1.Effects of ARB and β-blocker in heart function and left ventricular remodeling of Dilated Cardiomyopathy

ObjectiveTo investigate the effects of ARB or/and β-blocker in treatment of dilated cardiomyopathy(DCM).Methods95 cases with DCM were randomly divided into 3 groups.ARB group(A,32 cases) was treated with Irbesartan,150 mg/d without adverse drug reaction and then to 300 mg/d weekly; β-blocker group( B,30cases) was treated with Bisoprolol,1.25 mg/d without adverse drug reaction and then to maximal dose weekly; combination group( AB,33 cases) was treated with Irbesartan and Bisoprolol to maximal dose which patients were able to tolerate.The 3 groups were observed for a period of 12 months.ResultsComparing with pretreatment,the results of 6min walk,LVEDd,LVESd,LVEF were improved significantly( all P <0.05).There were no significant differences between A group and B group(P >0.05),but the differences between combination group and A group or B group were significant( P <0.05 ).ConclusionEffect of ARB combined with β-blocker was better than that of ARB or β-blocker alone in treatment of DCM and well tolerated.
2.Correlation between the serum ferritin levels and the post-stroke depression at 3 months after ischemic stroke

Objective To investigate the correlation between the serum ferritin levels and the post-stroke depression (PSD). Methods From July 2014 to October 2015, the inpatients with the first-ever acute ischemic stroke were colected consecutively. Chemiluminescence microparticle immune assay was used to measure the serum ferritin levels within 24 h after admission. Depressive symptoms were screened by using the 17-item Hamilton depression scale (HAMD-17) at 3 months after onset. In patients with a HAMD-17 score ≥7, the depression was further diagnosed according to The Diagnostic and Statistical Manual of Mental Disorders, 4th edition. Results A total of 200 patients with the first-ever acute ischemic stroke were enroled, 55 (27. 5% ) of them were diagnosed as PSD. There were significant differences in the body mass index (BMI), years of education, waist circumference, high sensitive-C-reactive protein, homocysteine, National Institutes of Health Stroke Scale score (at baseline, discharge, and day 90), mRs score (at discharge and day 90), BI (at discharge and day 90), and the proportions of widowed or solitary patients between the PSD group and the non-PSD group (al P < 0. 05 ). The serum ferritin level in the PSD group was significantly higher than that in the non-PSD group ( median [ interquartile range], 261. 90[142. 10-364. 90] μg/L vs. 164. 40[132. 50- 195. 10] μg/L; Z = - 4. 814, P < 0. 001 ). Multivariate logistic regression analysis adjusted for confounding factors showed that the baseline serum ferritin level >136. 375 μg/L was an independent risk factor for PSD (odds ratio 1. 041 per 1-quartile increase, 95%confidence interval 1. 009-1. 239; P = 0. 045). Conclusions The elevated baseline serum ferritin level is associated with PSD.
3.Determination of evodiamine and rutaecarpine of compound Wuzhuyu cataplasm in plasma by SPE-HPLC: application to its pharmacokinetics.

OBJECTIVETo establish a SPE-HPLC method for the determination and pharmacokinetic study of evodiamine and rutacarpine in rat plasma.
METHODA Kromasil C18 column (4.6 mm x 250 mm, 5 microm) was used with acetonitrile-water-tetrahydrofuran-acetic acid (51:48:1:0.1) as a mobile phase and at a flow rate of 1 mL x min(-1), and the UV detection was at 225 nm. The column temperature was 35 degrees C. After the analytes were extracted from the plasma of rats by solid phase extraction (SPE), the content of evodiamine and rutaecarpine was measuared by HPLC method using halcinonide as an internal standard solution.
RESULTAfter transdermal administration to rats, the pharmacokinetic behavior of evodiamine and rutaecarpine belongs to the one-compartment model. The main pharmacokinetic parameters was as follows: K(a) 0.224 h(-1) and 0.220 h(-1), K(e) 0.114 h(-1) and 0.118 h(-1), C(max) 0.211 mg x L(-1) and 0.272 mg x L(-1), T(peak) 6.132 h and 6.102 h, respectively.
CONCLUSIONThe method in this study is simple, rapid and sensitive. It is proved to be suitable for pharmacokinetic study of evodiamine and rutaecarpine.
