2.Biomechanical changes in the early period of flexor tendon repair: an experimental study

Objective To investigate biomechanical characteristics of flexor profundus tendon repaired by modified Kessler or Tsuge technique in the early of postoperative period, and to discuss the feasibility of using two techniques in flexor tendon repair in order to proceed early active or passive mobilization protocols. Methods 42 Sanhuang cocks were randomly divided into two groups. The second and fourth flexor digitorium profoundus tendons of cocks were severed at the middle phalanx transversely and repaired by modified Kessler or Tsuge technique respectively, postoperative plaster cast immobilization was maintained for 3 weeks. The repaired tendons were harvested at 7 time intervals-immediately after repair, 1, 4, 7, 10, 14, 21 days after operation and stored at 20 ℃ in a refrigerator until biomechanical test. A tensile strength-elongation curve was obtained for each tendon sample. Biomechanical parameters including tensile strength of rupture and elongation ratio of rupture were calculated from the curve automatically by testing machine. SPSS software was used for statistical analysis. Independent-samples t test for inter-group comparison; One way ANOVA and SNK q test for inter-group comparison. Results The tensile strength of rupture dropped to the lowest level between 10th and 14th day and resumed its original level immediately after repair at 21st day in modified Kessler group; while in Tsuge group, it decreased at 10th day, then slowly rised up at 14th day but still didnt reach the original level at 21st day. There was no significant difference between two repair methods after 4th day after operation. The elongation ratio of rupture was lowered significantly since 7th day in modified Kessler method, but 4th day in Tsuge method and decreased consistently until 3 weeks postoperatively. There was no significant difference between the two suture methods at any time-intervals. Conclusion The rather high biomechanical characteristics of Tsuge method loses quickly in the early period of tendon healing and resumes slowly; Modified Kessler method maintains its biomechanical properties at all time-intervals except 10th day.

Drug dependence has been a hot topic in neuroscience research due to its complicated pathogenesis. Recently, huge scientific efforts have been continuously put into understanding the association between drug dependence and polymorphism in ATP-binding cassette subfamily B member 1 transporter ( ABCB1 ) gene, especially a mutation in exon 26 at position 3435 (3435C>T). This article reviews the relationship between opioid dependence and analgesic tolerance and ABCB1 gene pol-ymorphism, which will shed light on the underlying mechanisms of drug dependence and optimal MMT programs for individuals.

Polyunsaturated fatty acids (PUFA) was a functional component extracted from fish oil. The experiment results indicate that PUFA might inhibit platelet agglomeration induced by Adenosine diphosphate (ADP) in rabbit and rat. The results also suggested that PUFA has extent effect of anti-coagulation in mice. These effects are related with dosage of giving drug.
5.Influence of Drug Price Reduction and Regulation of Purchase on the Use of Anti-infectives in Our Hos-pital

OBJECTIVE:To discuss the influence of drug price reduction and regulation of purchase on the use of anti-infectives in our hospital.METHODS:The use of anti-infectives in5times of drug price reduction was retrospective anal?ysed.RESULTS:The proportion of the sum of money for consumption of anti-infectives in the total sum for drug consumption dropped year by year but the total DDDc of anti-infectives increased year by year.The DDDs and the total DDDc of all kinds of anti-infectives showed different characteristics.CONCLUSION:The macroscopic price reduction for anti-infectives posi?tively affects the rational use of anti-infectives,yet the effect is limited.Microcosmic regulation of purchase offers an indis?pensable effect.
6.Development of self-locking cannulated compression anti-rotation blade for treatment of femoral neck fractures

Objective To develop self-locking cannulated compression anti-rotation blade and investigate its design characteristic and manipulation techniques. Methods Femoral neck fracture model was made by osteotomy on seven cadaver femurs and then fixed by self-locking cannulated compres-sion anti-rotation blade under fluoroscopic guidance according to designed procedures. After fixation, bio-mechanic test was performed to evaluate fixation stability. Results Cannulated self-locking compres-sion anti-rotation blade was tightly impacted in proximal femur and compression detected during the process of locking, with successful manipulation and stable fixation. Conclusion Because of advanced design characteristic and simple manipulation, self-locking cannulated compression anti-rotation blade can be a substitute for cannulated cancellous lag screw in treatment of femoral neck fractures.
7.Comparison of efficacy and safety between intensity-modulated radiotherapy combined with chemotherapy plus targeted therapy and intensity-modulated radiotherapy combined with chemotherapy for locally advanced nasopharyngeal carcinoma

Objective To retrospectively compare the efficacy and toxicity between intensity?modulated radiotherapy ( IMRT ) combined with chemotherapy plus targeted therapy and IMRT combined with chemotherapy in the treatment of patients with locally advanced nasopharyngeal carcinoma ( NPC) , and to preliminarily evaluate the necessity of adding targeted drugs to standard chemoradiotherapy . Methods Forty?two patients with stage Ⅲ?Ⅳb NPC who received IMRT combined with concurrent ± adjuvant chemotherapy plus targeted molecular therapy from January 2007 to December 2012 were assigned to experiment group,while 168 patients who received IMRT combined with concurrent ±adjuvant chemotherapy within the same period were assigned to control group. The experiment group was paired with the control group at a ratio of 1vs.4.The survival rates were caculated using Kaplan?Meier method and analyzed using log?rank method,other comparison was perfomed by χ2?test. Results The follow?up rate was 100%.The sample size of experiment group and control group were 42 patients and 168 patients. There were no significant differences in the 3?year OS, LRFS, or DMFS rates between the experiment group and the control group (94?3% vs. 87?3%, P=0?647;100?0% vs. 94?6%,P=0?193;92?2% vs. 89?1%, P=0?744).There were also no significant differences in the incidence rates of grade Ⅲ?Ⅳ gastrointestinal reaction or marrow suppression between the two groups ( 7?1%( 3/42 ) vs. 3?6%( 6/168 ) , P=0?388;26?2%( 11/42 ) vs. 17?3%(29/168),P=0?272).However,the experiment group had significantly higher incidence of grade Ⅲ?Ⅳoral mucositis than the control group ( 40?5%( 17/42 ) vs . 14?9%( 25/168 ) , P=0?000 ) . Conclusions The preliminary results indicate that IMRT combined with chemotherapy plus targeted molecular therapy is not able to substantially improve the OS, LRFS, or DMFS rates in patients with locally advanced NPC. Moreover, it may aggravate radiochemotherapy?induced oral mucositis.
8.Effect of varus deformity of caput humeri on shoulder joint function following surgical operation of proximal humeral fracture

Objective To analyze the varus deformity of caput humeri and its correlation with fuuctional outcome of unstable proximal humeral fracture managed with locking plate.Methods The study involved 62 patients (24 males and 38 females,at age range of 32-83 years,mean 63.7 years)with proximal humeral fracture who were treated by open reduction and locking plate internal fixation from May 2008 to June 2010 and had full follow-up data.According to Neer classification,there were 11 patients with two parts fractures,41 with three parts fractures,five with four parts fractures and five with dislocation.Their clinical results were evaluated with Constant-Murley scores.ResultsThe mean duration of follow-up was 20.3 months (range,14-37 months),which showed fracture healing in all the patients.There were 11 patients with varus deformity of the caput humeri. The significant difference was found in Constant-Murley score of shoulder function ( P =0.045 ),forward elevation ( P =0.006 ) and abduction (P =0.022) between the varus and non-varus groups.But there was no significant difference in shoulder external rotation( P =0.08 ).The fracture types ( P =0.037 ) and varus deformity of caput humeri ( P =0.006) were closely correlated with the shoulder function between two groups.ConclusionsThe varus deformity of caput humeri mav be predictive factor for poor outcome of the unstable proximal humeral fracture. Restoration of humeral head-shaft alignment and sound reduction should be done as possible during operation.
9.Comparison between the 7th edition of International Union Against Cancer staging system and the Chinese 2008 staging system for nasopharyngeal carcinoma and suggestions for the updating of the staging system

Objective To compare the 7th edition of International Union Against Cancer ( UICC) staging system with the Chinese 2008 staging system for nasopharyngeal carcinoma ( NPC) , and to provide evidence for further updating of the staging system. Methods A retrospective analysis was performed among 767 patients who were pathologically and newly diagnosed with non?metastatic NPC and treated with intensity?modulated radiotherapy from 2006 to 2012. Based on the main prognostic indices, overall survival ( OS) , locoregional failure?free survival( LFFS) local relapse?free survival ( LRFS) , and distant metastasis?free survival ( DMFS) rates, the value of T stage, N stage, and clinical stage in prognostic prediction was compared between the two staging systems. The Kaplan?Meier method was used for calculating survival rates. The log?rank test was used for survival difference analysis. The Cox model was used for multivariate prognostic analysis. Results In terms of T stage, the Chinese 2008 staging system was a significantly better predictor of the OS and LRFS rates than the 7th edition of UICC staging system. In terms of N stage, they were comparable in the prediction of the OS and DMFS rates. In terms of clinical stage, the 7th edition of UICC staging system was a significantly better predictor of the OS rate than the Chinese 2008 staging system. For the new staging system proposed based on the statistical results, the T, N, and clinical staging gave significantly better prognostic prediction. Conclusions The 7th edition of UICC staging system and the Chinese 2008 staging system for NPC have their own advantages in prognostic prediction. The new staging system proposed in this study could contribute to the updating of the current staging system for NPC.
10.A biomechanical comparison of cable tension band fixation for tibial avulsion fractures of the anterior cruciate ligament

Objective To compare cable tension band fixation with other 3 fixations in terms of biomechanical stability for tibial avulsion fractures of the anterior cruciate ligament (ACL).Methods Eighty fresh porcine knees were randomized into 4 equal groups (n =20).For each knee,all the soft tissues were removed only to keep the femur-ACL-tibia complex.The knee complexes were used to create fracture models of standard Meyers-Mc Keever type Ⅲ at the ACL attachment region using a swing saw.The fracture fragments in the 4 groups were subjected to fixation respectively with intramedullary lag screws,high-strength suture,tension band wire and cable tension band.All the specimens were subsequently tested on a Material Testing Machine at a load rate of 60 mm/mm.The 4 groups were compared in terms of ultimate failure load,yield load,and fragmental displacement under single-cycle loading and multi-cyclic loadings.Results Cable tension band fixation displayed significantly higher ultimate failure,higher yield load and lower fragmental displacement than all the other 3 fixations (P < 0.05).Conclusion As cable tension band fixation,which is convenient and economical,provides better biomechanical stability than other 3 fixations for tibial avulsion fractures of the ACL,it may allow earlier functional and weight-bearing exercise after operation.
