1.Enteral nutrition related complications in severe brain-injured patients

Objecive:To observe the incidence of early enteral nutrition-related complications in severe brain-injured patients in ICU and its associated factors.Methods:78 nutrition severe brain-injured patients(GCS 4-8) in ICU receiving enteral nutrition were all carried out for GCS scoring.The enteral feeding amount and speed,serum albumin(ALB) and intestinal feeding tolerance were observed.The incidence rate of all kinds of complications related to enteral nutrition was summarized.Results:The patients with lower GCS had lower total feeding amount,prolonged feeding duration and higher incidence rate of complications.The diarrhea was negatively related with serum ALB level and GCS. Conclusion :Enteral nutrition-related complications are related with the severity of disease and Alb level.Some critically ill patients can not sustain total enteral nutririon(TEN),and parenteral nutrition is necessary.
2.Risk Assessment on Decorated Indoor Environment and Human Health

Objective In order to know the harm of indoor environment pollutants to the human health. Methods Formaldehyde, benzene, dimethylbenzene, ? radiation and radon were selected as the factors of indoor environment pollution assessment, considering about the inhale and exposure path of indoor air, assessment model which from the production of inland and overseas was used to evaluate the health risk during Mar.-Oct.2005. Results The average risk of the indoor air pollution in Jiangxi province was 1.93?10-4/a, radiation (? radiation, radon) was 1.32?10-4/a, accounted for 68%. Formaldehyde was 4.64?10-5/a, accounted for of 24%. Benzene was 1.41?10-5/a, accounted for 7%. Dimethylbenzene was 1.32?10-9/a, which could be ignored. Conclusion The health risk of indoor environmental air pollutants in each city of Jiangxi province is higher than the acceptable lever(10-5/a).
3.The Value of 16-slice Spiral CT Perfusion in Diagnosis of Hyperacute Cerebral Infarction

Objective To assess the value of 16-slice spiral CT perfusion in the diagnosis of hyperacute cerebral infarction.Methods Routine and perfusion CT of brain were performed in 20 cases suspected with hyperacute cerebral stroke within 6 hours of symptom onset,the images of plain scan,rCBV,rCBF,MTT and TTP were observed and evaluated. All cases were followed-up with CT after one week.Results Ischemic lesions were found in routine CT in 5 cases,15 cases had negative results at routine CT, and ischemic lesions were showed in all of them by perfusion CT images.rCBV,rCBF distinctly decreased, MTT and TTP obviously prolonged in infarction area,and rCBF decreased, MTT and TTP prolonged and rCBV was normal or up lightly in the penumbra area. The infarction lesions were detected in 15 cases by follow-up CT.Conclusion 16-slice spiral CT perfusion in diagnosis of hyperacute cerebral infarction may be a valuable tool in predicting clinical prognosis.
4.Sulfentanil combined with bupivacaine for subarachnoid block on cesarean section patients

Objective To observe the anaesthetic effect of sulfentanil combined with bupivacaine for subarachnoid block on cesarean section patients and the influence on the patient and fetus.Methods Eighty patients who were scheduled for elective cesarean section under subarachnoid block,were distributed into two groups:bupivacaine with no sulfenlanil(group B)and bupivacaine with sulfentanil(group BS).Detected SBP,DBP,MAP,HR,SpO2 before and after anesthesia.The time of drug onset,motor recovery and pain recovery were recorded,the effect of muscle relaxation was evaluated,and the drag reaction,neonatal Apgar score and anesthesia complication were recorded.Results There was no significant difference in the time of drug onset and motor recovery,muscle relaxation,neonatal Apgar score between the two groups.MAP was lower after 3-10 min of anesthesia than before anesthesia,but there was no significant difference after 30-120 min of anesthesia in the two groups.The rate of drag reaction was 27.5% in group B,but no case in group BS.The time of pain recovery was longer in group BS than that in group B(P<0.01).The rate of temporary itch of skin after operation was 17.5% in group BS.Conclusion The addition of sulfentanil to hyperbaric bupivacaine provides adequate anesthesia for cesarean section and good postoperative analgesia.
5.Experimental study on the cryopreservation of LLC-PK1 epithelial cells with hypoxic UW solution.

The effects of oxygen partial pressure on cryopreservation of the cells with organ preservation solution were explored. Hypoxic UW solution was made by purging the UW solution with argon. The pig proximal tubule epithelial cells (LLC-PK1 cells) were cryopreserved in hypoxic UW solution (Ar-UW group) or standard UW solution (UW group) at 4 degrees C for 48 h. Trypan blue staining and LDH detection were performed to evaluate the injury of the cells. The results showed that the oxygen partial pressure in Ar-UW group was significantly declined from 242+/-6 mmHg to 83+/-10 mmHg. After cryopreservation at 4 degrees C for 48 h, LDH leakage rate and Trypan blue-stained rate in Ar-UW group were (11.3+/-3.4)% and (10.5+/-4.7)%, respectively, which were significantly lower than in UW group [(49.5+/-6.9)% and (47.6+/-9.3)% respectively, both P<0.01]. It was concluded that lower oxygen partial pressure of UW solution was more beneficial to the cryopreservation of LLC.

6.Preparation and preservation of hypoxia UW solution.

In order to explore the method to prepare hypoxia UW solution and the stability and preservation of hypoxia UW solution, UW solution was purged by argon or air for 15 min or 60 at a flow rate of 0.8 or 2 L/min, and the oxygen partial pressure of UW solution was detected. The hypoxia UW solution was exposed to the air or sealed up to preserve by using different methods, and the changes of oxygen partial pressure was tested. The results showed that oxygen partial presure of 50 mL UW solution, purged by argon for 15 min at a flow rate of 2 L/min, was declined from 242+/-6 mmHg to 83+/-10 mmHg. After exposure to the air, oxygen partial pressure of hypoxia UW solution was gradually increased to 160+/-7 mmHg at 48 h. After sealed up by the centrifuge tube and plastic bad filled with argon, oxygen partial pressure of hypoxia UW solution was stable, about 88+/-13 mmHg at 72 h. It was concluded that oxygen of UW solution could be purged by argon efficiently. Sealed up by the centrifuge tube and plastic bag filled with argon, oxygen partial pressure of UW solution could be stabilized.

7.Effect of thalidomide on the expression of VEGF mRNA in human breast cancer cell lines

Objective To assess the effects of thalidomide on VEGF mRNA level in human breast cancer cell lines. Methods Breast cancer cells were treated with thalidomide for 24~72h, RT-PCR was uced to detect the level of VEGF mRNA expression. Results When the concentration of drug is 50μg/L,thalidomide strongly inhibited the level of VEGF mRNA expression. Conclusion Within certain concentration range of thalidomide can inhibit the level of VEGF mRNA expression in breast cancer cell.
8.The application of rhinoscope in microsurgical treatment of intracranial aneurysms intraoperative clinical research.

This paper analysed the rhinoscope's clinical value in microsurgical treatment of intracranial aneurysms. Application of the rhinoscope in 87 patients, only 2 patients had ruptured during operation. However, 11 cases had ruptured in 94 cases without using rhinoscope, P < 0.05, they had a significant difference. By DSA follow-up review, 82 cases of used rhinoscope only 2 cases had remained the aneurysm neck, but 9 cases had the aneurysm neck in 77 cases which had not used the rhinoscope in the microsurgical treatment, P < 0.05, they also had significant difference. The application of rhinoscope in microsurgical treatment of intracranial aneurysms intraoperative, can reduce the risk of the intraoperative aneurysm rupture. It can achieve better clinical effect.

9.Effects of chemotherapy combined with Chinese herbal medicine Kangliu Zengxiao Decoction on tumor markers of patients with advanced non-small-cell lung cancer: a randomized, controlled trial.

Background: Tumor markers are widely used in clinical practice and have become important indicators in assessing cancer progress. There is increasing concern that chemotherapy combined with traditional Chinese medicine has effects in decreasing the level of tumor markers. Objective: To investigate the effects of chemotherapy combined with Kangliu Zengxiao Decoction (KLZX), a compound Chinese herbal drug, on tumor markers carbohydrate antigen 50 (CA 50), cytokeratin 19 fragment (CYFRA21-1) and carcinoembryonic antigen (CEA) in patients with advanced non-small-cell lung cancer (NSCLC) and to explore the relationships between clinical efficacy and tumor markers. Design, setting, participants and interventions: Patients were included from Punan Hospital of Shanghai Pudong New District and Longhua Hospital between October 2008 and December 2009. Seventy-four subjects with advanced NSCLC were randomly assigned into treatment group (n=37) and control group (n=37). Patients in the control group were treated with chemotherapy alone while patients in the treatment group were treated with chemotherapy combined with KLZX. Chemotherapy of NP (vinorelbine + cisplatin) was given for two cycles and patients in the treatment group were administered with KLZX during chemotherapy. Main outcome measures: Levels of CA50, CYFRA21-1 and CEA before and after treatment were evaluated and the relationship between changes in levels of tumor makers and tumor size, clinical symptoms and living condition score (Karnofsky score) was analyzed. Results: No patients achieved a complete remission. The disease control rates (complete remission (CR)+partial remission (PR)+no change (NC)) were 89.20% (33/37) and 70.30% (26/37) in the treatment and control group respectively (P<0.05). The levels of CA50, CYFRA21-1 and CEA were clearly decreased in the treatment group after treatment (P<0.05) while also decreased in the patients without progression of disease. There were no obvious changes of CA50, CYFRA21-1 and CEA in the control group, and there was even a trend of increase. Furthermore, the improvement rates of clinical syndrome were 51% (19/37) vs 11% (4/37) (P<0.05) in the treatment group and control group respectively. The total response rates of quality of life were 91.89% (34/37) vs 56.76% (21/37) (P<0.01) in the treatment and control group respectively. Conclusion: Combined chemotherapy with KLZX in treating advanced NSCLC can acquire better stabilizing tumor foci, decrease levels of tumor markers and improve the clinical symptoms and Karnofsky score.
10.Effects of TotalPanax Japonicus Extract onD-galactose Induced Aging Mice

Objective To research the protective effects of totalPanax Japonicus extract on learning memory, antioxidation, and anti-apoptosis of aging mice,and explore the mechanism. Methods Male mice were randomly divided into normal group, model group, Vitamin E (VE) group, Panax Japonicus extract low and high dose group. Except for the normal group, the other groups were injected withD-gal on the back of the neck subcutaneously to establish aging model. Normal group and model group were given a gavage with saline and each treatment group was given a gavage with totalPanax Japonicus extract and VE once a day for 7 weeks after the aging model established. All mice were be measured their learning and memory ability in the eighth week. After the test, the morphological changes of CA1 neurons were observed by HE stain. SOD, GSH-Px, MDA levels in brain tissue were measured by biochemical method, the expressions of Bcl-2 and Bax mRNA were evaluated by RT-PCR.Results Mice inPanax Japonicus extract low and high dose group could spend less time in searching for the platform, improve the learning and memory ability. TotalPanax Japonicus extract increased the activity of SOD and GSH-Px, while decreased the content of MDA. In addition, it could increase the expression level of Bcl-2 mRNA and reduce the expression level of Bax mRNA as well.Conclusion TotalPanax Japonicus extract has anti-aging effect.
