1.Clinical Research on External Application and Rectal Instillation with Chinese Medicine in Treatment of Chronic Renal Failure Combined with Acute Gouty Arthritis

This study was aimed to investigate the curative effect on external application and rectal instillation with Chinese medicine in the treatment of chronic renal failure (CRF) with acute gouty arthritis (GA). A total of 82 CRF patients with acute GA from the First People's Hospital of Hengyang City were randomly divided into the study group (n = 42), which was treated with external application and rectal instillation with Chinese medicine, and the control group (n = 40), which was treated with prednisone. The comparison was made on symptoms and body signs of GA, blood uric acid, creatinine, creatinine clearance, adverse reaction, and recurrence rate between two groups. The re-sults showed that there was no significant difference on alleviating of joint swelling and pain between the study group and the control group. Compared with pretreatment, the blood uric acid and creatinine were significantly decreased after treatment in the study group (P< 0.01). The creatinine clearance was obviously increased (P < 0.05). There were no significant differences on blood uric acid, creatinine and creatinine clearance in the control group before and after treatment. There was no adverse reaction in the study group. In the one-month follow-up, there were only three recurrent cases in the study group, which were obviously better than the control group (P< 0.01). It was concluded that external application and rectal instillation with Chinese medicine had a good curative effect on CRF with acute GA in the near future with low recurrence rate and no adverse reaction, which was worthy of popularizing in the clinical practice.

Objective:To explore the effect elements of teaching quality in a constitution,to learn the attitudes from different level of people towards effect elements of teaching quality and to examine the ways to guarantee the improvement of teaching quality and the complement of cultivating talents.Methods:The index of appraisal system of teaching quality originates from the transformation achievements of the study in our school "To apply the ISO9000 standard to construction assurance system of medical education quality".The investigation subjects are all the teaching and administration staff and students of the school.The investigation involves index of 4 levels,6 aspects and 84 observation points.The statistical analysis is carried on in 3 levels:the entire school,the teachers and the students.Results:The sequence of the index of 4 levels of influencing teaching quality is made clear and analyzed,and the improvement proposals are also given.
3.Practice and thinking of PBL in medical imaging

Objective To investigate whether problem-based learning (PBL) is suitable for medical imaging and whether PBL is more enlightening than lecture-based learning (LBL) mode in teaching of medical imaging.Methods Thirty-two students of clinical medicine specialty in Sun Yat-sen University were selected as teaching objects and were divided into 6 groups (5 students in four groups and 6students in 2 groups).Chapters of urinary and digestive system were taught by PBL mode while the rest chapters by LBL mode.Questionnaires were handed out to teachers and students by Email after class and the valid questionnaires were analyzed qualitatively.Results Thirty-one students and 5 teachers thought they were suitable for PBL teaching; 30 students and 5 teachers were willing to continue PBL.Thirty-one students and 5 teachers thought that PBL mode could let students learn more knowledge and was more revealing.Twenty-three students thought PBL teaching spent too much time can not promote in all chapters.Three teachers worried that PBL mode would cause poor organization form and teaching plans.Conclusions PBL is suitable for medical imaging teaching.PBL could let students learn more knowledge and is more revealing compared with LBL,however,PBL is time-consuming and laborious,therefore LBL and PBL should be combined.
4.Multi-slice Computed Tomography Enteroclysis in Evaluation of Active Ulcerative Colitis

Purpose To summarize the characteristics of computed tomography enteroclysis (CTE) in active ulcerative colitis (UC), and to explore the value of multi-slice CTE in the evaluation of UC. Materials and Methods Thirty-five patients with active UC confirmed by clinical manifestation, colonoscopy and pathology underwent CTE examination in the study. According to the modiifed Mayo-score, the patients were divided into mild group, moderate group and severe group, and the CTE manifestations were compared among the three groups. Results Among 35 patients, 6 patients were in the mild group, 13 in the moderate group, and 16 in the severe group. Submucosal bubbles had signiifcant differences between mild and moderate groups (P<0.05), bowel wall stratiifcation, disappearance of haustra and enlarged mesenteric lymph nodes were signiifcantly different between moderate and severe groups (P<0.05), and engorged vasa recta was significantly different between the 3 groups (P<0.05). However, bowel wall thickening, mural hyperenhencement, narrow lumen and fatty deposits around the rectum showed no difference between the three groups (P>0.05). Conclusion Multi-slice CTE can provide image features of bowel wall, intestinal tube and structures outside intestine in the evaluation of UC, thus it is useful in the diagnosis of active UC as well as in its clinical grading.
5.Visceral abdominal adiposity tissue volume quantification using noninvasive MRI in prediction of type 2 diabetes

Objective To investigate the feasibility of utilizing visceral abdominal adiposity tissue (VAT) volume quantification using MRI to predict type 2 diabetes mellitus (T2DM).Methods Forty-eight subjects including 15 T2DM (T2DM group),17 impaired glucose tolerance (IGT,IGT group) and 16 normal glucose tolerance (NGT,NGT group) were enrolled in this study.All subjects underwent upper abdominal iterative decomposition of water and fat with echo asymmetry and least square estimation-image quantification (IDEAL-IQ) MRI scanning.VAT volume of the second and third lumber vertebral body ranges (VATV L2,VATV L3),sum of VATV L2 and L3 (total VATV),hepatic and pancreatic fat were measured in fat fraction mapping of T1WI IDAEL-IQ sequence on post-processing workstation.The accuracy of predicting T2DM using VAT was evaluated by Logistic regression equation via ROC curve.Results The mean of VATV L2,VATV L3 and total VATV in T2DM group were significantly higher than those of IGT group and NGT group (P<0.05),while there were no significant difference of these metrics between IGT group and NGT group (P>0.05).Taking 460.34 ml as the cut-off value for VATV L2 to predict T2DM,sensitivity was 73.33%,specificity was 75.76% and accuracy was 75.00%,respectively.Taking 429.46 ml as the cut-off value for VATV L3 to predict T2DM,sensitivity was 86.67%,specificity was 72.73% and accuracy was 77.08%,respectively.Taking 887.83 ml as the cut-off value for total VATV to predict T2DM,the sensitivity,specificity and accuracy were 86.67%,72.73% and 77.08%,respectively.Only VATV L3 was enrolled by Logistic regression equation (P=0.01,OR=1.01),and the sensitivity,specificity and total accuracy of prediction for T2DM were 80.00 %,88.20 %,and 84.40 %,respectively.Conelnsion It is feasible to utilize VAT volume quantification with MRI to predict T2DM.VATV L3 is a better predictor.
6.Severe cases with hand, foot and mouth disease: data based on national pilot hand, foot and mouth disease surveillance system

Objective To investigate the clinical severity,etiological classification and risk factors of severe cases with hand,foot and mouth disease (HFMD).Methods A total of 1 489 records on severe and fatal HFMD cases reported to the national pilot surveillance system of HFMD were used to analyze the demographic,medical treatment,etiological classification of the cases.Treatment outcome related risk factors were also studied with multi-variable stepwise logistic regression method.Results Seven out of the 1 489 severe HFMD cases died of this disease.A total of 960 (72.9%) were under three years old and 62.9% were male and most of the cases (937,62.9%) resided in rural areas.Among all the cases,494 (33.2%) went to seek the first medical assistance at the institutions of village or township level.Durations between disease onset and first medical attendance,being diagnosed as the disease or diagnosed as severe cases were 0(0-1) d,1 (0-2) d and 2 (1-4) d,respectively.In total,773 (51.9%) of the severe HFMD cases were diagnosed as with aseptic meningitis,260 (17.5%) with brainstem encephalitis,377 (25.3 %) with non-brainstem encephalitis,6 (0.4%) with encephalomyelitis,1 (0.1%) with acute flaccid paralysis,4 (0.3%) with pulmonary hemorrhage/pulmonary edema and 68 (4.6%) with cardiopulmonary failure.Of the etiologically diagnosed 1 217 severe and fatal HFMD cases,642 (52.8%) were with EV71,other enterovirus 261 (21.5%),Cox A16 36 (3.0%),1 (0.1%) with both EV71 and Cox A16.However,277 (22.8%) showed negative on any pathogenic virus.Complication (Z=3.15,P=0.002) and duration between onset and diagnosed as severe cases (Z=3.95,P<0.001) were shown as key factors related to treatment outcomes.Conclusions Most severe HFMD cases appeared in boys,especially living in the rural areas.Frequently seen complications would include aseptic meningitis,non-brainstem encephalitis and brainstem encephalitis.EV71 was the dominant etiology for severe and fatal cases.Early diagnosis and complication control were crucial,related to the treatment outcome of HFMD.
7. Review on the etiology and complications of hand, foot and mouth disease, using data from the national sentinel surveillance program, in China, 2015-2016

Objective:
To understand the characteristics relating to the etiology and complications of hand, foot and mouth disease (HFMD) based on data from the pilot National Sentinel Surveillance (NSS) program so as to explore the feasibility, advantages and disadvantages of the NSS.
Methods:
Data were extracted from the NSS system, conducted in 11 provinces of China from November 2015 to October 2016. Characteristics regarding the etiology, complications of HFMD and factors related to the positive rates of HFMD specimens were analyzed under the logistic regression method by SPSS 20.0 software.
Results:
A total of 4 783 specimens were collected, including 3 390 from mild, 1 390 from severe and 3 from death cases. The overall positive rate was 81.43% (3 895/4 783). Other enteroviruses (non EV71/Cox A16 enteroviruses) appeared the major serotype (52.68%, 1 482/2 813) for mild infection of the disease while EV71 was for the severe cases (65.31%, 706/1 081). The serotype spectrum revealed by the pilot NSS was almost identical with the existing surveillance system. Other enteroviruses tended to infect younger children (
