1.Relationship between ABCG2 expression and chemotherapeutic efficacy and drug resistance of anthracyclines after breast cancer surgery

Objective To investigate the relationship between adenosine triphosphate-binding cassette superfamily G member 2 (ABCG2) mRNA and protein expressions and chemotherapeutic efficacy and drug resistance in breast cancer patients. Methods 24 patients with breast cancer were selected as study subjects, all patients were treated with anthracycline chemotherapy after chemotherapy, according to WHO criteria, the patients were divided into effective group, stable group and advanced group according to chemotherapy effect. The levels of ABCG2 mRNA and protein in the breast tissue were detected by qPCR and ELISA before and after chemotherapy. Results After chemotherapy, the total effective rate was 41.67%, 58.33% of patients still developed drug resistance. The expressions of ABCG2 mRNA and protein in patients after chemotherapy were significantly higher than those before chemotherapy (P<0.05). The expressions of ABCG2 mRNA and protein were significantly lower in the effective group than in the stable group and advanced group (P<0.05). The expressions of ABCG2 mRNA and protein in the stable group was significantly lower than that in the advanced group (P<0.05). Conclusion The expression level of ABCG2 was correlated with the chemotherapeutic efficacy and drug resistance of breast cancer patients. The higher the ABCG2 expression level, the stronger the ability of promoting the excretion of drugs, the more significant the drug resistance of tumor cells and the worsen the chemotherapy effect.
2.Analysis of the short and midterm results for total cavopulmonary connection

Objective To summarize the clinical experiences of total cavopulmonary connection (TCPC), investigate the applications and short and mid term effects. Methods From 1999 to 2007,31 patients with complex congenital heart disease underwent TCPC. All patients were divided into child group(≤16 years old ,n =21 ) and a-dult group( n = 10 ), including 15 cases of functional univentricle, 8 cases of complete atrioventricular septal defect with double outlet of right ventricle, 3 cases of transposition with double outlet right ventricle, 3 cases of tricuspid atresia, 1 case of double outlet of right ventricle with hypoplastic left heart. 1 case of corrected transposition of the great arteries with TOF. Among them, 1 patient had cortriatriatum, 10 patients had left superior vena cave, moderate or severe mitral valve insufficiency occurred in 1, moderate atrioventricular valve insufficiency occurred in 3.28 pa-tients underwent TCPC with cardiopulmonary bypass ( CPB ) and 3 patients without CPB ;Of them, 21 patients were treated with extracardiac conduit and 10 cases with intratrial lateral tunnel. Results There was no early death,post-operative death or large quantities of pleural effusion in child group;but two deaths and three patients with large quantities of pleural effusion in adult group. One case caught up in inferior venous stasis and was cured. Follow- up was carded out from 6 months to 6 years. 26 patients were in NYHA class Ⅰ ,2 patients in NYHA class Ⅱ and only one patient in NYHA class Ⅲ. Conclusions TCPC is effectively used for complex congenital heart disease which can not be corrected in anatomy;serious postoperative complications often occur in adults. Atrioventricular valve in-sufficiency decisively affects the short and midterm effects, even the long-term effects.
3.Short-term impact of fast track surgery combined with minimally invasive surgery on elderly patients with colorectal cancer

Objective To analyze the short-term impact of fast track surgery (FTS) combined with minimally inva-sive surgery on elderly patients with colorectal cancer. Methods 200 patients with colorectal cancer received laparo-scopic surgery from July 2013 to July 2015 were selected and randomly divided into control group and FTS group. Patients in control group received conventional perioperative treatment and patients in FTS group received perioper-ative management according to fast track surgery. Status of intraoperative, postoperative recovery and the postopera-tive complication rate were compared between the two groups. Patients' quality of life were analyzed after two weeks of operation. Results Compared with the control group, the blood loss, operation time and number of lymph node dissection of patients in FTS group showed no significant difference ( > 0.05). Flatus time, ambulation time, start eating time, the infusion time and length of hospital stay in FTS group was significantly shorter than the control group, the difference was statistically significant ( <0.05). The total cost of hospitalization of FTS group was signifi-cantly lower than the control group ( <0.05). The incidence of sore throat, abdominal distension, lung infections and overall complications incidence rate in FTS group were significantly lower than the control group, the difference was statistically significant ( <0.05). The satisfaction rate in FTS group was 96.0%(96/100), and was significantly high-er than the control group 82.0 %(82/100), the difference was statistically significant ( < 0.05). QLQ-C30 results showed that scores of overall health, emotional function and social function of FTS group were significantly higher than the control group ( <0.05), and scores of nausea and vomiting, pain, insomnia, anorexia, constipation were sig-nificantly lower than the control group ( < 0.05). Conclusion Laparoscopy combined with FTS is safe and effective in treatment of colorectal cancer in the elderly patients, which can promote postoperative rehabilitation, improve pa-tient satisfaction and quality of life in short term.
4.Brain protection role of sequential double-sided antegrade cerebral perfusion in arotic arch surgery

Objective To study the brain protection of sequential double-sided antegrade cerebral perfusion to central nervous system in arotie arch surgery. Methods 24 patients received aortic arch replacement under deep hyperthermic circulatory arrest (DHCA) ,with fight-sided, and following double-sided, and left-sided,if necessary, antagrade cerebral perfnsion through right axillary arterial cannula or right femoral arterial eannula homeochronously. Post-operation recovery and the central nervous complications (CNC) were recorded and analyzed. Results 2 cases died, one of whom died of refractory low cardiac output syndrome and the other died of late massive gastrointestinal tract hemorrhage. No patient suffered severe CNC. Conclusion Sequential double-sided antegrade cerebral perfusion combined with DHCA can provide good brain protection in arotic arch replacement.
