1.Clinical research on IL-2,IFN-γ,IL-10 and LDH in patients with acute leukemia

Objective To study the levels of interleukin(IL)-2 ,interferon-γ(IFN-γ) and IL-10 in peripheral blood and the effec-tive concentration of lactate dehydrogenase(LDH) in serum of patients with acute leukemia .Methods 65 patients with acute leuke-mia were divided into non-remission group(n=35) and remission group(n=30) .50 healthy volunteers were used as control group . The levels of IL-2 ,IFN-γ ,IL-10 in peripheral blood were measured by enzyme linked immunosorbent assay(ELISA) .The effective concentration of LDH was measured by automatic biochemical analyzer .Results The levels of IL-2 ,IFN-γin non-remission group were significantly decreased compared with control group and remission group(P<0 .05) ,but the level of IL-10 in non-remission group was significantly increased compared with control group and remission group(P<0 .05) .The effective concentration of LDH in non-remission group was significantly higher than that in control group or remission group(P<0 .05) .Conclusion The levels of IL-2 ,IFN-γand IL-10 in peripheral blood and the effective concentration of LDH in serum of patients with acute leukemia have changed .Clinical detection of IL-2 ,IFN-γ,IL-10 and LDH may contribute to the early diagnosis of patients with acute leukemia .
2.The diagnostic threshold value and influencing factors of high-sensitivity troponin I measurement in the diagnosis of patients with acute myocardial infarction

Objective To investigate the diagnostic threshold value and the influencing factors of high‐sensitivity troponin I measurement in the diagnosis of patients with acute myocardial infarction .Methods Collecting 9 236 patients who are the first time hs‐cTnI testing in the internal emergency department of our hospital(including 9 099 patients with non‐AMI and 137 patients with AMI) as participants ,then dividing non‐AMI patients into elder group(>60 years old) and younger group(≤60 years old) ,hs‐cTnI concentrations were compared between the elder and the younger group with different gender .The optimal threshold value of hs‐cT‐nI for acute myocardial infarction was determined by receiver operating characteristic(ROC) curve of AMI patients with different age and gender .Results The hs‐cTnI concentrations of non‐AMI patients in the elder male group were 39 .21 ng/L [(37 .48-40 .93)ng/L] ,which was higher than those of non‐AMI patients in the younger male group 22 .38 ng/L[(21 .16-23 .6)ng/L] and the difference was statistically significant(P<0 .05) ,the same result was seen in the elder female group and the younger female group .The total male group was 31 .26 ng/L [(30 .17 -32 .34)ng/L] ,it was higher than the total female group 26 .8 ng/L [(25 .73-27 .86)ng/L] .According to the ROC curve ,the optimal threshold value for AMI diagnosis was 45 ng/L in total AMI group and 135 ng/L in the elder male group and 45 ng/L in the elder female group ,which was different with the cut off value(120 ng/L) .Conclusion the hs‐cTnI levels were related with age and gender ,it is very important for us to consider these factors when we make the optimal threshold value with our own data .

BACKGROUND: Amniotic fluid cells have been widely used in antenatal diagnosis for gene mutation-related diseases. However,there are few reports concerning isolation, culture, surface character identification, differentiation and application perspective of fetal mesenchymal stem cells (MSCs) derived from amniotic fluid.OBJECTIVE: To investigate the differentiation of fetal mesenchymal stem cells (MSCs) derived from second-trimester amniotic fluid into osteoblasts in vitro.DESIGN, TIME AND SETTING: The cytological/n v/tro study was conducted at the Experimental Center of Xinhua Hospital from August 2005 to May 2006.MATERIALS: Ten amniotic fluid samples were obtained from pregnant women (18-22 weeks after conception) or aborted women,The informed consents were obtained from pregnant women.METHODS: Fetal MSCs were separated mechanically from amniocyte culture system and expanded in medium in vitro. At passage 3, fetal MSCs were induced in 100 nmol/L dexamethasone, 10 mmol/L β-glycerophosphoric acid and 50 mg/L vitamin C for 14 days.MAIN OUTCOME MEASURES: Collagen Ⅰ and alkaline phosphatase expression was detected by immunohistochemistry.Collagen Ⅰ protein expression was determined by Western Blot analysis. Calcium tuberoses were measured by Von Kossa staining. The cytoskeletal protein was detected by laser confocal microscopy.RESULTS: The isolated fetal MSCs were uniformly positive for CD44 and HLA-ABC, negative for CD34, CD45 and HLA-DR. After being induced with osteogenic medium for 14 days, 91% cells were positive for alkaline phosphatase, and 87% cells for collagen Ⅰ.Cells expressed collagen Ⅰ protein. Number of calcium tuberoses was increased and became big over time. Cytoplasm microfilament presented green fluorescence and the microfilament surrounding cells formed dense bundle.CONCLUSION: Fetal MSCs derived from amniotic fluid could be induced into osteeblasta and displayed a typical osteoblastic morphology and biological characteristic.
5.Early remedy for bulk trauma patients in China Wenchuan earthquake

Objective To explore methods of easy treatment of bulk trauma patients caused by Wenchuan earthquake.Methods Early treatment was done on 1123 patients including extremity injuries in 925,spinal injuries in 65,brain injuries in 46,thoracic injuries in 53 and abdominal injuries in 72.There were 809 patients with single part injury and 314 with multiple injury.Emergent operation was performed in 67 patients including 13 with liver repair,14 with spleenectomy,three with mesentery blood vessel repair,six with repair of multiple abdominal organ injuries,four with repair of raptured diaphragm and injured organs,12 with evacuation of intracranial hematoma,four with lung repair,five with partial resection of lung and bronchus,one with pericardium discission and five with internal fixation of ribs.Early operation was carried out in 166 patients including 105 with only debridement,36 with debridement plus external fixation and 25 with amputation.Results Of all,406 patients were cured,673 were transferred to higher level hospitals after fundamental treatment and three died.Conclusion Staged treatment is suitable for early management for bulk trauma patients.Damage control technique should be applied early for critically severe patients and those with severe disease of heart and lung.Repeated examination is efficient to avoid delayed diagnosis and treament.
6.CT Value in Examining Intestinal Tumor Obstruction

Objective To analyze the CT features of intestinal tumor obstruction, and explore its CT value.Methods CT manifestation and clinical materials of intestinal tumor obstruction proved by surgical findings or endoscopy biopsy in 35 cases were analyzed restrospectively, and compared with the results of surgery-pathology and endoscopy. Results CT findings were consistence with the results of surgery-pathology and endoscopy in 33 of 35 cases. The CT diagnostic accuracy of intestinal obstruction was 100%. And the accuracy of the cause was 94%, including colon carcinoma in 25, lyphoma in 5, gastrointestinal stromal tumor in 2, and lipoma in 3.Conclusion CT has unique advantage in examining intestinal tumor obstruction, not only for definiting the existence of the obstruction, but also locating the site of obstruction diagnosing the cause and chosing the appropriate treatment.
7.Application of Multi-slice Spiral CT Three Dimensional Reconstruction in Diagnosis of Mechanical Intestinal Obstruction

Objective To investigate the diagnostic Value of multi-slice spiral CT 3-Dimensional reconstruction in intestinal obstruction.Methods 28 Patients with malignant lesions of colon underwent volume scanning using multislice helical CT.Four types of reconstruction included multiple planner reconstruction(MPR),CT virtual colonoscopy(CTVC),shaded surface display(SSD)and raysum.The results were compared with those of colonoscopy and patholog.Results CT diagnostic accurate rate was 100%,and the accurate rate of the cause was 96% in intestinal obstruction,including primary or secondary neoplasms(18/28),abhesions(2/28),bowel torsion(2/28)and external or internal hernias(3/28) and gallstone ileus(3/28).Conclusion There were advantages and disadvantages in diagnosis of mechanical intestinal obstruction with MPR,CTVC,SSD and Raysum respectively.The combined use of the four post-processing technigues can be more helpful to precise localizing and qualitative diagnosis of lesions.MPR should be the optimal post-processing technigues for the diagnosis of intestinal obstruction
8.CT in Combination with MRI in Diagnosis and Differential Diagnosis of Chondrosarcoma

Objective To investigate CT in combination with MRI in diagnosing and differential diagnosing chondrosarcoma.Methods CT and MRI findings pathologically in 19 cases of chondrosarcoma proved were analyzed retrospectively.Results On CT,the tumors appeared as bony destruction in 15 cases,soft tissue masses in 14 cases,and with multiple calcifications in different shape.Postcontrast CT scan demonstrated peripheral and septal enhancement in 2 cases,and inhomogeneous enhancement in 3 cases.On MRI,the chondrosarcomas were hypointense or isointense on T1WI,heterogeneous hyperintense on T2WI.Postcontrast MRI demonstrated peripheral and septal enhancement with wreath-or honeycomb-like appearance in 9 cases,heterogeneous enhancement in 2 cases,obvious enhancement in 2 cases.On fat-suppression T2WI,the boundary and size of lesions were demonstrated clearly.The phylloid and cauliflower-like shape of tumors were displayed better on sagital and coronal position.Conclusion CT and MRI are both important in diagnosis and differetial diagnosis of chondrosarcoma,the combination of the two methods may improve the diagnosing accuracy of this tumor and minimize misdiagnosis.
