1.Glycoprotein B genotype of human cytomegalovirus analysis by nPCR-RFLP

Objective Aim to set up the method of glycoprotein B genotype of human cytomegalovirus (HCMV) analysis by using nested polymerase chain reaction(nPCR) and restriction fragment length polymorphism(RFLP). Methods Eleven blood samples and twenty-three urine samples were obtained from thirty-four HCMV-infected patients. A fragment of gB gene was amplified by nPCR. HCMV gB genotyping was carried out by RFLP, and the amplified DNA fragments were verified by DNA sequencing. Results Of the 34 patients, gB type Ⅰ was found in 13 patients, gB type Ⅱ in 12 patients, gB type Ⅲ in 9 patients, and none had the gB type Ⅳ sequence. The similarities of PCR products of HCMV gB Ⅰ, Ⅱ and Ⅲ amplified compared with the sequences of prototype strains in GenBank were 98.1%~99.6%, 98.9%~100%, 97.3%~98.9%. Conclusions The nPCR assay developed in this study was sensitive and specific for detection of HCMV, and RFLP analysis of HCMV gB genotype was definite and reliable.
2.The expression of heparanase and vascular endothelial growth factor-C in gastric carcinoma and its implication

Objective To investigate the expression of heparanase and vascular endothelial growth factor-C (VEGF-C) protein in human gastric carcinoma, and to evaluate its clinical implications during the progress of gastric carcinoma. Methods The expression of heparanase and VEGF-C protein in 97 cases of gastric cancer, adjacent tissues and normal tissues in 20 subjects was determined by immuno- histochemistry. Results The positive rate of heparanase and VEGF-C protein in tumor tissues (61.9% and 66.0%) were significantly higher than those in adjacent tissues of cancer (7.2% and 5.0%) and normal tissues (8.3% and 5.0%, P 0.05) . The positive expression of VEGF-C protein in the tumor tissues was significantly related with serosal infiltration, lymphatic invasion, lymph node metastasis and TNM stage Ⅲ-Ⅳ (P0.05). The 1, 3 and 5-year survival rates of patients with expression of heparanase or VEGF-C protein were significantly less than that of patients without expression of heparanase or VEGF-C (P
3.Influence of bifidobacterium on NF-?B and I?B in murine peritoneal macrophages

AIM: To explore the regulatory effect of bifidobacteria adolescence on NF-?B in murine peritoneal macrophages(PMs).METHODS: Macrophages were collected and were divided into tow groups,normal control group and bifidobacterium stimulation groups.The cells were fixed at 60 min after stimulated with bifidobacterium at 10~6,10~7, 10~8 and 10~9 cells/L,or fixed respectively at 15,30,60,120 and 180 min after stimulated with bifidobacterium at 10~8 cells/L,then the total protein and nucleoprotein were extracted.The activities of NF-?B and I?B? were measured with the methods of MSA and Western blotting.RESULTS: NF-?B activity markedly increased and reached the peak at 0.5 h after stimulation,while I?B? markedly decreased at same time.CONCLUSION: NF-?B activity is markedly activated by bifidobacterium.NF-?B pathway participates in the regulation of peritoneal macrophage in this process.
4.Plastic surgery for the serious breast asymmetry

Objective To evaluate mammaplasty for repairing the serious breast asymmetry.Methods Different mammaplasty was used to repair 38 cases of breast asymmetry,including 13 cases of unilateral breast augmentation,10 cases of breast reduction after different volume resection on each side,4 cases of unilateral partial breast resection,5 cases of breast reduction on one side and breast suspended on another side,4 cases of breast suspension on one side and augmentation on the other side,and 2 cases of only suspension on unilateral breast.The 38 cases were followed up for 3 to 12 months postoperatively to evaluate the results.Results One case had slight ptosis in half a year after breast suspension.Apart from mentioned above,the rest of 11 had favorable and satisfactory results.Conclusions The serious breast asymmetry deformities should be corrected with the method of the simple and minimum injuries to make the best result.
5.The value of esophageal intrapapillary capillary loop visualized by magnifying narrow-band imaging endoscopy in diagnosing esophageal mucosal pathology

Objective To investigate the diagnostic potential of magnifying narrow-band imaging endoscopy (NBI-ME) for different intrapapillary capillary loop (IPCL) for the diagnosis of esophageal lesion.Methods Patients with abnormal esophageal mucosa found by white light gastroscopy in digestive endoscopy center,Chinese PLA General Hospital during the period of November 2009 to November 2010 were enrolled in this study.IPCL was observed and divided into different types by NBI-ME.Histopathology of biopsy or endoscopic submucosal dissection (ESD) specimens was evaluated and used as the gold standard to evaluate the diagnostic value of NBI-ME for IPCL.Results A total of 146 lesions from 145 subjects with esophageal mucosa abnormal were collected. Among them, 88 were pathology-proven inflammation,5 were pathology-proven esophageal cancers,20 were pathology-proven low intraepithelial neoplasia (LIN) and 33 were pathology-proven high intraepithelial neoplasia (HIN) detected with NBI-ME.By a per-lesion analysis,the accuracy of inflammation and cancer were 100% (88/88) and 7/7.For the sensitivity,specificity,accuracy,positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio of LIN and HIN were 7/10,69.8% ( 30/43 ),69.8% ( 37/53 ),35.0% (7/20),90.9% (30/33),12.5% (70/559),2.3% (30/1290) and 87.1% (27/31),72.7% ( 16/22),81.1% ( 43/53 ),81.8% ( 27/33 ),80.0% ( 16/20 ),634.1% ( 837/132 ) and 35.2% ( 124/352 ),respectively.Conclusions NBI-ME can classify the different esophageal IPCL.Higher diagnostic accuracy of IPCL indicates the feasibility of NBI-ME for the efficacious diagnosis of esophageal inflammation and cancer.There is the higher diagnostic accuracy of HIN than LIN.
6.Comprehensive treatment for mid-inferior facial rejuvenation

Objective To summarize the experience of the integrated treatment on mid-face ageing for better cosmetic results.Methods A total of 56 cases were treated.With using subciliary approaches,the orbicularis oculi was resected to expose the orbital septum,the orbital fat relieved and orbital septum reseted.Hyaluronic acid was injected to some patients with obvious nasolabial fold after operation.Results 56 cases were all followed up from 6 to 26 months (11.8 months on average) postoperatively.The flabby tissue had been tightened and all got better effects.Conclusions Various combination of technologies can be applied to reach the purpose of rejuvenation by correcting the volume abnormality,resetting the tissue,elevating reasonably,and filling the facial depression.
7.A comparative study of cost-effectiveness between two curative means for treating early gastrointestinal carcinoma or precancerous lesions: endoscopic submucosal dissection (ESD) vs traditional surgery

Objective To compare the cost-effectiveness between endoscopic submucosal dissection (ESD) procedure and traditional surgery for the treatment of early gastrointestinal cancer or precancerous lesions.Methods A total of 866 early GI cancer/precancerous patients who had been admitted to Chinese PLA General hospital and Qilu hospital were included in this study.The medical records of all 866 patients were reviewed.Patients were divided into ESD group and traditional surgery group.Parameters of each patient,such as time of hospitalization,total treatment cost and incidence of complications,were documented and later compared in detail.Results A total of 607 patients were included in the ESD group and 259 in the traditional surgery group.There was no statistical difference in complete resection rate between the two groups (93.1% vs 93.0%,P > 0.05).The ESD group showed a shorter mean hospitalization time (d)(13.01 vs 18.88,P < 0.05) and lower treatment cost (RMB) than the traditional surgery group (22932.17 vs 57993.88,P < 0.05).The incidence of hemorrhage and perforation for each group were 3.63% and 3.10% respectively,which were not significantly different (1.98% vs 3.01%,P > 0.05; 2.80% vs 1.16%,P > 0.05).Conclusion ESD and surgery are both effective for early gastrointestinal cancer/precancerous lesions therapy,but ESD procedure is superior to surgery in terms of hospitalization time and expediture.
8.Combined strategies in treatment of breast ptosis and breast hypoplasia

Objective To seek an effective surgical procedure to treat patients with the varying degrees of breast ptosis and micromastia.Methods Patients were classified into Ⅰ-Ⅳ degrees based on different breast ptosis,and treated by different methods.Degree Ⅰ was treated with implanting prosthesis to the post-pectoralis major space; degree Ⅱ patients were repaired with avulsion of deeper mammary gland and pectoralis major and hanging fixed breast tissue,based on the degree Ⅰ procedure; degree Ⅲ was fixed with breast tissue flap,excised redundant epidermis by the method of double-rings and the complex of nipple and areola was shifted,based on the degree Ⅱ procedure; degree Ⅳ was repaired with implanting prosthesis and the method of lines to hanging fixed breast.Results In 116 cases of this study,there were no hematoma,infection and nipple and areola necrosis by the combined strategies.The follow-up period after the surgery was 6-31 months (mean 13.3 months).All cases had voluptuous and upright breasts,rectified breast ptosis,with the normal sense of nipple and areola.Conclusions For breast ptosis and hypoplasia,the combined strategies have better clinical therapeutic effects.
9.Endoscopic submucosal dissection for early gastrointestinal cancer and precancerous lesions

ObjectiveTo investigate the procedure,effect and complication of endoscopic submucosal dissection (ESD) in the management of early gastrointestinal tumors and precancerous lesions.MethodsESD was performed in 28 patients with 29 lesions of early cancer and precancerous lesions in esophagus,stomach,colon and rectum.First we made marks around the lesion 3-5 mm away from the margin with a needle knife or APC,then injected solution into submucosa to elevate the lesion,and cut the mucosa and submucusa along the margin with a needle knife or IT knife.The submucosa was carefully dissected until the lesions were completely removed with IT knife.Bleeding was stopped with thermocoagulation forceps,argon plasma coagulation or clip.The samples were collected for pathological examination.All patients were followed up with endoscopy as scheduled.ResultsOf the 29 lesinas,22 were en bloc resected,6 were piecemeal resccted,and 1 was partial removed.The resection rate of antral lesion was 100% (12/12),and that of lesions between angulus and cardia was 5/7,that of esophageal lesions was 3/5 and that of colorectal ncoplasmns was 2/5.Delayed bleeding occured in 1 patient.The mean operation time for the antral lesions was 48 minutes.Twenty patients were followed up for 1 to 12 months.No residue or recurrence of the lesions was found.ConclusionThe major advantage of ESD is that the resection area can be determined by the size and shape of the lesion,and resection can be achieved en bloc even in a large neoplasm.
10.Clinical effect of linear mastoplasty in treatment of mastoptosis

Objective To evaluate the effect of Lejours' procedure in the treatment of breast prolapse.Methods From July 2014 to July 2018,the Second People's Hospital of Guangdong Prov ince accepted 36 female patients with breast prolapse and correction failure,aged 25 42 years,with an average age of 32.1 years.According to the preoperative design,the pedicel epidermis,the redundant skin,the prosthesis and capsule were removed,and the mammary flap was fixed on the fascia of pectoralis major muscle,and the nipple and areola were fixed and sutured layer by layer.Results In 36 cases,the shape of breast was improved,the scar of incision was smaller,and the color of scar became lighter with time.There were no serious complications such as nipple and areola necrosis.The breast was straight and round after operation.After 6-24 months follow-up,one patient had delayed wound healing and received repeated dressing changes for 7 days.Conclusions The design of the op eration is simple,with little scar,few complications and good long term effect.The shape of the breast after repair is round and straight,which can be used as one of the feasible operation methods for the repair of breast ptosis.
