1.Exploration on the Cultivation of Country Doctors' Professionalism

At present,there are the following problems in the country doctors' professionalism education,the lack of country doctors' understanding of physician professionalism,the impact of the market economy and the neg-lect of the current physician professionalism education.On the basis of this,this paper suggested to improve the so-cial status and treatment of country doctors,to strengthen the education and training of professionalism-related subjects,to embody the humanistic idea and criterion in the rich specialized course teaching,and to promote the implementation of country doctors' professionalism education and training.
2.Controversy and analysis of vertebral augmentation for osteoporotic vertebral compression fracture

Vertebral augmentation (VA) is widely used in the clinical treatment of osteoporotic vertebral compression fracture (OVCF), but its clinical efficacy has always been controversial. Since 2009, a total of four high-quality prospective randomized double-blind placebo-controlled studies have been published, but their conclusions on the effectiveness of VA were inconsistent. Latest Cochrane review (2018) and American society for bone and mineral research task report (2019) believed that compared with sham surgery, there is no advantage supporting the use of VA as a routine treatment for patients with OVCF, which triggered a new round of debates. The effectiveness of the VA remains to be determined, which makes it difficult to guide the actual clinical work of surgeons. This paper reviews the key studies in the controversy, sorts out and analyzes the details and latest views in the controversy, summarizes the impact of VA on the mortality rate of OVCF patients, and puts forward the optimal application mode of VA based on the current evidence.
3.Analysis of influencing factors for prolonged postoperative hospital stay after Da Vinci robot-assisted mediastinal tumor resection with non-endotracheal intubation and the process optimization

Objective To analyze the risk factors for postoperative length of stay (PLOS) after mediastinal tumor resection by robot-assisted non-endotracheal intubation and to optimize the perioperative process. Methods The clinical data of patients who underwent Da Vinci robot-assisted mediastinal tumor resection with non-endotracheal intubation at the Department of Thoracic Surgery, General Hospital of Northern Theater Command from 2016 to 2019 were retrospectively analyzed. According to the median PLOS, the patients were divided into two groups. The univariate analysis and multivariate logistic regression were used to analyze risk factors for prolonged PLOS (longer than median PLOS). Results A total of 190 patients were enrolled, including 92 males and 98 females with a median age of 51.5 (41.0, 59.0) years. The median PLOS of all patients was 3.0 (2.0, 4.0) d. There were 71 patients in the PLOS>3 d group and 119 patients in the PLOS≤3 d group. Multivariate logistic regression showed that indwelled thoracic catheter [OR=11.852, 95%CI (2.384, 58.912), P=0.003], preoperative symptoms of muscle weakness [OR=4.814, 95%CI (1.337, 17.337), P=0.016] and postoperative visual analogue scale>5 points [OR=6.696, 95%CI (3.033, 14.783), P<0.001] were independent factors for prolonged PLOS. Totally no tube (TNT) allowed patients to be discharged on the first day after surgery. Conclusion Robot-assisted mediastinal tumor resection with non-endotracheal intubation can promote rapid recovery. The methods of optimizing perioperative process are TNT, controlling muscle weakness symptoms and postoperative pain relief.
4.Parkin promotes proteasomal degradation of p62: implication of selective vulnerability of neuronal cells in the pathogenesis of Parkinson's disease.

Mutations or inactivation of parkin, an E3 ubiquitin ligase, are associated with familial form or sporadic Parkinson's disease (PD), respectively, which manifested with the selective vulnerability of neuronal cells in substantia nigra (SN) and striatum (STR) regions. However, the underlying molecular mechanism linking parkin with the etiology of PD remains elusive. Here we report that p62, a critical regulator for protein quality control, inclusion body formation, selective autophagy and diverse signaling pathways, is a new substrate of parkin. P62 levels were increased in the SN and STR regions, but not in other brain regions in parkin knockout mice. Parkin directly interacts with and ubiquitinates p62 at the K13 to promote proteasomal degradation of p62 even in the absence of ATG5. Pathogenic mutations, knockdown of parkin or mutation of p62 at K13 prevented the degradation of p62. We further showed that parkin deficiency mice have pronounced loss of tyrosine hydroxylase positive neurons and have worse performance in motor test when treated with 6-hydroxydopamine hydrochloride in aged mice. These results suggest that, in addition to their critical role in regulating autophagy, p62 are subjected to parkin mediated proteasomal degradation and implicate that the dysregulation of parkin/p62 axis may involve in the selective vulnerability of neuronal cells during the onset of PD pathogenesis.
