1. Extracting coronary artery centerline based on improved skeleton thinning algorithm

Objective: To propose an automatic skeleton thinning algorithmfor accurate extracting the center line of coronary artery from CT images. Methods The region of coronary artery on CT images were segmented, and complete 3D data of coronary artery were obtained after 3D reconstruction. Then the center line of coronary artery was extracted using skeleton thinning algorithm. Dijkstra algorithm was introduced according to the characteristics of coronary artery to improve the accuracy of extraction. Results Compared with the skeleton thinning algorithm which didn't remove mini branches, the rate of overlap was increased by 2%, the average distance was reduced by 38.2%, and the average running time was 0.48 s. Conclusion Using improved skeleton thinning algorithm could effectively extract the center line of coronary arteries.
2.Value of intracranial pressure monitoring in minimally invasive surgery for hypertensive intracerebral hemorrhage

Objective To explore the value and advantage of intracranial pressure (ICP) monitoring in the treatment of hypertensive intracerebral hemorrhage (HICH) through minimally invasive surgery. Methods Seventy-three HICH cases were randomly selected and then divided into control group and treatment group. Thirty-four of them in control group received soft-channel minimally invasive hematoma removal. The head of the soft-channel was placed in the center of the hematoma. One third to half of the initial hematoma was extracted during the operation. Urokinase was injected into the soft channel to dissolve the hematoma. Thirty-nine of them in treatment group underwent the same operation procedure but with ICP monitoring to control aspirating hematoma during the operation. The target ICP was 15 mmHg. The aspiration of hematoma may stop once the ICP down to the target. Treatment such as urokinase injection was adjusted according to the value of ICP monitoring throughout the operation. The incidence of rebleeding, hematoma evacuation time and the average length of stay between two groups were compared. Results The results showed that one fifth of hematoma extracted was enough for a desired ICP. There found no rebleeding case in treatment group while 4 cases in control group and the difference had statistical significance. The hematoma evacuation time and the average length of stay between two groups had statistical significance. Conclusion Continuous ICP monitoring combined with directional software channel minimally invasive surgery changes the concept of empirically intraoperative hematoma evacuation and postoperative drug injection and proposes the idea of controlling drainage. It can shorten the course, provide sensitive and objective indicators and basis, reduce the secondary brain injury and improve prognosis.
3.Activation of mTOR signaling pathway in cancer stem cells of nasopharyngeal carcinoma and inhibitive effect of rapamycin against the cancer stem cells.

OBJECTIVE:
To study the mTOR expression of cancer stem cells(CSCs) in nasopharyngeal carcinoma and preliminarily explore the mechanism of inhibiting its proliferation with rapamycin.
METHOD:
Nasopharyngeal carcinoma spherical cells were gathered by using serum-free suspension culture method, CCK8 assay was used to detect cell proliferation, Western blot assay was used to detect the expression of CD44, OCT4, SOX2 and mTOR signaling. The spherical cells and CNE2 were treated with rapamycin in concentrations of 0, 0.1, 1.0, 10.0, 100.0, 1000.0 nmol/L, CCK8 assay was used to detect cell inhibition ratio, Western blot assay was used to detect the expression of mTOR signaling of nasopharyngeal carcinoma spherical cells.
RESULT:
Compared with CNE2, the spherical cells exhibited a high proliferation rate in RPMI 1640 medium supplemented with fetal bovine serum, and overexpressed in OCT4, SOX2 (P < 0.05), but not that of CD44 (P > 0.05). Although the expression levels of mTOR, P70S6, 4EBP1 were not significantly different between the two kinds of cells (P > 0.05) the proteins of phosphorylation activation form of them (P-mTOR, P-P70S6, P-4EBP1) were highly expressed in spherical cells (P < 0.05). The spherical cells and CNE2 were treated with rapamycin in different concentrations, the concentrations for 50% of maximal effect of spherical cells and CNE2 were 2.59 nmol/L and 78.12 nmol/L respectively, rapamycin inhibited the spherical cells more strongly compared with CNEZ. The expression levels of P-mTOR, P-70S6, P-4EBP1 in spherical cells were gradually decreased with increasing of the concentrations of rapamycin, but the difference of the expression levels of mTOR, P70S6, 4EBP1 were not significant.
CONCLUSION
The proteins of mTOR signaling pathway of CSCs in nasopharyngeal carcinoma are overexpressed, and rapamycin can effectively inhibit cell proliferation of CSCs in nasopharyngeal carcinoma by blocking mTOR signaling pathway.

4.The application of near-infrared spectroscopy in rehabilitation of stroke

Stroke is a cerebrovascular disease that damages brain tissue due to blockage or rupture of blood vessels, and is one of the leading causes of death and disability worldwide. Diagnosing, monitoring stroke and the treatment effects all rely on neuroimaging that detects changes in the brain. Near-infrared spectroscopy (NIRS) is a novel neuroimaging technique which is based on the non-invasively measuring of concentration changes of oxyhemoglobin and reduced hemoglobin. This review summarizes the underlying mechanisms of NIRS and its current clinical application in stroke, as well as the limitations of NIRS in broader clinical applications.

Objective To produce human papillomavirus type 6(HPV-6)virus-like particles with Escherichia coli expression system and study its immunogenicity.Methods HPV-6 L1 gene was inserted into pmkaryotic expression vector pTO-T7 and then expressed in Escherichia coli ER2566.The HPV-6 L1 protein was purified by ammonium sulfate precipitation,ion-exchange chromatography,and hydrophobic interaction chromatography.Then the purified HPV-6 L1 self-assembled into virus-like particle after removing 1,4dithiothreitol(DTr).The morphology of the virus-like particles was investigated with dynamic light scatter and transmission electron microscopy,and the immunogenicity was determined with in vitro pseudownons neutralization as8ay.Results HPV-6 L1 was expressed in soluble form in Escherichia coli.Following the removal of DTT,purified HPV-6 L1 protein could assemble into virus-like particles as 25 am in the radius.And the animal immunization test showed HPV-6 virus-like particles can elite hish titer neutralizing antibodies.Conclusion The bacterially expressed HPV-6 L1 VLP is highly immunogenieity and easy to produce.And it can be good candidate of HPV-6 vaccine.
6.AngioJet mechanical thrombectomy for lower deep venous thrombosis

Objective To evaluate percutaneous AngioJet thrombectomy in the treatment of iliofemoral deep venous thrombosis (DVT).Methods A total of 36 patients with 38 legs of lower extremity DVT treated by AngioJet mechanical thrombectomy from Feb 2016 to Dec 2016 were analyzed retrospectively.The effect of lower limb venous recanalization was evaluated by observing the intraoperativeangiography and the results of postoperative follow-up including complaints,signs,and lower extremity venous ultrasound or CT and Villalta scores.Results Thrombosis was completely dissolved by AngioJet thrombectomy device in 32 out of 38 legs (84.2%) at first stage.Slight bleeding occurred in 4 cases,1 patient could not tolerate the operation.23 patients were followed-up for 6 months,venous patency was present in 21 of 23 patients (91.3%).11 patients complete the one-year follow-up,9 patients (81.8%) were in the 0-4 Villalta group.Conclusions AngioJet can effectively and safely remove thrombus in the lower extremity deep venous system.It is especially advantageous for patients who have contraindications for thrombolysis with a satisfactory short term venous patency.

Immune imbalance is believed to play a dominant role in the pathogenesis of chronic urticaria.While Th1/Th2 imbalance used to be considered as the main contributing factor of the development of chronic urticaria.Recently,Th17/Treg imbalance is found to be an important immune mechanism leading to the development of chronic urticaria.To be more specific,according to traditional Chinese medicine(TCM)'s comprehensive understanding of the etiology of chronic urticaria.it is generally believed that the pathogenesis of chronic urticaria is due to a lack of innate endowment,a lack of solidity of the body surface,and repeated exposure to six pathogenic factors.Another possible reason lies with dietary disorders that generate heat and wind or chronic illness and weakness and loss of nourishment of qi and blood.Therefore,in terms of the treatment,from the perspective of sthenia syndrome,it is advisable to remove the wind and disperse the pathogenic factors and clear the dampness and heat.From the perspective of asthenia syndrome,it is advisable to nourish the qi and blood and support the righteousness.As for mixed excess and deficiency,both support the healthy atmosphere and dispel the pathogenic factors are important.Regarding the effects of TCM on the balance of Th17/Treg in chronic urticaria and immune diseases,it mainly involved herbal compounding,herbal active ingredients and single herbs.However,the research attention has been drawn to investigating the role of TCM in the treatment of chronic urticaria and various immune diseases based on the research outcomes in modern pharmacological research.This can not only provide scientific evidence for TCM treatment of chronic urticaria,but also bring benefits to more patients with immune diseases.Therefore,the author reviews the recent research progress of TCM on the effects of Th17/Treg immune imbalance in chronic urticaria and other immune diseases by explaining the effects of Th17 and Treg cells in chronic urticaria.
8.Analysis of the correlation of drug consumption index with DRG overspending and its threshold effect in a hospital

OBJECTIVE To explore the correlation between drug consumption index and diagnosis related groups (DRG) overspending cases, and provide a basis for hospitals to optimize the cost structure and strengthen the refined management. METHODS Based on the data of DRG patients enrolled in a third-grade class A hospital from September to November 2023, the multivariate Logistic regression model and restricted cubic spline (RCS) model were used to analyze the correlation of drug consumption index with DRG overspending cases and its threshold effect, respectively. At the same time, rational drug use evaluation was conducted based on the drug consumption index, precise cost control and management were carried out, and the changes in the main pharmaceutical indicators of the whole hospital were analyzed before control (January-June 2023) and after control (January-June 2024). RESULTS The results of multivariate Logistic regression analysis showed that long hospitalization days, high drug consumption index, transfer to other departments and combined diabetes mellitus were the risk factors for DRG overspending (P<0.05). The results of the RCS model showed that the drug consumption index had a non-linear relationship with DRG overspending. When the drug consumption index was ≥0.64, the drug consumption index was positively correlated with the risk of DRG overspending(P<0.05). Compared with the same period before the control, medical cost per time, drug cost per time and drug consumption index decreased significantly after the control (P<0.01). CONCLUSIONS The drug consumption index is a risk factor for DRG overruns, there is a non-linear relationship and threshold effect between it and DRG overruns. Each hospital can set a reasonable threshold and implement dynamic monitoring and intervention by comprehensively considering the actual drug usage, disease spectrum characteristics, and cost control targets, as well as factors such as medical quality, patient needs, and the payment capacity of medical insurance, which can effectively achieve precise control over drug usage.
9.Progress on fenestration during thoracic endovascular aortic repair.

Endovascular aortic arch repair remains challenging due to the complicated anatomy and the involved vital branches. Off-label techniques emerged during the last decade, including endografts with parallel stents (known as Chimney, Periscope, Sandwich), surgeon-modified endografts, and various other reported techniques. fenestration of standard endografts represents another off-label endovascular means to maintain perfusion to aortic side branches located in the excluded area. Its principle is based on fenestration of an endograft following its deployment inside the vascular system. As data are emerging regarding fenestration, the aim of this article is to review recent progress of technical descriptions, and clinic results of fenestration from the available literature.

10.Efficacy of integrated minimally invasive treatment for iliac vein compression syndrome with varicose veins of lower extremities.

OBJECTIVE:
To analyze the efficacy of integrated minimally invasive surgery for iliac vein compression syndrome with varicose veins of lower extremities.
METHODS:
From January 2017 to January 2018, 11 patients with iliac vein compression syndrome accompanied by varicose veins of lower extremities underwent left iliac vein stent implantation and radiofrequency thermal ablation of lower extremity veins in the First Affiliated Hospital of Zhejiang University School of Medicine. The left iliac vein stent was implanted through the puncture point approach of the main great saphenous vein, and then radiofrequency thermal ablation of the main saphenous vein was performed. Rivaroxaban and aspirin were administered from the day of surgery for 6 months and 12 months, respectively. After discharge, patients were followed up for more than 6 months. The lower extremity veins, iliac veins were reexamined by Doppler ultrasound or CT angiography at 2 weeks, 2 months and 6 months after surgery.
RESULTS:
The operations were successfully performed in 11 patients, and no complication was observed during the operation. The rates of soreness and swelling remission, pigmentation and skin quality improvement, and the iliac vein stent patency were 100%. No varicose vein recurrence, iliofemoral vein thrombosis and pulmonary embolism were found.
CONCLUSIONS
Integrated minimally invasive surgery is safe, effective and less invasive for iliac vein compression syndrome with varicose veins of lower extremities.
