1.Expression and clinical application of the mRNA of PI3K and Akt mRNA in leukemia

Objective To study the expression of PI3K and Akt mRNA in acute leukemia(AL) and chronic myelocytic leukemia (CML) and try to find out a relationship between the prognostic significance of acute leukemia and the expression level of PI3K and Akt. Methods The samples were collected from 63 ALpatients, 7 relapsed AL patients, 14 CML patients and 10 AL patients in complete remission(CR) patients, 11samples of normal controls(NC). The expression of PI3K, Akt mRNA were measured by semi-quantity reverse transcription polymerase chain reaction(RT-PCR). Results The expression of PI3K mRNA in AL and relapse group were significantly higher than that in normal control. In CML, the P13K mRNA expression level was higher than that in NC and CR. The similar results of the expression of Akt mRNA were observed in CML, NC and CR. At the same time, the expression level of Akt mRNA in relapse group was significantly higher than that in NC. The complete remission rate in PI3K (+) was lower than that in PI3K (-). Similar result was obtained in Akt (+) and Akt (-). Conclusion The PI3K/Akt pathway may contribute to the occurrence of leukemia. The patients with positive expression of the members of this pathway had a lower remission rate.
2.OBSERVATIONS ON THE INNERVATION RAT SUBCUTANOUS TISSUE BY MEANS OF HISTOCHEMICAL DEMONSTRATION OF CATECHOLAMINE FLUORESCENCE AND CHOLINESTERASES

The stretched preparation of subcutanous tissue of 120 male rats were observed by means of histochemical methods for separate and consecutive demonstration of norepinephrine and cholinesterase (ChE). In addition to adrenergic nerves, there are acetylcholinesterase (AChE) containing nerves surrounding the small arteries and arterioles. AChE is recongnized as a marker of cholinergic nerves in rat subcutanous tissue. After sympathetic gangliectomy, all adrenergic and most of cholinergic nerves disappeared, we suggested that both of them are terminals of sympathetic postganglionic nerve fibers. Using the method for the consecutive demonstration of norepinephrine (NE) and AChE, it showed a dual innervation at the same site of an arteriole, most of them are superimposed each other, their distribution were just the same; but the other one third were not. The latter is a separate sympathetic cholinergic system. Whether these superimposed terminals were coming from different neurons and travelling in the Schwann cell or they contained two kinds of neurotransmitters in the same neuron terminal were discussed.By the small arteries and arterioles of rat subcutanous tissue, there were many ChE-positive nerves, which were sensitive to iso-OMPA inhibition. Such thin unmyelineted nerve fibers are mainly non-cholinergic. They showed a variety of free nerve endings in the martrix of connective tissue, and they can be traced, in association with the whole course, to the small spinal nerve trunk that travelling in subcutanous tissue. We consider that these ChE-positive nerves and their terminals are sensory components of cerebrospinal fibers. It has been observed that some of free nerve endings are superimposed with adrenergic paravascular plexus by means of consecutive method.
3.Clinical characteristics of patients with ketosis-onset diabetes

A total of 105 patients with ketosis-onset diabetes were divided into four groups (A+B-,A + B +,A - B -,A - B +) based on the results of islet-associated autoantibodies (A +,A - ) and β-cell function (B +,B - ).Nearly 94% patients in the A -B + group and 1/10 patients in the A - B - group were non-insulin dependent.While in the A + B - and A + B + groups,the detection rates of subsequent non-insulin dependency were 20% and 58%,respectively.Among the four groups,patients in the A - B + groups had a lower level of IL-6 and a higher level of TNF-α. Patients in the A + B - and A + B + groups had a higher level of IL-18.Our study indicates that patients with ketosis-onset diabetes may present different clinical characteristics. Evaluate the islet-associated autoantibodies and pancreatic β-cell function may be helpful to the clinical diagnostic and therapeutic strategies in diabetes.
4.Diagnostic value of MRI versus 99Tcm-MDP bone scan in osseous metastasis of prostate cancer: a meta-analysis.

This paper is aimed to assess the diagnostic value of MRI versus 99 Tcm-methylene diphosphonate (99 Tcm- MDP) bone scan (BS) for osseous metastases in patients with prostate cancer. The computer-based retrieval was conducted on PubMed, EMBASE, EBSCO, Web of Knowledge, the Cochrane Library and Ovid data bases to search for trials about diagnosing osseous metastases of prostate cancer with MRI and 99Tc"m-MDP BS. Selected with time acceptance and time exclusion criteria, the data quality were evaluated with QUADAS quality assessment tool and collected. We used the Meta-Disc software to conduct meta-analysis, and then calculated the pooled sensitivity, specificity and diagnostic odds ratio (DOR), drew the summary receiving operating characteristic (SROC) curve, and measured the area under curve (AUC) and Q value. Then five studies were included, involving 353 patients. The pooled sensitivity of MRI and BS was 0. 95 (95% CI 0. 90~0. 98) and 0. 67 (95% CI 0. 58~0. 75), respectively. The pooled specificity was 0. 97 (95% CI 0. 94~0. 99) and 0. 88 (95% CI 0. 83~0. 91), respectively. The pooled DOR was 402.99 (95% CI 119. 05 ~1364. 15) and 23. 85 (95% CI 1. 32~431. 48), respectively. The AUC was 0. 990 1 and 0. 624 1, respectively. The Q was 0. 958 7 and 0. 593 8. It can well be concluded that MRI is more effective than 99 Tcm-MDP BS in the diagnosis of osseous metastases in patients with prostate cancer.

5.Effects of lidocaine on neutrophil apoptosis induced by tumor necrosis factor alpha

BACKGROUND: Lidocaine is deemed to can attenuate the inflammatory response, but the mechanisms are poorly understood. Nuclear factor-κB (NF-κB) is a key process of imflammatory response. OBJECTIVE: To determine the effect of lidocaine on activation and apoptosis of NF-κB in human neutrophiis in vitro. DESIGN, TIME AND SETTING: The controlled experiment was conducted at the Jiangsu Provincial Key Laboratory of Anesthesiology from October 2006 to February 2007. MATERIALS: Tumor necrosis factor (TNF)-α (0127:B8) was purchased from Sigma, USA. Peripheral blood samples were obtained from healthy donors after their informed consent. METHODS: Human neutrephilic granulocytes were assigned into five groups: ① saline control, ② TNF-α, ③ TNF-α+ lidocaine 1.0 mmol/L, ④ TNF-α+ lidocaine 2.0 mmol/L, and ⑤ TNF-α+ lidocaine 4.0 mmol/L groups. Incubation wasperformed for 3 hours.MAIN OUTCOME MEASURES: The effects of lidocaine on expression of NF- κB mRNA and I- κB mRNA in the cytosol extracts were analyzed by reverse transcription-polymerase chain reaction (RT-PCR), and content of p65 protein were analyzed by Western blot. Neutrophilic granulocyte apoptosis was detected on flow cytomatry after incubation 12 hours and 24 hours.RESULTS: The expression of NF- κB mRNA in the nuclear extracts was significantly decreased and I- κB mRNA in the cytosoI extracts was markedly increased in the lidocaine group. The expression of NF- κB was significantly better in the 2.0 mmol/L and 4.0 mmol/L lidocaine groups than in the 1.0 mmol/L lidocaine group (P< 0.05). No significant difference was detected between the 2.0 mmol/L lidocaine group and the 4.0 mmol/L lidocaine group (P0.05). Lidocaine could significantly inhibit peripheral blood neutrophilic granulocyte apoptosis induced by TNF-α (p < 0.05). The inhibitory effect was significantly better in the 4.0 mmol/L lidocaine group than in the 1.0 mmol/L lidocaine group (P < 0.05). CONCLUSION: Lidocaine (1.0, 2.0 and 4.0 mmol/L) can down-regulate the expressions of NF- κB subunit p65 mRNA and the content of p65 protein in human polymorphoneclear neutrophils, and can significant reverse the reduction of apoptosis induced by TNF-α.
6.Assessment of pain in rats with chronic pancreatitis using behaviour and the rat grimace scale

Objective To investigate whether rat grimace scale (RGS)could be used to assess pain in chronic pancreatitis,so as to provide evidence for pain research and clinical assessment of rat pain.Methods Twenty-eight adult male wister rats were evenly randomized into two groups (n =14):an experimental group and a control group.The experimental group was intravenously given 8 mg/kg body weight dibutyltin dichloride (DBTC)to induce chronic pancreatitis,and the control group was injected with ethanol and glycerin solution.Abdominal hypersensitivity,RGS scores and weight at different time points was detected.HE staining was used to detect the histological changes of pancre-atic tissue.Results Compared with the control group,the rats in the experimental group showed chro-nic inflammation in pancreatic tissue in two weeks.There was a significant increase in the number of abdominal withdrawals (P < 0.001 )and RGS in the experimental group.Conclusion Rat grimace scale might lead to a successful transition of basic science findings into clinical application.
7.Meta-analysis of PET/CT for diagnosis of residual/recurrent nasopharyngeal carcinoma.

OBJECTIVE:
To assess the diagnostic value of 18-fluoro-2-deoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in detecting residual/recurrent nasopharyngeal carcinoma.
METHOD:
The literatures published between January 1990 and September 2013 were searched in PubMed, EM-BASE, EBSCO, Web of Science, CBM, CNKI, VIP and Wanfang databases. Two researchers independently selected studies, extracted data and assessed the quality of included studies according to the QUADAS tool. Summary sensitivity, specificity, diagnostic odds ratios (DOR), and receiver-operating characteristic (SROC) curves were obtained using Meta-Disc software. Subgroup analysis was also conducted.
RESULT:
Twenty-six studies were included in this meta-analysis, involving 1203 patients. The pooled sensitivity, specificity and DOR were 0. 92 (95% CI:0.89-0.94), 0. 87 (95% CI:0.84-0.90) and 51. 10 (95% CI:34.29-76.15), respectively. The area under the curve (AUC) and Q index estimate for PET/CT were 0. 9494 and 0. 8897, respectively. The results of subgroup analysis showed no significant differences between subgroups(P>0.05).
CONCLUSION
In a word, 18F-FDG PET/CT performed well for diagnosis of residual/recurrent nasopharyngeal carcinoma, with relatively high sensitivity and specificity.

8.Localization of polypeptides release factors and ribosome protein L11 in Euplotes octocarinatus.

Protozoan ciliates are a group of unicellular eukaryotes. The special characteristics of stop codons usage in termination of protein biosynthesis in ciliates cells makes them an ideal model to study the mechanism of stop codon recognition of polypeptides release factors. To localize the functional positions of biomolecules in ciliates cell, we constructed a macronuclear artificial chromosome containing a gene encoding red fluorescence protein (EoMAC_R) based on the structural characteristics of ciliates chromosome. Three factors, L11, eRF1a, and eRF3 that are involved in termination process of protein synthesis were colocalized in Euplotes octocarinatus cells by using novel EoMAC_R and the previously constructed EoMAC_G. The results indicated that protein synthesis mainly occurred inside the "C" shape macronucleus, suggesting that EoMAC could be a useful tool for localizing biomolecules in ciliates cell.

9.Trend analysis between nursing human resources and the workload of emergency department and intensive care unit in a national public hospital in Shanghai

Objective:To analyze the gaps between the allocation of nursing human resources and the workload of emergency department and intensive care unit in Zhongshan Hospital, Fudan University in Shanghai in the past five years, so as to provide basis for the formulation of optimal countermeasures for the allocation of nursing human resources.Methods:Based on the data from January 2016 to December 2020, a retrospective study was used to analyze the trend and correlation between the allocation of nursing human resources (educational background, professional title and years of work experience) and the workload of emergency department and intensive care unit(emergency rescue, 120 vehicle pick-up, triage of level-1 patient, extracorporeal membrane oxygenation, central venous catheter intubation, organ transplantation, blood purification and endotracheal intubation). So as to explore the change trend of human resources and the workload of emergency department and intensive care unit in the past five years.Results:By the end of 2020, the proportion of the nurses with bachelor′s or master′s degree had reached 61.44% and the proportion of nurses with professional titles of nurse in charge and above reached 73.85% in emergency department. The structure of working years was relatively stable, and there was no obvious change trend. At the same time, the proportion of the nurses with bachelor′s or master′s degree has reached 57.62% and the proportion of nurses with professional titles of nurse in charge and above reached 63.42% in intensive care unit. The number of nurses who had worked for more than 5 years had shown an upward trend. However, the proportion of nurses with master′s degree or above was still small both in emergency department and intensive care unit. The workload of acute and critical care for nurses was increasing year by year. The average annual growth rates of nursing workload for triage of level-1 patient, blood purification, organ transplantation, central venous catheter intubation and extracorporeal membrane oxygenation were 8.94%, 9.06%, 11.07%, 18.50% and 28.32% respectively.Conclusions:The allocation of nursing human resources for emergency department and intensive care unit in Zhongshan hospital is changing towards personnel quality improvement and structure optimization, but there is still a shortage of high-level nursing personnel. And the workload of nurses in relevant departments is increasing year by year. Hospitals should actively explore the innovative way for the reform of the allocation of nursing human resource to ensure the balanced development of "quantity" and "quality" of nursing human resources for emergency department and intensive care unit.
10.Application of glass ionomer cement to orthodontic traction of the impact-ed tooth

Objective To study the treatment for orthodontic traction of the impacted tooth with glass ionomer cement to improve adhesion efficiency. Methods A total of 46 orthodontic traction of the impacted tooth cases (46 bonded or-thodontic attachments impacted teeth pulled) were selected from March 2005 to December 2012 with traction of im-pacted tooth in our hospital of stmatology department, 46 orthodontic patients were divided into two groups randomly.Following closed-eruption surgery. Experimental group were bonded with glass ionomer cement, the control group with enamel adhesive resin. Compared the failure rates for the first time and after half months. Results The success rate of the first bond, the experimental group was 100%, the control group was 52%. The expulsion rate after half months,0 for the experimental group, the control group was 25%. Initial success rate and after half months expulsion rates were significantly different between the two groups (P<0.01). Conclusion Impacted teeth should be routinely applied glass ionomer cement bonded orthodontic attachments to improve the success rate and reduce postoperative loss rate, to avoid the second surgery.
