular epidemiology study of MRSA infections.
3.Time-zero renal biopsy: Correlation analysis of clinical predonation parameters and histological abnormalities

BACKGROUND: The number of living renal donation has increased in China and abroad, thus, it is important to guarantee the safety of donors. How to accurately diagnose potential renal disease and provide guidance plays an import role in protecting safety of living renal donors.OBJECTIVE: To establish an evaluation method for analyzing the correlation between histological abnormalities and clinical predonation parameters.METHODS: The related data on renal transplantation of Fuzhou general Hospital of Nanjing Military Area Command of Chinese PLA were retrospectively reviewed. Paracentesis were performed when the vessels of kidney were mutilated and perfusions were finished. Time-zero renal biopsy was evaluated for following pathological changes: interstitial fibrosis, tubularatrophy, arteriolar hyalinosis, mesangial proliferation, and glomerulosclerosis. Predonation data were demography, body weight, body mass index' systolic/diastolic blood pressure, serum creatinine, glomerular filtration rate, and proteinuria.RESULTS AND CONCLUSION: There were no signs of kidney disease in preoperative examination of all the 62 patients, time-zero renal biopsy found there were 28 donors with histological changes, interstitial fibrosis with age and serum creatinine, tubularatrophy with diastolic blood pressure and protein excretion rate, arteriolar hyalinosis with serum creatinine and glomerular filtration rate, mesangial proliferation only with body mass index, and finally the presence of glomerulosclerosis did not correlate with any variable.
4.Identification of Medicinal Insect Vespa mandarinia from Its Adulterants Using DNA Barcode

This study aimed at exploring a fast method to accurately identify the medicinal insect Vespa mandarinia Smith from its adulterants using DNA barcode and COI sequences.The extracted DNAs from V.mandarinia and its adulterants V.soror were amplified by polymerase chain reaction (PCR) and sequenced bilaterally based on COI barcode sequence investigation.The information of the COI sequences of V.mandarinia and V.soror were gathered from GenBank.All the sequences were compared and analyzed,and their intraspecific and interspecific genetic distances were calculated using MEGA 6.06.In addition,the phylogenetic tree was established with neighbor-joining (NJ) method.As a result,the COI sequences of V.mandarinia and V.soror were successfully amplified.The minimum interspecific distance between V.mandarinia and its adulterants was 0.152 ± 0.017,being considerably larger than the maximal intraspecific distance between V.mandarinia,0.009±0.004.The constructed phylogenetic tree showed an independent branch for each species.It was concluded that the DNA barcode based on COI sequence can efficiently identify V.mandarinia and its adulterants.This study provided an innovative tool for the quality control and market regulation of Chinese materia medica,securing the safe medication of V.mandarinia.
5.Application effect of preoperative intranasal dexmedetomidine in pediatric anesthesia

Objective To observe the clinical effects of preoperative intranasal dexmedetomidine in pediatric anesthesia.Methods From April 2014 to April 2017,40 pediatric patients who accepted elective circumcision,ASA Ⅰ,aged 2 to 10 years in Wenjiang Branch of Sichan Provincial People's Hospital were divided into two groups,with 20 cases in each group.The test group received intranasal dexmedetomidine 1 μg/kg,and the control group received intranasal equal volume of saline 30 min before surgery.HR,SpO2,BP were monitored and recorded before intranasal (T0),5min after intranasal (T1),10min after intranasal (T2),20min after intranasal (T3),30min after intranasal (T4).The sedation score was assessed after 30 min of administration.The restless score was observed after waking.Results ANOVA analysis showed that there were significant differences in SBP (F =14.54,P < 0.05) and DBP (F =22.69,P < 0.05) between the two groups,and the SBP (F =13.77,P < 0.05),DBP (F =10.48,P < 0.05),HR(F =5.13,P < 0.05) had interaction effects.Compared with those of the control group,the heart rate and the diastolic pressure of the test group were decreased at T2 ~ T4 (all P < 0.05).The sedation score of the test group was superior than that of the control group(t =-9.131,P <0.05),and the postoperative agitation score was lower than that of the control group (t =3.387,P < 0.05).Conclusion Intranasal dexmedetomidine can provide satisfactory sedative effects without affecting the vital signs of children and significantly reducing the postoperative agitation.
6.The expression and clinical significance of TIMP-3 and FN1 in nasopharyngeal carcinoma tissue

OBJECTIVE To investigate the expression and clinical significance of tissue inhibitor of matrix metalloproteinase-3(TIMP-3)and fibronectin 1(FN1)in nasopharyngeal carcinoma patients.METHODS A total of 102 nasopharyngeal cancer patients admitted to Taihe Hospital from May 2018 to May 2019 were selected as the observation subjects(case group),and another 102 patients with chronic nasopharyngitis were selected as the control group.Immunohistochemistry was applied to determine the expression of TIMP-3 and FN1 in both groups.The prognosis of the patient is followed up.Kaplan-Meier method was applied to analyze the relationship between the expression levels of TIMP-3 and FN1 in nasopharyngeal carcinoma tissue with the 3-year prognostic survival of patients.The influencing factors were analyzed using Cox risk regression method.RESULTS The positive rate of TIMP-3 protein in the case group(25.49%,26/102)was lower than that in the control group(82.35%,84/102),with a statistically significant difference(χ2=66.369,P<0.05);the positive rate of FN1 protein(71.57%,73/102)was higher than that in the control group(20.59%,21/102),with a statistically significant difference(χ2=53.348,P<0.05).The expression of TIMP-3 and FN1 was related to lymph node metastasis and TNM staging(P<0.05).During the 5-year follow-up period,64 of 102 nasopharyngeal cancer patients with disease-free survival were categorized into the survival group,and 38 nasopharyngeal cancer patients who experienced death were categorized into the death group.The 5-year survival rate(56.58%,43/76)of patients with TIMP-3 negative expression in nasopharyngeal carcinoma tissue was lower than that of patients with TIMP-3 positive expression(80.77%,21/26)(χ2=8.921,P<0.05).The 5-year survival rate(23/29,79.31%)of patients with FN1 negative expression in nasopharyngeal carcinoma tissue was higher than that of patients with FN1 positive expression(41/73,56.16%)(χ2=8.596,P<0.05).FN1 positive,TNM staging,and lymph node metastasis were independent risk factors for mortality in nasopharyngeal carcinoma patients,while TIMP-3 was a protective factor for mortality in nasopharyngeal carcinoma patients(P<0.05).CONCLUSION TIMP-3 expression decreases and FN1 expression increases in tissues of nasopharyngeal carcinoma patients.Both are closely related to the progression and prognosis of nasopharyngeal carcinoma,and may serve as potential prognostic markers for nasopharyngeal carcinoma.

Skin modeling of transdermal drug delivery system refers to experimental models that mimic the structure and function of human skin to explore and evaluate absorption,penetration,and efficacy of medicines in transdermal drug delivery.It provides an alternative to traditional human skin experiments and reduces the use of human skin in medical research,which is convenient,controllable,and cost effective.For skin models of transdermal drug delivery systems,this article introduces commonly used animal skin models,artificial skin models,and recombinant human skin models from the perspective of the transdermal absorption pathway of medicines,and analyzes their advantages,disadvantages,and applications so provide references the research and development of transdermal formulations and topical therapies.
