AIM:To investigate changes of retinal nerve fiber layer ( RNFL) thickness and macular retinal thickness in patients with early diabetic retinopathy ( DR ) and disclose the changing trends of RNFL thickness and macular retinal thickness in different stages of early DR.
METHODS:It was a clinical case control study. Through selecting 60 patients ( 120 eyes ) with early DR diagnosed with type 2 diabetes were divided into non - diabetic retinopathy (NDR) group (20 cases, 40 eyes) and mild non-proliferative diabetic retinopathy (NPDR) group (20 cases, 40eyes), moderate NPDR group (20 cases, 40 eyes) . Twenty normal patients ( 40 eyes ) were enrolled as control group. The RNFL thickness of optical nerve ( with circle scan round optic nerve head, scan diameter of 3. 45mm) and the retinal thickness of macular ( around center point with 1000μm diameter ) were measured by optical coherence tomography ( OCT ) , to compare the RNFL thickness changes of the control and early DR patients.
RESULTS:Compared with the control group, the RNFL thickness of optical disc in the inferior quadrant was descended obviously in NDR group ( P < 0. 05 ), with statistically significant difference,; there were no statistically significant difference in other quadrants ( P>0.05). In mild NPDR group, the RNFL thickness of optical disc in the mean and inferior quadrant was significantly descended than that in the NDR group. In moderate NPDR group, the RNFL thickness of optical disc in the mean, superior and inferior quadrant was statistical significance descended than that in the NDR group ( P < 0. 05 ). Compared with the NPDR group, NDR group and control group, the RNFL thickness of optical disc in each quadrant were descended significantly. There was statistically significant difference in macular retinal thickness among the NDR group, mild NPDR and moderate NPDR group ( P<0. 05), the retinal thickness was increased gradually in mild NPDR and moderate NPDR group.
CONCLUSION:With the development in the degree of early DR, RNFL thickness is gradually decreased and retinal thickness is increased, OCT can be observed qualitatively and quantitatively in DR.

Objective To establish a HPLC method for determining the content of oleanolic acid in Radix araliae armatae.Methods Stationary phase was Lichrospher-C18(4.6 mm?250 mm,5 ?m),mobile phase was methanol-0.2% phosphoric(85:15),detection wavelength was 210 nm,flow rate was 1.0 mL/min and column temperature was 30 ℃.Results There was good linearity in the range of 0.059～0.59 ?g for oleanolic acid.The average recovery of oleanolic acid were 95.4% with RSD=0.67%(n=6).Conclusion This method is rapid and accurate.It can be applied to control the quality of Radix araliae armatae effectively.

We wished to screen the cell line that stably expresses the HPV18E5 protein, and to ascertain the influence of HPV18E5 protein on cell proliferation and the cell cycle. The HPV18E5 gene was amplified by the polymerase chain reaction. Then, the His-tag pSecTag-HPV18E5 eukaryotic expression vector was constructed by digestion ligation and connection. The recombinant plasmid was transfected into Balb/c3T3 cells with lipofectamine, and positive cell lines were screened by a culture medium containing bleomycin. HPV18E5 expression in cells was confirmed by western blotting and immuno-enzymatic methods. The influence of HPV18E5 on cell proliferation and the cell cycle were detected by Cell Counting Kit-8 and flow cytometry, respectively. The pSecTag-HPV18E5 eukaryotic expression vector was constructed. After 21-day selection in a culture medium containing 400 μg/mL bleomycin, stably expressing HPV18E5 protein cells were harvested. Compared with control groups, cell proliferation in HPV18E5 stably expressed cells was obviously increased, as was the S phase in the cell cycle. Our results suggested that HPV18E5 influences cell proliferation and the cell cycle. Our study has laid the foundation of the biologic properties of HPV18E5 protein, which will aid further studies on the mechanism of action of carcinogenesis.

AIM: To observe the role of confocal microscopy in infectious keratitis management.
METHODS:Totally 466 patients (467 eyes) diagnosed as infectious keratitis from January 2010 to December 2013 were retrospectively studied. the corneas were examined early by in vivo confocal microscopy. The characteristics of their images and clinical features were studied and summarized.
RESULTS:All patients were recorded, the average age was 54. 4±13. 0 years, in which 264 cases (56. 7%) were male, and 202 cases ( 43. 3%) were female. In the 466 patients, 190 (40. 8%) were fungal keratitis, 148 (31. 8%) were viral keratitis, 125 (26. 8%) were bacterial keratitis and 3 ( 0.6%) were acanthamoeba keratitis. There were fungal hyphae in the images of fungal keratitis. Amebic cysts were found in acanthamoeba keratitis.
CONCLUSION:Confocal microscope can help the early diagnose and treatment of infectious keratitis. It is a noninvasive imaging technique that provides high resolution images of ocular structures at a cellular level and infectious keratitis represents one of its most important clinical uses.

Objective To investigate the expression of transforming growth factor?1(TGF-?1)in human colorectal carcinoma and its value for predicting the prognosis.Methods The expression of TGF-?1 and vascular endothelial growth factor(VEGF)was measured in specimens of 52 coloreetal cancers by immunohistoehemistry.The features of clinical pathology were analyzed and the follow-up of all patients were conducted.The correlation between the expression of TGF-?1 and the survival time was studied with Log-rank test.Results Of 52 patients,no expression of TGF-?1 and VEGF was observed in 11 and 14 patients,and the expression was noticed in 41 and 38 patients,respectively.There was a signifi- cant positive correlation between expression of TGF-?1 and expression of VEGF(x~2＝0.633,P＜0.01). Furthermore,the expression of TGF-?1 was significantly correlated with Dukes staging(x~2＝19.866,P＜0.01)and metastasis of lymph nodes(x~2＝13.152,P＜0.01).The 3-year overall survival rates(OSR)in all patients was 49.1% and the 3-year OSR of patients with and without expression of TGF-?1 were 20.5% and 69.2% respectively(x~2＝11.64,P＝0.0006).Conclusion The expression of TGF-?1 could be served as an important predicator for prognosis of coloreetal carcinoma.

Objective To observe the clinical efficacy of Qingjinchangfei drink for treating AECOPD.Methods We Used random number table to separate 84 patients in to two groups:treatment group and control group.The treatment group used western medicine conventional treatment and Chinese medicine decoction Qingjinchangfei drink.The control group used western medicine conventional treatment.Both two groups were 14 days treatment.We recorded the improvement of symptoms and signs everyday.We compared the lung function,inflammatory cytokines,such as IL-8、IL-2 before and after treatment in the two groups.Results The total efficient in treatment group was obviously better than that in control group.The indicators such as clinical symptoms,signs,pulmonary function indicators were obviously better than that in control group.Compared with before treatment,IL-8,ET,MDA and TNF-? in sputum and blood were obviously lower but IL-2 was obviously higher.Conclusion Qingjinchangfei drink plus western medicine conventional treatment for treating AECOPD was obviously better than western medicine conventional treatment.

Exercise fatigue is a physiological phenomenon not only for athletes, but also for astronauts. It is vital for athletes with hard training and astronauts with the heavy exercises for the mission in manned space flight to know how to solve the exercise fatigue. This article reviewed current commonly used TCM protective measures against fatigue.

Objective To explore the effect of carbon nanoparticles-epirubicin suspension on proliferation and apoptosis in human breast cancer MCF-7 cells.Methods MCF-7 cells were cultured with different concentrations of CNP-EPI in vitro.CCK-8 assay was used for determinate inhibition effect of CNP-EPI on the proliferation of MCF-7 cells at different concentration and different time.According to the determination of IC90,5 μg/ml CNP-EPI was selected,and cell morphology and cell apoptosis rates were observed after 24 h.Results The inhibition effect of the CNP-EPI was stronger significantly in CNP-EPI group than in normal control group within 24 h,48 h,72 h when the concentration was from 1 μg/ml to 200 μg/ml (P＜0.01).The inhibition of CNP-EPI on the proliferation of MCF-7 cells was gradually strengthened in a dose-dependent relation within the same time,and the inhibition effect is reduced in the same concentration of drugs with the time extension,but it still has a strong inhibitory effect in 72 h,and the inhibition effect of different concentration of CNP was not obvious on MCF-7 cells.Obvious changes of cell morphology were observed under inverted microscope such as:a lot of dead cells,cell adherent growth poor,cell morphology became round and karyopycnosis etc,in 5 μg/ml CNP-EPI group after 24 h.However,no obvious abnormity of cell norphology was observed in normal control group and corresponding CNP group.Late apoptosis rate was (14.57±2.41) ％,the mortality rate could reach (78.63±-20.55)％ in 5 μg/ml CNP-EPI group after 24 h.The mortality rate and apoptosis rate of cells was higher significantly in CNP-EPI group than in CNP group and normal control group (P＜0.05).Conclusion CNP-EPI can obviously inhibit the proliferation or kill human breast cancer MCF-7cells,and the inhibition effect of CNP-EPI on proliferation of breast cancer cells might be the result of delayed releasing of EPI.

Objective To explore effects of endoscopic subtotal thyroidectomy on nodular goiter and its clinical significance.Methods From Jun.2004 to Dec.2009,72 patients with nodular goiter underwent endoscopic subtotal thyroidectomy in Southwest Hospital.Of the 72 cases,22 cases had left-side nodule,31 cases had right-side nodule and 19 cases had bilateral nodules.Single thyroid nodule occurred in 41 cases and multiple nodules in 31 cases.The total number of nodules was 113.The average nodule size was 2.8 cm(ranging from 0.4 to 6.3 cm).Results Of the 72 cases,19 patients underwent bilateral subtotal thyroidectomy and the mean operative time was 97 min(ranging from 80 to 150 min).53 patients underwent unilateral subtotal thyroidectomy and the mean operative time was 65 min(ranging from 25 to 120 min).The mean intraoperative blood loss was 45 ml (ranging from 5 to 120 ml).Of the 72 cases,68 cases were given cervical plexus block regional anesthesia and among them 66 cases(97%)acquired good anesthesia.Temporary hoarse voice occurred in 2 cases and postoperative bleeding occurred in the subcutaneous tunnel of breast in 2 cases.Patients were followed up from 3 to 5years and the cosmetic result was satisfactory.The postoperative review half year later showed that unilateral nodule recurred in 1 case and the recurrence rate was 1.4%.Conclusions Endoscopic subtotal thyroidectomy through anterior chest wall or breast approach under local anesthesia is a safe and effective treatment in nodular goiter.In the process of dissecting thyroid,blunt maneuver and proper use of ultrasonic knife instead of clamping thyroid nodules directly are recommended in order to reduce intraoperative blood loss and enhance safe practice.

To study the metabolite excretion of theophylline, a rapid and specific method by liquid chromatography with heated electrospray ionization tandem mass spectrometry (LC-HESI/MS/MS) method for simultaneous determination of theophylline, 1, 3-dimethyluric acid (1,3-DMU), 3-methylxanthine (3-MX) and 1-methyluric acid (1-MU) in human urine was developed using theophylline-d6 and 5-fluorouracil as internal standards. Selected reaction monitoring (SRM) with heated electrospray ionization (HESI) was used in the negative mode for mass spectrometric detection. After diluted with methanol and centrifuged, the analytes and ISs were separated on a XDB-Phenyl (150 mm x 4.6 mm, 5 microm) column with a mixture of water-methanol-formic acid (30 : 70 : 0.15) as mobile phase at a flow rate of 0.6 mL x min(-1). The linear calibration curves for theophylline, 1, 3-DMU, 3-MX and 1-MU were obtained in the concentration range of 1.0-250 microg x mL(-1), separately. The method herein described is effective and convenient, and can be used for determination of theophylline and its three metabolites. The results showed that urinary excretion ratio of theophylline, 1,3-DMU, 3-MX and 1-MU is approximately 1 : 3 : 1 : 2 in Chinese subjects, which is similar to the reported excretion pattern in Caucasian.
