1.Efficacy of Enteric-coated Capsule Containing Four Viable Organisms (Bafidobacterium,Lactobacillus,Enterococcus and Bacilluscereus) for Chronic Diarrhea

OBJECTIVE: To validate the efficacy and safety of the enteric-coated capsule of Four Viable Organisms(bafidobacterium,lactobacillus,enterococcus and bacilluscereus) in the treatment of chronic diarrhea.METHODS: In this multi-center,randomized,blind control study,124 patients(trial group) with chronic diarrhea were randomly assigned to receive the enteric-coated capsule containing bacillus bifidus,lactobacillus,enterococcus and bacilluscereus,while the control group(n=80) to receive the capsule of bacillus bifidus,bacillus acidophilus and enterococcus faecalis for 14 days.The days spent for the relief of diarrhea relief and the concomitant symptoms of diarrhea such as pain,bloating,and tenesmus were observed.RESULTS: At 3 days,the remission rate in trial group(41.13%) was higher than in control group(11.25%)(P
2.Relationship between different types of acute ischemic stroke in elderly patients with serum OX-LDL,blood sugar, CRP amd IL-6

Objective To investigate the relationship between different types of acute ischemic stroke in elderly patients and serum oxidized low-density lipoprotein (OX-LDL),blood sugar,C-reactive protein(CRP) and interleukin 6 (IL-6) levels.Methods 100 patients with acute cerebral infarction (group A) and 30 heathy patients (group B)were investigated.The levels of serum OX-LDL,blood sugar,CRP and IL-6 were detected by automatic biochemical analysis.Results The levels of OX-LDL(627.2 ± 174.7 ) μg/L,blood sugar ( 8.8 ± 1.7 ) mmol/L,CRP ( 107.5 ±19.4)mg/L and IL-6(79.5 ± 12.1 )ng/L in patients of group A were significantly higher than group B[ OX-LDL(506.1 ± 152.4) μg/L,blood sugar(6.9 ± 1.5) mmol/L,CRP(87.2 ± 13.6) mg/L and IL-6(58.5 ± 10.3) ng/L](t=2.74,3.18,2.61,2.91,allP<0.05).Conclusion Elderly patients with acute cerebral infarction were closely related to the elevated levels of plasma OX-LDL,blood sugar,CRP and IL-6.Increased plasma OX-LDL was a risk factor for the main artery atherosclerosis type,and hyperglycemia was primary factor for the small artery atherosclerosis type of acute cerebral infarction.
3.The clinical recent effects of two neoadjuvant chemotherapies in the treatment of advanced gastric cancer

Objective To observe the recent effects and toxicity in advanced gastric carcinoma by two kinds of neoadjuvant chemotherapies ( PF and FOLFOX).Methods A total of 60 patients with advanced gastric cancer ( Ⅲ ~ Ⅳ ),they were randomly divided into 2 groups; PF group ( 30 cases) and FOLFOX group ( 30 cases).All patients were given 3 cycles of docetaxel or L-OHP chemotherapy preoperatively,to observe the recent effects and toxicity in advanced gastric carcinoma.Results The total effective rate in PF group was 77.67%,while it was 50.00% in FOLFOX group,it had significant difference between in two groups ( x2 =4.59,P < 0.05 ).There is no difference in the incidence of neutropenia,transaminase elevation,nausea and diarrhea two groups.The incidence of hair loss in PF group is higher than in FOLFOX group,but there is no difference between two groups; The incidence of peripheral nerve paresthesia in FOLFOX group is higher than that in PF group( x2 =5.08,P <0.05).Conclusion PF as neoadjuvant chemotherapy is a very effective and well-tolerated regimen in patients with advanced gastric cancer.
4.The use of short dental implants in rigid fixed bridge for multiple missing teeth:a 2-year follow-up visit

BACKGROUND: Short dental implants have been widely used in the posterior area when one tooth loses. How effective is it when we use the short implants in the rigid fixed bridge for the multiple teeth lost area?OBJECTIVE: To observe the clinical effect of short dental implants used in the rigid fixed bridge.METHODS: Forty-three patients with multiple missing teeth were enrolled and underwent the rigid fixed bridge supported by short dental implants, including 26 males and 17 females, with an age range of 44-67 years. After 2 years of dental implantation, the success rate of dental implants, amount of alveolar bone loss and dental implant-related complication were followed up.RESULTS AND CONCLUSION: A total of 92 short dental implants, 6-8 mm in length, were implanted,including 44 in the maxilla and 48 in the mandible. There were two cases of peri-implantitis and the success rate of implants was 97.8%. There was no significant difference in the amount of alveolar bone loss between the 8 mm, 7 mm and 6 mm length implants as well as between the C/I ≤ 1, 1 < C/I ≤2, C/I > 2 groups at 1-2 years after implantation. There were four cases of central screw loose and one case of porcelain fracture after restoration. To conclude, the clinical use of short dental implants in the rigid fixed bridge has achieved good outcomes during the short period, but long-term observation is needed.
5.Epidemiological investigation on an epidemic of methicillin-resistant Staphylococcus aureus infection

Objective To investigate transmission routes of healthcare-associated infection(HAI)caused by methi-cillin-resistant Staphylococcus aureus (MRSA),and make effective measures for preventing and controlling the oc-currence and epidemic of HAI caused by multidrug-resistance bacteria.Methods From February 24 to March 29, 2012,12 MRSA-infected patients were performed epidemiological study,these patients underwent bronchoscopy be-cause of tracheal stenosis,strains were identified by amplifying the sequences of 16S rRNA ,femA and mecA with real-time quantitative polymerase chain reaction (PCR),homology analysis of strains were performed by Spa geno-typing.Results All 12 MRSA-infected patients were susceptible to multidrug-resistance bacterial infection,5 cases of MRSA infection occurred during this hospitalization.Detection of specimens from health care workers and envi-ronment were all negative;Spa gene of all 12 MRSA isolates was type t 030 ,which was the main epidemic strain in Asia;Spa gene of Staphylococcus aureus isolated from nurses’noses was type t1425 .Conclusion The assumption of MRSA spread among health care workers aren’t supported by the epidemiological investigation results,genotypes of 12 MRSA isolates are identical,but the result of gene typing can’t be as the evidence of homology of infection ;patients at high risk for MRSA infection should be screened as early as possible,early contact isolation should be performed,so as to prevent and control the occurrence of HAI.

AIM: To purify the water-extract of Sinisa recipe with macoroporous resigns. METHODS: Screening the purification condition by means of the content of active principle, fingerprint chromatography, the yield of dry extracts,et al. of the purified product. RESULTS: The purified product by type HP20 macoroporous resign is in good agreement with the active fraction of Sinisa in the composition of specific principles. CONCLUSION: HP20 type macoroporos resign is considered as optimum for active fraction of Sinisa recipe in purifying efficience.

Objective To study the anti-inflammatory,antitussive,and analgesia effects of Jinhua qingre capsules.Methods The anti-inflammatory effect was assessed by the methods include xylene-induced mouse auricular swelling and histamine-induced pigment oozing from skin vessel in rats;The antitussive and analgesic effect were assessed by ammonia water induced cough model and acetic acid-induced twisting method.Results In anti-inflammation experiment,the high dose (12 g·kg-1) and moderate (6 g·kg-1) groups of Jinhua qingre capsules showed significant inhibitory effect on auricular swelling and significant difference compared with control group (P < 0.01,P < 0.05.);the high dose (10 g· kg-1) and moderate dose (5 g·kg-1) groups of Jinhua Qingre capsules play a marked inhibitory role in the increase in mouse peritoneal capillary permeability (P < 0.01,P < 0.05).In antitussive experiment,high dose (12 g· kg-1) and moderate dose (6 g· kg-1) groups had significant inhibitory effect on cough caused by ammonia water (P < 0.01,P < 0.05) compared with control group.In analgesic experiment,the high dose (12 g·kg-1),moderate dose (6 g·kg-1),and low dose (3 g·kg-1) groups effectively reduced the writhing frequency of mice (P < 0.01,P < 0.05).Conclusion Jinhua qingre capsules have potential effects on anti-inflammatory,antitussive,and analgesic.
8.Effect of early combined functional exercise on lower limb rehabilitation of vessel bridge taken in coronary artery bypass surgery

Objective To investigate the effect of early combined functional exercise on lower limb rehabilitation of vessel bridge taken in coronary artery bypass surgery. Methods From January to October 2017,90 patients undergoing coronary artery bypass surgery were randomly divided into control group and experimental group by coin toss, with 45 patients in each group. The control group was given routine nursing care, and the experimental group was given early combined functional exercise (foot dorsiflexion, ring rotation exercise and straight leg raising exercise) on the basis of routine nursing care. The difference of correlation index between two groups was compared. Results In the experimental group, the number of swelling in the thigh, crus and ankle were 20, 25 and 35 respectively; the control group was 32, 35, 43, the difference was statistically significant(χ2=6.55, 5.00, 8.38, P<0.05). The remission of limb swelling in the experimental group was also better than that in the control group(P<0.01). The incidence of limb wound complications: 6 cases in the experimental group, 18 cases in the control group(χ2=11.79,P<0.01));limb muscle strength grade Lovett:experimental group 0 case with grade 3,19 cases with grade 4,26 cases with grade 5,control group 9 cases with grade 3,26 cases with grade 4,10 cases with grade 5, there were significant differences between two groups (χ2=17.20, P<0.05). The limb comfort score,postoperative bed time,postoperative hospital stay was(7.11±1.15)points,(5.28±1.07)d, (11.04±2.01)d in the experimental group and(5.18±1.13)points,(6.97±1.21)d,(12.67±1.59)d in the control group,the differences were significant(t=-8.02,6.97,4.23,P<0.01). Conclusions To carry out early functional exercise can not only take angiogenesis effectively reduce limb swelling rate after coronary artery bypass grafting,relieve the severity of limb swelling,but also can reduce the incidence of incision complications,promote the recovery of limb muscle strength,shorten the postoperative recovery time and postoperative hospitalization time,is worthy of promotion and use in clinical nursing work.
9.Brain metastasis in children with stage 4 neuroblastoma after multidisciplinary treatment.

INTRODUCTIONBrain metastasis is common in relapsed neuroblastoma patients, but the characteristics of brain metastasis remain largely unknown. This study aimed to investigate the status of brain metastasis with neuroblastoma in South China.
METHODSIn this retrospective case-based study, 106 patients with stage 4 neuroblastoma from the Department of Pediatric Oncology in Sun Yat-sen University Cancer Center between January 2004 and May 2013 were included. The incidence, risk factors, and survival status of these patients were reviewed and analyzed.
RESULTSOf the 106 patients, 11 (10.4%) developed brain metastasis, accounting for 20.0% of 55 patients with relapse or progression. The age at initial diagnosis of the 11 patients ranged from 2 to 10 years (median 4 years), which was younger than that of the patients without brain metastasis (median 5 years, range 1-10 years, P=0.073). The male to female ratio of the 11 patients was 8:3, which was not significantly different from that of the patients without brain metastasis (P=0.86). Patients with brain metastasis had higher lactate dehydrogenase levels than those without brain metastasis, but the differences were not significant (P=0.076). Eight patients died, and 3 patients survived. The median interval from the initial diagnosis to the development of brain metastasis was 18 months (range 6-32 months). The median survival was 4 months (range 1 day to 29 months) after the diagnosis of brain metastasis. The median interval from the manifestation of brain metastasis to death was 3 months (range 1 day to 11 months).
CONCLUSIONSHigh-risk factors for brain metastasis in cases of neuroblastoma include bone marrow involvement and a younger age at initial diagnosis. Nevertheless, multiple treatment modalities can improve disease-free survival.

10.Clinical factors associated with composition of lung microbiota and important taxa predicting clinical prognosis in patients with severe community-acquired pneumonia.

Few studies have described the key features and prognostic roles of lung microbiota in patients with severe community-acquired pneumonia (SCAP). We prospectively enrolled consecutive SCAP patients admitted to ICU. Bronchoscopy was performed at bedside within 48 h of ICU admission, and 16S rRNA gene sequencing was applied to the collected bronchoalveolar lavage fluid. The primary outcome was clinical improvements defined as a decrease of 2 categories and above on a 7-category ordinal scale within 14 days following bronchoscopy. Sixty-seven patients were included. Multivariable permutational multivariate analysis of variance found that positive bacteria lab test results had the strongest independent association with lung microbiota (R2 = 0.033; P = 0.018), followed by acute kidney injury (AKI; R2 = 0.032; P = 0.011) and plasma MIP-1β level (R2 = 0.027; P = 0.044). Random forest identified that the families Prevotellaceae, Moraxellaceae, and Staphylococcaceae were the biomarkers related to the positive bacteria lab test results. Multivariable Cox regression showed that the increase in α-diversity and the abundance of the families Prevotellaceae and Actinomycetaceae were associated with clinical improvements. The positive bacteria lab test results, AKI, and plasma MIP-1β level were associated with patients' lung microbiota composition on ICU admission. The families Prevotellaceae and Actinomycetaceae on admission predicted clinical improvements.
