1.Clinical Value of Neck Vascular Ultrasound and Homocysteine Level in Patients With Cerebral Infarction and Carotid Stenosis

Objective:To investigate the clinical value of neck vascular ultrasound and homocysteine (Hcy) level in patients with cerebral infarction and carotid stenosis.
Methods: Our research included 2 groups. Cerebral infarction group, n=115 patient who were treated in our hospital from 2012-01 to 2013-12, and Control group, n=110 healthy subjects from physical check-up. Neck vascular ultrasound was conducted to classify the degree of carotid stenosis as ①no stenosis, ②mild degree, the stenosis rate≤29%, ③moderate degree, the stenosis rate at 30%~69%,③severe degree, the stenosis rate at 70%~99%,④complete occlusion. Carotid plaque formation and vascular intima-media thickness (IMT) were examined by neck ultrasound. Blood levels of Hcy and triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) were
measured in clinical laboratory.
Results: Compared with Control group, Cerebral infarction group had the higher rate of carotid stenosis 89.56%vs 10.91%, more plaque formation 73.91%vs 6.08%and increased IMT (1.85 ± 0.42) mm vs (0.75 ± 0.21) mm;higher blood levels of Hcy (42.32 ± 12.02) μmol/L vs (6.32 ± 2.45) μmol/L and higher rate of high blood Hcy syndrome 71.30%vs 3.63%;higher levels of TC (4.25 ± 1.12) mmol/L vs (1.63 ± 0.74) mmol/L and LDL-C (5.96 ± 1.36) mmol/L vs (2.32 ± 0.82) mmol/L, P<0.05. In Cerebral infarction group, the patients with complete carotid occlusion and severe stenosis had the higher rate of plaque formation and increased IMT;higher levels of Hcy and more high blood Hcy syndrome;higher levels of TG and LDL-C than the patients with mild and moderate carotid stenosis, P<0.05. Logistic analysis indicated that the levels of IMT, Hcy and LDL-C were the independent risk factors for cerebral infarction and carotid stenosis.
Conclusion: The patients with cerebral infarction and carotid stenosis have obviously increased blood level of Hcy. Hcy may predict various degree of carotid stenosis in relevant patients.
2.Feasibility and surgery indications of endoscopic submucosal dissection for gastric cancers

Objective To evaluate the feasibility and indications of endoscopic submucosal dissection (ESD) by analyzing the early gastric cancer cases.Methods All the pathological materials of radical gastric resections in Huashan hospital from 2007 to 2011 were retrieved and the EGC cases were selected.All the cases in EGC group which met the extended ESD criteria were reviewed.Extended ESD criteria were cited from Treatment guidelines for gastric cancer in Japan edited by JGCA in 2001.The risk factors relevant with regional metastasis,such as invasive depth and differentiation degrees,were also analysed.Results In the recent five years,there were 1159 cases of gastric cancer undergoing radical gastrectomy in Huashan hospital including 210 EGC.According to the extended criteria of JGCA,151 cases could be candidates for ESD while 126 cases could be candidates for ESD according to the typical criteria for ESD.The local metastases of lymphatic or vascular involvement for mucosa cancer,muscularis mucosae cancer,sm1 and sm2 cancer were 6.75%,15.00%,23.25%,and 39.62% respectively (P <0.05).Twenty-five cases had local metastases that met the extended criteria for ESD.Within the 210 EGC cases,the local metastases of differentiated type and undifferentiated type were 13.95% and 47.37% respectively (P < 0.05).Conclusion Most EGC cases undergoing radical gastrectomy can be cured by ESD.Its metastases are highly correlated with invasion depth and differentiation,so indications of ESD should be assessed.
3.Passive mobilization for restoring hand function after surgical flexor tendon and ulnar nerve repair

Objective To explore the effects of early active mobilization on hand function after zone 5 flexor tendon and ulnar nerve repair.Methods Fifty-five patients who had received primary repair in zone 5 of a flexor tendon and the ulnar nerve were randomly divided into an observation group (26 cases, 88 digits) and a control group (27 cases, 91 digits).Both groups were given routine treatment after the operation, and started to do active and passive exercise 4 weeks later.The observation group was additionally forced to do active range of motion exercise training starting 8 days after the operation.Twelve weeks after the operation, the hand function of both groups was assessed using the total active motion (TAM) scoring system of the American Society for Surgery of the Hand, peripheral nerve function evaluation and the disabilities of arm-shoulder-hand (DASH) scale.Results At the end of the treatment, the average TAM score of the observation group was significantly better than that of the control group.The average active movement range of the wrist in palm flexion, dorsal extension, ulnar deviation and in radial deviation were all significantly better than in the control group.Grip strength, overall hand function and DASH score were also significantly better on average.Conclusion Early active mobilization following flexor tendon and ulnar nerve repair can effectively promote the recovery of hand function.
4.Pilomatricoma: a retrospective study of 399 cases.

OBJECTIVE:
To study the clinical and pathological features of pilomatricoma.
METHOD:
The authors retrospectively investigated the clinical and pathological materials of 399 patients with pilomatricoma.
RESULT:
Single lesion occurred in most patients (99%) and 56.39% of them were younger than 30 years. The male-female ratio was 1:1.33. The lesions which sizes average 1.22 cm were commonly emerged in the head, neck, and upper extremity.
CONCLUSION
Pilomatricoma is a slowly developed benign cutaneous tumour, but it can aggravate sometimes. It's manifestation is diversed and easily misdiagnosed. Early complete excision is recommended for hard or tenacious nodules on head, neck and upper extremity.

5.The improved application of artificial dermaiin repairing depth of burn wounds

ObjectiveTo explore the therapeutic efficacy of artificial dermal in repairing depth of burn wounds.MethodsTwenty-two cases of patients with depth bum who were admitted to our hospital during August of 2008 to August of 2010 were enrolled in this trial for retrospective study.The style of management was evaluated in these patients.Burn wound depth and severity was assessed immediately after patients admission,wound treatment was performed after patients with stable vital signs and wound edema peak period vanished.Artificial dermal was grafted onto the wound tissue after the primary debridement. Then,transplantation of artificial dermal was performed,with moist dressingonto it.After two weeks,the razor thick autoskin was grafted onto the surface of the artificial dermal after the removing of its silicon membrane during the secondary operation.ResultsThe wound healed completely with the survival of skin grafting and satisfactory appearance in all 22 patients. Furthermore,the scar in the donor site exhibited unconspicuous. Conclusion It's an optimal choice to repair deep burn with artificial dermal and thick autoskin.
6.Clinical observation on short-term effects of primary total knee arthroplasty

Objective To evaluate the short-term effects of total knee arthroplasty ( TKA) , and to detect the causes and prevention of major complications. Methods It was a retrospective study since January 2010 to December 2013. There were 63 patients with 70 knees re-ceived primary TKA. KSS knee scores, HSS knee scores, and range of motion ( ROM) before the operation and during the follow-up were as-sessed and analysed. Results According to the results of follow-up and the X-ray observation, the anteroposterior radiographs showed that the artificial prosthesis were placed well and the joint gap were balanced;the lateral radiographs showed that the artificial joint prosthesis we-rer of appropriate size, good tilt angle, and good package effect. There were two cases developed peroneal nerve palsy and they gradually re-stored in 4 to 6 months after surgery. 5 cases were of poor postoperative ROM (0~60 ± 10)°. Strong analgesic medication-assisted release were conducted in 2 to 3 weeks after surgery. 1 case had patellar pain and resorted by arthroscopic lateral ligament of patellar releasing sur-gery. During the follow-up period, TKA significantly improved HSS and KSS scores compared to preoperative scores. ROM was increased from (78. 3 ± 18. 6)° preoperatively to (109. 2 ± 19. 9)° postoperative (P<0. 01). Conclusion TKA was effective to release knee pain, improve ROM and reduce the incidence of postoperative complications.
7.Radiological protection effect of vanillin derivative VND3207 against radiation-induced cytogenetic damage in mouse bone marrow cells

Objective To study the protection of vanillin derivative VND3207 on the cytogenetic damage of mouse bone marrow cell induced by ionizing radiation.Methods BALB/c mice were randomly divided into five groups:normal control group,2 Gy dose irradiation group,and three groups of 2 Gy irradiaiton with VND3207 protection at doses of 10,50 and 100 mg/kg,respectively.VND3207 was given by intragastric administration once a day for five days.Two hours after the last drug administration,the mice were irradiated with 2 Gy γ-rays.The changes of polychromatophilic erythroblasts micronuclei (MN),chromosome aberration (CA) and mitosis index (MI) of mouse bone marrow cells were observed at 24 and 48 h after irradiation.Results Under the protection of VND3207 at the dosages 10,50,100 mg/kg,the yields of poly-chromatophilic erythroblasts MN and CA of bone marrow cells were significantly decreased(t = 2.36-4.26,P < 0.05),and the marrow cells MI remained much higher level compared with the irradiated mice without drug protection (t = 2.58,2.01,P < 0.05).The radiological protection effect was drug dose-dependent,and the administration of VND3207 at the dosage of 100 mg/kg resulted in reduction by 50% and 65% in the yields of MN and CA,respectively.Conclusions VND3207 had a good protection effect of on γ-ray induced cytogentic damage of mouse bone marrow cells.
8.Chromosome aberration and micronucleus formation in human peripheral blood lymphocytes induced by 12C heavy ions

Objective To investigate the biological effects of 12C heavy ions with different LETs,and to construct the dose-effect curves of chromosomal aberrations and micronucleus.Methods Human peripheral blood lymphocytes were irradiated with 0.5 - 5.0 Gy of 60Co γ-rays,29 and 148 keV/μm 12C with a dose rate of 0.5 Gy/min,respectively.The colchicine method and cytokinesis-blocking method were applied to measure chromosome aberrations and micronuclei formation,respectively. Results The chromosomal aberrations induced by 29 and 148 keV/μm of 12C ion had a linear dose response,while the dose-response of micronucleus induction followed to a linear-quadratic model.Chromosome aberration and micronucleus frequency increased with the LET of 12C ions. However, the micronucleus frequency increased slowly when the radiation doses exceeded 3.0 Gy..Chromosomal aberrations induced by 148 keV/μm 12C ions increased significantly when the time of colchicine treatment was prolonged.Conclusions Chromosome aberration and micronucleus induced by 12C ions increased with the LET,and the micronucleus frequency increased with dose less than 3 Gy.The biological effects of 12C ions were stronger than those of low LET irradiation of γ-rays.
9.A study of correlation between alexithymia and resilience in military personnel

Objective To explore correlation between alexithymia and resilience in military personnel.Methods 2802 subjects were chosen by random cluster sampling to outline the military personnel resilience scale and military alexithymia scale.The data were analyzed by t-test,chi-square test,ANOVA,correlation analysis and multiple linear regressions analysis.Results The prevalence rate of alexithymia was 6.85%,while resilience was 6.60%.Compared to sea force (45.62 ± 11.25 ) and air force (45.32 ± 9.98 ) subjects,land force (47.92 ± 9.92)had significantly higher total score in alexithymia(P< 0.05 ).Compared to sea force (87.35 ± 12.15 ) and air force ( 88.58 ± 10.39 ) subjects,land force ( 85.73 ± 10.70) had significantly lower total score in resilience(P < 0.05 ).The total score and every factor score of subjects with alexithymia were significant lower than those of control group in resilience(P < 0.05 ).Every factor of Resilience Scale were mild negative correlation with every factor of Alexithymia Scale (P<0.01).Problem solving,volition,optimism and family supports were the main factors which had affected on the alexithymia.Conclusion The prevalence rate of alexithymia in military personnel is lower than ordinary people,but there are still issues in alexithymia and resilience,particularly in land force.The research show mild correlation between alexithymia and resilience in military personnel.
10.Characteristics of the elderly fall cases in Jiaxing

Objective:
To analyze the trend and distribution characteristics of the elderly fall cases in Jiaxing from 2009 to 2018,and to provide evidence for injury prevention and control.
Methods:
The injury monitoring data of Jiaxing sentinel hospitals from 2009 to 2018 was collected and analyzed the demographic features,places,results,involved body parts and severity of the fall cases aged 60 years or over.
Results:
There were 22 494 fall cases aged 60 years or over,accounting for 7.51% of all injury cases,28.79% of all fall cases,and 41.25% of the elderly injury cases. Fall was the first cause for injury among the elderly in Jiaxing. The proportion of the elderly fall cases to all injury cases and all fall cases increased year by year. There were 9 419 males,accounting for 41.87%,and 13 075 females,accounting for 58.13%. The average age was(71.19±8.55)years old in males and(72.59±9.09)years old in females. The falls mainly occurred at home,with 15 874 cases accounting for 70.57%. The falls mainly happened in the free time,with 11 133 cases accounting for 49.49%. The most serious injuries were fractures,with 9 640 cases accounting for 42.86%. The injured parts were mainly lower limbs,with 7 720 cases accounting for 34.32%. Most old fall cases were moderately injured,with 14 223 cases accounting for 63.23%. The difference in the severity of fall among different genders was statistically significant(P<0.05).
Conclusion
Fall cases among the elderly in Jiaxing have an increasing trend. Senior age and women are risk factors. Fall mainly results in fractures in lower limbs.
