1.The clinical features of bone lesions in early congenital syphilis

Objective To evaluate the clinical and X-ray features of bone lesion in early congenital syphilis. Methods Between 1997 and 2005, 48 cases of early congenital syphilis were diagnosed. Among those 48 cases, there were 16 cases with bone lesions detected. The clinical and radiographic features of these 16 cases of bone lesion in early congenital syphilis were retrospectively analyzed. There were 10 males and 6 females with an age between 25-57 d. All patients were confirmed with serology and radiographs of limbs. They were given Penicilin G of 15-20 WU?kg-1?d-1, 3 times a day in a total of 14 days for one course of treatment. Results Rapid plasma reagent test(RPR) of all the cases was positive and they were 3 cases with 1∶128, 2 cases with 1∶64, 6 cases with 1∶32, 3 cases with 1∶16 and 2 cases with 1∶8. All the cases were positive for TPPA. 30 parents of 16 cases were tested for RPR and TPPA. The results showed that 16 mothers were positive for RPR, 13 fathers were positive for RPR. All the parents were positive for TPPA, and negative for HIV test. The main clinical features included active disorder, swelling of involved limbs, malnutrition, light body weight and multi-organ functional lesion. The main X-ray features were metaphysitis, periostitis and osteomyelitis involving all of the long bones of the extremities. Widen of the junction between epiphysis and diaphysis was the characteristic features of early congenital syphilis. After treatment, RPR became negative and sclerotic lesion was recovered and only remain periosteum proliferation. Hutchinson teeth were seen in some patients. Conclusion Bone lesions in early congenital syphilis has its characteristic clinical and radiographic features. The bone lesion could be cured after effective treatment.

PRP (Platelet-rich plasma) is an autologous concentrate of platelets extracted from the whole blood,which has the ability of hemostasis,relieving pain and accelerating wound healing.PRP can promote tissue repairing effectively,because it not only provides concentrated nutrition,but also builds a better microenvironment.With the wide spread of PRP,it has been gradually applied to the treatment of tendon,ligament,cartilage and muscle injury in orthopedics.And meanwhile,with the rapid development of foot and ankle surgery in recent years,such applications have been introduced into this area,reports of therapeutic effects of foot and ankle disorders are also emerging.This paper makes a summary of the clinical studies of foot and ankle disorders with PRP.
3.Open reduction and Y plate internal fixation to treat intra-articular calcaneal fractures

Objective To observe and analyze the outcomes o f open reduction and Y plate internal fixation in treatment of intra-articular calcaneal fractures.Methods Open reduction and Y plate internal f ix-ation by the extended L-shaped lateral approach were performed in 68pati ents with 82cases of intra-articula r calcaneal fractures under X-ray.Results82cases were followed up for an avera ge of 26months,and the clinical results were evaluated wit h the Maryland Foot Score.58cases ac hieved excellent results,16good,6fair,with the rate of excellent and good re sults being 90.24%.Conclusion Open reduction and Y plate internal fixation can obtain good clinical re sults in treatment of intra-articul ar calcaneal fractures.[
4.The diagnostic value of symptom index in gastroesophageal reflux-induced chronic cough

Objective To explore the diagnostic value and optimal cut-off point of symptom index (SI) in gastroesophageal reflux-induced chronic cough (GERC).Methods The recordings of multichannel intraluminal esophageal impedance and pH monitoring were retrospectively analyzed in 118 patients with suspicious GERC.SI for all the refluxes,acid reflux and non-acid reflux was calculated respectively by analyzing the temporal association between detected reflux and cough recorded on diary card.Based on the favorable response to the anti-reflux therapy,the diagnostic value for GERC of SI was evaluated and compared with that of the symptom association probability (SAP).Results GERC was definitely determined in 100 patients (84.7%).When SI for all the refluxes was used for the diagnosis of GERC,the cut-off point of ≥45% had the highest diagnostic efficacy,with the sensitivity of 56.0%,the specificity of 83.3%and Youden index of 0.393.SI for acid or non-acid reflux had the same optimal cut-off point of ≥30% and presented with the similar efficacy in the diagnosis of acid or non-acid GERC.Compared with SAP of ≥75%,SIforall the refluxes of ≥45% had a lower sensitivity (56.0% vs 75.0%,x2 =7.988,P=0.005),a higher specificity (83.3% vs 44.4%,x2 =5.900,P =0.015) and the comparable positive or negative predictive value in the diagnosis of GERC.The diagnostic accuracy for GERC was further improved when combining SI for all the refluxes with SAP.Conclusion SI for all the refluxes has a diagnostic value similar to SAP and its optimal cut-off point for GERC may be ≥45%.
5.Determination of ginsenoside Rb_1,Re,Rb_2 and Rd in Shenqi Granules by microbore liquid chromatography

AIM: To develop a method for determining ginsenoside Rb_1,Rc,Rb_2 and Rd in Shenqi Granules (Radix Rhizoma Ginseng,Radix Astragali,Rhizoma Atractylodis macrocephalae,Poria,etc.) by microbore LC.METHODS: Microbore liquid chromatography was applied.Microsil C_(18) column( 150 mm×1.0 mm) was used with the mobile phase containing acetonitrile-water for gradient elution.The flow rate was 50 μL/min,and the detection wavelength was set at 220 nm.RESULTS : The relationships between the concentrations and the peak areas of these for components were all linear.The recoveries were 98.14% for ginsenoside Rb_1,98.04% for ginsenoside Rc,98.79% for ginsenoside Rb_2,and 97.97% for ginsenoside Rd respectively.The relative standard deviations (RSD) were 0.67%,0.99%,0.75%,and 1.46%,respectively.CONCLUSION: This method is simple,convenient and can be used for quality control.

7.The Effect of Tiaozhixin NO/ET、6-PGF_2/TXB_2 Hemorrhedoge of Rats with Hyperlipoidemia

Objective: To study the influences of Tiaozhixin(TZX) on NO/ET, 6 keto PGF 1a /TXB 2 and hemorrheology rats with hyperlipoidemia. Methods: The rat hyperlipoidemia and early atheroscleorsis models were established by feeding high lipid diet for 40 days. Meanwhile TZX was taken by oral administration at the dosages of 40、80g/kg. The levels of NO、ET 1、6 keto PGF 1a and TXB 2 in serum were determind, and the hemorrheology markers were observed. Results: TZX could raise the levels of NO in serumobviourly ET 1 level of the normal and model rats. The large dose of TZX could increase 6 keto PGF 1a content remarkably which benefits maintenance of the balances of 6 keto PGF 1a /TXB 2. It could lower the whole blood specific viscosity, whole blood reduction specific viscosity, plasma specific viscosity; aggregation index of RBC; shorten RBC electrophoresis time; also decrease fibrinogen content; inhibit the platelet aggregation of normal rats induced by ADP. Conclusion: TZX can improve the abnormal hemorrheology and recover the balance of TXB 2/6 keto PGF 1a and NO/ET of rats with hyperlipoidemia, which might be one of mechanisms of antiatherosclerosis action.
8.Antibacterial Activity of Pazufloxacin in vitro Against Bacteria Isolated from Patients with Respiratory and Urinary Tract Infection

OBJECTIVE To study the antibacterial activity of pazufloxacin in vitro against bacteria isolated from(patients) with respiratory and urinary tract infection,compared with levofloxacin,ciprofloxacin,cefuroxime and(cefotaxime) to provide guideline for clinical application.METHODS The infected bacteria were identified by the(fully) automatic VITEK system.The susceptibility test was determined by MIC method and K-B disk diffusion method respectively.RESULTS The sensitivity of most Gram-negative bacteria to pazufloxacin was basically the same to levofloxacin,cefotaxime, etc.However,the sensitivity of Pseudomonas and Acinetobacter to(pazufloxacin) was higher than to other four antibiotics;the sensitivity of Gram-positive cocci,particularly(Streptococcus) to(pazufloxacin) was obviously lower than cephalosporins and no better than other quinolones.(CONCLUSIONS) The antibacterial activity of pazufloxacin in vitro against the bacteria isolated from patients with(respiratory) and urinary tract infection is comparatively strong,especially in the urinary system.Pazufloxacin has an excellent broad-(spectrum) and strong antibacterial activity.However,the antibacterial activity against Gram-positive cocci is(expected) to promote.
9.Phosphodiesterase 3 inhibitor attenuates cough response in a sensitized guinea pig model

AIM: To investigate the effects of selective phosphodiesterase 3 inhibitor olprinone on cough response in guinea pigs sensitized and challenged with ovalbumin. METHODS: Forty sensitized guinea pigs were randomly divided into control ( n = 10), challenged ( n = 10), olprinone ( n = 10) and aminophylline group ( n = 10 ). Two hours after challenged with the aerosol of 1% ovalbumin or saline, animals were intraperitoneally injected either with saline,25 mg/kg of olprinone or 25 mg/kg aminophylline. At 24 h, the injection was repeated with 2. 5 mg/kg and 25 mg/kg olprinone or 2. 5 mg/kg and 25 mg/kg aminophylline respectively in olprinone and aminophylline group, cough response to inhaled capsaicin and airway responsiveness to methacholine (PC150) were measured. Then, total cell number and differential counts were analyzed in bronchoalveolar lavage fluid. RESULTS: The cough frequency was (5 ± 2) times/3 min in control group and (24 ± 3 ) times/3 min in challenged group ( P < 0. 05 ), while PC150 was (659 + 57 ) mg/L in control group and (238 + 67 ) mg/L inchallenged group ( P < 0. 05 ). 25 mg/kg olprinone significantly inhibited the augmented cough response and airway hyperresponsiveness, the cough frequency and PC150 were (15 ±2) times/3 min and (580 ±45) mg/L (P < 0. 05 ), which differed significantly from (18 ± 2) times/3 min and (438± 52) mg/L in aminophilline group (P < 0. 05). However, olprinone failed to reverse the elevated total cell number and percentage of eosinophils in bronchoalveolar lavage fluid from guinea pigs challenged with ovalbumin (P > 0. 05 ). CONCLUSION: Phosphodiesterase 3 inhibitor attenuates cough response associated with eosinophilic airway inflammation by bronchodilatory effect.
10.Biphasic effects of propofol on the percentage of alpha2-band power of quantitative pharmaco-electroencephalography of rabbits

BACKGROUND: Quantitative pharmaco-electroencephalography (QPEEG) can reflect cerebral cortical function, which can be certainly affected by general anesthetics. Anesthesia depth has good correlation with the anesthetic dosage, so if we can find out the areas of brain and band of QPEEG which is relative to the anesthetic dosage, the band may be taken as the index to reflect the depth of anesthesia. OBJECTIVE: To observe the effects of propofol on the alpha2-band (α2- band) of QPEEG in rabbits. DESIGN: A randomized control animal experiment. SETTING: Jiangsu Provincial Key Laboratory of Anesthesiology, Xuzhou Medical College. MATERIALS: The experiment was carried out in the animal laboratory of Xuzhou Medical College from October 2004 to August 2005. Thirtysix healthy adult rabbits were randomly divided into propofol 2.5, 5 and 10 mg/kg groups with 12 rabbits in each, including 6 were used to observe the change of percentage of each band power of QPEEG, and the other 6 were used to observe the latency and duration for the disappearance of righting reflex in the rabbits. METHODS: The experiment was performed between 14:00-17:00 every day. Rabbits in the three groups were treated with intravenous injection of propofol of 2.5, 5 and 10 mg/kg respectively within 30 seconds. ① The conscious rabbits were fixed onto the platform in a prone osition, and the QPEEG was recorded with the method of power spectrum analysis before administration and at 20, 30, 40, 50, 60, 70, 80, 90, 100 and 110 s and 2, 5, 10, 15, 20 and 30 minutes after administration respectively. The sampling time for each time point was 5 s. ② The latency and duration for the disappearance of righting reflex in the rabbits were recorded. RESULTS: ll the 36 rabbits were involved in the analysis of esults. ① After the intravenous injection of propofol, the righting reflexes all disappeared within 1 minute. The greater the dosage, the shorter the latency and the longer the duration r=0.79, P < 0.01). ② Compared with before administration, propofol of 2.5 mg/kg had no obvious influence on the percentage of α2-band power (P > 0.05); The percentages of α2-band power in the brain areas were increased after administration in the propofol 5 mg/kg group (P < 0.05); Except that there were no significant differences in the left and right parietal regions between the propofol 10 mg/kg group and the propofol 5 mg/kg group, the percentages of α2-band power in the other brain areas in the propofol 10 mg/kg group were decreased as compared with those before administration and those in the other two groups (P < 0.05), and the changes above were more obvious in the frontal and temporal regions.CONCLUSION: The influence of propofol on the percentage of α2-band power of QPEEG is biphasic, it is suggested that α2-band would be an index to reflect the anesthesia depth of propofol.
