1.Epithelial stem cells marker keratin 19 expression in residual skins after abrasion treatment of deep second-degree burn wounds

BACKGROUND: The wound eschars are often excised from the large wounds of deep second degree bum, but the tangential excision may remove excessive tissues of the epithelial regeneration in residual skin wounds.OBJECTIVE: To investigate the therapeutic effect of electro-driven abrasion treatment in deep second-degree bum wounds on keratin 19 expression, the marker of the epithelial stem cell in residual skin wounds.DESIGN, TIME AND SETTING: Randomized controlled trials were performed from October 2002 to January 2004 in the Guangxi Medical University.MATERIALS: Forty patients suffered thermal (heat or fire) burns were recruited from Department of Burn and Plastic Surgery,aged 18-37 years. The average burn size approaches 15% -45% total body surface area, while deep second-degree burn size was 15%-30%. The patients were divided randomly into two groups, abrasion treatment group (n=20) and tangential excision treatment group (n=20).METHODS: Abrasion treatment group: The wound eschers were removed from superficially to deeply by the electro-driven abrasion instrument, until the wound beds were red hyperaemia or bead bleeding. Tangential excision treatment group: The wound eschars were excised by the skin graft knife, until the wound beds were chalkiness and wet. There were not brown tissue and reticular vascular thrombi. The wound beds were bead bleeding after the hemostasia strap was relaxed. At times the operations were so excessively deep as that fat tissues on the wound beds were revealed. The wound beds in two groups were covered by porcine skin postoperation.MAIN OUTCOME MEASURES: Tissue specimens were taken from the wounds to detect keratin 19 expression in regenerative tissues by using S-P immunohistochemical method before and after operation. Assisted by optical microscope (100x), five view areas were randomly selected to count cells. The healing time of the wounds in two groups was recorded, the granulation wounds that failed to heal required secondary operation.RESULTS: Electro-driven abrasion treatment could preserve more skin attachment, such as dermis, hair foltides, sebaceous glands,and sweat glands. Tangential excision treatment could preserve less skin attachment, such as lamina network tissue residuals, some hair follicles and sweat glands. At times the fat tissues on the wound beds were revealed, rather than dermis. Results of cell counts expression, keratin 19 expression in the residual skin wounds could be seen in both two groups before and after operations. No significant difference was found in the keratin 19 expression before and after abrasion treatment (P > 0.05), while keratin 19 expression counts in the tangential excision treatment group were decreased compared with before operation (P < 0.05). The healing in the abrasion treatment group occurred eadier than in the tangential excision treatment (P < 0.05). There were 2 wounds in the abrasion treatment group and 8 wounds in the tangential excision treatment group requiring secondary operation (P < 0.05).CONCLUSION: Applying electro-ddven abrasion treatment to heal deep second-degree burn wounds is efficient and induces less damages to tissues. Compared with tangential excision treatment, it can preserve more regenerated skin tissues, and is helpful to repair of epithelial stem cells through the regeneration and to shorten the healing time of wounds.
2.Effects of ribs in high intensity focused ultrasound beam path on formation of coagulated necrosis in cattle liver in vitro

Objective To investigate the effects of ribs in high intensity focused ultrasound (HIFU)beam path on formation of coagulative necrosis in cattle liver in vitro in the same acoustic power. Methods The HIFU ablation was carried out in the manner of a single dot in the liver in vitro. The liver was ablated by HIFU in free field,then the ribs were placed between the focal plane and transducer,directing the beam axis through the intercostals space, lied at the rib edge and on the centre of middle rib. For each of these axial arrangements,the plane of the front surface of ribs was placed 3 cm,6 cm or 9 cm in front of the focus. After HIFU exposure,the tissue necrosis rate(TNR),the maximum length,width,depth and position of the coagulated necrosis in the liver were measured. Results ①The TNR was 100% in the free field. When the ribs were placed 3 cm in front of the focal plane,directing the beam axis through the intercostals space, the TNR was 100%. When directing the beam axis lied at the rib edge and on the centre of middle rib,the TNR decreased sharply. When the ribs were placed 6cm and 9cm in front of the focal plane, the TNR was more than 50%. ②When the ribs were placed in HIFU beam path, the volume of necrosis decreased compared with those in free field. The relationship between the ribs and beam axis, the location of ribs placed away from the focal plane different,the volume,length,width and position of the coagulated necrosis different. ③ When the ribs were close to the focal plane,unacceptable damage could been observed on the surface of ribs.When the ribs were far away from the focal plane, the ribs were safe. Conclusions The volume of necrosis decrease when the ribs are placed in HIFU beam path. The relative location of ribs and HIFU beam axis, the location of ribs away from the focal plane are relative with the volume of the coagulated necrosis. When directing the beam axis through the intercostals space, the volume of necrosis is large, and the shape of coagulated necrosis is regulated. When the ribs are closed with the focal plane,ribs are damaged probably.
3.The hemispheric lateralization of the auditory cortex after being stimulated by pure tone: a 1H-MRS study

Objective To explore the asymmetry of metabolic products of the auditory cortex in normal human brain after being stimulated by the pure tone with the proton magnetic resonance spectroscopy(1 H-MRS). Methods Twelve healthy volunteers, including 7 males and 5 females, were performed MRS tests before and after the stimulation of the pure tone of 90 dB in intensity and 1000 Hz in frequency. The the metabolites peak of the N-acetylaspartate (NAA) ,creatine(Cr) ,choline( Cho) , glutamine/glutamate ( Glx) and GABA in auditory cortex were investigated, The content of metabolic products were conducted with half-quantitative analysis. The differences of the NAA/( Cho + Cr) ,Glx/Cr and GABA/ Cr ratios before and after pure tone stimulation in the bilateral auditory cortex were analyzed. The difference of distribution of the metabolic products between the left and right auditory cortex was also analyzed to explore the asymmetry of the auditory cortex. Results The NAA/( Cho + Cr) and GABA/Cr ratios increased,the Glx/Cr ratio decreased in the left auditory cortex after being stimulated by the pure tone. There were significant differences in the NAA/( Cho + Cr) , Glx/Cr and GABA/Cr ratios before and after pure tone stimulation (P<0.05) ;the GABA/Cr ratio decreased,the Glx/Cr and NAA/( Cho + Cr) ratios increased in the right auditory cortex after being stimulated by the pure tone, and statistical difference of the GABA/Cr ratio was found before and after pure tone stimulation (P<0. 05) ,however the difference of the Glx/Cr and NAA/(Cho + Cr) ratios before and after pure tone stimulation was not significant (P>0.05). There were statistically significant differences in the Glx/Cr ratio of the auditory cortex between two sides after being stimulated by the pure tone. Conclusion The metabolic lateralization exists in auditory cortex of normal human brain after being stimulated by the pure tone, which may be the bases of the functional asymmetry.
4.The effects of Kidney-deficiency on auditory cortex metabolism in the elderly: a 1H-MRS study

Objective To investigate the effects of kidney-deficiency on auditory cortex metabolism in the elderly by using the proton magnetic resonance spectroscopy(1H-MRS).Methods 50 volunteers,including 20 young subjects and 30 older subjects matched for gender,were measured by pure tone audiometry、“kidney-deficiency”-related test and were executed 3.0 T multi-voxel 1H-MRS tests,The ratios of N-acetylaspartate (NAA),choline (Cho) and lactate (Lac) to creatine (Cr) were measured and were compared between the two groups.The t-test analysis were used for statistic process.The relationship between auditory cortex metabolism and the symptom of kidney-deficiency was also analyzed.Resnlts The result of kidney-deficiency-related test and pure tone audiometry revealed that there was significant difference between the two groups and the older subjects had the more severity kidney-deficiency and high-frequency hearing loss (t=6.335、19.558,P<0.05).The NAA/Cho ratio in the auditory cortex in the older group was significantly lower than that in the younger group (t=2.90,P<0.05).There were no significant differences between two groups for the ratios of NAA/Cr and Cho/Cr (t=1.415、0.927,P>0.05).Decrease of the ratio of NAA/Cho in the older group is significantly related with kidneT-deficiency(r=- 0.773,P=0.025).Conclusion kidney-deficiency play a very important role in the hearing loss and reduction of the number of neurons in auditory cortex of older healthy subjects.It suggests one possible underlying mechanism for the speech perception difficulties exhibited by aging adults.
5.Clincal study of treatment for refractory diabetic wound

Objective To explore the repair method for refractory diabetic wound. Methods A total of 206 patients with refractory diabetic foot ulcers were treated with proper surgical treatments.Results Of all, 106 patients were treated by skin flap (51.5 % ), with one stage wound healing rate of 85.8%; 122 patients were repaired with split-thickness skin graft ( 59.2% ), with survival rate of the graft for 79.5%. Simple toe amputation was made in 34 patients (46 toes). The high level amputation was performed in 56 patients (27.2%). Of all, 132 patients were followed up for 6-18 months, which showed that ulcer recurred in 12 patients (9.1%). Conclusion Timely and effective treatment as well as flap and skin graft repair could reduce high level amputation rate of diabetic foot ulcer and promote the quality of life.
6.Effect of highly selective nerve injury on skin wound healing in rats

ObjectiveTo establish a rat model of pure sensory nerve injury or pure motor nerve injury combined with skin defect to examine the effect of highly selective nerve injury on skin wound healing.MethodsA total of 90 male Sprague-Dawley rats were randomly and equally divided into three different groups including Group A (posterior rhizotomy + cutaneous excision wounding group),Group B (anterior rhizotomy + cutaneous excision wounding group) and Group C ( sham operation + cutaneous excision wounding group).The wound healing rate was detected at days 2,7,14 and 21 after injury.The CGRP mRNA expression was determined by reverse transcription-polymerase chain reaction (RT-PCR) at days 1,3,7 and 14 after injury.The Bcl-2 protein expression was determined by immunohistochemistry at days 3,7,14 and 21 after injury.ResultsThe wound healing rate in the Groups A and C was significantly higher than that in the Group B at day 2 ( P < 0.05 ),with no statistical difference between Group A and Group C (P > 0.05 ).The wound healing rate in the Group C was higher than that in the Groups A and B at days 7 and 14 (P <0.05),with no statistical difference between Group A and Group B (P >0.05).The wound healing rate in the Groups B and C was higher than that in the Group A at day 21 ( P < O.05 ),with no statistical difference between Group B and Group C ( P > 0.05 ).The CGRP mRNA was expressed in three groups and the expression was up-regulated after wound.The CGRP mRNA expression in the Groups B and C was significantly higher than that in the Group A at days 1,3 and 7,with no statistical difference between Group B and Group C ( P > 0.05 ).The CGRP mRNA expression was distributed as Group C > Group B > Group A at day 7 ( P < 0.05 ) and Group B > Group A > Group C at day 14 (P < 0.05 ).Immunohistochemistry indicated that there was no statistical difference in Bcl-2 protein expression at day 3 after injury in all groups; that the Bcl-2 protien expression in the Group C was significantly higher than that in the Groups A and B at day 7 ( P < 0.05 ),with no statistical difference between Group A and Group B ( P > 0.05 ) ; that the Bcl-2 protien expression was distributed as Group C > Group B > Group A at day 14 ; that the Bclo2 protien expression in the Group B was higher than that in the Groups A and C at day 21 (P <0.05),with no statistical difference between Group A and Group C (P > 0.05).ConclusionsIn wound healing process,the sensory nerve plays a more important role than the motor nerve.Denervation of the nerves ( especially the sensory nerves) is not conducive to wound healing,and complete innervation is necessary for normal wound healing.
7.Scanning skill and quality control in prostate magnetic resonance spectroscopy

Objective To investigate the factors influencing the image quality of prostate magnetic resonance spectroscopy (MRS),and to put forward quality control measures to improve MRS in success ratio and image quality.Methods Totally 1 255 patients with prostate diseases confirmed pathologically from October 2009 to December 2015 had their MRS data analyzed retrospectively.MRS was executed with multi-voxel 3D chemical shift imaging technique,and special-purpose software was involved in for post processing.Re-scanning would be performed in case of baseline clutter,low SNR and etc.Results There were 1 218 patients had MRS executed well in preparedness,positioning,parameters setup,post processing,baseline and chemical shift,one patient underwent unsuccessful MRS due to incorrect bed mode,9 ones due to unstable baseline resulting from unsatisfactory preparedness,26 ones due to unstable baseline resulting from bad shimming and 1 case due to non-standard post processing.Conclusion Prostate SRS depends on patient preparedness,positioning,parameters setup,shimming and post processing.
8.Study about target-network of anti-cerebral infarction neuropathy based on theory of neurovascular unit and network pharmacology.

OBJECTIVEPotention drug-targets on anti-neuropathy of stroke were summarized, and it will provide materials for developing innovation components traditional Chinese medicine on anti-cerebral infarction neuropathy.
METHODThis article had done a series of researching work about neurovascular unit which includes three kinds of cells: neuron, gliacyte,brain microvascular endothelial cell, then signal mechanism of cell death or apoptosis of each section of stroke neuropathy was analysised by the historical documents.
RESULTThere are five important pathways: inflammatory factor-MMPs pathway- Caspases, Ca2+ -mitochondrial pathway-Caspases, Ca2+ -Phospholipase-PI-3K/AK pathway, Ca2+ -radical-MAPK pathway, Ca2+ -NO-protease pathway, among all the nodes, Caspases, Ca2+, NO were the most important ones.
CONCLUSIONDeveloping the multi-mechanism and multilevel of traditional chinese medicine under the guidance of the theories of network pharmacology and neurovascular unit will play an important role in studying the key links of signal-network of stroke neuropathy.

9.Application of network pharmacology and high through-put technology on active compounds screening from traditional Chinese medicine.

OBJECTIVETo solve the difficult in the active compounds screening from traditional Chinese medicines (TCM).
METHODAccording to preliminary lab results and related literature, systematic analysis of the high-throughput technology was made.
RESULTTechnologies of rapid preparation, high-throughput screening and biochip, together with the thought of drug target network, will contribute to TCM research on active ingredients screening, toxic components exclusion and molecular mechanisms. They are key technologies and ideas for modernization of TCM and research of TCM network pharmacology.
CONCLUSIONHigh throughput technology and network pharmacology-related technologies will provide new ideas and key methods for active compounds screening from TCM.

10.Complete genome sequence characteristics of 3 strains of Kaeng Khoi virus isolated from bat flies

Objective:To investigate the infection status of Kaeng Khoi virus (KKV) in bat flies in Yunnan province.Methods:Specimens of the ectoparasitic bat flies on bats in Baoshan city in 2014 and Ruili city in 2017 were collected and identified, virus isolation was performed by cell culture. The positive isolates were amplified and sequenced to obtain the complete genome sequences, and homology comparison and phylogenetic analysis were carried out.Results:Three positive isolates (WDBC1704, WDBC1710 and BSBC1406) were identified from bat flies in Ruili city and Baoshan city. The supernatant was inoculated into BHK-21 cells for virus isolation, all of which could cause cytopathic effects (CPE) included shrinking, rounding and falling off of BHK-21 cells. The complete genome sequences of the three positive isolates were produced by RT-PCR and high-throughput sequencing. Phylogenetic analysis showed that the S, M, and L genes of all three positive isolates were in the same evolutionary branch as the original KKV strain PSC-19 isolated in Thailand. In this study, three KKV strains isolated were most closely related to the previous isolate WDBC1403 from Yunnan province, of which the BSBC1406 strain formed a small independent branch. The sequence comparison showed that there were significant differences in the M gene among the three isolates. The similarity between BSBC1406 and the other two strains (WDBC1704 and WDBC1710) was only 78.0%-78.1%, the amino acid similarity of the Gc protein in the open reading frame region was 85.3%.Conclusions:Three KKV strains from bat flies were quite different from the prototype strain, especially the BSBC1406 strain was somewhat different from other isolates and may have undergone variation. Surveillance of KKV carried by vector insects in Yunnan province and its relationship with human diseases should be strengthened.
