1.Relation between coagulation-fibrinolysis indexes and biological behavior of breast cancer

Objective To study the relation between coagulation-fibrinolysis and metastasis, hormone receptor and TNM stage of breast cancer. Methods The plasm levels of six coagulation-fibrinolysis indexes of 30 breast cancer patients were tested, which included activated partial thromboplastin time(APTT), fibrinogen(FIB), D-dimer(D-Di), tissue type plasminogen activator (t-PA)and plasminogen activator inhibitor-1(PAI-1). Statistic analysis of the relation between the changes of these indexes and metastasis, hormone receptor and breast cancer cell proliferation was performed. Results There were statistically significant differences between the malignant and benign breast disease in FiB(3.05±0.44)g/L vs(3.39±0.52)g/L(P<0.05),D-Di(0.27±0.06)μg/ml vs(0.36±0.16)μg/ml(P<0.05) and PAI-1(26.14±3.30)ng/ml vs(34.59±3.68)ng/ml (P<0.01). The levels of plasma FIB (3.70±0.47)g/L and PAI-1 (37.36±2.71)ng/ml in metastasis group were higher than those in the non-metastasis group (P<0.01). Significantly higher levels of PAI-1(36.40±3.57)ng/ml(P<0.05)in expression of Ki67≥30% in the patients with lymph node involvement were seen. Conclusion Coagulation-fibrinolysis indexes were related to the proliferation, invasion, metastasis and clinical stage of breast cancer and it can serve as an important marker for predicting breast cancer's biological behavior and prognosis.

Objective The expressions of PI3K in invasive squamous cell carcinoma of cervix and their clinical significance were investigated .Methods The expressions of PI3K ,Ki‐67 and CD34 protein in 28 cases of normal cervical epithelium (NCE) ,36 cases of cervical intraepithelial neoplasm (CIN) and 68 cases of squamous cell carcinoma of cervix (SCC) were detected by immunohisto‐chemistry SP method .Results The positive expression rates of PI3K in NCE ,CIN and SCC were 25 .00% ,55 .56% and 85 .29% , respectively .The positive expression rates of PI3K increased remarkably from NCE and CIN to SCC(P<0 .05) .The expression of PI3K in SCC was significantly positive correlated with PI and MVD .The expression of PI3K in SCC was related with FIGO stag‐ing ,histological grading and pelvic lymph node metastasis (P<0 .05) ,but was not related with age ,depth of stroma infiltration and intravascular infiltration (P>0 .05) .In the cases with FIGO staging Ⅱ ,poorly differentiated tissue and pelvic lymph node metasta‐sis ,The positive expression rate of PI3K significantly higher than those in the cases without them(P<0 .05) .Conclusion The o‐ver‐expression of PI3K may lead to up regulation of tumor angiogenesis and cancer cell proliferation in SCC ,and may promote tumor Metastasis and progress of SCC .
5.Perioperative changes in serum levels of TNF-?,IL-6 and IL-10 in peri-operative period of liver orthotopic transplantation

Objective To study perioperative changes in serum levels of TNF-?, IL-6 and IL-10 in orthotopic liver transplantation without veno-venous bypass. Methods Sixteen patients with terminal liver disease underwent orthotopic liver transplantation under general anesthesia. Samples from arterial blood were obtained at beginning of surgery, before the anhepatic phase, 5min before recirculation, 15min and 60min after recirculation, 4h and 24h after operation, respectively. This study aimed to induce the differentiation of isolated and purified adipose-derived stromal cells (ADSCs) into myoblasts, which may provide a new strategy for tissue engineering in patients with stress urinary incontinence (SUI). ADSCs, isolated and cultured ex vivo, were identified by flow cytometry and induced to differentiate into myoblasts in the presence of an induction solution consisting of DMEM supplemented with 5-azacytidine (5-aza), 5% FBS, and 5% horse serum. Cellular morphology was observed under an inverted microscope. Ultrastructural changes occurring during the differentiation were observed by transmission electron microscopy and confocal laser scanning microscopy. Cellular immunohistochemical staining was applied to determine the expression of desmin protein in cells with and without induced differentiation. Reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting were used to detect mRNA and protein expression, respectively, of sarcomeric and desmin smooth muscle proteins. The results showed that ADSCs were mainly of a spindle or polygon shape. Flow cytometry analysis revealed that ADSCs did not express CD34, CD45, and CD106 but high levels of CD44 and CD90, which confirmed that the cultured cells were indeed ADSCs. After induction with a 5-aza-containing solution, morphological changes in ADSCs, including irregular cell size, were observed. Cells gradually changed from long spindles to polygons and star-shaped cells with microvilli on the cell surface. Many organelles were observed and the cytoplasm was found to contain many mitochondria, rough endoplasmic reticulum (rER), and myofilament-like structures. Cell immunohistochemical staining revealed different levels of desmin expression in each phase of the induction process, with the highest expression level found on day 28 of induction. RT-PCR and Western blot results confirmed significantly higher desmin gene expression in induced cells compared with control cells, but no significant difference between the two groups of cells in sarcomeric protein expression. It was concluded that under specific induction setting, ADSCs can be induced to differentiate into myoblasts, providing a potential new option in stem cell transplantation therapy for SUI.
7.Clinical characteristics of gastric mucosa-associated lymphoid tissue lymphoma and the effects of Helicobacter pylori eradication therapy

ObjectiveTo review the clinical characteristics of patients with gastric mucosa-associated lymphoid tissue(MALT) lymphoma and to investigate the effects of Helicobacter pylori (Hp) eradication therapy in these patients.MethodsClinical data of ninety-five patients with gastic MALT lymphoma were reviewed with respect to their clinical manifestations,endoscopic features,histopathological features and Hp infection.The follow-up data of patients treated by Hp eradication therapy were analyzed.The survival curve was calculated with Kaplan-Meier,while the predictive factors for resistance to Hp eradiation were then analyzed by using Cox proportional hazards model.ResultsThe clinical manifestations and endoscopic features of 85 patients with gastric MALT lymphoma were non-specific.Hp positive was found in 97.9% (93/95) patients at presentation,and 36 patients were treated by Hp eradication with a median follow-up duration of ( 58.1 ± 29.9) months.Total remission was achieved in 94.4% ( 34/36),including 24 complete remission 10 partial remission.Therapy failure occurred in 2 patients.3-year survival rate of patients treated by Hp eradication were 86.2% (25/29). Cox multivariate analysis showed that age ( ≥60 years),multiple lesions and non-superficial lesions were independent predictors of resistance to Hp eradication therapy.ConclusionGastric MALT lymphoma is associated with Hp infection.Eradication of Hp can effectively induce remission in these patients.

9.Morphology and Immunohistochemistry of Sarcomatoid Carcinoma of the Fallopian Tube

Objective To investigate the pathologic morphology and immunohistochemical phenotypes of sarcomatoid carcinoma(SC) of the fallopian tube.Methods One case of SC of the fallopian tube was studied under the light microscopy for morphology.Immunohistochemistry was used to detect the expression of CK,EMA,S-100,Desmin,SMA,Leu-7,CD68,actin,and Vimentin in SC tissues.Pathologic features and biological behaviors of SC were analyzed in combination with the review of literature.Results Microscopically carcinomatous and sarcomatous elements co-existed.Usually the sarcomatous element formed the bulk of the lesion.In the sarcomatous tissues,there was no distinguishable bone,cartilage,rhadomyosarcoma,etc. Immunohistochemically the strong expression of CK7,EMA and Vimentin,and the partial expression of S-100,SMA,Leu7,CD68 and actin were detected.Conclusion SC of the fallopian tube is lack of specific symptoms,and its preoperative diagnosis is difficult.B-ultrasonography,CT and MRI are helpful to the staging of SC.Final diagnosis of SC depends on pathological examination and immunohistochemistry.SC is rare,and undergoes blood metastasis in early stage.Prognosis of SC is worst.Early detection of SC is the key to improve the prognosis.

Objective To investigate the characteristics,treatment and prognosis of the gastrointes-tinal iatrogenic perforation resulting from endoscopic operations.Methods The clinical data of the 107 pa-tients with gastrointestinal iatrogenic perforations caused by endoscopic operations from October 2003 to Octo-ber 2013 were retrospectively studied,and the characteristics,treatment and prognosis of these patients were analysed.Results The incidence of the gastrointestinal iatrogenic perforation resulting from endoscopic oper-ations was 0.041%(107 /263 549,among which 0.006% was diagnostic(13 /232 011),and 0.298% was therapeutic (94 /31 538).A total of 107 patients with gastrointestinal iatrogenic chose conservative medical management,endoscopic clipping or surgery according to different conditions after perforations.All patients had good prognosis.Conclusion Endoscopic therapeutic operations are more likely to lead to the occurrence of iatrogenic gastrointestinal perforations compared with diagnostic operations.And most patients could get good prognosis as long as the appropriate treatment is performed when the perforation occurs.
