1.Contamination and dietary exposure assessment of zearalenone in cereals and their products

Abstract
Zearalenone (ZEN), a nonsteroidal mycotoxin produced by fusarium with estrogen-like effects, and it contaminates various types of cereals and their products to varying degrees. Wheat and rice are the staple foods of Chinese residents, and understanding the contamination status and dietary exposure risk of ZEN in grains and their products is of great significance for maintaining food safety and promoting population health. This article reviewed the publications between 2012 and 2023 on ZEN contamination and dietary exposure assessment in cereals and their products, and found that the dietary exposure risk of ZEN contamination in cereals and their products was low. Continued efforts are needed to strengthen food safety monitoring and regulatory oversight to ensure food safety.
2.Discussion of interrupted circular suture dming caesarean section with placenta previa

ObjectiveTo discuss the value of interrupted circular suture in hemostasis of placenta previa during cesarean section. Methods We summarized 54 caesarean section patients with placenta previa. Results The hemostasis was succeeded in all of the 9 patients and uterus was retained without postpartum complications. The duration of operation was obviously shorter than that of hysterectomy( P<0.05). Bleeding and blood transfusion were less than that of hysterectomy, but without statistical difference (P>0.05). ConclusionInterrupted circular suture is one of the efficient methods in controlling postpartum bleeding during caesarean section with placenta previa.
3.Recent advance in role of cyclic GMP-AMP synthase-stimulator of interferon gene signal pathway in brain tumors

The cyclic GMP-AMP synthase (cGAS)-stimulator of interferon gene (STING) signal pathway is a classical pathway of STING activation, and in recent years, its role in stimulating innate immunity has gradually attracted wide attention. Besides, cGAS can recognize and combine endogenous or exogenous DNA, then catalyze ATP and GTP to synthesize cyclic guanosine monophosphate-adenosine monophosphate (cGAMP), subsequently activate STING signal to promote type I interferon and inflammatory factors, finally induce natural and adaptive immunity. Existing studies have indicated that cGAS-STING signal pathway plays an important role in infections, inflammations and tumors, especially in high-grade gliomas with poor clinical treatment efficacy. Here, we briefly summarize the cGAS-STING signal pathway and its mechanism in brain tumors to provide new ideas for exploring therapeutic targets and drugs for brain tumors.
4.Perineum nursing with traditional Chinese medicine for preventing catheter associated urinary tract infection based on literatures

Objective To analyze the current literatures on perineum nursing with traditional Chinese medicine for preventing catheter-associated urinary tract infection (CAUTI).Methods The literatures were retrieved with free words and key words from the Chinese biomedical literature database (CBM), China National Knowledge Infrastructure (CNKI), Wanfang database, VIP database (VIP) before November 2016. The retrieval results were evaluated by 2 reviewers independently. Data was extracted using modified JADAD scale.Results Nine literatures were included in the study, with low quality (0-3 points). No high quality literatures (4-7 points) were involved. Conclusions The studies on preventing CAUTI with traditional Chinese medicine (TCM) are at the first step. High quality literatures are seldom. Methodology studies are required to provide evidences for preventing CAUTI, especially for high risk and special patients.
5.The study of effect of Human macrophage polarization on Mouse Pericyte-to-Myofibroblast Transition in vitro

Objective:To explore the role of macrophage polarization on pericyte-to-myofibroblast transition and renal allograft fibrosis after kidney transplantation(KT).Methods:Allograft tissues were harvestedfrom recipients with chronic allograft dysfunction(CGD)and normal kidney tissues.The expression and distribution of M1/M2 macrophages in kidney tissues were detected by routine and immunofluorescent staining; mRNA of CD68, CD206 and iNOS detected by polymerase chain reaction(PCR); Murine vascular pericytes subjected to TGF-β1 in vitro and the expressions of α-SMA and PDGFR-β in perivascular cells detected by immunoblotting and cellular fluorescence; The co-culturing models of vascular pericytes and M1/M2 macrophages were constructed.The expressions of α-SMA and PDGFR-β in pericytes were detected by immunoblotting, cellular fluorescence and PCR.Results:A marked infiltration of CD68+ iNOS+ M1 macrophages was present in allograft tissues of recipients with CGD while no obvious infiltration of CD68 + CD206 + was observed.The mRNA levels of CD68, iNOS and CD206 were significantly higher in CGD group than those in control group( P<0.05); In CGD allograft tissues, protein expressions of α-SMA and PDGFR-β spiked markedly( P<0.05)while cells with double staining of α-SMA and PDGFR-β were markedly infiltrated in interstitial area of CGD allograft.TGF-β1 could induce a marked elevation of PMT-related markers in a time-dependent manner( P<0.05); Immunoblotting and cellukar fluorescence indicated that M1 macrophages could promote the elevations of α-SMA and PDGFR-β in pericytes in vitro while M2 macrophages showed no effect on pericyte-to-myofibroblast transition in pericytes. Conclusions:M1 macrophage polarization may promote the formation of renal allograft interstitial fibrosis through promoting PMT.
6.Advances in surgery in neonatal complicated congenital heart disease

With the research and summary of the cases, the surgical indications, surgical procedures, and reference values for material selection for the surgical treatment of neonatal complex congenital heart disease are further refined, and the diagnosis and treatment system is further standardized and completed. In order to increase the success rate of surgery, reduce postoperative mortality and postoperative complications, many researchers have proposed improved surgical procedures, some of which have become mature technologies. In addition, in recent years, the surgical age of newborns has gradually advanced and the surgical weight has gradually decreased. In particular, the increasing application of Hybrid therapy has enabled some critically ill, premature, and low birth weight children to survive smoothly to accept surgical correction. This article will review the recent advances in the surgical treatment of neonatal complicated congenital heart disease.
7.Exploration and discussion on homogeneous development of medical quality across multiple campuses in cancer specialty hospitals—a case study of sun yat-sen university cancer center

Objective To summarize the management challenges faced by multi-campus hospitals both domestically and internationally,to delineate the experiences in medical administration across multiple campuses,and to propose the"Zhong-zhong"multi-campus medical management pattern based on the practices of Sun Yat-sen University Cancer Center,aiming to pro-vide reference and evidence for domestic peers in the field.Methods This study uses the methods of literature research and case study analysis,and summarizes and condenses the experience of medical management in multi-campus and provides policy sug-gestions through key person interviews and expert consultation.Results SYSUCC carries out multi-campus medical management through six major measures,namely,the organizational system combining vertical and territorial management,the layout of disci-plinary clusters and specialties,multi-channel flow attraction measures,the dynamic adjustment of medical resources,the homog-enization of medical quality,and the high-effective information system.It achieves the high-quality development goals with differ-ential development of various specialties,improved resource utilization efficiency,homogeneous and incremental medical serv-ices,smooth operation of information system,and steady and orderly development of new techniques.Conclusion Domestic public specialized hospitals can achieve homogeneous management and high-quality development across multiple campuses through streamlined organizational structures to enhance management efficiency,optimizing the layout of specialized departments to pro-mote coordinated development,strict control of standards to ensure medical quality,using dynamic adjustments to guide orderly competition among specialized departments,and leveraging information systems to support homogeneous development across multi-ple campuses.
8.Establishment and application of medical performance evaluation index system for clinicians competing for senior professional titles in cancer hospitals

Following the reform of the professional title system for"Breaking Four One-sided Evaluation Criteria"—focu-sing on more than just papers,titles,academic qualifications,and awards-establishing a scientific,systematic,and comprehen-sive medical performance evaluation index system has become essential for developing medical talent teams and conducting profes-sional title assessments fundamentally.This study establishes a medical performance evaluation index system tailored for clinicians in various departments of a cancer hospital who are competing for senior professional titles.This system comprises six primary in-dicators and 18 secondary indicators,with results presented in a ranked format of medical performance.Additionally,we have al-so analyzed the corr-elation between clinicians'medical performance rankings and their professional title evaluation outcomes through practical application.The results indicate that clinicians with higher performance rankings have significantly higher suc-cess rates in evaluations(P<0.05).This index system underscores clinical practice,enhances classification-based evaluations,and supports advanced information management and precision in hospital administration,thereby providing a solid foundation for strengthening the hospital's core competitiveness.
9.Evaluation of robotic system for mandibular reconstruction based on intelligent preoperative planning

Objective:To evaluate the performance and the accuracy of surgical robot for mandibular reconstruction based on intelligent surgical planning.Methods:115 CT scanning images of normal mandible (57 males, 48 females, 40.3±9.1 years old, from February 2010 to May 2019) and 115 CT scanning images of mandible with tumor (62 males, 53 females, 55.6±7.2 years old, from March 2008 to August 2019) from Shanghai Ninth People’s Hospital were selected. The surgical robot system including work station, UR robot, optical navigation system, 6 dimensional force senor and surgical instrument. A 3D V-Net for mandible preoperational segmentation from CT scans was proposed and used to segment the mandible of a 54-year-old male patient who received mandible reconstruction with fibular flaps. The machine learning algorithm was used to aid surgical planning for maxillo-mandibular defect by detecting landmarks. The accuracy was defined as the distance between corresponding landmarks on the intact mandible. The robot could locate the target according to surgical planning and perform fibula osteotomy through force-motion control. The CT scanning of limb and head from the patient (male, 54 years old) was used for phantom experiments. 30 osteotomies on 5 3D-priented resin phantom were carried out. The pre- and post-operative images were compared to calculate the accuracy. The descriptive results were in the format of Mean±SD.Results:The average accuracy of V-Net for mandible segmentation was 96.581% and the time cost was less than 30 seconds. The average error of feature points on mandible was (2.24±1.74) mm. The residual length error was (1.02±0.45) mm and angle error was (0.96±0.42) degree in robotic-assisted osteotomy according to 3 cases of phantom experiments. The surgical robot could perform osteotomy safely and steadily within 15 min.Conclusions:Intelligent surgical planning can precisely segment the mandible and determine its landmarks. Robot for mandibular reconstruction can perform fibular osteotomy precisely with the pre-operative planning.
