1.Single stage and staged cochlear implant for chronic suppurative otitis media suffers.

OBJECTIVE:
To explore and summarize the operation method and operation stage for cochlear implant with chronic suppurative otitis media, to provide the clinical reference for cochlear implant with chronic suppurative otitis media.
METHOD:
The clinical data of 6 cases of cochlear implant with chronic suppurative otitis media from Jun 2006 to Mar 2009 in our hospital was analyzed retrospectively. The operation stage. surgical skill, possible risk and prognosis was analyzed and summarized.
RESULT:
3 of 6 cases received single stage subtotal petrosectomy and cochlear implant. 3 of 6 cases received subtotal petrosectomy, they received staged cochlear implant 1 to 6 months later. No complications occurred, all of the cochlear implanted had good open set speech perception.
CONCLUSION
Staged operation was the first choice for cochlear implant with chronic suppurative otitis media. Single stage operation took potential risks, it should be done cautiously. The key points for the operation was the clearance of the pathological tissue totally, this required good surgical skill and operation instrument.

2.Phylogenetic relationships of Dendrobium candidum and its related species based on DNA ITS sequences

AIM: To apply molecular systematic techniques to revealing the genetic diversity of medicinal plant of Dendrobium candidum and its related species in Dendrobium.METHODS: The internal transcribed spacer(ITS) as well as 5.8S rDNA sequences of 40 samples of 25 species of Dendrobium were carried out on 25 dendrobi-um samples and amplified using PCR method and sequenced.Mega 3.1 was used to analyze the genetic diversity within the genus.Phylogenetic relationships among species in Dendrobium were estimated by maximum parsimony and neighbor-joining methods to construct similar ITS trees.RESULTS: The phylogenetic analysis indicated that Dendrobium candidum had a markedly difference in the interspecies and possessed the characteristic sequence site.CONCLUSION: ITS sequences as a good molecular marker for authentication between Dendrobium and outgroup is feasibility.
3.Peripheral Mohs micrographic surgery for the treatment of extramammary Paget′s disease Wan Miaojian, Ma

Objective To evaluate effectiveness of peripheral Mohs micrographic surgery for the treatment of extramammary Paget′s disease (EMPD). Methods A total of 28 patients with EMPD were treated with peripheral Mohs micrographic surgery. The depth and extent of tumor infiltration were evaluated before the surgery. One day before the surgery, 20% aminolevulinic acid hydrochloride was topically applied to determine and label surgical margins under a Wood′s lamp. After fluorescence-based localization, peritumoral skin tissues were resected and underwent frozen-section examination according to the protocol for Mohs micrographic surgery. Meanwhile, the tumor was resected. After surgery, patients were followed up every 3 - 6 months to detect local recurrence and metastasis. Results Of the 28 patients, 25 were male and 3 were female. Six patients each underwent 3 sessions of frozen-section examination, and 12 patients each received 2 sessions, with an average of 1.86 sessions for each patient. During the follow-up for 5 - 72 months, local recurrence occurred in 3 cases, and 1 patient died of tumor metastasis and uremia after 2 years of follow-up. Conclusion Peripheral Mohs micrographic surgery is a time-saving and effective treatment for EMPD.
4.Effects of interleukin-34 on prostaglandin E2 expression of fibroblast-like synoviocytes in patients with rheumatoid arthritis

Objective To investigate the effects of interleukin-34 (IL-34) on prostaglandin E2 (PGE2)/cyclo-oxygenase-2 (COX-2) expression on fibroblast-like synoviocytes (FLS) in patients with rheumatoid arthritis (RA). Methods FLS was isolated from 6 RA patients and stimulated with IL-34 (50 ng/ml), IL-34 receptor antagonist (25 ng/ml) and IL-34 (50 ng/ml), inhibitors of signaling pathway (10 μmol/L) and IL-34 (50 ng/ml) in vitro respectively. The expression of COX-2 mRNA was detected by reverse transcription polymerase chain reac-tion (RT-PCR). The level of PGE2 in the supernatant of RA FLS culture was measured by Enzyme linked immunosorbent assay (ELISA). Statistical analysis between groups were performed by t test. Results Com-pared to unstimulated FLS, COX-2 and PGE2 expression was increased dramatically on IL-34-stimulated FLS, most evidently in 48 hours [(139±24) pg/ml vs (201±8) pg/ml, t=-6.177, P<0.01]; Moreover, the level of PGE2 was decreased when anti-IL-34 antibody was added to the IL-34-stimulated RA FLS at 24 hours, 48 hours, 72 hours [(250 ±58) pg/ml vs (100 ±28) pg/ml, t=5.742, P<0.01; (375 ±24) pg/ml vs (97 ±23) pg/ml, t=20.564, P<0.001; (357 ±21) pg/ml vs (94 ±18) pg/ml, t=22.353, P<0.01]; In the presence of SB203580 and IKK-16, PGE2 level produced by IL-34-stimulated FLS was obviously decreased [(279 ±37) pg/ml vs (63 ±17) pg/ml, t=12.806, P<0.01;(279±37) pg/ml vs (77±16) pg/ml, t=6.177, P<0.01]. Conclusion Binding of IL-34 with its receptor may promote the secretion of PGE2 via NF-κB and P38 MAPK signaling pathway in RA FLS, suggesting that it might be involved in the pathogenesis of RA.

Objective To investigate the effects of interleukin (IL)-34 on the proliferation and chemokines expression of fibroblast-like synoviocytes (FLS) in patients with rheumatoid arthritis (RA).Methods RA FLS were isolated and cultured in the presence or absence of IL-34.FLS proliferation was determined by methyl thiazlyl tetrazoliu method (MTT) method.The levels of chemokines from RA FLS supernatant were detected by protein chip AAH-CYT-G1000 assay.The IL-34 receptor (IL-34R) expression on RA FLS was analyzed by flow cytometry (FCM).Statistical analysis between groups was performed by t test.Results Compared to the control group, the proliferation of IL-34-stimulated RA FLS was obviously increased, and up to the maximum at 72 hours.In addition, the levels of chemokines [epithelial neutrohil-activating peptide 78 (ENA-78), IL-8, growth-related oncogene (GRO), macrophage chemoattractant protein-1 (MCP-1)] on RA FLS supernatant in IL-34 stimulated group were significantly elevated than those in the control group [ENA-78: (397.1±8.2) pg/ml, (54.0±2.9) pg/ml (t=127.61, P<0.01);IL-8:(2 017±36) pg/ml, (778±102) pg/ml (t=36.67, P<0.01);GRO: (4 935±160) pg/ml, (2 746±188) pg/ml (t=43.60, P<0.01);MCP-1: (46 798±1 293) pg/ml, (27 813±2 329) pg/ml (t=22.43, P<0.01)].IL-34R was highly expressed on the RA FLS.Conclusion IL-34 promotes RA FLS proliferation and chemokines expression, which may be one of the important mechanisms involved in the pathogenesis of RA.
6.Levels of interleukin-1 family and interleukin-34 in serum of patients with ankylosing spondylitis

Objective To preliminarily investigate the levels of interleukin (IL)-1 family and IL-34 in serum of patients with ankylosing spondylitis (AS) and their roles.Methods Serum IL-1 family levels were detected from 6 AS patients and 4 healthy controls by using protein-chip technique.Enzyme-linked immunosorbent assay (ELISA) method was used to detect the levels of serum IL-34 from 65 AS patients and 85 healthy controls and the relationships of serum IL-34 levels and clinical or laboratory features were analyzed.T test and Spearman correlation were used for statistical analysis.Results IL-1Ra [(3302±1352) pg/ml vs (10778±2764) pg/ml]and IL-36Ra [(1363±194) pg/ml vs (3875±996) pg/ml] levels were significantly down-regulated in AS patients compared with that of healthy controls (t=5.363 and 4.289 respectively,both P<0.05).The levels of IL-1α,IL-18,IL-36α and IL-37 were increased more remarkable in AS patients than in healthy controls (t=-2.532,-5.400,-5.023 and-5.783 respectively,both P<0.05).Moreover,serum IL-34 levels were elevated more significantly in AS patients than in healthy controls [(169±153) pg/ml vs (54±31) pg/ml,t=6.722,P<0.01] and were positively correlated with the levels of CRP and ESR.Serum IL-34 levels were markedly up-regulated in human leukocyte antigen (HLA)-B27 positive patients than in HLA-B27 negative patients(P<0.05).Conclusion Part of IL-1 family and IL-34 may be involved in inflammatory or immunological process of AS.
7.Analysis of environmental endocrine disrupting activities in wastewater treatment plant effluents using recombinant yeast assays incorporated with exogenous metabolic activation system.

OBJECTIVETo measure the endocrine disrupting chemicals (EDCs) in wastewater and evaluate the EDCs removal efficiencies in the municipal wastewater treatment plants (WWTP).
METHODSA battery of in vitro recombinant yeast bioassays incorporated with exogenous metabolic activation system (rat liver preparation, S9 mix) was conducted to assess the estrogen receptor (ER), androgen receptor (AR), progesterone receptor (PR), and thyroid receptor (TR) ant/agonistic activities of effluents collected from Datansha WWTP.
RESULTSThe indirect estrogenic, anti-androgenic, anti-progesteronic, and anti-thyroidic activities were observed in the influent. The removal efficiencies of EDCs were above 74%, suggesting that the present wastewater treatment processes were good enough to remove most of these indirect endocrine disrupting chemicals.
CONCLUSIONThe incorporation of exogenous metabolic capacity into the test system was valid for the study of indirect effects on ER, AR, PR, and TR.

8.The clinical features of nested variant of bladder urothelial carcinoma

Objective To analyze the clinical features of nested variant of bladder urothelial carcinoma.Methods The clinical data of 13 patients with nested variant of bladder urothelial carcinoma treated in our hospital from July 2014 to April 2018 were retrospectively analyzed.There were 10 males and 3 females and the mean age was 64.6(37-81) years.All 13 cases had symptom of hematuria,3 cases with anemia,1 cases with urinary frequency,urgency and dysuria,1 case with all the above symptoms.Six cases underwent transurethral resection of the bladder tumor (TURBT),7 cases underwent radical cystectomy.Results All 13 cases had nested structures in pathology.Six cases did immunohistochemistry but none of them were specific.Twelve cases were high-grade invasive urothelial carcinoma,of which 2 cases were associated with carcinoma in situ,and 1 case was low-grade invasive.Two patients' pathological stages were ≤T1,4 patients in T2 phase,and 7 patients in T3-4 phase.Four patients who underwent TURBT received intravesical instillation chemotherapy,and 3 patients who underwent radical cystectomy and 1 patient with TURBT received intravenous chemotherapy.One patient with TURBT received both intravesical chemotherapy and intravenous chemotherapy.The remaining 4 patients who underwent radical cystectomy did not receive special treatment.The progression-free survival time of 13 patients was 2-39 months,of which 2 patients relapsed,1 patient metastasized,1 patient with tumor progression,and 2 patients with non-tumor specific death.Conclusions The clinical features of nested variant of urothelial carcinoma special.However,the pathology of nested variant of bladder urothelial carcinoma has a unique nested structure.At present,TURBT and radical cystectomy are still the main treatment modalities for nested variant of bladder urothelial carcinoma,and the prognosis of which is poor.
9.Survey analysis of idiopathic scoliosis in 12 881 junior middle school students from Shantou city,Guangdong province

Objective To study the survey outcome of idiopathic scoliosis ( AIS) in 12 881 junior middle school students from Shantou city ,Guangdong province.Methods From October 2013 to January 2015,the survey of AIS was carried out in 12 881 junior middle school students from 10 middle schools in Shantou city ,Guangdong province.Three methods such as school screening ,hospital review and X -ray film examination were used.The two five surface checking method was used as physical examination method .X-ray was used to slicing a positive lateral view of the whole spine as standing position.The persons whose Cobb angles ≥10°were diagnosed as AIS.Results The amount of individual with suspicious signs were 1 543 students,the suspected rate was 11.98%.Meanwhile,a further checking was conducted by hospital to 263 students,the response rate was 17.04% and 136 students were diagnosed as AIS.The positive rate of AIS was 51.71%.Conclusion The prevalence of AIS in junior middle school students in Shantou city is higher than national average level.Therefore,it is necessary to attach attention to the census for AIS.
10.Dynamic Variation of Serum Neuron-Specific Enolase and Its Clinical Significance in Patients with Acute Or-ganophosphate Poisoning

Objective:To explore the dynamic variation of serum neuron‐specific enolase (NSE) and its clinical significance in patients with acute organophosphate poisoning .Methods:Ninety patients with acute organophosphate poisoning were enrolled into case group ,and other 90 healthy subjects were enrolled as control group .Venous blood samples of 3 .0 mL were collected from the all 180 subjects at admission .Apart from the 8 patients died on day 1 ,the other 172 subjects underwent collection of 3 .0 mL venous blood samples on day 2 ,day 3 ,day 5 ,day 7 .Serum NSE concentration was measured by enzyme‐linked immu‐nosorbent assay ,while cholinesterase(ChE) was determined by photoelectric colorimetry .Results:Eight patients in case group died on admission day ,of whom the average concentration of NSE and ChE were (131 .72 ± 10 .84) ng/mL ,(199 ± 228) U/L , respectively .Concentration of NSE of the rest 82 patients on day 1 ,day 2 ,day 3 were (54 .56 ± 5 .46) ng/mL ,(36 .18 ± 5 .27) ng/mL ,(20 .56 ± 4 .64) ng/mL ,respectively ,and concentration of the control group on the corresponding time points were (9 .72 ± 3 .92) ng/mL ,(9 .48 ± 3 .66 )ng/mL ,(9 .86 ± 3 .62)ng/mL ,respectively .Statistically significant differences were de‐tected between two groups(P< 0 .01) .Average concentration of ChE of the rest 82 patients on day 1 ,day 2 ,day 3 were (1628 .60 ± 112) U/L、(2113 .59 ± 324) U/L、(2474 .75 ± 711) U/L ,respectively ,and those in the control group on the corre‐sponding time points were (5982 ± 215) U/L ,(5651 ± 136) U/L ,(5783 ± 764)U/L ,respectively .Statistically significant differences were detected between two groups(P<0 .01) .The concentrations of NSE were in significantly negative correlation with that of ChE in mild ,middle and severe poisoning patients on day 1 ,day 2 ,day 3 ,and in positive correlation with the poi‐soning severity .The correlation coefficients rs>0 .05 ,P<0 .05 .Conclusions:Serum NSE may be used as an reference indica‐tor in the diagnosis and treatment of acute organophosphate poisoning .
