1.Localization of motor language functional area in normal people whose native language is Chinese

BACKGROUND: Chinese character is the only non-alphabetic system of writing, the patterns of semantic symbol, phonetic symbol and sign in the cognition of Chinese characters, it is the combination of form, sound and meaning, and its cognitive pattern, pathway, analytical judgement of semantic and storage are different from those of western alphabetic system of writing, and the processing of Chinese characters maydifferent from that of alphabetic system of writing.OBJECTIVE: To localize the motor language center of the healthy volunteers whose native language is Chinese by means of the equivalent current dipole (ECD) and synthetic aperture magnetometry (SAM) of magnetoen cephalography (MEG).DESIGN: A single-sample univariate analysis.SETTING: Department of Neurology, Beijing Tiantan Hospital affiliated to Capital University of Medical Sciences.PARTICIPANTS: Ten healthy volunteers whose native language is Chinese were selected from the postgraduates and training participants in the Department of Neurology, Beijing Tiantan Hospital affiliated to Capital University of Medical Sciences between July 2003 and April 2005, they were all right handedness and had no speech disorders and cognitive impairments, and all accepted training of language task.METHODS: All the healthy subjects were given the language task stimulation of reading words and pseudowords silently, the number of the silently read nouns, appearing time for each noun and the interval between two tasks were strictly matched, ① Words were the common high-frequency characters, accorded to the law of Chinese characters components, and had 4-13 strokes. ② Pseudowords, composed by Chinese characters made-up software, accorded to the made-up law of Chinese character, but could not compose the stroke assemble of the character, and had 4-13 strokes. The evoked magnetic field produced after the stimulation was recorded with MEG, the collected data were overlapped with MRI to obtain the localization of language functional areas.RESULTS: All the 10 volunteers finished the test and entered the analysis of results. Both words and pseudowords evoked obvious late magnetic reaction waves in bilateral cerebral hemispheres, the motor language centers both located at posterior inferior frontal gyrus.CONCLUSION: The motor and classic motor language centers are general concordant in the normal people whose native language is Chinese, that was to say, the motor language center localizes at posterior inferior frontal gyrus of normal people whose native language is Chinese.

Objective To observe the effectiveness of dexamethasone combined with azasetron for the prevention of postoperative nausea and vomiting(PONV) in patients after laparoscopic cholecystectomy(LC).Methods A total of 150 ASA(Ⅰ~Ⅱ) patients undergoing an elective LC were randomly divided into three groups with 50 patients in each group: Group D+A was given intravenous dexamethasone 5mg and azasetron 10mg(2ml) at the end of surgery,Group A received intravenous azasetron 10mg(2ml) at the end of surgery,and Group C received normal saline(NS) 2 ml as the control.Episodes of nausea and vomiting were recorded for 24 h following the surgery.Results The incidence of nausea was 4% in the Group D+A(2/50),which was significantly lower than in the Group A(16%,8/50) and the Group C(34%,17/50)(?~2=4.00 and 14.62;P=0.046 and 0.000).The incidence of vomiting was 2% in the Group D+A(1/50),which was significantly lower than in the Group A (14%,(7/50)) and the Group C(32%,16/50)(?~2=4.89 and 15.95;P=0.027 and 0.000).The incidences of nausea and vomiting were significantly lower in the Group A than in the Group C(?~2=4.32 and 4.57;P=0.038 and 0.033).Conclusions Use of a low dose of dexamethasone in combination with azasetron is more effective than azasetron prophylaxis alone for a successful control of PONV after LC.
4.Multislice computed tomography angiography in the diagnosis of coronary artery disease

Multislice Cr angiography represents one of the most exciting technological revolutions in cardiac imaging and it has been increasingly used in the diagnosis of coronary artery disease.Rapid improvements in multislice CT scanners over the last decade have allowed this technique to become a potentially effective alternative to invasive coronary angiography in patients with suspected coronary artery disease.High diagnostic value has been achieved with multisfice CT angiography with use of 64-and more slice CT scanners.In addition,multislice CT angiography shows accurate detection and analysis of coronary calcium,characterization of coronary plaques,as well as prediction of the disease progression and major cardiac events.Thus,patients can benefit from multislice CT angiography that provides a rapid and accurate diagnosis while avoiding unnecessary invasive coronary angiography procedures.The aim of this article is present an overview of the clinical applications of multislice CT angiography in coronary artery disease with a focus on the diagnostic accuracy of coronary artery disease;prognostic value of coronary artery disease with regard to the prediction of major cardiac events; detection and quantification of coronary calcium and characterization of coronary plaques.Limitations of multislice Cr angiography in coronary artery disease are also briefly discussed,and future directions are highlighted.
5.The change of calcitonin gene-related peptide immunoreactive nerve fibers in acute inflammatory pulpitis in rats

Objective:To investigate the changes of calcitonin gene-related peptide immunoreactive (CGRP-IR)nerve fibers in rat dental pulps during acute inflammation. Methods: Rat acute pulpitis model was established by silk thread ligation and the change of CGRP-IR nerve fibers was observed with immunohistochemical method.Results: In radical pulp,the CGRP-IR nerve fibers became denser and more heavily stained;in the coronal pulp,the number of CGRP-IR nerve fibers decreased,but the background staining was heavier. Conclusion: During acute inflammation,the amount of CGRP increases in dental pulps, and is released into the surronding tissue in a large scale in the coronal region.
6.The clinical application of pulse indicator continuous cardiac output monitoring in early fluid resuscitation for patients with severe acute pancreatitis

Objective To evaluate the therapeutic effect of early fluid resuscitation under the guidance of pulse indicator continuous cardiac output (PiCCO) on patients with severe acute pancreatitis (SAP).Methods Clinical data of 18 SAP patients (research group),who had undergone fluid resuscitation under the guidance of PiCCO in the Department of Critical Care Medicine of the Second Affiliated Hospital of Anhui Medical University from October 2011 to October 2013,were analyzed prospectively.At the same time,clinical data of 25 cases (control group) that had undergone fluid resuscitation without the guidance of PiCCO from January 2009 to September 2011 were collected retrospectively.The volume of fluid and clinical data were compared between two groups.Results During the first 6 hours,0-24 hours,24-48 hours,and 0-72 hours after intensive care unit (ICU) admission,the research group received larger volume of fluid than that of the control group (mL:2 133 ± 1 593 vs.1 024 ± 421,t=3.337,P=0.002; 5 960 ±2 951 vs.3 767 ± 854,t=3.531,P=0.001; 4 709 ± 1 508 vs.3 863 ± 1 122,t=2.112,P=0.031 ; 14 601 ± 5 095 vs.11 409 ± 2 667,t=2.673,P=0.007).Compared with the control group,the incidence of application of blood purification was lowered [5.56% (1/18) vs.44.00% (11/25),x2=7.688,P=0.006],the duration of the systemic inflammatory response syndrome (SIRS) was shortened (days:3.54 ± 2.44 vs.5.62 ± 3.62,t=2.113,P=0.041),acute physiology and chronic health Ⅱ (APACHE Ⅱ) score was significantly declined at 24 hours after admission (11±4 vs.14 ± 5,t=2.104,P=0.042),the blood lactic acid was decreased more significantly after 72 hours (mmol/L:3.10 ±0.55 vs.2.40 ± 1.12,t=2.442,P=0.019),and the length of ICU stay was shortened (days:10 ±9 vs.20 ± 10,t=3.371,P=0.002) in research group.But there was no significant difference in the percentage of the use of vasoactive drugs [16.67% (3/18) vs.24.00% (6/25),x2 =0.340,P=0.560],the incidence of invasive mechanical ventilation [50.00% (9/18) vs.52.00% (13/25),x2 =0.017,P=0.897],72-hour urea nitrogen changes (mmol/L:-0.33 ± 4.71 vs.-0.09 ± 5.37,t=0.152,P=0.880),and the percentage of abdominal infection [16.67% (3/18) vs.16.00% (4/25),x2=0.003,P=0.953] between research group and control group.The mortality in research group was lower than that in control group [5.56% (1/18) vs.20.00% (5/25)] without statistical difference (x2=1.819,P=0.178).According to the 2012 Atlanta classification,patients were re-evaluated after 48 hours fluid resuscitation.Six patients in research group developed moderately severe acute pancreatitis,and the incidence was significantly higher than that in control group [33.33% (6/18) vs.8.00% (2/25),x2=4.435,P=0.034].The time of mean PiCCO installation was 4.5 days in 18 cases of the research group,and no related complications occurred.Conclusions The PiCCO device may be a useful adjunct for fluid resuscitation monitoring in patients with SAP within 72 hours.Early fluid resuscitation under the guidance of PiCCO may be helpful in improving tissue perfusion,reducing the application of blood purification,as well as shortening length of ICU stay.This program did not increase the risk of invasive mechanical ventilation,and no obvious change in mortality rate was observed.
7.Effect of cold-sensitive channel activity induced by cold air on expression of inflammatory cytokines in rat lungs

Objective To investigate the effect of rapid repeated cold air inhalation stimulation on cold-sensitive chan-nel transient receptor potential ankyrin 1(TRPA1) associated inflammatory cytokines in living lungs .Methods A total of 20 male Wistar rats were randomly and evenly divided into 4 groups:cold air inhalation group , warm air inhalation group , TRPA1 channel agonist inhalation group and normal group , respectively . Tracheal intubation was carried out after anesthesia in rats of cold air inhalation group , and the tubes were linked to the air temperature controlled device with the temperature controlled at ( 2 ±1 )℃.The rats of warm air inhalation group were treated in the same way as cold air inhalation group except for the temperature at (36 ±1)℃.The rats of both groups were treated for 3 times, 1 hour each time.There were 12 hour intervals between treatments .The rats of TRPA1 channel agonist inhalation group were treated with atomizing inhalation of 60 mmol/L acrolein at room temperature (24 ±2)℃for 3 times, 1 hour each time.There were 12 hour intervals between two treatments .The rats were sacrificed after the last treatment .The tracheas and left lung tissues of all rats were taken and total RNA was extracted .The mRNA expressions of TRPA1, interleukin 1 beta ( IL-1β), interleukin 5(IL-5),and neutrophil chemoattractant chemokine (CXCL-1/KC)in the rat lungs were detected by real-time quantitative PCR .Results TRPA1 was expressed in rat lung tissues .In the short term cold air inhalation group , the expressions of IL-1β,IL-5 and CXCL-1/KC mRNA in the rat lungs were higher than those of the warm air inhalation or the normal groups .Similar results were also found in the specific TRPA 1 channel positive agonist ( acrolein ) inhalation group . Conclusion In living animals, the TRPA1 channels of the lungs can be activated by short-term cold air(<17℃),which results in the increasing expression of some inflammatory cytokines in lungs .
8.Two thermoplasticized gutta-percha filling techniques and cold lateral condensation: a comparison of root filling results

Objective:To compare the root filling results of 2 thermoplasticized gutta-percha filling techniques(Ultrafil 3D and ObturaⅡ) with cold lateral condensation of gutta-percha.Methods: Sixty freshly extracted maxillary mandibular single-rooted teeth were instrumented and randomly divided into 3 groups: Ultrafil 3D,ObturaⅡ and cold lateral condensation groups.The root canal was routinely prepared and the obturation time was recorded.Three days later,10 teeth of each group were randomly stained and the micro effusion length was subsequently measured.The micro-chink between filling materials and the wall of root canal were measured with scanning electron microscope in the other 10 teeth of each group.The data were analyzed using unpaired Student's t-test.Results: The longest obturation time was in cold lateral condensation group,then ObturaⅡ and Ultrafil 3D group in order,with significant difference found between the cold lateral condensation group and the latter 2 groups((P
9.Expression of IL-8 in pulpal tissue injured by maxillofacial firearm trauma

Objective: To provide theoretical basis and experimental evidence for diagnosis and therapy of dental pulp indirect injuries by gunshot wounds. Methods: The levels of IL 8 in different stages were studied using immunohistochemical method to evaluate its functions in inflammation process of dental pulps. Results: IL 8 expression was detected by immunohistochemical assay in postwound conditions. The results showed that positive staining of IL 8 was observed predominantly on infiltrating inflammatory cells, vascular endothelial cells and some fibroblasts, which was consistent with the recruitment and activation of inflammatory cells, and with the severity duration of pulpal tissue injury and response. Conclusion: These results indicate that IL 8 is involved in regulating physiologic responses of dental pulp injuries in maxillofacial trauma by a projectile. [
10.A retrospective study of 500 mg fulvestrant in recurrent or metastatic breast cancer patients pretreat-ed with the third generation of aromatase inhibitors

Background and purpose:The third generation of aromatase inhibitors (AI) in postmenopausal hormone receptor-positive patients is the routine treatments in endocrine therapy. The 500 mg fulvestrant showed clini-cal beneifts in patients with previous AI treatment. This study aimed to access the effcacy and safety of 500 mg fulves-trant in estrogen receptor (ER) positive postmenopausal patients who had previous AI treatments with locally advanced and metastatic breast cancer.Methods:This study retrospectively analyzed the clinical data from 188 post-AI ER positive and (or) progesterone receptor (PR)-positive locally advanced and metastatic breast cancer patients treated with 500 mg fulvestrant in Fudan University Shanghai Cancer Center from Jul. 2011 to Dec. 2015. Primary end point was progression-free survival (PFS). Secondary end points were objective response rate (ORR), clinical beneift rate (CBR) and safety proifle.Results:After the median follow-up of 11.3 months, median PFS was 5.9 months (95%CI: 4.2-7.5), CBR was 40.0% and ORR was 3.4%. COX proportional hazards regression analysis indicated that PFS was correlated with the number of metastatic sites (HR=1.92, 95% CI: 1.2-2.9,P =0.002) and previous lines of chemotherapy (HR=1.52, 95%CI:1.0-2.1,P=0.022). Six patients stopped the treatment for intolerable adverse events.Conclusion:The treatment of 500 mg fulvestrant has a favorable effcacy and safety in treatment of post-AI ER positive postmenopausal patientswith metastatic breast cancer.
