Objective To summarize the perioperative nursing of patients with post-thoracotomy atelectasis treated with bronchoalveolar lavage combined with pulmonary insufflation.Methods Before operation,the nursing measures included the full preoperative preparation,full anesthesia and mental care.During the operation,the nursing focused on intensified monitoring,close cooperation with doctors in manipulation of bronchoalveolar lavage and pulmonary insufflation.After operation,the postoperative nursing was performed.Results The lung recruitments were successful in all the 12 patients.Compared to pre-operation,the arterial oxygen saturation(SaO2)and arterial oxygen partial pressure(PaO2)6 hours after treatment were significantly improved(P<0.05) while the CO2 partial pressure(PaCO2)was significantly decreased(P<0.05).There were no severe complications and no recurrence of pulmonary atelectasis.All the patients were successfully discharged.Conclusions The bronchoalveolar lavage combined with pulmonary insufflation is effective in treating post-thoracotomy atelectasis.To ensure the smooth treatment,perioperative nursing is also contributive.

Objective To investigate the expression of serum amyloid A (SAA) in patients adipose tissue with gestational diabetes mellitus (GDM) and the correlations between SAA and insulin resistance (IR) and body mass index (BMI).Methods A total of 60 single full-term pregnant women underwent cesarean section from June 2013 to December 2013 was enrolled in this study (GDM group,n =30;control group,n =30);serum SAA level was detected with Enzyme-Linked Immunosorbent Assay (ELISA);and mRNA expression of SAA1 in adipose tissue was determined by reverse transcription PCR (RT-PCR);SPSS software was used to compare these markers,and the correlations between SAA and HOMA-IR,BMI were analyzed with Pearson correlation method.Results SAA,mRNA expressions in omental and subcutaneous fat in GDM group (0.447 ± 0.069,0.291 ± 0.067) were significantly higher than those in control group (0.194 ± 0.070,0.231 ± 0.068,P ＜ 0.01).Serum SAA levels [(21.038 ± 6.648) mg/L] and homeostasis model assessment of insulin resistance(HOMA-IR) (4.168± 2.416) in GDM group were significantly higher than those in control group [(14.384 ± 12.770) mg/L,2.045 ± 1.008,P ＜ 0.05];SAA1 mRNA expression levels in omental and subcutaneous fat were positively correlated with serum SAA (r =0.353,0.342,P ＜ 0.01).SAA1 mRNA expression levels in omental were positively correlated to pregestational BMI,late gestational BMI,weight gain in pregnancy and HOMA-IR (r =0.543,0.644,0.340,0.473,P ＜ 0.01),and SAA1 mRNA expression levels in subcutaneous fat were positively correlated to pregestational BMI,late gestational BMI,and HOMA-IR (r =0.788,0.693,0.504,P ＜ 0.01),but was no correlation with weight gain in pregnancy(r =0.013,P ＞ 0.05).Conclusions SAA mRNA expressions in omental and subcutaneous fat in GDM group and serum SAA levels increase,which is positively correlated with BMI and the degree of insulin resistance,SAA may participate in the formation of GDM by increasing insulin resistance.SAA may be used as a new monitor of GDM.

To explore the clinical diagnosis methods of SARS virus infection, and analyze the value of ELISA by which the SARS-Ab was detected. The results show ed that the control group was negative, the positive detection rate was respective ly 2.7% and 92% before and after 12 days when the patients got the SARS in the t est group, and the total positive rate was 94.4%. But the detection rate was 3 1% among 29 suspected patients. In a word, the ELISA method was high specified. Th e detection rate was lower before 12 days and higher after 13 to 16 days when th e patients got the SARS.

Objective To evaluate the clinical significance of combining clinical parameter staging and magnetic resonance imaging (MRI) staging for prostate cancer before radical prostatectomy. Methods The data of 53 patients with organ confined prostate cancer were reviewed to assess the accuracy of combining clinical parameter staging and MRI staging in correlation with pathological staging results after radical prostatectomy. Results Of the 53 prostate cancer cases,20 were of stage T 2 preoperatively,but 5 (25%) of the 20 were pathologically diagnosed as stage T 3 after prostatectomy when serum PSA

Objective To explore the characteristic and pattern of children injury of Zhejiang Province,and provides scientific evidence for the prevention and control strategy of children injury. MethodChildren diagnosed as injury at emergency department in sentry hospitals (Tnere are 5 monitoring sites for injury surveillance in Zhejiang in 2005: Jinhua, Shengsi, Tongxiang, Yinzhou dislrict of Ningbo, Hailing; each monitoring site has 3 sentry hospitals: one county level hospital, one town center hospital and one town level hospital) from January to December,2005 were investigated by trained doctors or nurses in emerncy department. The investigation included basic information, when injury happened, where injury happened and type of injury. Children were investigated when they were conscious, and their parents or accompanies could answer if they were not able to. The contents of investigation were kept private. The distribution of variables which included sex, age, when injury happened, where injury happened, why injury happened and severity level were described mainly with proportion and means, and chisquare test was used to analyze the proportion difference. RemitsA total of 1 794 injury cases were recorded in 2005 among which 67.56% were male and 32.44% were female. The majority were native people and the average age was 7.33±3.84 years old. The injury occurred mostly at home and was of high frequent occurrence from April to November. The most common causes of injury were tumble (47.94%).traffic accident (17.17%) and blunt injury(10.26%). The injury happened mostly during spare time(67.66%), and then school aetivities(11.25%)ant]sports activities(9.96%). Totally 90.33% of the injured children went home after been treated, 1.40% stayed for obsrvafion, 7.04% were in hospital, 0.56% were transferred to other hospitals, 0.22% died and 0.45% unknown. The slight superficial injury and moderate injury were the most common injuries. The most common three types of injury were haemaloma and stasis injury(31.29%), superficial abrased wound(23.69%) and bone fracture(8.67%). Among the injury, 39.47% were head injury, 25.80% were upper limb, 28.55% were lower limb, 4.81% were trunkand 1.38% wereothers. Conclusions The most common three types of injury in Zhejiang province were haemawma and stasis injury, superficial abrased wound and bone fracture. The main reasons of injury were falling injury,traffic accident injury and blunt injury. The injury happened mostly during spare time, and scbool activities and sports activities. Thus, safety education and safety protection should be strengthened to reduce children injuries.
