Objective To investigate the methods of treating great saphenous varicosity by inserting optical fiber straight into EVLT.Methods Wuhan Linyun Laser Therapeutic System was introduced to treat 205 cases (257 limbs)of great saphenous varicosity.Optical fiber instead of blood vessel sheath and catheter was inserted straight into endovenous laser treatment(EVLT)through arteriovenons permanent needle was adopted.Results Operations lasted 35～55 minutes,averaging 45 minutes.Patients were hospitalized for 5～7 days.Postoperative vis- its were followed for 3～12 months,averaging 6 months.Two months after the operation,appraisal according to the curative effect evaluation criteria was carried on.The cure rate for superficial varicosity is 97.3%(250/257),for lower extremities swelling asthenia is 93.7%(150/160),for skin pigmentation is 91.8%(56/61),and for swelling is 90.9%(20/22).10 cases(3.9%)suffered from skin ambustion.9 cases(3.5%)suffered from shank anesthesia and stabbing pain.15 cases(5.8%)suffered from hypodermic tubercle.The patients had not deep venous thrombosis after the operation and the wounds healed well.Conclusion Inserting optical fiber straight into EVLT through arte- riovenous permanent needle brings about more accurate treatment.It can be operated simply,causing small wounds and less pain.

Bone mineral density (BMD) of lumbar spines, femoral neck, Ward′s triangle and greater trochanter was lowered, serum calcium, phosphate, alkaline phosphatase and bone Gla-protein levels were raised, and vitamin D receptor (VDR) contents in lymphocyte was lowered in hyperthyroid patients. Both FT 3 and FT 4 were negatively correlated with BMD, suggesting that hyperthyroidism results in high-turnover type of bone loss which causes the abnormalities of bone metabolic indices and VDR content.

Objective To establish a gene diagnosis assay for spinal muscular atrophy(SMA) in children. Method Analysis of the survival motor neuron (SMN) gene in 19 SMA patients and in 21 normal controls were performed by using polymerase chain reaction - fragment length polymorphism (PCR - RFLP) method. Result Deletion of exon 7and 8 in SMNt gene were found in all 19 SMA patients, while no such changes were found in normal controls. Conclusion The SMNt gene exon 7 and 8 examine can be applied to SMA gene diagnosis, and the PCR- RFLP method have higher sensitivity and particularity to the SMA diagnosis.

BACKGROUND:How to construct tissue-engineered periodontal tissue through efficient combination of scaffold materials and seed cells in vitro is an important direction of current research.
OBJECTIVE:To compare the cellseeding efficiency of traditional static seeding method and col agen enclose seeding method to select a more appropriate seeding method for cellattach.
METHODS:Gingival fibroblasts from dogs at certain concentrations were seeded onto polylactic acid-chitosan-gelatin scaffolds with gradient and uniform pore sizes using the traditional static seeding method and col agen enclose seeding method, respectively. The number of adherent cells was counted to calculate the seeding efficiency for data comparison. RESULTS AND CONCLUSION:The cellseeding efficiency of the col agen enclose seeding method was higher than that of the traditional static seeding method (P<0.01). The col agen enclose seeding method can significantly improve the seeding efficiency and increase cellinitial concentration on the scaffold, and this method can be used for gingival tissue engineering.

Objective To evaluate the feasibility and indications of endoscopic submucosal dissection (ESD) by analyzing the early gastric cancer cases.Methods All the pathological materials of radical gastric resections in Huashan hospital from 2007 to 2011 were retrieved and the EGC cases were selected.All the cases in EGC group which met the extended ESD criteria were reviewed.Extended ESD criteria were cited from Treatment guidelines for gastric cancer in Japan edited by JGCA in 2001.The risk factors relevant with regional metastasis,such as invasive depth and differentiation degrees,were also analysed.Results In the recent five years,there were 1159 cases of gastric cancer undergoing radical gastrectomy in Huashan hospital including 210 EGC.According to the extended criteria of JGCA,151 cases could be candidates for ESD while 126 cases could be candidates for ESD according to the typical criteria for ESD.The local metastases of lymphatic or vascular involvement for mucosa cancer,muscularis mucosae cancer,sm1 and sm2 cancer were 6.75％,15.00％,23.25％,and 39.62％ respectively (P ＜0.05).Twenty-five cases had local metastases that met the extended criteria for ESD.Within the 210 EGC cases,the local metastases of differentiated type and undifferentiated type were 13.95％ and 47.37％ respectively (P ＜ 0.05).Conclusion Most EGC cases undergoing radical gastrectomy can be cured by ESD.Its metastases are highly correlated with invasion depth and differentiation,so indications of ESD should be assessed.

Objective To analyze the content, quality, the type of nursing documents and the actuality of domestic emergent percutaneous coronary intervention (PCI) in patients with acute myocardial infarction(AMI), and to provide basis and reference for the development of more objective and scientific preoperative nursing guidelines. Methods Wanfang DATA, China National Knowledge Infrastructure (CNKI), CqVIPwere retrieved by computer, and supplemented by manual searches.Then the articles which in accordance with inclusion criteria were analyzed by descriptive analysis. Results A total of 114 literatures from 2004 to 2016 were involved, most of them were presented from 2010 to 2014, all of them were published in nearly 70 kinds of journals. There were merely 9 kinds of nursing journals, 26 literatures in all. Literatures came from different levels of hospital, including 47 provincial hospitals, 50 municipal hospitals, 17county hospitals. Only 3 of them had fund support. According to the types of research, there were 61 quasi- experimental studies, 35 retrospective analysis and 18 experience summarization. The most of literatures about perioperative nursing and whole nursing were 39. Nursing experience literatures were 27, nursing cooperation or nursing intervention or nursing study were 15, others were 12. Only preoperative nursing were 4. All of this content were much the same, with minor innovativeness. Conclusions Researches of domestic nursing scholars on intraoperative and postoperative nursing of emergent PCI are more than preoperative nursing, and the attention to preoperative nursing is not high, we should strengthen the importance to preoperative nursing and increase the study of nursing before the operation in order to strive for the precious time when rescuing patients with acute myocardial infarction.

We carried out a new unit-based method in the teaching process of epidemiology for improving undergraduate prevention-oriented medical students' comprehensive ability. The new method integrated heuristic teaching and case teaching, and focused reform on the system of teaching and assessment. The results showed new teaching method can significantly improve students' capacity of analysis and oral expression, and contributes to the comprehensive ability.

Objective To explore the mechanisms of synergic action of commonly-used herb pair of Radix Sophorae Flavescentis (Kushen, KS) and Fructus Cnidii (Shechuangzi, SCZ) for the treatment of pruritus. Methods We predicted and analyzed the potential targets of KS and SCZ based on network pharmacology method, and then established Chinese herbs-compound skeletons-targets networks to reveal anti-pruritus targets, predicting targets, and the interaction of KS and SCZ, as well as the components of the herb pair. The RAW264.7 inflammatory cell model was established to observe the synergistic anti-inflammatory activity of KS and SCZ. Results KS and SCZ had possible synergic action on the pruritus-related targets such as histamine receptors, cannabinoid receptors and proteinase-activated receptor 2. Additionally, KS and SCZ probably had synergic regulation of inflammation-related pathway (Toll -like receptor signaling pathway , chemokine signaling and Fc epsilon RI signaling pathway) and nerve-related pathway(neurotrophin signaling pathway) during the treatment of pruritus. Flavones from KS and coumarin of SCZ had various synergistic anti-inflammatory activities(P<0.05), indicating that they played an important role in inhibiting pruritus induced by inflammation. Conclusion The method may reveal the molecular mechanism of KS and SCZ in inhibiting pruritus, which is important for the modernization of Chinese medicine and new drug development.

Objective To study the relationship between fragile histidine traid (FHIT), pituitary tumor transforming gene-1 (PTTG-1) in thyroid tumor tissue. Methods The expression of FHIT and PTTG-1 were detected by immunohistocbemistry in 96 eases (56 carcinoma,40 adenoma). Results Compared with thyroid adenoma, the expression of FHIT decreased (P <0.01) ,PTTG-1 increased in thyroid carcinoma(P <0.01). The expression of FHIT is different in thyroid carcinoma in eancerometastasis to non-cancerometastasis (P < 0. 01), prognosis index (≥65) and prognosis index(< 65) (P < 0.01 and P < 0.05) ; There also was statistically significant differences between the expression of PTTG in thyroid carcinoma (P <0.05 and P <0.01). Conclusion FHIT and FTTG-1 may be an important reference significance in the differentiation of benign and malignant thyroid tumor tissue, and may serve as useful prognostic markers.

Objective To study the clinical efficacy of recombinant human epidermal growth factor (rhEGF) in the treatment of sacrococcygeal decubitus ulcers grade Ⅲ with direct polymerizing suture after appling rhEGF to reinforce wound bed preparation (WBP). Methods From January 2007 to October 2009, 60 patients with sacrococcygeal decubitus ulcers Grade Ⅲ, were divided into control group and treatment group. The ulcer size was 3 cm × 4 cm to 10 cm × 12 cm and all ulcers were infected for 15-70 d. Treatment group received traditional dressing change and appling rhEGF to reinforce wound bed preparation. Control group received traditional dressing change only. The operative technique that we used in two groups was direct polymerizing suture. Cure rate of stage Ⅰ and complication morbidities were analyzed. Results Cure rate of stage Ⅰ was 87% in treatment group and 70% in control group. Complication morbidities were 13% in treatment group and 30% in control group. The difference between these two groups was statistically significant (P ＜ 0.05). Conclusion Appling rhEGF to reinforce wound bed preparation before operation could make subsequent treatment more effective and improve the cure ratio of operation with decreasing complications and morbidities. And more, dissecting under fascia possesses the advantages of easiness to perform and rich blood supply.The method of appling rhEGF with direct polymerizing suture is a simple, high efficient approach for the first repairment of sacrococcygeal decubitus ulcers grade Ⅲ, especially desirable for the elderly.
