1.Analysis on distribution characteristics and drug resistance of pathogenic bacteria in ICU

Objective To investigate the distribution characteristics and drug resistance of pathogenic bacteria in ICU . Methods The bacteria identification and drug sensitive test were carried out on all pathogenic bacteria in ICU of our hospital from January 2013 to December 2013 .Results A total of 187 strains of pathogenic bacteria were separated ,among them 37 strains (19 .79% ) were Gram‐positive bacteria ,which was dominated by staphylococcus aureus ,with drug resistance rate to vancomycin , teicoplanin and linezolid of 0 .00% ;135 strains were Gram‐negative bacteria (72 .19% ) ,which was dominated by pseudomonas aeruginosa ,acinetobacter baumannii ,klebsiella pneumoniae and escherichia coli ,the drug resistant rate of acinetobacter baumannii to cefoperazone/sulbactam and minocycline was lower (43 .59% ,46 .15% ,which of pseudomonas aeruginosa to amikacin ,cefotaxime , cefoperazone/sulbactam were 27 .91% ,32 .56% and 30 .23% ,respectively ;15 strains(8 .02% ) of fungi were isolated ,which were mainly candida albicans and candida tropicalis .Conclusion Regular detection of distribution types and drug resistance change can provide the basis for climical rational use of antibacterial drugs .
2.HMGB1 RNAi inhibits TGF-β1 induced epithelial-mesenchymal transition in A 549 cells

Objective To investigate the role of HMGB 1 in epithelial-mesenchymal transition .Methods Specific siRNA of HMGB1 was designed and synthesized .Cultured typeⅡalveolar epithelial cell line-A549 cells were divided into 4 groups:1)control group, 2)model group induced by TGF-β1, 3)HMGB1 RNAi group, 4)RNAi negative con-trol group .Cellular morphology changes were observed by phase-contrast microscope .HMGB1 andα-SMA expression in A549 cells was detected by RT-PCR and Western blotting respectively .Results mRNA and protein expression of HMGB1 andα-SMA in TGF-β1 group increased significantly than that in control group (P<0.01).HMGB1,α-SMA mRNA and protein expression in siRNA-treated cells decreased significantly as compared with that in TGF -β1 group (P<0.01).Conclusions HMGB1 may be involved in the TGF-β1 induced EMT.
3.Simultaneous Determination of Eight Compounds in Shegan Mixtures by UHPLC-MS/MS

Objective:To establish an UHPLC-MS/MS method to determine the contents of baicalin,scutellarin,wogonin,caffeic acid,ephedrine,belamcandin,irisflorentin and baicalein in Shegan mixtures. Methods:The chromatographic separation was performed on a ZORBAX SB-C18 column(2. 1 mm × 50 mm,1. 8 μm)with the mobile phase consisting of acetonitrile and water(0. 1% formic acid),the flow rate was 0.3 ml·min-1,and the column temperature was 35℃. Electrospray ionization source(ESI)was used with multiple reaction monitoring( MRM)combined with positive and negative scanning switch. üegative ion mode detection was used for caf-feic acid,belamcandin and scutellarin ,while positive ion mode detection was used for baicalin,wogonin,irisflorentin,baicalein and e-phedrine. Results:The quantification limit of baicalin,scutellarin,wogonin,caffeic acid,ephedrine,belamcandin,irisflorentin and ba-icalein was 1. 44 × 10-4 ,4. 20 × 10-3 ,2. 95 × 10-4 ,7. 80,4. 90 × 10-3 ,4. 6 × 10-2 ,3. 18 × 10-4 ng·ml-1 and 4. 85 ng·ml-1 . The detection limit was 4.32 ×10-5,1.3 ×10-3,8.84 ×10-5,0.77,2.90 ×10-4,3.33 ×10-4,9.5 ×10-5 ng·ml-1 and 1.46 ng· ml-1 ,respectively. The correlation coefficients( R2 )were all higher than 0. 992 3 within the linear ranges. Both intra- and inter-day RSD were less than 5%. The average recoveries of the eight components were within the range of 80%-120%. Conclusion:The method is simple,rapid,sensitive and accurate. It can be used to determine the contents of baicalin,scutellarin,wogonin,caffeic acid,ephed-rine,belamcandin,irisflorentin and baicalein in Shegan mixtures for the quality control.
4.Effect of family-of-origin paternal parenting on father involvement of 1-12 years old children: the mediating effect of role perception and moderating effect of depression

Objective:To identify the distal and proximal influencing factors of father involvement, and explore the mediating role of father's role perception and the moderating role of depression in the effect of his family-of-origin paternal parenting on father involvement.Methods:Totally 572 fathers with children aged under 12 years old completed the father involvement questionnaire, role of the father questionnaire (ROFQ), parental bonding instrument (PBI) and the center for epidemiologic studies depression scale (CES-D). The Pearson correlation analysis and non-parametric percentile Bootstrap method were conducted via SPSS 21.0.Results:(1)Father involvement (2.86±0.58) was found to be negatively correlated with his own family-of-origin paternal indifference (7.30±3.86) ( r=-0.175, P<0.01), positively correlated with his family-of-origin paternal care (10.46±3.98) ( r=0.291, P<0.01), negatively correlated with his depression (7.07±5.09) ( r=-0.266, P<0.01) and positively correlated with his role perception (61.52±5.92) ( r=0.522, P<0.01). (2)Father's role perception partially mediated the relationship between his family-of-origin paternal care and his own paternal involvement (the mediating effect was 0.015, which was 35.71% of the total effects) and fully mediated the relationship between his family-of-origin paternal indifference and his own paternal involvement (the mediating effect was -0.016). (3)Father's depression played a moderating role between his family-of-origin paternal care and his own role perception ( β=-0.069, P<0.05). And it also played a moderating role between his family-of-origin paternal indifference and his own role perception ( β=0.121, P<0.001). The results of J-N methods showed that the values and significance of simple slopes varied according to father's depression levels in both two models. Conclusion:The paternal parenting that fathers had experienced in his family of origin is an important factor affects his own paternal involvement.In the relationship between father's family-of-origin paternal parenting and his own paternal involvement, father's role perception plays a mediating role and his depression level plays a moderating role.
5.Anesthetic management for repair of tetralogy of Fallotin adults

Two hundred and thirty-five patients of both sexes (123 male,112 female) aged 18-54 yr,weighing 35-62 kg underwent repair of tetralogy of Fallot from January 1996 to July 2009 in Fuwai hospital.Direct BP, ECG,CVP,SpO2,naso-pharyngeal temperature and TEE were continuously monitored during operation.Anesthesia was induced with midazolam and/or etomidate, fentanyl and pipecuronium and maintained with isoflurane/sevoflurane inhalation and intermittent iv boluses of fentanyl and pipecuronium. The total amount of fentanyl administered ranged from 30-50 μg/kg. Cardiac function was supported and hemodynamic stability was maintained with vasoactive and inotropic drugs. Measures were taken to strengthen blood conservation and respiratory function support.Seven patients(2.9%) needed urgent CPB during operation because of serious cyanotic spells. Perfusion-induced lung injury occurred in 18 patients (7.6%). Circulation was assisted by ECMO in 2 patients (0.9%). Three patients (1.3%) died of serious low cardiac output and perfusion-induced lung injury.
6.Imaging Evaluation of Xiaoding Ointment in the Treatment of Acute Patellar Bursitis

OBJECTIVE:To evaluate clinical efficacy through comparing the change of CT image in infrapatellar fat pad before and after Xiaoding ointment in the treatment of acute patellar bursitis of knee joint. METHODS:73 patients with acute patellar bur-sitis were randomly divided into observation group(39 cases)and control group(34 cases). Observation group was given Xiaoding ointment for local application,qd,7 d as a courses,3 courses in total;control group was given triamcinolone acetonide 30 mg af-ter the extraction of articular cavity effusion,once a week,totally for 3 times. All patients of two groups underwent knee CT exami-nation for observation of the infrapatellar fat pad and articular cavity effusion volume change before and after treatment. Clinical ef-ficacies were compared between 2 groups. RESULTS:CT image alterations of treatment group showed that infrapatellar fat pad den-sity were decreased,anteroposterior diameter,vertical diameter,internal to external diameter were significantly reduced. The total effective rate of treatment group was 92.31%,which was better than that of control group(88.24%),with statistical significance (P<0.05). CONCLUSIONS:Xiaoding ointment demonstrate markedly curative effects in the treatment of acute patellar bursitis, and CT image is an effective method for diagnosis of infrapatellar fat pad.
7.Expression of mice interlecukin-23 in pichia and preliminary studies on its biology function

Objective:To clone the mice interleukin-23(mIL-23) and express it in Pichia efficiently.Methods:Two subunits of IL-23 were amplified by PCR from pcDNA3/mIL-23,and ligated together with adaptor by over-PCR.The IL-23 cDNA confirmed by sequencing was inserted into expressing vector pPICZαA and expressed in Pichia X-33 strain.IL-23 protein expression was induced by methanol and ammonia.The recombinant IL-23 was identified by immunoblot and its biological activity was analyzed.Results:DNA sequencing confirmed that cloned cDNA was identical to the published sequence of mIL-23.The recombinant plasmid pPICZαA/mIL-23 was electroprated into X-33.An expected 72 kD protein of mIL-23 was founded both in the induced pellets and supermatants by SDS-PAGE and coomassie blue staining.The 72 kD protein could be recognized in Western blot.While we could detect bands at 72 kD in cell pellets induced more than 24 h by Western blot.Detected by Western blot using anti-His antibody,we could see positive band at 72 kD from supernatants induced 24 and 36 h.While we could detect band at 72 kD in cell pellets induced between 24 h and 96 h.Meanwhile,we could see obviously proliferation of mice peripheral blood mononuclear cells(PBMC) treated with the induced pellets and supermanants.Conclusion:We have successfully expressed mIL-23 protein in Pichia and the expressed product has its bioactivity in promoting the proliferation on PBMC.

At the same time of targeted drugs working, the side effects on associated rash also gradually attracted people's attention.Traditional Chinese medicine has its own specific advantage of treating sample acne skin rash.Thisarticle from the understanding of acne, andvarious of acne syndrome differentiation of traditional Chinese medicine experience of ancient doctorsto sample acne skin rash was summarized. At the same time, the paper wassummarized forEGFR-TKI induced sample acne skin rash in the treatment with traditional Chinese medicine from the etiology and pathogenesis, differentiation of symptoms and signs,principle and prescription of treatment,in order to cure the current disease by learning the ancient method.
9.Evaluation of the efficacy and usefulness of low temperature hydrogen peroxide gas plasma sterilization system for endoscopies

Objective To evaluate the efficacy of STERRAD (r) 100S (low temperature hydrogen peroxide gas plasma sterilization system) by comparing with formaldehyde steam. Methods From 2005.1 to 2005.3, totally sterilization of 2 760 pieces of operating tools were carried out with formaldehyde steam. From 2005.1 to 2005.3, totally sterilization of 4 950 pieces of operating tools in 180 circles were carried out with STERRAD (r) 100S. The instrument damage rate, operating infection rate and satisfaction rate of medical workers of 2 groups were compared. Results All data were in favor of the efficacy of STERRAD (r) 100S. Conclusions STERRAD (r) 100S is a safe, effective, low temperature, and cost-effective sterilization system..
10.Impact of subclinical hypothyroidism on macro-vascular complications in elderly type 2 diabetic patients

Objective To investigate the impact of subclinical hypothyroidism (SCH) on macrovascular complications in elderly type 2 diabetic patients.Methods A total of 1170 hospitalized elderly patients with type 2 diabetes mellitus were enrolled in the study through systematic sampling and underwent testing for blood biochemical indicators, thyroid function and C peptide.Parameters for macro-vascular complications, including the ankle/brachial index (ABI), transcranial Doppler vascular ultrasound (TCD), electrocardiogram (ECG), ejection fraction (EF), history of coronary heart disease, and hypertension grading were also monitored.Results All the subjects were divided into two groups based on the thyroid stimulating hormone (TSH) level: the euthyroid group (4 mU/L≥TSH>0.4 mU/L) and the SCH group (TSH>4 mU/L), and the latter was further sub-grouped into the mild SCH group (10 mU/L≥TSH>4 mU/L) and the severe SCH group (TSH>10 mU/L).ABI was significantly decreased in SCH (R/L: 0.86/0.92, P<0.01).Levels of basal C-peptide (CP0) and post glucose-challenge C-peptide (CP1-3) were higher in the SCH group than in the euthyroid group [(2.16±0.93)pg/L vs.(1.56±1.05)pg/L, (0.53±0.25)pg/L v, (0.38±0.37),(0.72±0.23) pg/L vs.(0.56 ±0.32) pg/L, (6.21± 2.69) pg/L vs.(4.46 ± 2.62) pg/L,respectively, P<0.01 for all].EF was higher in the SCH group than in the euthyroid group[(70.87± 6.66)% vs.(65.10 ± 8.08%), P< 0.01].There were no significant differences in other biochemical indicators, ECG, TCD, history of coronary heart disease, hypertension grading and intervention treatment (P>0.05 for all).Conclusions Lower extremity atherosclerotic disease has a higher incidence in elderly type 2 diabetic patients with SCH and occurs earlier than other macrovascular complications.Elevated TSH levels and insulin resistance may be the major causes.
