1.Dilemma and policy proposal for the ethical value of new health care reform:From the per-spective of Diagnosis Related Groups

Health care policy should follow principles of total efficiency, no-harm, fairness and justice as ethi-cal standards for public policy. Diagnosis Related Groups( DRGs) as a sign of new health care reform ( NHCR) is implemented as a pilot program across the country, with the obvious function of suppressing excessive growth in medi-cal costs and over-treatment. From the dimension of ethical values, the DRGs has avoided unethical individuals, fol-lowed the principle of total efficiency and achieved the equalization of social security. However it has also increased ethical risks and been adverse to the development of medical research. To realize the reasonable development of DRGS, the construction of DRGs must be improved, and security policies must be reasonable and feasible; orderly measures must be established to ensure promotion and implementation;the government’s financial investment must be increased to protect the social welfare of public hospitals;a sound communication mechanism must be implemented, and interaction between medical institutions and insurance institutions must be strengthened.
2.Treatment of traumatic anterior sternoclavicular joint dislocation with Fastin anchors

Objective To discuss the treatment of traumatic anterior sternoclavicular joint disloca-tion with Fastin anchors. Methods A retrospective analysis was done for 14 patients with traumatic anterior sternoclavicular joint dislocation who had been treated with Fastin anchors from March 2006 to April 2008. They were 9 males and 5 females, with an average age of 35 years (range, 21 to 46 years) . Their diagnoses were confirmed by X-ray and CT scan. After reduction and temporary Kirschner wire fixation of the sternoclavicular joint, the anchor was driven into the manubrium sterni, with the suture at the end of the anchor securely knotted after passing through the internal extremity of clavicle. The Kirschner wire was re-moved after 3 weeks. Results All the patients were followed up for 12 to 22 months (average, 17 months). The results were excellent in 11, good in 3 according to Rockwood scale. There was no loss of re-duction or implant loosening. Conclusion Fastin anchor can fix the sternoclavicular joint reliably, avoiding potential risk of Kirschner wire migration and ensuring fine functional recovery of the joint.
3.Progress in research on correlation between dampness disease and aquaporin.

Pathogenic dampness can cause many diseases, and that is why traditional Chinese medicine pays close attention to pathogenic dampness. In recent years many researches discovered that dampness diseases and aquaporin have close correlation. By analyzing the progress in research on correlation between dampness diseases and aquaporin, it is discovered that aquaporin has close correlation with urinary system, digestive system, respiratory system and central nervous system. Normal expression of aquaporin may be the molecular biological basis of dispelling dampness, while abnormal expression may be one of the main causes for dampness diseases. Studying the correlation between dampness diseases and aquaporin has quite important significance in understanding mechanism of dampness diseases and formulating treatment protocols.
4.Comparison of external fixation and locking compression plate for intra-articular distal radial fractures

[Objective]To compare the results of external fixation and locking compression plate(LCP) in the treatment of intra-articular distal radius fractures.[Method]A retrospective analysis was done for 62 cases of intra-articular distal radius fractures treated from May 2006 to September 2008.Twenty-seven cases were treated with external fixation,and 35 with LCP.The study analyzed the differences between the two groups in Sarmiento scale and functional recovery of wrist.[Result]All the patients were followed up for 13 to 40 months(average,17 months).There was no significant difference in the Sarmiento scale and range of motion 1 year later,and the grip strength was significantly higher in external fixation group.Two cases in external fixation group had pintract infection.One case had tendon irritation in LCP group.[Conclusion]External fixation and LCP can be used reliably to treat intra-articular fractures about the wrist.It is essential that the surgeon have a complete understanding of the injury pattern before operation.

6.Effect of subculture at logarithmic phase on kinetics of cell cultures of Taxus chinensis var. mairei

Th euptake of carbohydrate, nitrogen and phosphate from the medium was examined, th e growth rate of Taxus cell and the synthesis of taxol were anal y zed too during the whole growth period Results The Taxus cell subcultured at logarithmic phase uptook carbohydrate and nit rate earlier than that subcu ltured at stationary phase. The special growth rate of the former was 1.5 folds of that of the latter. The production rate of taxol was increased by nearly 4 ti m es Conclusion Subculturing at logarithmic phase will benefi t the biomass accumulation and the taxol synthesis
7.Primate models of diabetes induced by streptozotocin

BACKGROUND: It is of great importance in improving the clinical effect of human islet allograft to study and design models of such large animals as pigs or primates preclinically.OBJECTIVE: To evaluate the effect of different doses of streptozotocin (STZ) on inducing diabetes type Ⅰ models of nonhuman primates.DESIGN, TIME AND SETTING: A contrast observational animal experiment was performed in the Cell Transplantation and Gene Therapy Center, the Third Xiangya Hospital of Central South University from October 2007 to December 2008. MATERIALS: A total of 21 adult male rhesus monkeys were divided into a 125 mg/kg STZ group (n =5), a 75 mg/kg STZ group (n=5) and a 50 mg/kg STZ group (n=11).METHODS: STZ weighed with regard to body mass of animals was prepared into 25 g/L STZ solution with buffer that was prepared in advance. After being filtered and degermed, the new-prepared STZ of 125 mg/kg, 75 mg/kg and 50 mg/kg were administered by intravenous injection into the experimental monkeys respectively, which took 1-5 minutes.MAIN OUTCOME MEASURES: Liver and renal function, glucose metabolism and histomorphological changes of animals during 1-16 weeks following administration.RESULTS: In 125 mg/kg STZ group, two rhesus monkeys died, in 8 hours following STZ administration, of serious hypoglycemia caused by severely damaged pancreas β cells; All rhesus monkeys in this group had got significantly increased liver transaminase, serum creatinine and urea nitrogen at week 1 following STZ administration, which reached a peak during 2-4 weeks; One rhesus monkey in this group showed severe shortage of endogenous trypsin and hyperglycemia irreversible by exogenous insulin following STZ administration, and finally died at day 13 following STZ administration due to the glucose metabolic disorder, ketoacidosis, liver and renal failure; The other two survivors in this group kept high level of liver transaminase,urea nitrogen and serum creatinine throughout the observation period. In 75 mg/kg STZ group, rhesus monkeys presented significantly increased liver transaminase, serum creatinine and urea nitrogen at week 1-2 following STZ administration; After 4 weeks following administration, their liver and renal function presented with abnormality of different degrees; One rhesus monkey in this group had got injured renal function, decreased power of resistance, eyelid edema, general dropsy and irreversible infected rump after injection of STZ, and finally died at the end of week 5 following administration; Another rhesus in this group presented with irreversible continuous hyperglycemia, inappetence and significantly decreased weight, and finally died ofsystemic failure at week 9 following administration. In the 50 mg/kg STZ group, renal function of monkeys were slightly affected, with a transient mild rise which return to the normal level by the end of week 4 following administration; Only 3 animals in this group appeared eyelid edema during 1-4 weeks following administration which disappeared afterwards.CONCLUSION: STZ of 50 mg/kg is possibly the optimal dose for inducing diabetes models in most rhesus monkeys.
8.Separation of Cyclotetramethylenetetranitramine Synthetic Intermediates by Online Molecular Imprinted Solid Phase Extraction and Liquid Chromatography

An online solid phase extraction ( SPE ) coupled with high performance liquid chromatography (HPLC)-mass spectrometry (MS) method was developed for the separation of 1,3,5,7-tetraacetyl-1,3,5,7-tetraazacyclooctane ( TAT ) and 1 , 3 , 5-triacetyl-1 , 3 , 5-triazacyclohexane ( TRAT ) which are the synthetic intermediates of cyclotetramethylenetetranitramine ( HMX) . In this experiment, molecularly imprinted polymers with TAT as the template were used as SPE sorbents. PC HILIC column was employed in liquid chromatographic separation. The parameters of SPE-HPLC were optimized. Acetonitrile was selected as the loading solution with flow rate of 0. 1 mL/min. After flushed by ethyl acetate, the TAT adsorbed on SPE was eluted by methanol, which was also used as the mobile phase in HPLC separation. The mass spectrometry was coupled with HPLC to identify the corresponding peaks. Under the optimized conditions, the linear detection range of this method was 6. 0 mg/L to 500. 0 mg/L, with the detection limit of 1. 8 mg/L (3σ). The enriching factor was 400 times and TAT recovery was 79%–93% in the standard addition experiment.
9.Combined detetion of SCC ,CEA and CA125 in early diagnosis of lung cancer by chemiluminescence

Objective To explore the clinical application effect of squamous cell carcinoma antigen (SCC) ,cancer embryo antigen (CEA) and carbohydrate antigen 125(CA125) chemiluminescence combined detection in early diagnosis of lung cancer .Methods The SCC ,CEA and CA125 levels in the patients with early stage lung cancer ,benign lung diseases and healthy subjects undergoing physical examination were detected by using the automatic chemiluminescence immune analyzer .Results The concentrations of three tumor markers in the lung cancer group were significantly higher than those in the benign disease group and control group (P<0 .05);t he concentrations of CEA and CA125 in the adenocarcinoma patients with were significantly higher than those in the patients with squamous cell carcinoma and small cell carcinoma (P<0 .05) .The concentration of SCC in the patients with squamous cell carcinoma was significantly higher than that in the patients with adenocarcinoma and small cell lung cancer (P<0 .05) .The sen-sitivity ,specificity ,positive predictive value and negative predictive value of combined detection of three tumor markers were signifi-cantly better than that of single tumor marker ,and the difference was statistically significant (P<0 .05) .Conclusion The combined detection of SCC ,CEA and CA125 can be used in the early diagnosis of lung cancer and has the promotion value .
10.Proteome analysis of nuclear matrix proteins during arsenic trioxide induced apoptosis in K562 cells

Objective: To investigate arsenic trioxide (As 2O 3) -target interactions at the level of nuclear matrix (NM) in chronic myelogenous leukemia cell line K562 by proteomics. Methods: DNA fragmentation analysis was used for As 2O 3 induced apoptosis of K562 cells. The nuclear matrix proteins were analyzed by high-resolution two-dimensional gel electrophoresis and computer-assisted image analysis. Results: While more than 200 protein spots were shared among the nuclear matrices, about 18 distinct spots were found characteristic of As 2O 3 treated cells. Onset of mass mange apoptosis, and the profiling of nuclear matrix proteins had been alternated and it was a more sensitive indicator than nucleosomal DNA fragmentation against As 2O 3 treatment. Conclusion: As 2O 3 induced apoptosis in K562 cells in a dose-time-dependent manner. As 2O 3 might be clinically useful in treatment of chronic myelogenous leukemia and the changes of nuclear matrix proteins in the treated cells can be used as a useful indicator for the treatment.
