OBJECTIVETo evaluate the outcomes of T3a prostate cancer with unfavorable prognostic factors treated with permanent interstitial brachytherapy combined with external radiotherapy and hormone therapy.
METHODSFrom January 2003 to December 2008, 38 patients classified as T3a prostate cancer with unfavorable prognostic factors were treated with trimodality therapy (brachytherapy + external radiotherapy + hormone therapy). The prescription dose of brachytherapy and external radiotherapy were 110 Gy and 45 Gy, respectively. The duration of hormone therapy was 2-3 years. The endpoints of this study included biochemical failure-free survival (BFFS), distant metastasis-free survival (DMFS), cancer-specific survival (CSS), and overall survival (OS). Survival curves were calculated using the Kaplan-Meier method. The Log-rank test was used to identify the prognostic predictors for univariate analysis.
RESULTSThe median follow-up was 71 months. The serum pre-treatment prostate-specific antigen (PSA) level ranged from 10.0 to 99.8 ng/ml (mean 56.3 ng/ml), the Gleason score ranged from 5 to 9 (median 8), and the percentage of positive biopsy cores ranged from 10% to 100% (mean 65%). The 5-year BFFS, DMFS, CSS, and OS rates were 44%, 69%, 82%, and 76%, respectively. All biochemical failures occurred within 40 months. The percentage of positive biopsy cores was significantly correlated with BFFS, DMFS, and OS (all P=0.000), and the Gleason score with DMFS (P=0.000) and OS (P=0.001).
CONCLUSIONST3a prostate cancer with unfavorable prognostic factors presents not so optimistic outcome. Hormone therapy should be applied to prolong the biochemical progression-free or metastasis-free survival. The percentage of positive biopsy cores and the Gleason score are significant prognostic factors.

The effects of Guizhi Fuling capsule and its active ingredient combination within different concentration on SPL proliferate were observed by MTT method. The ratio of CD80/86, CD3CD25 and CD3CD69 was used to evaluate cell activation effects of Guizhi Fuling capsule and its active ingredient combination by FCM. Guizhi Fuling capsule with concentration of 400 mg · L(-1)can promote spleen lymphocyte proliferation, as well as the active ingredient combination, which showed the obvious dose-effect relationship. Compared with control group, the difference has statistical significance (P≤0.01). The result of FCM showed that Guizhi Fuling capsule and its active ingredient combination can promote CD80 and CD86 expression on spleen lymphocyte, and also can increase CD25 and CD69 ratio between spleen CD3+ cells. Compared with control group, the difference has statistical significance (P≤0.01). Thus, Guizhi Fuling capsule and its active ingredient combination may have immune-modulate effects, and the mechanism may have a close relationship with the lymphocyte activation.

OBJECTIVETo study the clinical therapeutic effect of integrative Chinese and Western medicine on patients with hyperemesis gravidarum (HG) and its influence on serum level of motilin (MOT).
METHODSSixty patients with HG were randomized equally into two groups, the treated group and the control group. They all received the Western medical supportive treatment mainly by fluid infusion, but for patients in the treated group, Chinese herbal medicine, the Yuntu recipe was given orally in combination. The course of treatment was 6 days. Besides, a group of 30 women with regular pregnancy was set up for normal control. Serum levels of MOT and beta-HCG were measured before and after treatment. And patients were followed up for 2 weeks.
RESULTSThe curative rate and the relapse rate was 73.3% (22/30) and 16.7% (5/25) in the treated group; 56.7% (17/30) and 33.3% (10/20) in the control group respectively, the curative rate was significantly higher and the relapse rate was significantly lower in the treated group than those in the control group (P>0.05). Blood 3-HCG levels in the two HG groups were similar to that of the normal control, and changed insignificantly after treatment (P>0.05), while serum level of MOT in them were 67.15 +/- 15.94 ng/L and 70.28 +/- 13.56 ng/L respectively before treatment, significantly lowers than that of the normal control (145.35 +/- 30.12 ng/L, P<0.05), but it increased significantly after treatment in the treated and the control groups (P<0.05), being 151.50 +/- 54.84 ng/ L vs 103.28 +/- 38.31 ng/L and the increasing in the treated group was more significant than that in the control group (P<0.05).
CONCLUSIONThe occurrence of HG is correlated with the lowering of serum MOT level. Integrative medical treatment is effective in treating HG by way of increasing the serum level of MOT and promoting the peristalsis of stomach.
