AIM:To observe the transplantation of acellular porcine corneal stroma on the treatment of superficial keratitis by drug-resistant fungal.METHODS:We performed a retrospective analysis of 16 cases of fungal keratitis received the transplantation of acellular porcine corneal matrix from June 2015 to March 2016 with a follow-up of 6mo.We analyzed on items as postoperative visual acuity, corneal graft status, postoperative recurrence and postoperative complications.RESULTS:We observed a healing time of corneal epithelium in 7 to 10d postoperatively generally and the absence of corneal edema in 1mo, while the cornea gradually returned transparent in the 16 cases.Two cases required medication for an epithelial recovery and 3 cases received intervention for decreasing intraocular pressure to a certain level.During the follow-up we observed no cases of cornea degeneration, recurrence of infection or rejection.The vision acuity showed 1.27±0.22, 1.11±0.13, 0.79±0.22 in 1, 3 and 6mo after operation respectively.There was no statistical difference between vision in 1mo and the vision before surgery (P=0.06);while we found a statistical difference when comparing the vision of 3 and 6mo with vision before surgery (P=0.01,0.001).The vision in 6mo increased with a statistic difference to the vision at 1 and 3mo (P<0.001) while no statistic difference was observed between 1 and 3mo(P=0.11).CONCLUSION:Transplantation of acellular porcine corneal matrix is a safe and efficient treatment for fungal keratitis.

AlM:To discuss the protective effect ofα-mangostin on retinal light damage in mice.METHODS:Totally 30 Balb/c mice, aged 6~8wk, were randomly divided into the control group, light-exposure group and α-mangostin group. Every group contained 10 mice. Mice of α-mangostin group were treated with alpha-mangostin at the dose of 30mg/( kg · d ) body weight by intragastric administration daily for 7d, and then exposed to white light at the 5th d. The light-exposure group and α-mangostin group were exposed to 5 000 ± 200lx white light-emmiting diodes (LEDs) for continuously 1h to establish the mice model of retinal light damage. Flash -electroretinograme was recorded 72h after light exposure. The changes in retinal morphology of mice were observed by light microscopy. Retinas were extracted to detect the malondialdhyde ( MDA ) content change of the retinal homogenate.RESULTS: Flash-electroretinogram ( F-ERG ) showed that retinal dysfunction was less severe in α-mangostin group than in light-exposure group ( P<0. 05 ). Light microscopy test showed that retina structural damage was less severe in α-mangostin group than in light-exposure group (P<0. 05). The level of MDA in retinal tissue of α-mangostin group was significantly lower when compared with light-exposure group (P<0. 05).CONCLUSlON: α-mangostin inhibits lipid peroxidation induced by light damage and protect retina against light damage.

To construct a rabies virus mutant, the psi region was replaced by the coding region of human cytochrome c gene, and the coding region for cytoplasmic domain of glycoprotein G was deleted in the full-length of genomic cDNA of rabies virus strain SRV9. The mutant plasmid and the plasmids with N, P, L and G structural proteins of wild type SRV9 were co-transfected into BHK-21 cells. It was shown by IFA that there were many specific fluorescence in the BHK-21 cells, and typical rabies virus virions were observed by electronic microscope. These results demonstrated that the mutant rabies virus was successfully rescued. The genetically modified SRV9 stain has promise to provide invaluable experimental tool to develop attenuated live rabies vaccine.

OBJECTIVETo explore the regional anatomy of the rectum including the perirectal fasciae and spaces.

METHODSTwenty-one cadavers (15 males and 6 females) were embalmed and their vessels were visualized by injection with color dye. From the cadavers, 30 hemipelvis and 6 three-quarter pelvis were harvested. The perirectal fasciae and spaces and the pelvic autonomic nerves were dissected and examined.

RESULTSThree tissue layers were dissected from the inside to the periphery including the proper rectal fascia enveloping the mesorectum, the presacral fascia, and the piriformis fascia fused with the sacral periosteum. The mesorectum comprised 2 parts with the classical posterolateral fat covered by the proper rectal fascia posteriorly and the anterior fat covered by the posterior layer of Denonvilliers fascia anteriorly. Extending anteriorly to the anterior layer of Denonvilliers fascia, the presacral fascia bisected the space between the mesorectum and the piriformis fascia into the retrorectal space and the presacral space. The retrorectal space extended cranially to the left retrocolic space, anterior to the space between the 2 layers of Denonvilliers fascia(prerectal space).

CONCLUSIONSFrom the inside to the periphery, the proper rectal fascia, the presacral fascia, and the muscular fascia are distributed in an annular pattern around the mesorectum. The presacral fascia divides the perirectal space into 2 annular parts, the central retrorectal space and the peripheral presacral space. The retrorectal space is the ideal surgical plane for total mesorectal excision.

Objective To compare the effects of 20 mg qd and 10 mg bidadministration of iprrazole enteric-coated tablets on the control of gastric acid in healthy subjects.Methods A randomized,single-center,parallel controlled trial was designed to include 8 healthy subjects.Randomly divided into 2 groups,20 mg qd administration group:20 mg enteric-coated tablets of iprrazole in the morning;10 mg bid administration group:10 mg enteric-coated tablets of iprrazole in the morning and 10 mg in the evening.The pH values in the stomach of the subjects before and 24 h after administration were monitored by pH meter.The plasma concentration of iprazole after administration was determined by HPLC-MS/MS.The main pharmacokinetic parameters were calculated by Phoenix WinNonlin(V8.0)software.Results The PK parameters of iprrazole enteric-coated tablets and reference preparations in fasting group were as follows:The Cmax of 20 mg qd group and 10 mg bid group were(595.75±131.15)and(283.50±96.98)ng·mL-1;AUC0-t were(5 531.94±784.35)and(4 686.67±898.23)h·ng·mL-1;AUC0-∞ were(6 003.19±538.59)and(7 361.48±1 816.77)h·ng·mL-1,respectively.The mean time percentage of gastric pH＞3 after 20 mg qd and 10 mg bid were 82.64％and 61.92％,and the median gastric pH within 24 h were 6.25±1.49 and 3.53±2.05,respectively.The mean gastric pH values within 24 h were 5.71±1.36 and 4.23±1.45,respectively.The correlation analysis of pharmacokinetic/pharmacodynamics showed that there was no significant correlation between the peak concentration of drug in plasma and the inhibitory effect of acid.Conclusion Compared with the 20 mg qd group and the 10 mg bid group,the acid inhibition effect is better,the administration times are less,and the safety of the two administration regimes is good.

OBJECTIVESTo investigate possible abnormalities of the sperm neck in eight infertile painters observed by transmission electron microscope.

METHODSSpermatozoa from 8 infertile painters (21-32 years old) were observed by transmission electron microscope.

RESULTSThe abnormalities of the connecting piece could be divided into 6 types, among which the anomaly of the connecting piece concomitant with a true nuclemalamic condition was the most serious.

CONCLUSIONSThe abnormalities may be related to expression obstacles of nuclear genome and mitochondrial genome, and to anomalous cellular signal transduction during spermiogenesis as well.

OBJECTIVETo investigate the clinical efficacy, surgical indications and postoperative complications of mid urethral sling procedures in treatment of female stress urinary incontinence.

METHODSA multicenter clinical trial was conducted from April 2002 to April 2008 in five hospitals, 304 cases of genuine stress urinary incontinence and 8 cases of mixed incontinence were included. TVT procedures were carried out in 134 patients, TVTO procedures in 167 patients, Monarc procedures in 11 patients. Perioperative evaluations included: operating time, bleeding volume, and perioperative complications. Operative efficacy was classified into three categories: cure, improved and failure and evaluated before discharge, 3 months after surgery and then every year.

RESULTSTVT group had longer operating time [(18.5 + or - 9.6) min] and more bleeding volume [(32.2 + or - 12.6) ml] than those in TVTO group [(11.5 + or - 3.1) min, (12.8 + or - 8.5) ml] and in Monarc group [(11.1 + or - 2.6) min, (12.3 + or - 3.5) ml] with P < 0.05. Monarc and TVTO procedures had higher cure rates and improve rates comparing with TVT, but the differences were of no significance. The cure rate (95.7%) in patients with genuine stress incontinence were significantly higher than that in patients with mixed incontinence (37.5%). No significant differences of total intra- and postoperative complications were noted for all of the three procedures. However, bladder injury tended to occur in TVT group and obturator nerve injury and vaginal injury tended to occur in TVTO group. Transient voiding dysfunction and urinary retention were the most common complications.

CONCLUSIONSMid urethral sling procedures have excellent clinical outcomes in the treatment of female stress urinary incontinence.

OBJECTIVETo assess the association between polymorphisms within the interferon-Gamma receptor gene(IFN-GammaR) and systemic lupus erythematosus (SLE) in Chinese people.

METHODSThe IFN-GammaR genotypes of 94 SLE patients and 80 healthy subjects were examined by the reverse transcription-polymerase chain reaction (RT-PCR)-single-strand conformation polymorphism (SSCP) method, RT-PCR-restriction fragment length polymorphism (RFLP) method and DNA sequencing.

RESULTSThere was no statistically significant difference in the IFN-GammaR genotype frequencies between the two groups. The IFN-Gamma R2 Arg64/Arg64 genotype decreased the risk of SLE (OR = 2.481, 95% CI 0.992 - 6.203, P = 0.047). The decreased risk of the development of SLE was detected in the individuals who had the combination of IFN-GammaR2 Arg64/Arg64 genotype and IFN-GammaR1 Val14/Val14 genotype (OR = 2.481, 95% CI 0.992 - 6.203, P = 0.047).

CONCLUSIONThe IFN-GammaR2 Arg64/Arg64 genotype does not determine susceptibility to SLE in Chinese people, and the combination of IFN-Gamma R2 Arg64/Arg64 genotype and IFN-Gamma R1 Val14/Val14 genotype does not, either.

Objective To determine the prevalence rates and risk factors of hyperuricemia(HUA) and gout among residents aged over 20 years in Foshan areas. Methods A randomly stratified cluster sampling was conducted, and 7403 inhabitants were investigated on their prevalence rates of HUA and gout. Results (1) The prevalence of HUA was 15.09%, and the standardized rate was 15.27%, in which the prevalence in males was 19.90% and females was 10.54%. The prevalence of gout was 1.04% and the standardized rate was 1.08%,in which the prevalence in males was 1.73%and females was 0.39%. The prevalence of gout in patients with HUA was 6.89%. (2) Average serum uric acid was (336.4 ± 81.5) μmol/L, with (347.1 ± 88.6) μmol/L in males and (289.7±78.6) μmol/Lin females. The serum uric acid levels in male patients with HUA was higher than those in women.(3) Age, body mass index, systolic blood pressure, diastolic blood pressure, serum uric acid, blood sugar, triglyceride (TG), total cholesterol were significantly higher in patients with HUA and gout than in the normal group (P＜0.05-0.01). The incidence rates of patients with hyperuricemia and gout in the following indices as: overweight and obesity, high blood pressure, high blood sugar were significantly higher than those in the normal group (P＜0.05). Patients having gout in the following indices as age, TG, serum uric acid levels were significantly higher than the HUA group (P＜0.05).(4) Data from non-conditional logistic regression analysis showed that age, overweight, hypertension,diabetes, hyperlipidemia, use of diuretics, family history, alcohol uptake, eating seafood and drinking meat broth, post-menopausal women, and other factors were similar to those factors as patients with hyperuicemia. Tea, fresh vegetables, fruits seemed to be the protective factors. Conclusion Both the prevalence rates of HUA and gout had significantly increased in Foshan areas in recent years.Restricting the intake of food with rich purine, alcohol intake as well as controlling obesity and blood pressure, improving the status of lipid metabolic disorder together with programs as hypertension control etc. were important measures in the strategies on prevention and treatment on hyperuricemia and gout.

·AIM:To explore the effectiveness of a new non-inverted pedicle internal limiting membrane ( ILM ) flap transposition technique in the treatment of large macular holes (MH). ·METHODS: This was a prospective pilot study which included 9 patients with 10 eyes in Jiangsu Province People's Hospital from December 2016 to February 2017. All patients was diagnosed with large MH (size >400μ m) by the spectra- domain optical coherence tomography (SD-OCT) and underwent the non-inverted pedicle ILM flap transposition surgery. Best-corrected visual acuity (BCVA), SD-OCT images, and MP-1 microperimetry tests were performed pre-operation, 1d, 1wk, 1, 3, and 6mo post-operation. ·RESULTS:The macular hole closure rate after 6mo was 100%. The averaged BCVA improved from 1. 19 ± 0. 54 (LogMAR) pre-operation to 0.70 ± 0.50 (LogMAR) post-operation (P=0.005). The mean retinal sensitivity within 8° and 2° improved from 3.14±4.52dB to 8.91±5.53dB(P=0.008), and 1.46 ± 2.94dB to 6.33 ± 4.90dB (P=0.008) respectively. Preoperative unstable fixation in seven eyes resolved at the last postoperative follow-up.·CONCLUSION: Our non-inverted pedicle internal ILM flap transposition technique shows effectiveness in the treatment of large macular holes with high MH closure rate and improving visual function.
