1.Discussion of the concept of the modernized hospital and its appraisal framework

By summarizing the results of research on the modernized hospital by the academic circles and using system theories and expert consultations, the paper proposes the technical lines for constructing a framework of modernized hospital appraisal and defines the basic concept of the modernized hospital and its appraisal dimensions, arguing that a humanized, intelligence- and science-based, law-driven, and environment-friendly hospital constitutes the basics of the modernized hospital. At the same time the paper further clarifies several relationships that need to be well handled in appraising the modernized hospital on the basis of theoretical systems.

Hospital modernization is closely related to hospital information systems.The paper presents the notion of information flow and argues that the quality,speed,coverage,and efficiency of information flow in a hospital are the basic elements of its development.Hospital information systems provide systematic,primary and service support for various activities in the hospital and bring about changes in the essence of hospital culture.In their efforts to strengthen their information infrastructure,modernized hospitals should observe the principles of people first,cost-effectiveness balance and sustainable development and lay equal stress on cultivating specialized information personnel and improving the level of the entire staff in manipulating information and science and technology.
4.The ten essential elements about building the improving long-term mechanism of modern medical quality

Medical quality management is the core of hospital management.Concerning how to build the improving long-term mechanism of modem medical quality,the article is guided with the theory of modern medical quality management,and tested with the reform of medical quality management about one hospital.It puts forward 10 essential elements on how to construct the long-term mechanism on improving the medical quality,which is based on consulting all kinds of experts.Namely,“qualifications,responsibilities,complete path,reverence for life,reverence for integrity,reverence for rules,value on evaluation,value on case history,value on sum-up,value on teamwork”.Then it will take active effect on hospital workers and hospital managers in strengthening medical quality control,and improve the medical quality persistently.
5.Expression of type Ⅰ transforming growth factor beta receptor and type Ⅰ collagen in the peripheral and central area of human skin hypertrophic scar

BACKGROUND: Many studies focus on transforming growth factor β (TGF β) and its receptors, however, the distdbution of type Ⅰ TGF receptor (TGF-βR Ⅰ) in peripheral region of hypertrophic scars remain poorly understood. OBJECTIVE: To determine the expression and distribution of TGF-βR Ⅰ and type Ⅰ collagen in the peripheral and central areas of human skin hypertrophic scar. METHODS: A total of 30 cases with human cutaneous scars admitted at the Department of Plastic Surgery, First Affiliated Hospital and Department of Mammary Gland, Head and Neck Surgery, Tumor Hospital of Xinjiang Medical University from 1999 to 2002, were selected, including 20 cases with hypertrophic scar and 10 cases with normal scars. A total of 180 scars were obtained from central and peripheral areas of scars as well as normal skin tissues. The protein contents of TGF-βR1 and type Ⅰcollagen was detected by immunohistochemistry. In addition, the immunostaining positive in these samples was analyzed by semiquantitative analysis. RESULTS AND CONCLUSION: Compared to non hypertrophic scar and normal skin tissues, the TGF-βR1 expression of hypertrophic scar was obvious greater with strong positive reaction. The TGF-β R Ⅰ content was 100% in peripheral region of hypertrophic scar, which was notably 20% greater than that of central area (P < 0.05). The content of type Ⅰ collagen was both 100% in peripheral and central areas. The differences of positive TGF-β R Ⅰ and type Ⅰ collagen had no significance between peripheral and central areas of non hypertrophic scars (P > 0.05). There were few contents of TGF-βR Ⅰ and type Ⅰ collagen in normal skin tissues. The expression of TGF-β R Ⅰ is higher in peripheral than central areas of hypertrophic scar. Therefore, the peripheral area would be emphasized in the clinic work.
6.The clinical significance of peripheral blood T cell subsets,NK cells in NHL

Objective:To investigate the relationship of the changes of peripheral bloos T cell subsets and NK cell with non-Hodgkin's lymphoma(NHL).And to investigate the change of the cell immune function of NHL and chronic lymphadenitis.Methods:Peripheral blood T cell subsets,NK cell with NHL,chronic lymphadenitis patients and 50 normal controls were detected quantitatively with FCM.Results:Compared with the normal controls,the number of CD3~+,CD4~+T cell,CD4~+/CD8~+ ratio markedly reduced,while those of CD8~+T cells increased.These were no significant changes in NK cell.Compared with the group of chrnic lymphadenitis,the number of CD8~+T cell,NK cell markedly increased,while these were no significant changes in CD3~+,CD4~+T cell and reduction of CD4~+/CD8~+ ratio.Conclusion:NHL patients cell function was suppressed.The detection of T cell subsets and NK cells are clinically valuble on the diagnosis,treatment and prognosis of NHL.

8.Analysis of qualified military flying cadets by comprehensive assessment

Objective To provide an effective theoretical basis for future selection and recruitment of military flying cadets.Methods Two hundred military qualified flying cadets by comprehensive assessment samples and 200 military flying cadets by medical fitness samples were interviewed with questionnaires and subjected to a physical examination.The comprehensive assessment samples were classified by symptoms and attributive specialties for statistical analysis.A database was established, cleaned and analyzed by EpiData 3.02, SPSS 16.0 with double checking.Results There was no differ-ence between the comprehensive assessment team and the medical fitness team in age, areas, academic scores and psycho-logical performance.The ten top symptoms according to the comprehensive assessment were abnormal chest circumference, abnormal body mass,iris cyst,abnormal hypersplenotrophy,gallbladder polyps,flat foot,skin disease, intrahepatic calcifica-tion focus, joint disease and reproductive system disease.The attributed specialties included surgery, ultrasonics, ophthal-mology,ENT,radiology, ECG and internal medicine departments.The surgery and ultrasonics department contributed the most from the comprehensive assessment and accounted for 55.52%and 16.37%, respectively.Conclusion The compre-hensive assessment for military flying cadets has an important effect on military aviator selection.Continuous observation should be carried out among the military flying cadets by comprehensive assessment in their aviation training.
9.Value of Vienna Test System in psychological screening of flying cadets

Objective To study the applicability of Vienna Test System ( VTS) to psychological screening of flying cadets,and to establish a corresponding standard model for flying cadets .Methods VTS and pilot psychological selection system were used to evaluate the mental health of 134 cases of flying cadets , SPSS 13.0 was used to compare their consistency and relevance.Results The results of VTS included the raw score of overall mean duration [(46.68 ±21.06)s] on 2hand test, the correct reaction times (246.54 ±25.88) on DT test, the normal values of overall mean duration [(27.84 ±21.98)s] on 2 hand test,and the correct reaction times (60.57 ±18.61) on DT test and other indexes. Pearson coefficient was calculated as predictive validation .The composite score of VTS was significantly positively correla-ted with the first platform(r=0.46,P<0.0001), the second platform(r=0.27,P<0.05) and the psychological perform-ance of the pilot psychological selection system (r=0.31,P<0.05).Kappa-test showed that the two methods were highly consistent(Kappa=0.53).Conclusion The application of VTS to screening the mental health of flying cadets can yield and quick but accurate results , which are of high reliability and validity .It is undoubtedly of great significance for improving the level of aviation security ,objectivity of selection results and predictive validity .
10.An empirical study on medical selection of flying cadets with ear diseases between PLAAF and USAF

Objective To determine whether the candidates who are disqualified because of ear diseases during medical selection of flying cadets in China are qualified or not according to United States Air Force ( USAF ) Medical Standards Directory in order to raise suggestions for revising PLAAF medical standards for flying cadets.Methods Data of final selections from 2013 to 2015 and the conclusions were compared by different standards.Results and Conclusion There was a marked difference (x2 =7.99,P<0.05) in the disqualification rate of cadets that rose each year.There was no difference (x2 =5.39,P>0.05) in the disqualification rate related to ear diseases.Vestibular dysfunction and dysacousis were the leading causes of disqualification among ear diseases, making up 45.54% and 38.84% respectively, with other diseases less than 5%.Medical standards for ear diseases in China are more strict than in the USAF, so we should revise some PLAAF standards according to the USAF standards for reference, such as those for vestibular neuritis, dysacousis and tympanic disease in order to enlarge the source of cadets.
