1.Cost-effectiveness Analysis of Three Therapeutic Schemes for Sudden Deafness

OBJECTIVE: To evaluate three therapeutic schemes for sudden deafness with cost-effectiveness analysis for guiding rational use of drugs. METHODS:Using pharmacoeconomic cost-effectiveness analysis to evaluate 61 patients with 62ears of sudden deafness treated by Salvia Miltiorrhiza(groups A), Salvia Miltiorrhiza and cerebroprotein hydrolysate(groups B) or cerebroprotein hydrolysate(groups C) . RESULTS: The effective rates of three groups were 73.9%, 75.0% and 80.0% respectively without significant difference among them(P > 0.05) . The average drug costs for one patient in three groups were 617.89 yuan, 2 651.29 yuan and 2 647.99 yuan respectively. The costs for 1% increase of effective rate in three groups were 8.36 yuan, 35.35 yuan and 33.10 yuan respectively. CONCLUSION: Salvia Miltiorrhiza scheme is the most rational one for sudden deafness.
2.A case of large foreign body in hypopharynx and upper esophagus.

A case of large artificial teeth in hypopharynx and upper esophagus was reported. Physical signs of laryngeal obstruction were detected. CT showed foreign body in right pyriform sinus. Acute tracheotomy was performed before removal of the large foreign body with the help of esophagoscope and mouth-gag.

4.Synthetic Evaluation Method of Mental Workload on Visual Display Interface in Airplane Cockpit

Objective To establish a system to evaluate the mental workload on visual display interface in airplane cockpit after synthesizing three kinds of mental workload assessment techniques,i.e.main task measure method,physiology measure method and subjective evaluate method.Methods The evaluation indices of every kind of assessment techniques was defined,and the amount of each index through experiment and subjective inquiry was gained,weighted average was calculated and a quantification evaluation value of each kind of evaluation methods was got.At last,with weighted calculation of above three evaluation values,the evaluation value of mental workload on visual display interface in airplane cockpit was obtained.Results In M kinds of different designs of visual display interface in airplane cockpit,the synthetic evaluation value of one design was the smallest and the mental workload of this design was the smallest.The design of the smallest mental workload was chosen prior as the final scheme of visual display interface in airplane cockpit.Conclusion The evaluation system is combined with present main evaluation methods of mental workload and utilizes their advantages.The evaluating result can be quantified more directly and clearly.With this evaluation system,the mental workload can be evaluated during the design of visual display interface of airplane cockpit and the best scheme can be confirmed.
5.Human Movement Characteristics of Target Acquisition

Objective This paper deals with the problem of human movement characteristics of target acquisition. Method A hypothetical model was posed by using experimental data. Result The conception of final target size was put forward, an equation for calculating the movement time of target acquisition was obtained, and a new definition of index of difficulty was given. Conclusion Analysis of experimental data showed that this equation could give a better description of target acquisition in a wide range.
6.Clinical profile analysis of laryngeal nonsquamous cell malignancies.

OBJECTIVE:
To observe clinical characteristics of 15 cases of laryngeal nonsquamous malignancies.
METHOD:
The clinical data of 15 patients with laryngeal nonsquamous malignancies were collected. Details about pathology, CT, treatment and follow-up results were carefully taken into account.
RESULT:
There were more male patients than females while the sex ratio was 4:1. Supraglottic malignancies accounting for 53.33% seemed to be the most common type. In all the 15 patients, 1 patient gave up other treatment except tracheotomy to relieve laryngeal obstruction. Three patients took radiotherapy and chemotherapy. The rest patients underwent surgical procedure after strict endoscopic, radiologic staging. Radiotherapy and (or) chemotherapy were chosen after operation. The follow-up results show that 1 year survival rate was 79.4% and the 3 year survival rate was 57.8%, 5 year survival rate was 27.5% which was not optimistic.
CONCLUSION
Being a kind of rare type laryngeal nonsquamous malignancies have difference clinical characteristics with laryngeal squamous cell carcinoma. Laryngeal nonsquamous cell carcinoma have much lower 5 year survival rate and are easier to suffer recurrence and metastasis compared with laryngeal squamous cell carcinoma.

7.Expression of homosapiens chromosome segregation gene in homogenous nasopharyngeal carcinoma tissue cells

OBJECTIVE To study the expression of CSE1L gene sifted from the whole genome expression profiling in the aRNA from homogenous human nasopharyngeal carcinoma (NPC) and non-NPC biopsy tissue samples using semi-quantitative RT-PCR (sqRT-PCR). METHODS CSE1L gene was sifted from the whole genome expression profiling from homogenous NPC and non-NPC biopsy tissue samples. RNA later RNA Stabilization Reagent was used to preserve the tissue samples harvested from the nasopharynx of NPC and non-NPC patients. The samples were microdissected to get the homogenous tissue cells and then the total RNA was isolated from them. The antisense RNA (aRNA) was amplified from the total RNA by “in vitro transcription” (IVT). CSE1L gene expression in the homogenous tumor cells of NPC and the pure epithelial pillar cells of normal nasopharyngeal tissue was investigated by sqRT-PCR. RESULTS The high quality total RNA could be harvested from the microdissected homogenous tissue cells, and then the sufficient aRNA was amplified from it. The expression of CSE1L gene was identified using these aRNA by sqRT-PCR. There were significantdifference between the average expression value of CSE1L in NPC tissue (1.740?1.105) and in non-NPC tissue (0.618?0.183; df=30, t=3.159, P=0.004). The expression of CSE1L gene in the whole expression profiling were 1.056?0.296 in NPC tissue and 0.465?0.835 in non-NPC tissue respectively (df=16, t=4.317, P=0.001) . CONCLUSION The whole genome expression profiling with sqRT-PCR could be used to sift the marker genes from biopsy tissue samples. CSE1L may be as a candidate oncogene in NPC.

Objective To resolve the problems of comprehensive evaluation on pilot's situation cognition basing on studying the mission situation. Methods The model of fuzzy comprehensive evaluation on pilots' situation cognitive was established on the basis of researches on concepts and methods of environmental quality evaluation in environment science.Results On the basis of theoretical consideration and analysis of experiments, the state of pilot's situation cognition was revealed preliminarily.Conclusion It is showed that the fuzzy comprehensive evaluation method on pilot's situation cognition can provide real and complete flight cognitive information. It is reliable basis for pilot's efficacy of training and flight safety.
9.Thoracoscopic internal mammary artery harvesting approach in MIDCAB

Objective To analysis of preoperative preparation,operation,character,advantage and deficiency of two non-robotic TIMAH approach in MIDCAB,and to observe the short-term follow up result.Methods 7 male CAD patients with classic unstable angina pectoris,the age ranged from 52 to 75 years,average (63.8 ± 8.5) years,underwent TIMAH and MIDCAB,in which 6 single LAD disease patients and one patient for reoperation with saphenous vein graft (SVG) graft failure to LAD and progressive obtuse marginal(OM) coronary artery disease.These patients were intubated with a double-lumen endotracheal tube,and one-lung ventilation were used to facilitate the procedure.In the approach of two incisions TIMAH for 4 patients,the thoracoscope was placed at the third intercostals space(ICS) on the anterior axillary line,and LIMA was dissected with endo-instruments placed from the two angles of mini-thoracotomy at fifth ICS on the midclavicular line.In the approach of three incisions TIMAH for 3 patients,LIMA was dissected with endo-instruments placed from two ports at the fourth ICS on the anterior axillary line and at the fifth ICS on the midclavicular line,and the thoracoscope was placed at the second or third intercostals space (ICS) on the anterior axillary line.Anastomosis of LIMA and LAD followed through mini-thoracotomy at fifth ICS with the heart stabilizer after TIMAH.SVG graft was used from LIMA to OM in the reoperation patient.Results All patients underwent TIMAH and MIDCAB safely without transferece to stenotomy,only one LIMA was extended with 2 cm SVG for injure at the distal.In 6 single vessel disease patients the length of mini-thoracotomy incision was (6.0 ± 0.9) cm,TIMAH time was (112 ±18) min,operation time was (293 ± 75) min,bleeding volume was (233 ± 52) ml,endotracheal tube time was (14.2 ± 10.7) h,ICU time was (1.8 ±0.4) d and hospital stay time was (10.1 ±6.7) d.All patients were uneventful discharged and with no recurrence of cardiac symptoms in short-term follow up.Conclusion TIMAH can perform safely in both approaches for LIMA prepare to MIDCAB as described before.The minimally invasive procedure need not enlarge incision for LIMA harvesting with good short term results.
10.The current situation of willingness to receive prophylactic treatment among students with latent tuberculosis infection and its influencing factors in Jiangsu Province

Objective To assess the willingness of students with latent tuberculosis infection (LTBI) in Jiangsu Province to undergo preventive treatment and identify factors influencing their decision, aiming to provide insights for tuberculosis prevention and control strategies in school. Methods The physical examination information of tuberculosis latent infection cases was collected from screenings of new school enrollment and contacts of tuberculosis patients in 6 cities of Jiangsu Province from December 2022 to December 2023. Data on past medical history and understanding of preventive treatment were gathered through an online questionnaire survey on the website of Juanxing, and the influencing factors related to the willingness to take preventive medication were analyzed by logistic regression analysis model. Results In December 2022 to December 2023, a total of 13 school tuberculosis outbreaks occurred in 6 cities, and 1 661 contacts were screened, among which 162 cases met the criteria for prophylactic medication, 96 cases were included in the study by filling in the questionnaire. A total of 22 600 new students from 56 schools participated in the TB screening upon enrollment, of which 358 tested positive for the tuberculin skin test alone, meeting the criteria for preventive medication, and 251 of them completed the willingness survey. Finally, 347 students who met the criteria for preventive treatment were included in the study, with 164 expressing to accept preventive treatment representing a treatment acceptance rate of 47.3%. The results of multivariate analysis showed that university (OR=17.950, 95%CI: 3.078-104.686, P=0.001) and contact with the source of school tuberculosis epidemic (OR=19.542, 95%CI: 6.289-60.726, P<0.001) were associated with increased willingness to receive preventive treatment, while unclear whether to pay for the drugs themselves (OR=0.349, 95%CI:0.133-0.916, P=0.032) was associated with decreased willingness to receive preventive treatment. Compared with Huai'an City, the willingness to receive preventive treatment was significantly lower among students from Nantong City (OR=0.005, 95%CI:0.000-0.063, P<0.001), Nanjing City (OR=0.022, 95%CI: 0.003-0.703, P<0.001) and Lianyungang City (OR=0.074, 95%CI:0.008-0.703, P=0.023). Conclusions The acceptance rate of preventive treatment among LTBI students in Jiangsu Province is not high and is affected by multiple factors. Health education and medication mobilization for preventive medication are essential.
