OBJECTIVE:To review the status quo and problems in the study of TCM formula granule.METHODS:Recent literature concerning the preparation technology and the normalization of quality criteria,etc.of TCM formula granule reported were reviewed and analyzed.RESULTS&CONCLUSION:TCM formula granule was of many merits such as safe and sani-tary,accurate in dosage measurement,portable etc.However,there are also some problems yet to be solved.
2.Construction of Harmonious Doctor-patient Relationship under the Background of the New Criminal Law Amendment

Under the background of current strained doctor-patient relationship,the new Criminal law Amendment stipulates that people who makes medical troubles shall be persecuted for their criminal liability in order to reconstruct harmonious doctor-patient relationship.Through questionnaire survey,this paper investigated the cognitive status,cognitive channel,and improvement measures of doctor-patient relationship after the new Criminal law Amendment in college students in Nanjing,analyzed the causes,and discussed the possible path to construct harmonious doctor-patient relationship.Doctor-patient communication should be enhanced;patients should seek medicine service rationally;hospitals should effectively deal with complaints and disputes and vigorously strengthen the enforcement strength of the new Criminal law Amendment.
3.CT Diagnosis of Retinoblastom

Objective To investigate CT diagnosis and differentiating diagnosis of the retinoblastoma . Methods CT findings of 25 cases with pathologically proved retinoblastoma were analyzed.Results CT finding included:high density of vitreous body entirely or partly in all case. Calcification in 23, augment in 8, optic never demaged in 6, ectopia retinoblastoma in 7.Conclusion Retinoblastoma of eyeball can be diagnosis qualitativly by CT.

8.Dose optimization of continuous renal replacement therapy in sepsis-induced acute kidney injury

Sepsis is a life-threatening organ dysfunction caused by dys-regulated host response to infection. Acute kidney injury (AKI) caused by sepsis is one of the most common and severe clinical disease, which incidence and mortality remains high level, and has poor clinical outcomes. Continuous renal replacement therapy (CRRT) is an indispensable tool for the treatment of critically ill patients with severe AKI, but there is no consensus on the appropriate treatment dose of CRRT. By reviewing the process of high volume hemofiltration (HVHF) and standard volume hemofiltration (SVHF), comparing their benefits and risks, and making suggestions for therapeutic schedule improvement, reference for clinical work was provided.
9.The therapeutic effect of the isolated duodenal exclusion for type 2 diabetes mellitus rats

Objective Measuring the therapeutic effect of the isolated duodenal exclusion for type 2 diabetes mellitus rats by observing the post-surgery change of glycometabolism-related indicators which could be contributed to the isolation between food and the duodenal mucosa with an endoluminal sleeve placing into the duodenum intestine duodennal of type 2 diabetes mellitus Rats.Methods In this experiment,12 male type 2 diabetes mellitus Goto-Kakizaki (GK) rats were randomized to endoluminal exclution as the experimental group and 12 to sham surgery as the control group,The body mass,the average daily food intake and fasting plasma glucose were determined before (0 week) and 1,3,6,12 weeks after operation;For testing the hematoglobinA1c(HbA1c) level,blood samples were collected before and 6,12 weeks after operation.Results No statistical significant differences between two groups before operation index.In both groups,The body mass and average daily food intake decreased markedly at 1 week after surgery:the experimental group body mass preoperative(262.6 ± 5.6 g)vs postoperative(224.0 ± 6.3 g) ;the average daily food intake preoperative(25.5 ± 2.7 g) vs postoperative (16.5 ± 3.0 g),P < 0.05.Changes in the experimental group with control group,P <0.05.The 3 week,6 week,12 week body weight was gradually increased in rats of two groups.The experimental group daily feed intake is always lower than the preoperative postoperative,P < 0.05.In the 3 weeks,the control group's body weight and daily food intake were higher than those before operation,P < 0.05.The experimental group post-FPG level (7.5 ± 1.1) mmol/L,(7.2 ± 1.2) mmol/L,(7.3 ± 0.9) mmol/L,(7.1 ± 1.0) mmol/L vs preoperation (12.2 ± 1.2) mmol/L were decreased significantly,P < 0.05.Compared to the preoperation,the HbA1c concentration of the experimental group decreased significantly,6 w (7.8 ± 0.9) %,12 w(8.2 ± 1.2) % vs the preoperation (10.3 ± 1.4) %,P <0.05.These indicators of the control group showed no significant changes,P > 0.05.Conclusion The isolated duodenal exclusion can achieve the therapeutic effect for type 2 diabetes mellitus on GK rats,and which might be attributed to the improvement of the glycometabolism.
10.Analysis on the mutation patterns of HBV polymerase gene in patients with Lamivudine resistant chronic hepatitis B

Objective To analyze the clinical features and mutation patterns of HBV polymerase gene in patients with chronic hepatitis B(CHB) after the emergence of drug-resistance during Lamivudine(LAM) therapy.Methods LAM-resistant mutations were detected by direct sequencing of the HBV polymerase gene in hospitalized patients and outpatients of CHB with LAM-resistance in Changhai Hospital from Dec.2005 to Dec.2007.Clinical features after the emergence of LAM-resistant mutations were retrospectively analyzed.Results Two hundred and fifteen patients with CHB were diagnosed as LAM-resistant.Among them 192 patients were found to have LAM-resistant-associated mutations in the HBV polymerase gene.The mean value of serum HBV DNA was 6.25?1.31(log10copies/ml),the mean value of alanine aminotransferase(ALT) was 75U/L(ranged 19-821 U/L).ALT elevation and hepatitis recrudescence were found in 139 among 192(72.4%) patients.99.0%(190/192) patients had YMDD mutations.Four major mutation patterns of LAM-resistant HBV were identified as rtM204I(33.9%),rtL180M+rtM204V(26.0%),rtL180M+rtM204I(21.9%) and rtV173L+rtL180M+rtM204V(11.5%).The rtM204V mutation was accompanied more frequently by the rtL180M mutation compared with the rtM204I mutation(P0.05).Conclusions YMDD is the major mutation pattern of HBV polymerase gene after emergence of LAM-resistance.The mutation patterns of HBV polymerase gene are possibly not related to the clinical severity of CHB patients during LAM therapy.
