Objective To explore the distribution and clinical significance of hepatitis B core antigen( HBcAg) in the hepatocytes of chronic hepatitis B virus infected patients. Methods Paraffin sections were made from 41 liver biopsy samples obtained from chronic hepatitis B virus infected patients. The immuno-fluorescence confocal technique was utilized to analyze the expression level and localization of HBcAg in hepatocytes. The data were analyzed by using Kruskal Wallis test and chisquare test. Results HBcAg expression were detected in 36 (87. 8%) patients, among whom 23 cases had moderate abnormal liver function, 10 with mild abnormal liver function and 3 with normal liver function. Among the cases with moderate abnormal liver function, 6 cases showed the simple membrane-type HBcAg expression, 17 cases showed mixed cytosolic-type and membrane-type HBcAg expression without the nuclear-type expression. Twelve cases with mild abnormal liver function only showed simple cytosolic-type HBcAg expression without membrane-type or nuclear-type expression. In the three patients with normal liver function, HBcAg was expressed in cytoplasm and nuclear but not on membrane. The correlation between HBcAg expression pattern and liver function was statistically significant (χ2 =10. 60, P<0.01). Conclusion HBcAg expression is directly correlated with liver injury in chronic hepatitis B virus infected patients, which indicates that membrane expressed HBcAg is the target antigen during the immuno-attack of liver.

By reviewing name, type, function and indication of Chinese herbTing-Li-Zi (Lepidii seu Descurainiae) in literatures, there were northernTing-Li-Zi and southernTing-Li-Zi from Song dynasty. It was also known that northernTing-Li-Zi has strong effect and southernTing-Li-Zi had moderate effect. However, there was no strict classification between two types in current clinical practice. The disease prevention and treatment effect of Chinese herbs were depended on their internal features. Disease treatment must be based on syndrome differentiation with corresponding selection of herbs to achieve desired results. Therefore, it is necessary to combine properties with actions in the study of Chinese herbs to provide guidance in clinical practice.

Journal of Chinese Physician 2002;0(S1):-
Objective To summarize the expreiences of management of triplet and quaternion pregnancy mothers and their in our hospital. Methods The clinical data of seven cases of multiple pregnancy (6 case of triplet and 1 case of quaternion) in our department from 1992 to 2003 were analyzed retrospectively. Results All seven patients had middle or severe anemia and hypoproteinemia in second and third trimester. The preterm sign happened in 2/3 cases in 32-36 gestation weeks. The patients were admitted in hospital in 27-34 gestation weeks because of obstetric complication. 6 triplet cases delivered during 35th-37th gestation weeks, except the quaternion one delivered at 33 gestation weeks. There were 22 newborns in those cases, the all babies Apgar's scores were above 9, the birth weights of all babies were above the 3th percent of Chinese singleton gestation and newborn estimated curves, and the birth weights of half babies were above the 10th percent position. Conclusion The key points for managing the multiple pregnancies, especially larger than 3 babies were the following ⑴All pregnant women had a good prenatal care. ⑵Pregnant women rested well, anemia must be corrected, and the adequate nutrition was supplied, especially in third trimester. ⑶Using ultrasound to determine the cervix length and evaluate the risk of preterm delivery, when the preterm delivery sign happened, the inhibiting drugs must be used cautiously and rationally. ⑷ After the antenatal dexamethasone therapy was applied around 36 gestation weeks, selective cesarean section was preferred to deliver the babies. If good prenatal care can be applied, a good outcome for triplets and quaternion can be achieved, close to that of same gestation weeks singleton.

Objective To study the long term results of condylar reconstruction using pedicled patella in the treatment of giant cell tumor of lower femur. Methods From August 1964 to December 1996, 6 patients with giant cell tumor in the lateral condyle of femur were treated with unicondylar resection curettage and reconstruction with pedicled patella autograft. There were 3 males and 3 females. The average age of the pateints was 30.5 years. The operative indications were as the following: tumor was located in one condyle, complicated with pathological fracture or the tumor had invaded underneath cartilage which was difficult for curettage. Surgical procedure included: Lateral condyle resection according to X ray or CT image, curettage of the possible remaining tumor tissues in the medial condyle. A muscle pedicle of 2.5 cm wide from the quadriceps was raised with the patella, then the articular surface of patella was used to reconstruct the articular surface of femoral condyle, ilium graft(sometimes allograft) was used to fill the defect proximal to the patella. The grafts was fixed by screws and(or) plates. Anterior cross ligament was fixed to the patella. Results 6 patients were followed up for a mean period of 136 months. There were no infection, recurrence, metastasis and instability. All patients could walk for a long distance. The range of motion of the involved knees was between 80? and 120?. There was no varus deformity under stress test, but about 5 degrees valgus deformity under stress test could be found in some, muscle strenth was 5 grade. Conclusion Pedicled patella autograft is a good method for the treatment of giant cell tumor that invades unilateral condyle of lower femur, it is specially suitable to the patients with pathological fracture or severe destruction underneath the cartilage. Thorough resection and curretage of the tumor, preserving the blood supply of patella, refixing the cross ligament, and lower placement of patella are the key points to obtain the satisfactory results.

Objective To summarize the achievement and experience in leprosy control in Jiangxi Province and provide scientific basis for formulation of leprosy strategies. Methods Based on the National Leprosy Recording and Reporting System, epidemiological data of leprosy in Jiangxi Province were analyzed using computer. Results Jiangxi Province was a leprosy medium endemic area with a higher distribution of leprosy in the south than the north. The prevalence, incidence and detection rates were highest in the 1959s or 1960s and had decreased annually since then. Average incidence rate for the recent 5 years (0.1604 per 100 000) and prevalence rate (0.049 per 10 000) in 1998 reached the criteria of basic elimination of leprosy at provincial level, however, still 15 counties or cities not reaching the criteria. Conclusion The comprehensive measures including early detection of cases, immediate treatment with multidrug therapy and effective health education are helpful and important for leprosy control.
