Objective: To investigate the efficacy and safety of clinical use of sodium nitroprusside.Methods: The data of 3894 hospitalized patients who had ever been treated with sodium nitroprusside between 1995 and 2004 in our hospital were retrospectively collected.The therapeutic dose,time of therapy,effective rate and side effect were assessed.Results: The minimum time of continuous infusion with sodium nitroprusside was 0.5hour,the maximum 368 hours,average 125.5?24.2 hours.The minimum dose of sodium nitroprusside infusion was 0.2?g/(kg?min),the maximum 9.8?g/(kg?min),average 3.1?1.9?g/(kg?min).The minimum dose of continuous infusion was 2.5mg,the maximum 2050mg,average 210?26.7mg.Among the 3894 patients,3524 cases were effective,accounting for 90.4%;278 cases were ineffective,accounting for 7.1%;95 cases were death,amounting 2.4%,among which the cases caused by ssodium nitroprusside toxicity accounted for 0.02%.The total incidence of caused by sodium sodium nitroprusside toxicity accounted for 0.02%.The total incidence of common side effect was 13.7%.The incidence of hypotention,rebound and tolerance were 10.2%,3.2%,and 0.3% respectively,which might be prevented by some protective measures.Conclusion: It is effective and safe for patients to undergo treatment with sodium nitroprusside.Sever toxicity is rare.Although the common side effect is frequent,but it can be controlled.
3.Effect of uric acid reduction on blood pressure in hypertensive patients with asymptomatic hyperuricemia

Objective To investigate the effect of the reduction of serum uric acid(SUA) on blood pressure(BP) control and re‐nal function protection in hypertensive patients with asymptomatic hyperuricemia (HUA) .Methods One hundred and forty‐eight hypertensive patients with asymptomatic HUA were randomly divided into two groups ,namely HUA reduction group and control group .Patients in HUA reduction group was provided with benzbromarone and low purine food ,while patients in control group was given placebo and ordinary diet .All patients were given the same method of antihypertensive drugs .Follow up was preformed for 6 months ,data of BP ,SUA ,creatinine(Cr) ,creatinine clearance(Ccr) ,24 h microalbuminuria quantity(MAU) ,serumβ2 microglobulin (β2‐MG)and adverse reactions were recorded and analyzed .Results The systolic blood pressure(SBP) and diastolic blood pressure (DBP) of patients in HUA reduction group significantly decreased immediately after being treated ,while SUA decreased 1 month after being treated .Compared with control group ,the average SBP and DBP of patients in HUA reduction group was significantly lower 3 months after being treated;the SUA was significantly lower 1 months after being treated ,the levels of Cr ,Ccr ,MAU andβ2‐MG was significantly lower at 3 months and 6 months after being treated (P<0 .05) .Conclusion The SUA therapy can effec‐tively lower BP and SUA levels of primary hypertensive patients with asymptomatic HUA while protecting renal function .
4.Multiple linear stepwise regression of fiver lipid levels: proton MR spectroscopy study in vivo at 3.0 T

Objective To analyze the correlations between liver lipid level determined by liver 3.0 T 1H-MRS in vivo and influencing factors using multiple linear stepwise regression. Methods The prospective study of liver 1H-MRS was performed with 3.0 T system and eight-channel torso phased-array coils using PRESS sequence. Forty-four volunteers were enrolled in this study. Liver spectra were collected with a TR of 1500 ms ,TE of 30 ms, volume of interest of 2 cm ×2 cm ×2 cm, NSA of 64 times. The acquired raw proton MRS data were processed by using a software program SAGE. For each MRS measurement, using water as the internal reference, the amplitude of the lipid signal was normalized to the sum of the signal from lipid and water to obtain percentage lipid within the liver. The statistical description of height, weight, age and BMI, Line width and water suppression were recorded, and Pearson analysis was applied to test their relationships. Multiple linear stepwise regression was used to set the statistical model for the prediction of Liver lipid content. Results Age (39.1 ± 12. 6) years, body weight (64.4 ± 10. 4) kg,BMI (23.3 ±3.1) kg/m2, linewidth (18.9 ±4.4) and the water suppression (90.7 ±6.5)% had significant correlation with liver lipid content (0.00 to 0.96%, median 0. 02% ), r were 0.11,0. 44,0. 40,0. 52, - 0. 73 respectively(P < 0. 05 ). But only age, BMI, line width, and the water suppression entered into the multiple linear regression equation. Liver lipid content prediction equation was as follows: Y =1.395-(0.021 × water suppression) + (0.022 × BMI) + (0.014 × line width) - ( 0. 064 × age),and the coefficient of determination was 0.613, corrected coefficient of determination was 0.59. Conclusion The regression model fitted well, since the variables of age, BMI, width, and water suppression can explain about 60% of liver lipid content changes.
5.Effect of body mass and BMI on proton hepatic MRS water suppression at 3.0T MR

Objective To explore the effect of body mass and body mass index (BMI) on proton hepatic MRS water suppression at 3.0T. Methods The prospective study of hepatic proton MRS was performed with GE Signa Excite HD 3.0T system and eight-channel torso phased-array coils using PRESS sequence in 44 healthy subjects. Liver spectra were collected with TR of 1500 ms, TE of 30 ms, VOI of 2 cm×2 cm×2 cm and NSA of 64 times. Areas and heights for metabolites resonances were caulculated. Results Group with small mass has lower height ([161.2±8.5] cm vs [167.7±6.2])cm, lower BMI ([20.8±2.3] kg/m~2 vs [25.6±2.6]kg/m~2), better water suppression effect (min-max: 90-96 vs 65-94;median: 94 vs 93), smaller height (min-max: 1.41×10~4-5.76 ×10~5 vs 3.45×10~4-1.75×10~6;median: 9.00×10~4 vs 2.58×10~5) and integrated area (min-max: 4.27×10~4-2.00×10~7 vs 1.24×10~5-5.00×10~7;median: 2.64×10~5 vs 1.19×10~6)of Lip2 than larger weight group. Standardized lipid content (min-max: 0-0.11 vs 0-0.96;median: 0.01 vs 0.04) was less. Group with lower BMI had lower weight ([55.2±8.2]kg vs [71.2±7.8]kg), smaller age ([33.2±11.9]years vs [45.6±9.4]years), better water suppression effect(min-max: 90-96 vs 65-95;median: 94 vs 93) smaller of height (min-max: 1.41×10~4-5.76×10~5 vs 3.45×10~4-1.75×10~6;median: 7.37×10~4 vs 2.11×10~5) and integrated area (min-max: [4.27×10~4-2.00×10~7] vs [1.24×10~5 -5.00×10~7];median: 2.64×10~5 vs 1.19×10~6) of Lip2 than larger weight group. Standardized lipid content (min-max: 0-0.08 vs 0.01-0.96;median: 0.01 vs 0.04) was less. There was significant correlation among water suppression, weight (r=-0.478, P=0.001) and BMI (r=-0.494, P=0.001). Conclusion Lipid accumulation in the liver may be the result of increased fat portion of the body depending on mass and BMI, and hinder to achieve effective water suppression.
6.Effect of intravenous Gd-DTPA on 3.0T MR spectroscopy prescan of different organs

Objective To evaluate the effect of intravenous Gd-DTPA on 3.0T proton MR spectroscopy (MRS) water suppression and shimming. Methods Prospective study of proton MRS was performed with GE Signa Excite HD 3.0T system and eight-channel phased-array coils with PRESS sequence (head, liver and kidney, respectively). Routine auto prescan program was operated to record full width half maximum (FWHM) and water suppression (WS%). Routine scan was performed after injection of Gd-DTPA, then prescan program was reoperated to record FWHM and WS%. The data of FWHM and WS% in head, liver and kidney were compared between before and after injection of Gd-DTPA with the Wilcoxon matched pairs signed test. Results WS% of spectroscopy of head and liver after administration of Gd-DTPA decreased significantly (T_+=12, T_-=66, P=0.02; T_+=0, T_-=45, P=0.007). The effect of shimming of kidney after administration of Gd-DTPA was poor (T_+=0, T_-=435, P<0.001) and WS% of spectroscopy of kidney after administration of Gd-DTPA decreased significantly (T_+=0, T_-=435, P<0.001). Conclusion WS% of spectroscopy in head, liver and kidney can be impacted negatively by Gd-DTPA. Gd-DTPA has great influence on shimming of spectroscopy of kidney, but has little influence on shimming of spectroscopy of head and liver. It is better to acquire MRS data before administration of contrast medium in kidney.
7.Effect of laser-assisted hatching on birth weight in blastocyst cultureof low-grade cleavage embryos

Objective:
To examine the impact of laser-assisted hatching (LAH) technique on perinatal outcomes in blastocyst culture of low-grade cleavage embryos, so as to provide insights into improving the utilization of low-grade cleavage embryos and embryo quality.
Methods:
A total of 369 single live births after transfer of thawed blastocysts following in vitro fertilization or intracytoplasmic sperm injection at Zhejiang Provincial People's Hospital were selected as subjects, and they were divided into 51 conventional culture blastocysts and 318 LAH blastocysts based on whether LAH was performed on day 4 of blastocyst culture. Gestational age, birth weight, birth defects and maternal perinatal information were collected, and the prevalence of premature birth, birth weight and birth defects were analyzed after propensity score matching (PSM).
Results:
After PSM, 98 matched cases were included in the study. There were no statistically significant differences in maternal age, body mass index, type of infertility and blastocyst age (P>0.05), indicating effective matching. The birth weight of offspring in the LAH group was lower than that in the conventional culture group [(3 261.08±432.24) g vs. (3 464.18±444.46) g; P<0.05].
Conclusion
The birth weight of offspring can be reduced by using LAH during blastocyst culture of low-grade cleavage embryos.
8.Influences of escitalopram on cognitive function and cardiac function in elderly patients with chronic heart failure complicated with depression disorder

Objective To explore the short-term influences of conventional internal medical treatment combined with escitalopram on cognitive function and cardiac function in elderly patients with chronic heart failure (CHF) complicated with depression disorder.Methods A total of 97 patients with CHF complicated with depression disorder in Chongqing Ninth People's Hospital,from October 2014 to October 2015,were selected and randomly divided into the antidepressant group and control group.Both groups were undergoing conventional internal medical treatment.Additionally,patients in the antidepressant group were administrated with escitalopram,while patients in the control group were treated with placebo.The degree of depression and anxiety and cognitive function were assessed by using 24-item Hamilton depression scale (HAMD-24),14-item Hamilton anxiety scale (HAMA-14) and Montreal cognitive assessment(MoCA) scale,and the plasma level of NT-proBNP,left ventricular end-diastolic dimension (LVEDD) and left ventricular ejection fraction (LVEF) were measured before and after 6-week treatment.Results After 6-week treatment,the HAMD-24 and HAMA-14 scores and plasma level of NT-proBNP in antidepressant group were lower than those in the control group,while the attention score and LVEF were greater than those in the control group,there were statistically significant differences(P<0.05).After 6-week treatment,no statistically significant difference was found in other observed indicators between the two groups(P>0.05).Conclnsion For elderly patients with chronic heart failure complicated with depression disorder,it is indicated that escitalopram could not only relieve their anxiety and depression,but also improve their cardiac function and attention.
9.Changes of serum soluble apoptosis factors and their relations with cognition disorders in the patients with vascular dementia

Objective To investigate the changes of serum soluble factor-related apoptosis (sFas) and soluble Fas ligand (sFasL) and their relations with cognition disorders in the patients with vascular dementia (VaD). Methods Serum concentrations of sFas and sFasl were detected by enzyme-linked immunosorbent assay (ELISA) and compared between 70 patients with VaD aged (72.5± 7.5)years and 50 healthy elderly people aged(72.5 ± 7.5)years.The VaD patient's cognitive functions were evaluated by activity of daily living scale (ADL),mini mental state examination (MMSE) and hachinski ischemia score (Hachinski). Results The serum levels of sFas and sFasL in VaD patients were (228.0±60.7)μg/L and (146.8±30.1)μg/L,and in the healthy elderly were (62.4±22.6)μg/L and (82.3 ± 18.7)μg/L,respectively.The serum levels of apoptosis factors in VaD patients were significantly higher than in the healthy controls (t=20.883,14.453,P<0.01).sFas level was negatively correlated with age,the scores of ADL and Hachiuski while positively with the scores of MMSE (r=-0.956,-0.943,-0.950 and 0.904,all P<0.01). sFasL level was negatively correlated with the scores of MMSE while positively with age,the scores of ADL and Hachinski (r=-0.899,0.963,0.948 and 0.939,a11 P<0.01). Conclusions Apoptosis may be involved in the pathological change during VaD and the serum levels of sFas and sFasL might be related with cognition disorders.
10.Magnesium sulfate combined with compound polyethylene glycol electrolyte in the bowel preparation before capsule endoscopy

ObjectiveTo evaluate magnesium sulfate and compound polyethylene glycol electrolyte in bowel preparation before capsule endoscopy.MethodsA total of 81 patients were randomly divided into two groups.Group A (n =41 ) received compound polyethylene glycol electrolyte,and group B (n =40) received magnesium sulfate combined with compound polyethylene glycol electrolyte. Positive detection rate,intestinal preparation score,the incidence of capsule retention and solution quality,liver and kidney function were compared between those two groups.ResultsThe positive detection rates of capsule endoscopy were 65.9% (27/41) in group A and 62.5% (25/40) in group B,which were nonsignificantly different (x2 =0.099,P > 0.05).The intestinal preparation scores of 19 patients in group A were ≤4 and those of 22 patients were ≥6,while those of 28 patients in group B were ≤4 and of 12 patients were ≥6,which were significantly different (x2 =4.653,P < 0.05). The rates of capsule retention of group A and B were 2.4%( 1/41 ) and 5.0% (2/40) respectively,which were not significantly different (x2 =0.372,P > 0.05 ).Obvious abdominal pain,nausea or vomitting occurred in no patients of the two groups.There was no difference in electrolyte level or liver and kidney function between those two groups (P > 0.05 ).ConclusionMagnesium sulfate combined with compound polyethylene glycol electrolyte is applicable for the bowel preparation before capsule endoscopy.
