China Pharmacist 2016;19(2):316-318
Objective:To research the expense distribution of discharged patients during hospitalization to find out the period with the greatest weight, and provide reliable evidence for the increase of turnover rate by controlling hospitalization of patients and medical security. Methods:Totally 2 444 discharged patients of our hospital from September to November in 2013 with stay day of 1 to 20 d were analyzed, and then divided into the internal medicine group and surgical groups including surgical operation group and surgical without operation group. The hospitalization expenses and drug expense in all the groups were counted and compared. Results: The daily expense in the internal medicine group showed significant difference when compared with that in the surgical operation group and surgical without operation group (P<0. 01), meanwhile, that in the two surgical groups had no significant difference (P>0. 05). The daily expense in the surgical operation group was with linear decrease, and that in the other groups was with logarithmic decline. The 10-day expense in each group had significant difference (P<0. 05) compared by median test. The expense proportion of drugs in all groups was generally increased along with the hospitalization days. Conclusion:The effective method for the decrease of drug pro-portion is shortening hospitalization days, especially in surgical departmens, the drug proportion is increased along with the length of stay.

Chinese Journal of Immunology 2015;(6):809-813
Objective:To explore C-terminal tensin-like(CTEN) gene in breast cancer tissue and its significance. Methods:Explore the prevalence and clinical significance of CTEN expression in invasive breast cancer cases ( n = 1409 ) using immunohistochemistry and tissue microarray. Results: The staining pattern for CTEN in breast tumour cells was homogenous cytoplasmic staining. Moderate to strong cytoplasmic immunoreactivity for CTEN was observed in 90% of the studied cases(1268 cases) . CTEN expression was significantly associated with poor prognostic variables including larger tumour size(χ2=4. 254,P=0. 044), higher histological grade(χ2=7. 555,P=0. 019),axillary nodal involvement(χ2=5. 842,P=0. 035),poor Nottingham Prognostic Index (χ2=7. 578,P=0. 016),Local recurrence(χ2=5. 211,P=0. 039),Regional recurrence(χ2=4. 778,P=0. 042),Distant metastases (χ2=4. 780,P=0. 041) and Histological type(χ2=7. 634,P=0. 012). Significant associations were observed between increased CTEN expression and up-regulation of phosphorylated-Akt(P-Akt)(χ2=27. 23,P<0. 001),Phosphoinositide 3 kinase(PIK3)(χ2=37. 22,P<0. 001) and N-cadherin proteins(χ2=26. 91,P<0. 001). Kaplan-Meier survival analysis demonstrated that patients with high CTEN ex-pression had significantly shorter Breast Cancer Specific Survival(P=0. 004) and Metastasis-Free Survival(P=0. 041) than those with low-CTEN expression. Multivariate analysis showed that CTEN was not an independent prognostic marker in breast cancer. Conclusion:CTEN expression increase is a cancer gene, its association with poor prognostic parameters, and the expression of CTEN associated with poor prognosis of breast cancer parameters.

Objective The human IFN-inducible protein absent in melanoma 2 (AIM2) has been regarded as a tumour sup-pressor, however, the molecular mechanisms of its antitumor activity still remain unclear. This research is to study the molecular mech-anisms of AIM2 inducing breast cancer cell apoptosis. Methods Tet-Off induction model system was established to induce AIM2 ex-pression in great quantities, MCF-7 tTA-AIM2 cells as the experimental group and MCF-7 tTA-Luc cells as control group.Western blotting was used to detect AIM2 expression in breast cancer cell lines and subcellular localization.XTT assay was applied to analyze the effects of AIM2 on cell proliferation.Apoptosis were detected by Annexin V-FITC and propidium iodide staining, and the apoptosis mechanism were investigated by west blot. Results The established Tet-off guidance system could combine tTA to TRE, thereby promoting AIM2 gene transcription and inducing great expression of AIM2 protein.4 days after induction, the expression of AIM2 could be detected in cytoplasm and nucleus, and AIM2 expression increased with the increasing days.XTT assay detected the growth speed of MCF-7 tTA-AIM2 cell lines slowed down 6 days after induction. After abundant expression of induced AIM2, the percentage of FITC fluorescence apoptosis increased significantly (2.36%vs 14.45%, P<0.01) .Increased AIM2 expression inhibited the expression of the anti-apoptotic protein Bcl-xL, increased the expression of the apoptosis proteins Bad and Bax, and activated caspases, resulting in the cleavage of DNA repair protein PARP. Conclusion In breast cancer Tet-off induction system, AIM2 will express in cytoplasm and nucleus, influence cell proliferation, and stimulate the mitochondria to cell apoptosis.

Objective: The aim of this study was to develop and assess the computer-assisted fabricated individual titanium mesh technique applied for reconstruction of fractured orbital walls to correct post-traumatic enophthalmus and diplopia. Methods: Twenty patients (12 males and 8 females) were inclu-ded in this study. All of patients suffered from unilateral impure orbital fractures. Group A included 8 patients who were reconstructed for the fractured orbit with computer-assisted fabricated titanium mesh technique. Group B had 12 patients treated with traditional methods (titanium meshes in 2, autogenous bone grafts in 7 and Medpor implantation in 3). CT data of group A was input into a image software to rebuild a 3-D orbit and then mirror the unaffected side onto the affected side to replace the demolished orbit. A resin model of the reshaped orbit was then produced and was used to develop custom-fabricated titanium mesh for repair of the fractured orbital wall. The fabricated mesh was used in operation to reconstruct the orbital wall. Treatment was assessed in comparison with traditional way by means of clinical examinations and CT measurements. Results: The difference between pre-operative and post-operative enophthalmos: the mean value of group A was 2.6 mm, group B was 2.0 mm;the difference between the pre-operative and post-operative enlargement volume of traumatic orbit: the mean value of group A was 4.3 mL, group B was 3.7 mL; Index of Medial orbital wall of both groups was 100% and Index of inferior wall reconstruction of group A was 84.7%, group B was 53.3%; The mean depth of implants of group A was 31.2 mm, of group B was 28.3 mm; Improvement of diplopia of group A was 1.5 quadrants, that of group B was 1 quadrant. There were no infection, gaze restriction, or graft extrusion in all patients after operation. Conclusion: Computer-assisted fabricated titanium mesh technique could provide more accurate orbital reconstruction and improve the correction of enophthalmus and diplopia.
