1.Purification of patchouli alcohol in volatile oil of Pogostemon cablin by molecular distillation

Objective The volatile oil was further purified to get the patchouli alcohol by molecular distillation taking patchouli oil which contained 23.97% of patchouli alcohol as raw material.Methods The best process parameters,including temperature,feed rate,and scraping rate,of molecular distillation technique for separating patchouli oil were studied by orthogonal test.The content was detected by GC under the conditions of elastic quartz capillary column HP-5,detector temperature 280 ℃,gasification temperature 280 ℃,distributary refer 20∶1.The effects of evaporation temperature on content and extraction rate of the patchouli oil had been investigated and the difference between the molecular and vapor distillations was studied.Results Content and extraction rate of patchouli alcohol under the best evaporation temperature 65 ℃ by molecular distillation reached to 40.71% and 76.55%,respectively.Conclusion The molecular distillation technique that could get the product with higher purity and extracting rate than that by vapor distillation in patchouli oil purification has a very good prospect in industry.
2.Clinicopathological analysis of solitary fibrous tumor in 11 patients

Objective To evaluate the clinicopathological characteristics and immunophenotype of solitary fibrous tumor (SFT),which is able to provide the basis for clinical diagnosis and prognosis,differential diagnosis and predictable prognosis.Methods Data from 11 cases of SFT were investigated by immunohistochemical.Results Histologically,SFT was composed of spindle cells,which arranged in fascicles pattern,as well as the collagen and branch vessels.Malignant SFT had higher and crowed cellularity with significant pleomorphism,numerous mitoses and putrescence.Positivity for bcl-2 and CD34 was observed in all SFT.Eight tissues showed CD99 positivity,one showed s-100 positivity,all 1 1 tissues showed Ckpan negativity.Conclusions SFT has originated from different sites,which should be distinguished with fibrosarcoma and other spindle cell neoplasms.More attentions should be paid to malignant SFT and focus on the follow-up visit.
3.Inhibition of the invasion and migration of hepatocellular carcinoma cells by miR-148a and the mechanisms

Background and purpose: Primary liver cancer is the malignant tumor of liver cells or intrahepatic bile duct epithelium with familiar metastasis and postsurgical recurrence. The purpose of this study was to investigate the effects of miR-148a on the invasion and migration of hepatocellular carcinoma cells and the underlying mechanisms. Methods: The supernatant containing LV-miR-148a lentivirus particles was used to infect SMMC-7721 cells. The expression of miR-148a was determined by RT-PCR. Wound healing assay and transwell assay were performed to detect the effects of miR-148a on the invasion of hepatocellular carcinoma cells. Gelatin zymography assay was used to detect the effects of miR-148a on the enzyme activities of matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9). The expression of MMP-2, MMP-9, E-cadherin and vimentin proteins was detected by Western blot assay. Results:RT-PCR showed the expression of miR-148a was upregulated in the infected SMMC-7721 cells. Transwell assay and wound healing assay showed ectopic expression of miR-148a suppressed cell migration and invasion abilities. miR-148a overexpression led to the decrease of the enzyme activities of MMP-2 and MMP-9 (P<0.05). Western blot assay showed that the protein expression of MMP-2, MMP-9 and vimentin proteins was signiifcantly decreased, the expression of E-cadherin had no changes. Conclusion:miR-148a is able to inhibit the migration and invasion of human SMMC-7721 cells in vitro, and the possible mechanisms may be related to decrease the enzyme activities of the MMP-2 and MMP-9 and the down regulation expression of MMP-2, MMP-9 and vimentin.
4.Life Style of the Elderly in empty-nest Family and Need of Community Health Service in community of Shanggang

Objective To explore the life style of the elderly in empty-nest family and need of community health service in shanggang community.Methods Using random sampling method, a group of 634 the aged more than 60 from empty-nest family in the community were investigated with questionnaires.Results In daily activities,40.9% of the elderly empty-nest took part in physical exercise,36.3% had hobbies.Really empty-nest group had rarely physical exercise and prepared staying alone.In community health service,about 82.65% ,72.82% ,55.21% of the aged empty-nest needed regular physical examination,health guidance and psychological counseling respectively.Conclusion Life style of the elderly in empty-nest family was reasonable,and empty-nest group,especially really empty-nest group had advanced demand about the community health service.Medical staff should often carry out physical exercise and psychological counseling,and also popularize common medical knowledge.Besides,regular physical examination and paying close attention to the healthy information of the elderly in empty-nest family was needed.
5.Prevalence and antibiotic resistance proifle ofHelicobacter pylori in Shanghai

Objective To examine the prevalence ofHelicobacter pylori in Shanghai and relevant risk factors, evaluate the resistance proifle ofH. pylori isolates to antibiotics used in ifrst-line therapy in two hospitals in Shanghai.MethodsH. pylori were isolated from the biopsy samples of gastric mucosa collected from September 2013 to January 2015. Antimicrobial susceptibility test was performed by E-test method for 131H. pylori strains to 4 antibiotics, clarithromycin, metronidazole, amoxicillin and tetracycline. Results A total of 955 patients receiving gastroscopy were enrolled. And 248 (26.0%)H. pylori strains were isolated from the biopsy samples of gastric mucosa. Overall, 14.5%, 64.1%, 0 and 0.8% of the strains were resistant to clarithromycin, metronidazole, amoxicillin and tetracycline, respectively. Resistance to two drugs was found in 10.7%(14/131) of the strains, and majority (92.8%, 13/14) of which were resistant to clarithromycin and metronidazole.Conclusions The prevalence ofH. pylori in gastric mucosa is rather lower compared with the data reported previously. It is associated with the sex, age and clinical outcome of patients, however, antibiotic resistance profile is not related to these factors.H. pylori eradication therapy should be individualized according to the results of susceptibility test in Shanghai.
6.The value of different inflammatory indexes in early diagnosis of bacterial bloodstream infection in patients with malignant tumor

Objective To explore the value of procalcitonin,C-reactive protein and neutrophil percentage in early diagnosis of bacterial bloodstream infection in patients with malignant tumor.Methods Retrospectively analyzed the clinical data of 413 cases with tumor fever who were performed PCT,CRP,blood test and blood culture in the same day in our hospital from January to December in 2015.According to blood culture results,they were divided into observation group(211 cases) and control group(202 cases).The observation group was divided into Gram-negative(G-) group and Gram-positive(G+) group.The diagnostic efficacy of PCT,CRP and NEUT% on bacterial bloodstream infection was evaluated by receiver operating characteristic curve (ROC).Results The specificity of PCT in the observation group was the highest and the sensitivity of CRP was the highest.The levels of PCT,CRP and NEUT% in the observation group were significantly higher than those in the control group(P<0.05).PCT level in the G-group were apparently higher than that of G+ group(P<0.05),while CRP and NEUT% levels had no statistical significance between the two groups(P>0.05).According to ROC curve,In the observation group,The AUC of PCT,CRP and NEUT% were 0.848,0.796 and 0.718,respectively.In G-and G+ group,the AUC of PCT were 0.877 and 0.767,respectively.Conclusion PCT,CRP and NEUT% were significantly increased in the malignant patient with bloodstream infection,PCT has showed better diagnostic efficiency compared to CRP or NEUT% and better clinical value in distinguish between G-and G+ bacterial infection type.Combined detection of PCT,CRP,NEUT% can help for early diagnosing malignant tumor with bacterial bloodstream infection.
7.Correlation analysis between thrombelastography and conventional coagulation indexes in breast cancer patients

Objective To analyze the correlation between thrombelastography (TEG) and conventional coagulation indexes in breast cancer patients, and to compare the differences between the two methods in the detection of coagulation function in breast cancer patients.Methods Retrospectively analyzed the clinical data of180patients with breast cancer who were performed TEG, coagulation and blood test in the same day in our hospital from November 2016to May 2017.Linear correlation and regression analysis were performed among the parameters.The differences of positive rates of TEG parameters and conventional coagulation indexes were compared.Results The R value of TEG parameters of breast cancer patients was positively correlated with K and APTT, and negatively correlated withα-Angle, MA, CI, DD, FDPs.K was positively correlated with APTT and TT, and negatively correlated withα-Angle, MA, FIB, DD, FDPs and PLT.α-Angle was positively correlated with MA, CI, FIB, DD, FDPs and PLT.MA was positively correlated with CI, FIB, DD, FDPs and PLT.CI was positively correlated with FIB, DD, FDPs and PLT.α-Angle, MA and CI were all negatively correlated with APTT and TT, the difference were statistically significant (P<0.05).A linear regression equation of TEG parameters and coagulation indexes was obtained.There was no significant difference between TEG detection positive rate and conventional coagulation test (P>0.05).Conclusion There are significant correlation between the TEG parameters and routine coagulation or platelet, and the results are consistent.

Levothyroxine is a class of thyroid hormone medication mainly used in the clinical treatment of thyroid hormone replacement therapy and thyrotropin suppression therapy. In recent years, studies have found a close correlation between the human gut microbiota and the occurrence and development of thyroid diseases, as well as changes in thyroid hormone levels. Therefore, understanding the impact of levothyroxine on the gut microbiota, as well as the effects of the gut microbiota on the metabolism and absorption of levothyroxine, is of great significance for the treatment of thyroid diseases and the rational use of clinical medication. This article explores the interaction between the gut microbiota and levothyroxine and summarizes the current clinical findings of the gut microbiota in levothyroxine therapy.

Lung cancer is the leading type of cancer death, and most patients with lung cancer are diagnosed at an advanced stage and have a very poor prognosis. Although low-dose computed tomography (LDCT) has entered the clinic as a screening tool for lung cancer, its false-positive rate is more than 90%. As one of the epigenetic modifications of research hotspots, DNA methylation plays a key role in a variety of diseases, including cancer.Hypermethylation of tumor suppressor genes and hypomethylation of proto-oncogenes are important events in tumorigenesis and development. Therefore, DNA methylation analysis can provide some useful information for the early screening, diagnosis, treatment and prognosis of lung cancer. Although invasive methods such as tissue biopsy remain the gold standard for tumor diagnosis and monitoring, they also have limitations such as inconvenience in sampling. In recent years, there has been a rapid development of liquid biopsy, which can detect primary or metastatic malignancies and reflect the heterogeneity of tumors. In addition, the blood sample can be collected in a minimally invasive or non-invasive format and is well tolerated in older and frail patients. This article explores some of the emerging technologies for DNA methylation analysis and provides an overview of the application of DNA methylation in the diagnosis and treatment of lung cancer.

Objective To analyze the correlation between thrombelastography(TEG)and coagulation, platelet count(PLT)in patients with malignant tumor.Methods Retrospectively analyzed the clinical data of 241 cases with tumor who were performed TEG,coagulation and blood test in the same day in Chongqing Cancer Institute from November 2016 to March 2017.Linear correlation and regression were carried out to an-alyze relationship among the parameters.The number of patients with positive blood clotting,PLT and TEG parameters were counted,and the χ2test was used to compare the difference between them.Using Mann-Whit-ney U test to compare the differences between multiple parameters of liver cancer,breast cancer,and pancreatic cancer.Results The R value of TEG parameters in patients with malignant tumor was positively correlated with APTT,negatively correlated with TT,DD and FDPs.K was positively correlated with APTT and TT, and negatively correlated with FIB and PLT.The relationship between α and FIB,PLT were positive,between APTT and TT were negative.MA was positively correlated with FIB and PLT,negatively correlated with TT, CI was positively correlated with FIB and PLT,and negatively correlated with APTT(P<0.05).The correla-tion between FIB,PLT and MA was the highest.And the linear regression equation of TEG parameters and coagulation indexes was obtained.The positive rate of TEG was lower than that of coagulation(P<0.05). Same certain differences of TEG and coagulation parameters were existed in liver cancer,breast cancer and pancreatic cancer patients.Conclusion TEG is significantly associated with PLT and conventional coagulation test,and the results of TEG and conventional coagulation test are consistent to a certain degree,but the overall agreement is generally not interchangeable.TEG might be play a complementary role with coagulation tests and platelet counts.And the TEG of different cancer types is not exactly the same as the coagulation parameters.
