OBJECTIVE To investigate the clinical distribution and resistance of Acinetobacter baumannii in our hospital in order to provide evidence to proper use of antibiotic drugs in clinics. METHODS A. baumannii isolates were collected in our hospital from Jan 2004 to Dec 2006. Antimicrobial susceptibility testing was performed by disk-diffusion method,according to the standards of CLSI/NCCLS, the data were analyzed by WHONET5.4 software. RESULTS The isolating rate of A. baumannii from 2004 to 2006 was 2.5%,3.8%, and 10.9%, respectively.It was distributed mainly from ICU (29.2%), respiratory ward (19.3%) and emergency ward (10.9%). The highest appearing rate was the sputum, up to 78.0%. More than 60% of isolates were resistant to antimicrobial agents tested such as PIP, CTX, CRO and ATM. A. baumannii showed the highest susceptibility to carbopenems with about 80%. CONCLUSIONS The infection of A. baumannii is one of the most complex problems. Therefore, monitoring A. baumannii constantly and regularly, finding out resistant strains timely, and adjusting the treatment regimen are very important to the prevention of nosocomial infection. The disinfection should be strengthened for hospital environment and medical staff in order to control existence and spread of A. baumannii.

OBJECTIVE To investigate the drug susceptibility of clinical isolates in local region for using antibiotic reasonably. METHODS Totally 3 170 strains of clinical isolates were identified by API and Microscan and tested for drug resistance against antimicrobial agents by K-B method. WHONET5.4 was applied for analysis. RESULTS The commonly encountered bacteria were Pseudomonas aeruginosa (24.3%), Acinetobacter baumannii (10.9%), Klebsiella pneumoniae (10.4%), Escherichia coli (8.9%), and Staphylococcus aureus (SA,8.0%). In Gram-negative isolates, the resistance rate to meropenem was 19.7%, and to piperacillin-tazobactam was 26.5%. The incidences of E.coli and K. pneumoniae isolates producing extended spectrum beta-lactamases (ESBLs) were 49.1% and 33.5%, respectively. In Gram-positive isolates, the susceptibility rate to vancomycin and teicoplanin both was 100.0%. The oxacillin resistant rates of SA and coagulase negative Staphylococcus (CNS) were 54.2.0% and 82.3%. CONCLUSIONS The production ratio of ESBLs and oxacillin resistance of bacteria in local region are high. It is important to promote the rational use of antimicrobial agents and take effective contaminant methods to reduce resistant rates of bacteria and dissemination of multi-resistant bacteria.

This review introduces the mechanism and advantages of laser acupuncture and moxibustion, the biological effect of low-level laser, the characteristics of acupuncture point area and the experimental laws of biological effect.On this basis, the existing problems in clinical application of laser acupuncture and laser acupuncture apparatus are pointed out.The combination therapy method with various wavelength are also discussed, and future development directions in the study are prospected.

Objective In order to further improve the noninvasive measurement precision of human blood glucose and achieve clinical requirements,experiments were conducted to measure human blood glucose by a novel measurement method our group proposed.Methods The blood flow of the tested parts was stopped by pressure,and dynamic distribution of transmission spectra was measured by time-correlated single-photon counting (TCSPC) technology under the dual wavelength light incident.Oral glucose tolerance test (OGTT) was conducted on three human subjects,and the early arriving photons were selected by Laplace transform.Meanwhile,the human blood glucose concentration was measured using the Roche glucose meter.Results The best curve fitting was got when the Laplace parameter was 1Gs-1 (determining parameter R2=0.0922).Conclusions The experimental results showed that better measurement accuracy can be obtained by selecting appropriate Laplace parameter,and noninvasive measurement of human blood glucose under flow control of time gate was feasible.

Objective Nasal administration,as a common means of treatment of respiratory diseases,may normally cause bronchospasm,cough and other adverse reactions,the efficiency of nasal nebulizers has been criticized as well.This paper aims to develop a medicine atomizing device,pharmaceutically atomized in solving the existing problems,providing a modern instrument for treatment of respiratory diseases by traditional Chinese medicine.Methods The device could prepare medicine by decoction and filtration,which means this unit is divided into two functional modules,medicine preparation and herbal atomization.1 200 W heating plate is placed in decoction unit and is programmed to fit traditional decoction protocol.Steam and ultrasonic nebulised medicine are combined in atomizing unit.Another non-contact infrared heating unit also located in to control the temperature precisely.Different forms of traditional Chinese medicine and different filtration methods were selected to optimize the atomizing effect.Results An implementation of two sequence of filtration enabled optimization of atomizing.After 15 volunteers'trial,this device functioned well on Bama nasal medical protocol,and could effectively avoid the adverse reactions of the traditional way of cold fog atomizing.Conclusions The nasal nebulizer equipment on which Chinese herbal medicine treatments could directly performed,produced atomized mist particle size suitable for nasal treatment,and by generating warm steam to avoid the common clinical administration caused choking to hypoxia and other adverse reactions.Meanwhile,instrument has a friendly interactive feature to meet the clinical requirements for medicine nebulizer treatment.

Objective To explore the applicability of the childhood autism spectrum test (CAST) of children in Mainland China according to autism screening,in order to provide the effective reference for the identification and diagnosis of autism spectrum disorders of children.Methods Two hundreds and fifty-two typical developing children,and 61 autistic children who were confirmed by psychological clinic of Child Healthcare Clinic,Shenzhen Luohu District Maternal and Child Health Care Hospital who were conformed with diagnosis standards of Diagnostic and Statistical Manual of Mental Disorders Fifth Edition(DSM-5) by using CAST Chinese version were assessed,and the reliability and validity analysis were made on the CAST scale.The receiver operating characteristic curve was drew to determine the optimal threshold value,and the sensitivity,specificity and positive predictive value were calculated.Results CAST Chinese version had good reliability,Cronbach's α was 0.880.Split-half reliability coefficient r was 0.821,and the whole scale calibration reliability coefficient was 0.902.There was statistically significant difference between items and the total score(P＜0.05).The consistency between the CAST ≥4 years old children and diagnosis standards of DSM-5 reached 0.884(P=0.000).The consistency 3-＜4 years old children and diagnosis standards of DSM-5 reached 0.867 (P=0.000).Among the screening of the children who were ≥4 years old,the cut off was 15 scores that the sensitivity and specificity were best.With the children of 3-＜4 years old,a designated cut-point should be 17 scores.Conclusions There are good reliability and validity of CAST,which can be used to mainland China,as well as to screen autistic children more than 3 years old.

Objective To detect the expression of long non-coding RNA (lncRNA) CCAT1 in human papillary thyroid cancer, and to observe the effect of CCAT1 down-regulation on the invasion and migration of human papillary thyroid cancer.Methods The expression of CCAT1 was detected in human normal thyroid Nthy-ori 3-1 cells and human papillary thyroid cancer TPC-1 cells.CCAT1 siRNA plasmid was transfected into TPC-1 cells.The effect of CCAT1 down-regulation on cell invasion and migration was observed by Transwell chamber assay and scratch test, and the expressions of BRAF, MUC15 and RKIP proteins were detected by Western blot.Results The level of CCAT1 in human papillary thyroid cancer TPC-1 cells was significantly higher than that in human normal thyroid Nthy-ori 3-1 cells.CCAT1 down-regulation significantly inhibited the invasion and migration of TPC-1 cells.The Transwell invasion assay revealed that the number of migrated TPC-1 cells in the CCAT1 down-regulation group was significantly lower than that in the control group.The scratch test showed an increased distance between cells in the CCAT1 down-regulation group compared to the control group, suggesting a reduced cell motility.The expressions of BRAF and MUC15 proteins were decreased in the CCAT1 down-regulation group, while that of RKIP protein was increased.Conclusions The expression of CCAT1 in papillary thyroid cancer cells is significantly higher than that in normal human thyroid cells.Down-regulation of CCAT1 in papillary thyroid cancer cells may inhibit the cell invasion and migration by regulating the expression of BRAF, MUC15 and RKIP proteins.

Objective To study the causes, diagnosis and treatment of pancreatogenic portal hyperten-sion.Methods 20 cases of panereatogenic portal hypertension were analyzed retrospectively.Clinical features were analyzed.Results The causes of them included chronic panereatitis in 15 cases, pancreatic carcinoma in 3 cases, pancreatic tuberculosis and pancreatic injury in 2 eases.Lieneetomy was performed in all 20 cases and all with a satisfactory results.Conclusions Lienectomy is the first choice in treatment of pancreatogenic portal hy-pertension.If the primary disease being early and properly treated, surgery may be avoided.

OBJECTIVE To determine the clinical distribution and drug resistance characteristics of pathogens from nosocomial infection cases to provide the gist for clinical therapy.METHODS Pathogens isolation,identification and drug resistance tests were conducted to samples,which were gathered from respective departments in our hospital between Jan 2007 to Dec 2007.Then,MRSA tests were performed for Staph Staphylococcus and ESBLs-producing G-bacilli were also detected.RESULTS Among 5382 pathogen strains from 12571 samples,3390 strains were G-bacilli(62.98%),953 strains were G+ bacteria(17.71%) and 1039 strains were fungi(19.31%).The most common species among 4343 strains pathogens except fungi were Pseudomonas aeruginosa(20.5%),Acinetobacter baumanni(11.2%),Klebsiella pneumoniae(10.8%),Escherichia coli(8.6%) and Staphylococcus aureus(8.3%).The clinical departments with higher infective rate were Department of Neurosurgery,Department of Respiratory Disease,ICU,Department of Hepatobiliary Surgery,and Department of Pediatrics in order.Drug resistance results showed that the resistance rates of S.aureus to clindamycin,Quinolones and Tetracyclines were about 50%,but no S.aureus was resistant to vancomycin,linezolid and teicoplanin.The resistance rates of G-bacilli to imipenem,meropenem and azithromycin were lower,but with more than 30% of resistance rates to others common antibiotic drugs.CONCLUSIONS The pathogens in nosocomial infection are mainly the opportunistic pathogenic bacteria and mostly G-bacilli.The infection due to fungi shows an increasing trend.It should pay attention to the pathogenic detection and rational use of antimicrobial agents.

ObjectiveTo investigate the effectiveness and toxicity of CyberKnife in the treatment of lung metastases.MethodsTreatment details and outcomes were reviewed for 93 targets of 48 histologically verified patients treated by CyberKnife at the CyberKnife Center of Tianjin between September 2006 and June 2010.The median tumor volume was 6.0(0.2 - 135.2) cm3,the median biological equivalent dose was 140.8(53 - 180) cGy (α/β =10),the median fraction was 3(1-7) times and the median isodose line was 81％ (71％-91％ ).ResultsThe rate of follow-up is 96％.33 cases were followed up for more than 2years.The effective rate was 90.3％.Two targets of 2 patients locally progressed.The 1-and 2-year local control rates,overall survival (OS) rates and progression-free survival (PFS) rates were 98％ and 98％,83％ and 63％,and 64％ and 37％,respectively.Univariate analyses showed that age older than 60 versus ≤60 years tended to be predictor for PFS ( x2 =3.45,P =0.063 ) ;The PFS of patients who had single lesion was better than patients with multiple lesions ( x2 =4.49,P =0.034 ) ; patients with disease-free interval longer than 18 months had better OS ( x2 =6.50,P =0.011 ).Five patients were reported to experience treatment-related grade 1 radiation pulmonary injury,and one each for subcutaneous fibrosis with pigmentation,grade 2 and grade 3 adverse event.ConclusionsFor patients with lung metastatic lesion,CyberKnife is an effective option with high local control rate and little acute reaction.The long-term outcome and toxicity need further study.
