1.The prevention and management of complications during and immediately after percutaneous balloon mitral valvuloplasty

Objective To approach the cause and treatment of complications during and immediately after percutaneous balloon mitral valvuloplasty. Methods One thousand three hundred and eleven patients with mitral stenosis were treated by percutaneous transseptal balloon mitral valvuloplasty. Among them, 42 patients with complications were retrospectively analyzed. Results The overall complications rate was 3.2% (42/1 311) including atrial fibrillation 0.8%(10/1 311), acute pericardial tamponade 0.31%(4/1 311), severe mitral insufficiency 0.46% (6/1 311), femoral arterial venous fistula 0.69% (9/1 311), acute pulmonary edema and iatrogenic atrial septal defect 0.23%(3/1 311), respectively. Coronary air embolism, arterial thrombosis and transient cerebrovascular accident was 0.15%(2/1 311) for every other one. Balloon rupture was 0.08%(1/1 311).Conclusions The complications of percutaneous balloon mitral valvuloplasty rarely occur. It is a safe and efficient nonsurgical method for treating rheumatic mitral stenosis.

Objective The research aimed to probe into the complications of the treatment that endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy (ERCP/EST) combined with laparoscopic cholecystectomy (LC) for the patients suffered from gallstone disease combined with common bile duct stones and investigate the nursing methods.Methods One hundred and twenty-four patients suffered from gallstone disease combined with common bile duct stones were included in this retrospective analysis.At first,they underwent the treatment of ERCP/EST,and underwent LC in 1 to 5 days later.All the clinical data were collected and analyzed.Results Of all the patients,104 patients successfully underwent the treatment of ERCP/EST.And of the 104 patients,100 patients successfully underwent LC,the other 4 patients underwent open cholecystectomy.Nineteen complicated with acute pancreatitises,one with hemorrhage and two suffered from infections of bile duct were observed in this study.Conclusions Combination of ERCP/EST with LC is a safe and effective treatment for gallstone disease combined with common bile duct stones.The preoperative psychological counseling,and the surveillance and nursing of complications play significant roles in the successful treatment.

To assess the roles of glutamate and its ionotropic receptor in processing noxious sensory input from dental pulp,glutamate NMDA receptor subtype antagonist,MK-801 and non-NMDA receptor subtype antagonist,CNQX were administrated separately into left lateral cerebral ventricle prior to exposing pulp cavity of left maxillary first molar tooth.The number of Fos-positive neurons in medulla oblongata was measured and compared with that of the group with simple exposure of dental after same survival time pulp.Fos protein expression was reduced dose-dependently by an administration of MK-801,but CNQX had little effect.The results showed that glutamate may be an excitatory transmitter of noxious dental pulp sensory input to neurons in medulla oblongata.In pain nodel of mechanical exposing dental pulp,NMDA receptor subtype has an important role in this process.
4.Prevention and management of complications during and after catheter closure of ventricular septal defect

Objective To approach the cause and treatment of complications during and after catheter closure of ventricular septal defect(VSD).Methods Catheter closure of ventricular septal defect was attempted in 415 patients.Among them,74 patients with complications were retrospectively analyzed.Results The overall complication rate was 17.83%(74/415).The complications during procedure were seen in 13.98%(58/415) which included arrhythmias in 11 patient,aortic regurgitation in 15 patients,residual shunt in 26 patients,displacement of closure device in 1 patients,cardiac temponade in 1 patient,tricuspid valve injury in 1 patient,mitral valve injury in 1 patient,iatrogenic VSD in 1 patient and respiratory complication in 1 patient.The complications after procedure were noted in 4.19% of the patients(16/382),including arrhythmias in 8 patients,LV enlargement in 2 patients,thrombus fomation in femoral artery in 2 patients,Ⅲ?AVB in 2 patients whom required temporary and permanent pacemaker implantation,respectively,headache in 4 patients,haemolysis due to residual shunting in 2 patients,tricuspid valve injury in 1 patient and puncture site bleeding in 1 patient.There was no death recorded in our study.Conclusion The incidence of complications during and after catheter closure of ventricular septal defect was relatively low but careful long term follow-up is necessary for the monitor of delayed complications.

Objective To investigate the correlation of the prevalence of periodontitis with age and course of disease in patients with type 2 diabetes.Methods A total of 884 patients with diabetes (both sexes,aged 35-79 yr) were involved in present study.Diagnosis of periodontitis was made according to the 1999 WHO standard,and of type 2 diabetes to the 1997 WHO standard.Age,gender,course of diabetes and periodontal state of those patients were recorded.The patients were grouped according to their age (10 yr interval) and course of disease (5 years interval).Based on the grouping,the collected data were input into ACCESS data bank and statistically analyzed with SAS software,and the prevalence of periodontitis were then compared.Results The prevalence of periodontitis in patients with type 2 diabetes increased with the increasing of age and with the prolonging of disease course (P

Objective To investigate the expression of heat shock protein 5 (HSPA5) in breast carcinoma and to study its relationship with clinical and pathological factors.Methods HSPA5 from 97 cases of breast carcinoma and 40 cases of adjacent tissues of breast carcinoma were examined by immunohistochemical method.Results The overall positive rate of HSPA5 in 97 cases of breast cancer was 81.4 % (79/97).The positive rates of HSPA5 in cancer tissues and paired normal tissues were 32.5 % (13/40) and 7.5 % (3/40),respectively.The expression rate of HSPA5 in tumor tissues was significantly higher than that in normal tissues (x2 =7.813,P =0.038).The expression of HSPA5 in breast carcinoma was correlated with histologic grade and lymph node metastasis (x2 =22.233,P =0.007; x2 =5.342,P =0.024).Conclusion The occurrence of breast carcinoma and the formation of invasion may be related with overexperssion of HSPA5.
7.Regional differences of methylation of metallothionein-3 gene in tissues of esophageal cancer

Objective:To investigate the methylation of CpG island of metallothionein-3 (MT-3) gene in esophageal cancer tissues and normal tissues in middle and south area of Hebei Province and Chaoshan area of Guangdong Province and compared the results with those in low risk area of esophageal cancer. Methods:The blood samples from 10 normal volunteers, 10 embryonic esophageal tissues, 20 esophageal mucosa tissues from normal subjects in low risk area as well as 30 fresh surgical specimens of esophageal cancer and 30 normal marginal tissues in the high risk middle-south Hebei Province and Chaoshan area were collected. Methylation-specific polymerase chain reaction (MSP) was used to detect the methylation status of the CpG island of MT-3 gene in these samples. Its relationship with clinicopatho-logical features was analyzed. Results:There were 20 (33.3%) cases with MT-3 methylation in the marginal tissues of esophageal cancer from high-risk area, which was higher than that in the normal mucosa from low-risk area (P=0.013). And there were 49 (81.7%) cases with MT-3 methylation in esophageal cancer tissues, which was higher than that in normal marginal tissues (P<0.001). But there was no significant difference in the methylation degree between middle-south of Hebei Province and Chaoshan area (P=0.739). Conclusion:MT-3 methylation widely exists in esophageal mucosa and carcinoma tissues. Acquired stimulus may be the main cause of these methylations.

Objective To investigate the proliferation inhibition of chlorpromazine combined with taxol on Hep-2 cells (human laryngeal carcinoma cell line) and the effects on cell cycle progression. 9.A metabonomic approach to the early prognostic evaluation of sepsis using HPLC/MS in rat model

Objective To innovate an early, rapid and efficient approach to the pmgnestic evaluation of sep-sis in order to lower the mortality. Method Forty-five septic rats, induced by cecal ligation and puncture, were divided into surviving group (n=23) and non-survival group (n=22) on six days after onset of sepsis. Serum samples were taken from septic and sham-operated rats (n=25) at 12 hours after surgery. HPLC/MS assays were performed to acquire the serum metabolic profiles, and radial basis function neural network (RBFNN) was em-ployed to build predictive model for prognostic evaluation of sepsis. Results The principal component analysis al-lows differentiating the rots of survive,non-survive and sham-operated from one another in respect of the pathologic characteristics. Six metabolites, linolenic acid, linoleic acid, oleic acid, stearic acid, docosahexaenoic acid and do-cosapentaenoic acid, related to the outcomes of septic rats were then structurally identified. A RBFNN model for outcome predication was built based upon the metabolic profile data from rat sera with the sensitivity of (96.1 ±3.6)% (n=10) and specificity of (91.0±4.3)% (n=10). Condusions HPLC/MS-based metabonomic approach combined with pattern recognition permits accurate outcome prediction of septic rats in the early stage. The proposed approach has advantages of rapid, low-cost and efficiency, and is isph-ing to be applied in clinical prognostic evaluation of septic patients.
10.Assessment of transcatheter closure of coronary artery fistula by transthoracic echocardiography

Objective To investigate the efficacy of transcatheter closure of coronary artery fistula (CAF) by transthoracic echocardiography(TTE) and the role of TTE in this therapy. Methods CAF were occluded with transcatheter closure techniques in 17 patients. TTE was performed before and after the treatments. The key points were retrospectively analyzed including: the sites of CAF, the position and diameter of CAF,the shape and position of the devices after the intervention,the residual shunt,and cardiac chamber size. Results Before the therapy,TTE made definite diagnosis of CAF. All patients had lesions in single coronary artery, and the diameter of CAF was 2-14 (6.4 ± 3.5)mm. All patients underwent transcatheter closure successfully. TTE revealed the shunts disappear 1-4 days after the occlusion. During a follow-up period of 1- 29 (11.7± 7.9)months,no residual shunts, no complications, and normal size of thecardiac chambers were recognized by TTE. Conclusions The transcatheter closure of CAF has emerged as a less invasive, safe and effective strategy. Echocardiography has important role in primary screening of patients and the follow-up after the treatments.
