Objective To explore the effects of early active mobilization on hand function after zone 5 flexor tendon and ulnar nerve repair.Methods Fifty-five patients who had received primary repair in zone 5 of a flexor tendon and the ulnar nerve were randomly divided into an observation group (26 cases, 88 digits) and a control group (27 cases, 91 digits).Both groups were given routine treatment after the operation, and started to do active and passive exercise 4 weeks later.The observation group was additionally forced to do active range of motion exercise training starting 8 days after the operation.Twelve weeks after the operation, the hand function of both groups was assessed using the total active motion (TAM) scoring system of the American Society for Surgery of the Hand, peripheral nerve function evaluation and the disabilities of arm-shoulder-hand (DASH) scale.Results At the end of the treatment, the average TAM score of the observation group was significantly better than that of the control group.The average active movement range of the wrist in palm flexion, dorsal extension, ulnar deviation and in radial deviation were all significantly better than in the control group.Grip strength, overall hand function and DASH score were also significantly better on average.Conclusion Early active mobilization following flexor tendon and ulnar nerve repair can effectively promote the recovery of hand function.
3.Analyse selective non-operative management of closed splenic injuries in 98 cases

Objective To discusses the methods of the case selection and monitoring and diagnosis and treatment to selective non-operative management of closed splenic injuries .Methods The clinical data and results of 98 cases from October 1999 to Octo-ber 2011 with closed splenic injuries managed non-operative treatment were analyzed retrospectively .Results There were 90 cases (91 .8% ) conservative successfully ,5 cases(5 .1% ) transit operation because hematoma increase and bleeding continuously during therapeutic observation and 2 cases(3 .1% ) died from multiple injuries merge infection and multiple organ failure .Conclusion Most of the closed splenic injuries below level Ⅱcan be treated by non-operative management ,the effect and prognosis determined by the reasonable case selection and monitoring conditions and diagnosis and treatment .
4.The improved application of artificial dermaiin repairing depth of burn wounds

ObjectiveTo explore the therapeutic efficacy of artificial dermal in repairing depth of burn wounds.MethodsTwenty-two cases of patients with depth bum who were admitted to our hospital during August of 2008 to August of 2010 were enrolled in this trial for retrospective study.The style of management was evaluated in these patients.Burn wound depth and severity was assessed immediately after patients admission,wound treatment was performed after patients with stable vital signs and wound edema peak period vanished.Artificial dermal was grafted onto the wound tissue after the primary debridement. Then,transplantation of artificial dermal was performed,with moist dressingonto it.After two weeks,the razor thick autoskin was grafted onto the surface of the artificial dermal after the removing of its silicon membrane during the secondary operation.ResultsThe wound healed completely with the survival of skin grafting and satisfactory appearance in all 22 patients. Furthermore,the scar in the donor site exhibited unconspicuous. Conclusion It's an optimal choice to repair deep burn with artificial dermal and thick autoskin.
5.Comparison between clinical results of peripheral blood stem cell transplantation from HLA-matched sibling donors mobilized with different regimens

Objective To retrospectively review and compare the clinical results of allogeneic peripheral blood stem cell transplantation (allo-PBSCT) from HLA- matched sibling donors mobilized with different regimens. Methods Seventy-one patients with hematological malignant diseases received allo-PBSCT from HLA-matched sibling donors in our department. Among them, 24 received allografts mobilized with G-CSF (group G), and the remaining (47 cases) were mobilized with G-CSF and GM-CSF (group G+ M). CD34+ subsets and T cell subsets in the allografts were analyzed, and the time of hematopoietic reconstitution and the incidence of graft versus host diseases (GVHD) were compared. The adverse effects on the donors after mobilization were also observed. Results The enough targeted CD34+ cells in all donors were harvested by 1-3 aphereses. Ninety-six h after mobilization, WBC counts of the donors were significantly higher in group G than in group G + M [(49. 6± 19. 5) 109/L vs (25.4 ± 10. 4) 109/L, P<0. 05]. Analysis of the CD34+ subsets showed that the percentage of cells with the CD34+/CD38- phenotype was significantly higher in group G + M than in group G [(37. 7 ± 5. 7) % vs (31.4 ± 4. 5) %, P<0. 05]. There was no significant difference in T cells and subsets of grafts. There was no significant difference in the number of total CD34+ cells and CD34+ CD38- cells, and infusion of T cells between two groups. The days required for the recovery of neutrophils and platelets was inversely correlated with the infused CD34+ and CD34+ /CD38- cell number. There was no significant difference in incidence of acute and chronic GVHD between two recipient groups. Seventeen cases and 10 eases among 71 eases died of relapses of primarydiseases, and complications of transplantation such as severe GVHD and infections respectively.Fourteen cases in group G (58.3 %) and 31 cases in group G+ M (66.0 %) survived. The most common adverse events in the donors were bone pain and fever, which mostly occurred 36 h after mobilization and could be relieved by non-steroidal anti-inflammatory drugs. Conclusion Two mobilization regimens showed equivalent clinical results. But the combined regimen of G-CSF and GM-CSF demonstrated a significantly greater mobilization of cells with the CD34+/CD38- phenotype.Meanwhile in allogeneic PBSCT, a greater number of total CD34+ cells and CD34+ CD38- cells infused may be associated with faster hematopoietic reconstitution of recipients.
6.Influence of protein phosphatase 5 on the lipid metabolism in mice

Objective To investigate the effect of protein phosphatase 5 (PP5) on lipid metabolism in the PP5 knockout (KO) mice.Methods Male PP5 KO and wild type (WT) mice at the age of 6 weeks were used in this study. In order to study the effect of high fat diet ( HFD) feeding, the body weight was measured.The liver histology was examined by HE and oil red O staining.To further verify PP5 functions in the adipogenesis, in vitro experiment was carried out using mouse embryonic fibroblasts (MEF).Western blotting and real-time PCR were performed to quantified the expression of lipid metabolism-related genes in the liver tissues.Results Compared with the WT mice, the body weight gain was slower in the KO mice.The size of the lipid droplets was smaller and the quantity was less in the KO mouse liver tissue.In vitro study revealed that the KO mouse MEF cells showed less differentiated adipocytes with smaller lipid droplets than the WT MEF cells.This observation was further confirmed by detecting the expression of adipogenesis-related genes in the HFD liver.The markers of adipocyte differentiation, such as CD36, AP2, PPARγ2, and Glut4, were significantly decreased, while energy expenditure-related markers, such as phosphorylation of GR and expression of UCP1, were significantly increased.Conclusions Protein phosphatase 5 may play a regulatory role in the mouse lipid metabolism through regulating the de-phosphorylation of p-GR and enhancing the expression of UCP1.
7.Roles of probiotics in regulating serum ghrelin levels and protecting pulmonary function of children with acute lung injury

Objective To investigate the regulatory effects of probiotics on ghrelin and their pro-tective effects on pulmonary function of children during acute lung injury.Methods Fifty children with acute lung injury were recruited form Henan Provincial People′s Hospital during 2010 to 2013 and randomly divided into two groups.In addition to regular therapy, those who received probiotics treatment were set up as the probiotics treatment group and the others were considered as the control group.Levels of ghrelin, sur-factant protein A( SP-A) , TNF-αand IL-6 in serum samples were measured before and 10 days after the cor-responding treatments.Each patient was analyzed by pulmonary function test and echocardiography.The vol-ume to peak expiratory flow ( VPEF) , ratio of time to reach peak tidal expiratory flow to total expiratory time ( TPTEF/TE) , mean arterial pressure ( MAP) and pulmonary artery pressure ( PAP) were recorded for sta-tistical analysis.Results The levels of SP-A, TNF-αand IL-6 in serum samples from the probiotics treat-ment group were decreased on day 10 as compared with those of the control group [(195.9±54.3) ng/ml vs (245.5±56.4) ng/ml, (382.0±56.4) pg/ml vs (440.7±46.8) pg/ml, (221.5±62.6) pg/ml vs (303.5 ±83.9) pg/ml;P<0.05] .Children from the probiotics treatment group showed higher values of VPEF and TPTEF/TE on day 10 as compared with those without probiotics treatment [(25.3±4.5) ml vs (23.8±4.6) ml, (29.5±4.3) %vs (26.8±4.7) %;P<0.05].Treatment of children with probiotics increased MAP and decreased PAP [(74.5±6.7) mmHg vs (71.9±6.9) mmHg, (25.5±6.4) mmHg vs (29.1±6.2) mmHg] .The levels of ghrelin in patients received probiotics treatment were higher than those of children without probiotics treatment [(329.3 ±49.6) pg/ml vs (296.6 ±48.5) pg/ml].The levels of ghrelin showed negative correlations with the levels of SP-A, TNF-α, IL-6 and PAP on day 10, but positive correla-tions with TPTEF/TE and MAP (P<0.05).Conclusion Use of probiotics to children with acute lung inju-ry could regulate the level of ghrelin, alleviate the inflammation of lungs and improve the recovery of lung function and circulation.Therefore, probiotics treatment had positive effects on the protection of pulmonary function.
8.Determination of the content and the molecular weight of Mussel Polysaccharide by GPC

Objective To develop a gel permeation chromatography method for determination of content and molecular weight ( Mw ) of Mussel Polysaccharide.Methods Using GPC method, the sample was separated with TSK-gel GMPWXL(7.8 mm ×300 mm) chromatography column which was set at 35℃.The mobile phase was 0.05 mol/mL NaNO3(including 0.05%Na2N3) and the flow rate was 0.6 mL/min.The detector was RID-20AT. Results The average molecular weight of the polysaccharide of Mytilus coruscus was 1 261 411 and the average content was 88.6%by using of the calibration curves of dextrans.The average molecular weight of the polysaccharide of Mytilus edulis was 1 244 062 and the average content was 87.4%. Conclusion The method established in this paper is simple and rapid, accurate and reproducible, which can be used for the quality control of Mussel Polysaccharide.
9.Value of walking speed at the evaluation of motor function recovery in hemiparetic patients after stroke

ObjectiveTo identify the value of walking speed at the evaluation of motor function recovery in hemiparetic patients after stroke.MethodsFifty-five cases were assessed in the study. The walking speed, motor function, strengths of the paretic lower limb,balance and ambulation of ADL function were evaluated with the 10m Maximum Walking Speed Test,Fugl-Meyer Assessment, Motricity Index, Berg Balance Scale and ambulation item of Functional Independence Measure. The level of association between walking speed and the clinical variables were examined with Pearson's correlation coefficients.ResultsIn the slow group, the gait-related clinical scores were related to walking speed (Pearson's r=0.581-0.770,P<0.05),while that in the moderate group and fast group were not (Pearson's r=0.016-0.380,P>0.05).The correlation between the gait-related clinical scores and walking speed was weakened when walking speed above 44.8m/min(or 41.3% of normal values).ConclusionWalking speed plays an independent role as an indicator of improved performance.
10.Optimization of extraction procedure of tongmai granules by orthogonal design with pharmacodynamic index.

OBJECTIVETo optimize the extraction technology for anti-myocardial ischemia active component group in tongmai granules.
METHODThe extracting conditions were optimized by using orthogonal experiment and pharmacodynamic index. Employing puerarin, ferulic acid, tanshinone IIA, and salvianolic acid B as indexes, the effects of ethanol concentration, solvent volume, extraction times and extraction time on the extracting process were investigated.
RESULTThe optimum extraction process was adding 8 times amount of 70% alcohol, refluxing and extracting for 3 times, 1.5 h each time.
CONCLUSIONThis extraction process shows good stability and is available for extracting anti-myocardial ischemia component group in tongmnai granules.
