2.Multi-organ preservation solution: progress in laboratory research and prospects of clinical application

UW solution has bean considered the gold standard for abdominal organ preservation. Celsior solution and HTK solution, with low potassium and low viscosity, are becoming more widely used in the preservation of the kidney, liver and pancreas, and are gradually accepted as an ideal alternative for UW solution. Recent studies have found that addition of trophic factors can attenuate hypothermic ischemic injury to the donor organ, thus improving the preservation outcome. This paper reviews the progress in laboratory research and looks foward to the clinical application of multi-organ preservation solution.
3.Influence of a self-designed multiple organ preservation solution on apoptosis of renal cortical cells during hypothermic storage

Objective:To observe the influence of a self-designed multiple organ preservation solution,namely Shanghai multi-organ preservation solution(SMO),on the apoptosis of canine renal cortical cells during cold storage and the related mechanism.Methods: Canine kidneys were freshly perfused and separately stored with cold(0-4℃) SMO,hypertonic adenine citrate solution(HC-A),and University of Wisconsin solution(UW solution)(the latter 2 as control).At defined time points during storage,the morphologic changes of renal cortical samples were observed;apoptosis of renal cortical cell was determined by TUNEL;changes of malonldialdehyde(MDA) in cortex homogenate were measured by thiobarbituric acid method;and the activity of superoxide dismutase(SOD) was determined by xanthine oxidase method.Results: Pathologic assessments indicated that the kidney damage was similar in 3 groups within 24 h.The kidney tubular cell swelling and necrosis in SMO group and UW group were slighter than those in HC-A group after stored for 2 days.With the prolongation of storage time,apoptosis increased in all groups,with the apoptotic index of SMO group significantly lower than that of HC-A group(P
4.Health economic research on tertiary stroke rehabilitation treatment

Objective To economically investigate tertiary stroke rehabilitation treatment.Methods 3 tertiary rehabilitation networks were set up in Beijing.82 patients with acute cerebrovascular diseases were enrolled into the study according to standardized criteria in all centers and randomized into rehabilitation group and control group.Rehabilitation training was implemented according to standardized procedures.The patients in rehabilitation group were given rehabilitation training in acute stage for 28 days and then underwent long term rehabilitation'program for 6 months in rehabilitation centers or community rehabilitation settings.The patients in control group were only given rehabilitation training in acute stage for 28 days. Altogether 4 evaluations were conducted in one week after the onset and at the end of 1,3,6 months, respectively.Results Significant differences of motor function,ability of daily activities,quality of life evaluation and cost were found in the 1st,3rd and 6th month after the onset of diseases.The rehabilitation group surpassed the control group significantly.At the end of 6 months,the average of Barthel Index of rehabilitation group was 83.18?18.22 while that of control group was 63.69?12.73.At the end of 6 months,total cost of rehabilitation group was(56 420.0?17 831.8)Yuan and that of control group was (64 864.2?18 267.5)Yuan.The rehabilitation group had better cost/effectiveness and cost/utility.There was a significant difference of cost between two groups.The total rehabilitation cost of rehabilitation group was 4613 Yuan,accounting for 15.3%of all the cost.Conclusion The tertiary rehabilitation treatment for stroke patients has achieved better cost/effectiveness and cost/utility.In China pharmaceutical cost constitutes the major part of the stroke therapy while the rehabilitation cost only a small portion,which is significantly different from the situation in developed countries.
5.Influence of collagenase with different degrees of activity on the isolation of rats' myocardiocytes

Objective To investigate the influences of 4 kinds of collagenase with different degrees of activity on the survival rate,contraction and relaxation functions of isolated rats' myocardiocytes.Methods Rats' myocardiocytes were isolated by enzymolysis with 4 kinds of collagenase with different degrees of activity.The survival rate of myocardiocytes was observed immediately after isolation(D),one hour after loading with calcium(E time point) and 10 minutes after electric stimulation(F time point).The contraction and relaxation functions of myocardiocytes,including contraction amplitude(ph),the proportionality of ph/single cardiocyte length(bl),maximal velocity of contraction(+dL/dt) and maximal velocity of relaxation(-dL/dt),were measured with IonOptix video edge tracker.Results From 203U/mg to 299U/mg,with lowering of the activity of collagenase,the isolation time became longer,the survival rate of myocardiocytes declined 1 hour after loading calcium and 10 minutes after electric stimulation(P
6.Research on the Equity of Secondary Allocation for Hospital Performance Based on Gini Coefficient

Objective:To evaluate the equity of the secondary performance allocation in target hospital.Methods:Through the comparison on the Gini coefficient calculation,the calculation method of Gini coefficient developed by Jianhua Zhang was applied to estimate the secondary allocation result of performance salary in the target hospital from 2014 to 2015.Results:From 2014 to 2015,the Gini coefficient remained at 0.22 or so,the overall allocation was average.Conclusion:Estimating the Gini coefficient was beneficial for the hospital managers control the structure and level of the second performance distribution and modulate the program of the sencond performance distribution in time,so that the distribution of performance salary would be accordant to the designed program and acts as the direction of motivation.
7.Progress in treatment of peripheral T-cell lymphoma

Peripheral T-cell lymphoma (PTCL) is a kind of highly aggressive lymphoma which contains a collection of subgroups. The patients with PTCL have poor prognosis under current chemotherapy regimens, so the treatment of PTCL remains a big challenge. Brentuximab vedotin combined with CHP regimen, pralatrexate in combination with romidepsin and other regimens have set up more options for patients with PTCL. Novel agents, such as PI3K inhibitors, which entered into clinical trials, bring good news to PTCL patients.
8.THE EFFECT OF REMOTE SEGMENTAL ELECTRO-ACUPUNCTURE ON THE FLEXION REFLEX POTENTIAL AND THE DORSAL ROOT POTENTIAL

Deseription was given of the effect of electric acupuncture of Neiguan point on the flexion reflex potential (FRP) of the hindlimbs and the dorsal root potential (DRP). On the intact and awake rats, the graded train stimulation (three pulses, 5ms interval, 0.5ms duration) was given to the sural nerve (a pure cutancous n(?), the C-FRP and C-DRP with their proper amplitudes (70~80% maximum) were obtained on the hamstring nerve (a group of nerves innervating the hindlimb flexors) and L5 dorsal root respectively. Then the electric acupuncture with low freqency and low intensity (4Hz, 1~3V)and low freqency and high intensity ( 4Hz, 20~40V), lasting 5 minutes, was given to the ipsilateral Neiguan point. After acupuncture, the amplitudes of both C-FRP and C-DRP decreased without change of their latebcues. The former one declined heavier and recovered slower than the latter one did. The A-FRP and A-DRP were not affected. Te result of the experiment suggests that the ipsilatcral Neiguan point cletric acupuncture can induce hindlimb's analgesia, the presynaptic inhibition might be involved in the mechanism of such analgesia.
9.EFFECTS OF STRETCHING URINARY BLADDER ON DORSAL HORN NEURONS IDENTIFIED BY SOMATIC AFFERENTS

Single unit activity were recorded from 157 dorsal horn neurons in the spinal rats by stimulation of left surat nerves. Stretching urinary bladder were performed after the neurons were identified by somatic afferents. 32 out of the 157 could also be activated by stretching urinary bladders. These recorded dorsal horn neurons which responded to both somatic and visceral inputs are called somatovisceral convergent neurons (SVCN). The afferent visceral fibers were N group according to conductive velocity in our experiment. It has been shown that C-fiber component in 9 WDR neurons were incresed by stimulating the left sural nerves after stretching urinary bladders. 15 spontaneous- discharge WDR neurons were recorded and 13 out of them were excited, meanwhile, 2 out of them were inhibited after urinary bladders were stretched. The study provides the new data for the convergence from the somatic and visceral afferents in the dorsal horn of spinal cord and supplies the first evidence for central mecha nism of visceral referred pain.
10.THE FLEXION REFLEX POTENTIAL AND DORSAL ROOT POTENTIAL INDUCED BY STIMULATION OF THE CUTANEOUS NERVE IN THE RAT

On the intact and awake rat, the flexion reflex potential (FRP)and dorsal root potential (DRP) induced by stimulation of the cutaneous nerve with graded intensity stimulations to the sural nerve (a pure cutaneous nerve) were observed, there appeared A-FRP and C-FRP one after the other on the hamstring nerve (a group of nerves innervating the hindlimb flexors), and A-DRP and C-DRP on the L_5 dorsal root at the same time. The threshold and the latencices of A-FRP and A-DRP had no differences, and had significant positive correlations. The amplitudes of A-FRP and A-DRP increased simultaneously with the increase of the stimulating intensity in certain range. The threshold and latency of C-FRP are larger than those of C-DRP. The threshold and the latency of the C-FRP and C-DRP showed significant positive correlation. The amplitudes of C-FRP and C-DRP increased simultaniously with the increase of the stimulating intensity in certain range. The train stimuli (three pulses, 5ms interval, 0. 5ms duration) induced larger FRP and DRP than those induced by single stimulus. Thus, FRP and DRP both have the property of temporal summation.
