1.Disease burden of tumors among children aged 0 to 14 years in Chinain 1990 and 2019

Objective:
To investigate trends in the disease burden of tumors among children aged 0 to 14 years in China in 1990 and 2019, so as to provide insights into management of pediatric tumors in China.
Methods:
The Global Burden of Disease 2019 data were retrieved from the Global Health Data Exchange, and the mortality and disability adjusted life years (DALYs) of pediatric tumors were evaluated among children at ages of 0 to 14 years in China in 1990 and 2019, and the disease burdens due to pediatric tumors in China were compared with the regions with different social population index (SDI).
Results:
The mortality of tumors decreased from 13.10/105 in 1990 to 4.96/105 in 2019 (a 62.17% reduction) among children aged 0 to 14 years in China, and the DALY rate decreased from 1 118.93/105 to 424.77/105 (a 62.04% reduction). The mortality and DALY rate of tumors decreased from 13.48/105 to 5.38/105, and from 1 147.09/105 to 458.65/105 among male children, and from 12.69/105 to 4.46/105, and from 1 088.22/105 to 384.94/105 among female children. The disease burden of pediatric tumors was concentrated among children at ages of 0 to 4 years. The three highest disease burdens of pediatric tumors were measured in leukemia, brain and nerve system tumors, and lymphoma in 2019. Compared with the regions with different SDI, the largest reductions were seen in the mortality and DALY rate of tumors among children at ages of 0 to 14 years in China, which were still higher than in middle, high-middle and high SDI regions.
Conclusions
The disease burden of tumors declined among children at ages of 0 to 14 years in China in 2019, compared with 1990; however, it is still higher than in middle and higher SDI regions. The disease burden of pediatric tumors was high among children at ages of 0 to 4 years and among male children, with leukemia, brain and nerve system tumors and lymphoma as predominant types.
2.The roles of otolith organs in the recurrence primary benign paroxysmal positional vertigo.

OBJECTIVE:
To explore the roles of otolith organs in the occurrence and recurrence of primary benign paroxysmal positional vertigo (BPPV) by vestibular evoked myogenic potential (VEMP) test.
METHOD:
We enrolled 17 recurrent primary BPPV patients and 42 non-recurrent primary BPPV patients between September 2014 and November 2014. All patients underwent VEMP tests, including cervical vestibular evoked myogenic potential (cVEMP and ocular vestibular evoked myogenic potential (oVEMP) tests. The abnormal case was defined as non-elicitation or asymmetry rate between bilateral sides is larger than 29%.
RESULT:
Significant difference was found in abnormal rate between cVEMP and oVEMP (P < 0.05 ) in BPPV patients. The abnormal rate of oVEMP was higher than that of cVEMP. Significant difference was found in abnormal rate in oVEMP test between recurrent and non-recurrent groups (P < 0.01) but not in cVEMP( P > 0.05). No significant difference was found in sex and age between recurrent and non-recurrent groups (P > 0.05).
CONCLUSION
The impairment of otolith organs, especially the utricle, is related to primary BPPV. Dysfunction of utricle may play a role in recurrence of BPPV. Recurrence of BPPV is not correlated with sex and age.

3.Comparison of the characteristics of the medical humanities education between Chinese and American medical colleges and its enlightenment

American medical colleges attach great importance to the humanistic education.The rich humanities curriculum is fully integrated into the teaching practice thus the mutual penetration of medicine and the humanities is achieved.They have specific and detailed examination evaluation system and the teaching method was more practical and participatory.By contrast,nost of the medical colleges in our country neglect the humanities construction and the cultivation of medical students' humanistic spirit.The coverage of the humanities curriculum is relatively narrow and the faculty is insufficient.We should promote the development of humanistic cducation,attach importance to the construction of medical humanities,set up appropriate curriculum system,increase the investment in faculty and reform the teaching method and examination method.
4.Pedicle screw placement combined with filler for lumbar vertebra fractures

BACKGROUND:Conventional cross-segment pedicle screw fixation wil cause some complications such as kyphosis and loss of corrective angle. Fixation of pedicle screw placement of the injured vertebral body improves the disadvantages of conventional posterior screw placement, and obtains ideal outcomes. OBJECTIVE: To investigate the clinical application value of pedicle screw placement combined with filer for lumbar vertebra fracture. METHODS: Clinical data of patients with pedicle screw placement combined with filer for lumbar vertebra fracture were retrospectively analyzed. They were repaired with pedicle screw placement combined with bone graft as wel as pedicle screw placement combined with bone cement. The ratio of anterior border to posterior border of the injured vertebral body after surgery and changes in Cobb angle were observed. Patients were folowed up and results were compared. RESULTS AND CONCLUSION: Pedicle screw placement combined with filer has many advantages, and obtained good outcomes in the treatment of lumbar vertebral fracture. However, the extensive application has some problems and disputes. Clinical physicians should pay more attention on choice of surgical indications, prevention and treatment of perioperative and long-term complications and postoperative treatment of osteoporosis. The pedicle screw fixation combined with calcium phosphate bone cement has a broad prospect, can maintain the effect of spine reduction, and is worth to spread in the clinical treatment of lumbar fractures.
5.Comparative study of seamless welding in cholecystectomy

Objective To explore the feasibility of high-frequency electric welding systemin cholecyste-ctomy when compared with ultrasound scalpel , Ligasure and traditional scalpel. Methods Ninety-six rabbits were enrolled in the study and divided into high-frequency electric welding system group , ultrasound scalpel group , Ligasure group and traditional scalpel group. The working temperature was recorded. Eight rabbits of each group were killed to observe the occlusion at 1st, 4th, 7th day postoperatively. Tissues were collected for observation under HE staining. Results High-frequency electric welding system group excelled in operative time , bleeding, working temperature, granulation tissue and inflammatory reaction. No significant difference was found in preope-rative and postoperative liver function test. Conclusion High-frequency electric welding system in cholecyste-ctomy has similar effect as ultrasound scalpel and Ligasure.
6.Effect of Shudi Jisheng Zhuanggu Decoction on Morphology of Articular Cartilage of Knee Osteoarthritis Model Rats

Objective To observe the morphology changes in articular cartilage of knee osteoarthritis ( KOA) model rats after intervention with Shudi Jisheng Zhuanggu Decoction (SJZD). Methods SD rats were randomized into 7 groups, namely blank control group, model group, Paracetamol group ( 0.270 g/kg) , Zhuangu Guanjie Pills group ( 1.08 g/kg) , and high-, middle- and low-dosage SJZD groups ( 27.450, 13.725, 6.863 g/kg, respectively). Rats were injected with papain solution into the right knee joint to build models of KOA before administration of drugs. Thickness of right knee joint was measured to evaluate the effect of SJZD on the swelling degree of knee joint. Histological changes of knee cartilage were evaluated by Mankin scoring method after the cartilage sections being stained with hematoxylin-eosin (HE). Results Swelling degree of knee joint and Mankin scores were significantly increased in the model group compared to the blank control group ( P<0.01). In comparison with the model group, the swelling degree of knee joint and Mankin scores of SJZD groups were significantly decreased (P<0.05 or P<0.01), and the pathological changes were relieved. Conclusion SJZD can ameliorate the pathological changes of knee articular cartilage in rats with KOA, so as to delay cartilage degeneration and protect the cartilage.
7.Clinical Study on Displacement of Atlas axis Joint,

By analysing the anatomy of upper cervical segment, the atlas - axis relation in plain X - ray film in 3-1 cases revealed that the joint displacement is one of the important causes for vertebral artery type or sympathetic type cervical spondylopathy. The main manifestations for diagnosing atlas - axis displacement included headache, dizziness, unilateral protrusion and tenderness of the affected vertebra, displacement of tooth protrusion up to 1 mm at anterior mouth -open positions. The most effective therapy was accurate and dexterousmanipulation for reduction.
8.Optimal Precision Decocting Process of Codonopsis Radix through Combination of AHP-Entropy Weight Method and Box-Behnken Design-Response Surface Method

Objective To optimize the granularity and decocting process of Codonopsis Radix decoction.Methods Adding water,soaking time and decoction time were set as examine factors,and flavonoids,total polysaccharides,and extract yield were set as the indexes,AHP-entropy weight method combined with the Box-Behnken design-response surface method were used to determine the best particle size and the technological parameters of Codonopsis Radix decoction.Results The optimum size of precise Codonopsis Radix decoction was 4.00-4.75 mm(4-5 mesh);the optimum decocting process was 13 times the amount of water added;the soaking time was 15 min;the decocting time was 10 min.Conclusion The granularity of Codonopsis Radix decocting powder optimized in this study is scientific and reasonable,which is suitable for practical production.The established decocting process is convenient,reasonable and feasible,and provides a theoretical basis for the industrial production of decocting powder.
9.Effects of magnesium sulfate on neuron apoptosis and expression of caspase-3, bax and bcl-2 after cerebral ischemia-reperfusion injury.

OBJECTIVETo investigate the effects of magnesium sulfate on neuron apoptosis and the expressions of caspase-3, bax and bcl-2 after cerebral ischemia-reperfusion injury.
METHODSThirty-six gerbils were randomly divided into three groups: Sham-operation group (So, n = 12), ischemia-reperfusion group (I-R, n = 12) and magnesium sulfate group (Ms, n = 12). The neuron apoptosis was detected by the terminal deoxynucleotidyl transferase (TdT)-mediated dUTP-fluorescence nick end labeling (TUNEL stain), and the protein expressions of caspase-3, bax and bcl-2 were detected by immunohistochemisty stain.
RESULTSApoptotic neurons and the expressions of caspase-3 and bax were significantly increased in I-R and Ms groups, compared with those in So group. Apoptotic neurons and the expressions of bax and caspase-3 in Ms group were significantly less than those in I-R group. There was no significant difference in the expression of bcl-2 among three groups.
CONCLUSIONSMagnesium sulfate decreases neuron apoptosis after cerebral ischemia-reperfusion injury, which may be related with the suppressed expressions of caspase-3 and bax.

Steaming is one of the commonly used methods in the processing of Chinese materia medica,which can be used to change the medicinal properties,expand the scope of use,enhance the efficacy,reduce side effects and facilitate slicing.By reviewing ancient herbal texts and literature,this article summarized the historical development of TCM steaming:from the Spring and Autumn period to the Warring States period,it has mainly gone through the stages of germination,growth,maturity,and prosperity.It further sorted out the research progress of modern steaming from the aspects of steaming technology,chemical composition and drug effect changes before and after steaming,and analyzed and discussed the establishment of quality standards of steamed products and the development of traditional and modern processing techniques,with a view to clarifying the mechanism of steaming,and providing basis for standardizing steaming technology,improving the quality standards of steamed decoction pieces and rational clinical application.
