3.Risk factors for ischemic stroke in Uygur population in Xinjiang, China

Objective To investigate the risk factors for ischemic stroke in Uygur population in Xinjiang, China. Methods Using a case-control study mode, 504 Uygur patients with first-ever ischemic stroke and 507 Uygur healthy subjects w ere investigated in accordance w ith the principle of the ethnic, sex, and age matching. The risk factors for ischemic stroke w ere screened. Results Multivariate logistic regres-sion analysis showed that the obesity (odds ratio [OR] 4.82, 95% confidence interval [CI] 1.80-12.94;P=0.002), hypertension ( OR 8.20, 95%CI 4.93-13.51; P<0.001), and heart disease ( OR 2.40, 95%CI 1.07-5.39;P=0.033) w ere the independent risk factors for ischemic stroke in Uygurs. While the education level of junior high school or above ( OR 0.38, 95%CI 0.18-0.81;P=0.012), tea drinking habit ( OR 0.22, 95%CI0.06-0.70; P=0.021), siesta habit ( OR 0.38, 95%CI 0.24-0.59; P<0.001), high levels of high-density lipoprotein cholesterol ( OR 0.34, 95%CI 0.20-0.59; P<0.001), and apolipoprotein A ( OR 0.23, 95%CI 0.07-0.77; P=0.017) were the protective factors for ischemic stroke. Conclusions The risk factors for ischemic stroke are more and typical in Uygur population in Xinjiang, China. Adjusting the diet structure of Uygurs, less taking meats and high-salt diet, intaking high fiber foods and fish, and appropriate physical exercise w il play an important role for the prevention of stroke in Uygur population.

5.Effect of high-fat diet before and during pregnancy on growth and serum insulin-like growth factor-1 level in neonate rats

Objective To establish neonate rate models induced by high-fat diet so as to explore the effect of high-fat diet before and during pregnancy on growth and serum insulin-like growth factor-1 (IGF-1) level in neonate offspring.Methods Forty female SD rats were divided into high-fat group and control group that were fed with 35% high-fat diet and standard chow,respectively.After 8 weeks,8 rats from each group were euthanatized for liver pathology and the other female rats were mated with male rats and fed continuously with 35% high-fat diet and standard chow throughout gestation,respectively.Offspring from both groups were studied within 24 hours after birth.Their birth weight and body length (from apex nasi to end of tail) were measured.Enzyme-linked immunosorbent assay was used to detect serum IGF-1 level.Serum biochemical indexes were tested by biochemical analyzer.Liver pathologic changes were observed under the light microscope.The expression of IGF-1 in liver samples was detected by Western blot.Results 1.Rats in the high-fat group showed a significant less birth weight and shorter body length compared with those in the control group(all P < 0.05).2.The level of serum IGF-1 of offspring in the high-fat group decreased by 20.1% in comparison to offspring in the control group,but there was no significant difference between the 2 groups (P > 0.05) because of the limited sample sizes.3.The aspartate transaminase(AST) of offspring in the high-fat group increased compared with those from control group(P < 0.05),while other serum biochemical indexes between the 2 groups showed no significant differences(all P >0.05).4.Fatty degeneration could be found in livers of both dams and offspring in the high-fat group under light microscope,while the samples of both dams and offspring from the control group all showed normal.5.The IGF-1 expression in livers of offspring from high-fat group increased in comparison to that from control group(P < 0.05).Conclusions A maternal high-fat diet before and during pregnancy decreases the birth weight and body length of offspring in utero,which may be associated with the decreased IGF-1.However,the pathogenesis of decreased IGF-1 in this study can not been found.Further study is needed to clarify the mechanism of intrauterine growth retardation induced by high-fat diet.
6.Research on the association of TIRAP coding region polymorphism with susceptibility to tuberculosis in Chinese Han population

Objective To detect specific polymorphisms in Toll-interleukin 1 receptor domain containing adaptor protein(TIRAP) coding region for Chinese Han population, and verify whether they are associated with susceptibility to tuberculosis. Methods Search TIRAP polymorphisms by sequencing in small sample; detect single nucleotide polymorphism(SNP) by ligase detection reaction technique in large sample; analyze whether polymorphisms are related to tuberculosis by statistic methods. Results Four polymorphisms were present in the TIRAP coding region. 394A had higher frequencies in the tuberculosis(TB)group than the control. But allelic and genotypic analysis showed that there were no significant difference in statistic between TB patients and controls(P>0.05). The SNP G164A mutation related with TB patient's condition. Comparing to controls, retreatment patients' allelic frequencies had significant difference in statistic(P<0.05), sputum positive patients and lung cavitation patients had lower 164A frequencies. Conclusion TIRAP coding region polymorphisms may be risk factors for TB occurrence and development in Chinese Han population.
7.Changes of immune function in liver cirrhosis patients after splenectomy combined with resection of hepatocellular carcinoma.

OBJECTIVETo study the changes of immune function in liver cirrhosis patients after splenectomy combined with resection of hepatocellular carcinoma (HCC).
METHODSSixteen patients with HCC associated with liver cirrhosis were divided into two groups: splenectomy combined with hepatectomy (n = 7) and hepatectomy (n = 9). T-lymphocyte subsets such as CD4, CD8, CD4/CD8 and Th-lymphocyte cytokines such as IFN-gamma, IL2, IL10 in 7 ml peripheral venous blood before operation and two months after operation were examined and compared between the two groups.
RESULTSThere was no significant difference in pre-operative CD4, CD4/CD8, IL2, IFN-gamma, IL10 levels in the two group. Two months after operation, the levels of CD4 (38.2% +/- 3.7%), CD4/CD8 (1.7 +/- 0.3), IFN-gamma [(104.4 +/- 14.9) pg/ml], IL2 [(98.6 +/- 18.6) pg/ml] were increased and those of CD8 (23.7 +/- 13.7) pg/ml and IL10 [(55.5 +/- 11.2) pn/ml] were decreased in the two groups, but the changes in the group of splenectomy combined with hepatectomy were more obvious than those in the hepatectomy group. The levels of CD4 (32.5% +/- 4.0%), CD4/CD8 (1.1 +/- 0.1), IFN-gamma [(70.5 +/- 12.6) pg/ml], IL2 [(80.9 +/- 13.5) pg/ml] in the group of splenectomy combined with hepatectomy, were much higher than those in the hepatectomy group; those of CD8 (29.4% +/- 4.0%), IL10 [(89.4 +/- 10.0) pg/ml] level were significantly lower than those in the hepatectomy (P < 0.05).
CONCLUSIONSplenectomy combined with hepatectomy for HCC associated with liver cirrhosis donor decrease but promote the recover T-lymphocyte subsets and Th1/Th2 cytokines from imbalance and improve the patient's antitumor immune function.

Objective To summarize the experience in prevention and treatment of iatrogenic bile duct injury Methods Clinical data of 112 cases with iatrogenic bile duct injury in ten hospitals of Songhua river drainage area from January 1978 to January 2003 were analyzed retrospectively Results The main cause of iatrogenic bile duct injury was wrong identifying the anatomy of the Calot′s triangle before cholecystectomy accounting for 55 4% (62/112) Diagnosis depended on clinical features, celiac puncture and imaging examination Ultrasonography was among the most sensitive diagnostic means (diagnostic rate=97 5%) Six types of injury were identified according to their locations and type Ⅲ damage was most common in clinical practice (92/112) The curative rate in this group was 95 5% (107/112) Eighty seven cases (77 7%) underwent Roux en Y choledochojejunostomy, with cure rate of 94 3%(82/87) Conclusion Iatrogenic bile duct injury prevention lies in identifing the topography of extrahepatic bile ducts Roux en Y choledochojejunostomy is usually the therapy of choice
9.Effect of Notch1 signaling pathway activation on pancreatic cancer cell proliferation in vitro.

OBJECTIVETo observe the effect of activation of Notch1 signaling pathway by Notch intracellular domain (NICD) plasmid transfection on pancreatic cancer cell proliferation and explore the underlying mechanism.
METHODSThe transfection rates were observed under microscope with fluorescence stimulation, and mRNA expression levels of Hes1 were detected by real-time PCR. Cell proliferation changes were evaluated by CCK-8 after NICD and control plasmid transfection in pancreatic cancer cells. Caspase 3 activity was examined using a caspase 3 detection kit.
RESULTSThe transfection rates of NICD plasmid were up to 80% by fluorescence stimulation observation. Hes1 expression was significantly increased after NICD plasmid transfection, suggesting the activation of Notch1 signaling pathway. NICD plasmid transfection significantly promoted cancer cell proliferation compared to control plasmid transfeciton. The activities of caspase 3 were obviously decreased after NICD plasmid transfection in 3 pancreatic cancer cell lines.
CONCLUSIONActivation of Notch1 signaling pathway by NICD plasmid transfection can promote the proliferation of pancreatic cancer cells by inhibiting the apoptosis pathway.

10.Relationship between patients′ trust in family physicians and medication adherence of patients with type 2 diabetes mellitus

Objective To understand the status of type 2 diabetes mellitus (T2DM) patients′ trust in their family physicians, and to explore the role of the trust on patients′ medication adherence. Methods A total of 1078 patients with T2DM were recruited by cluster and systematic sampling method, from ten rural communities in Sanlong township in Yancheng city and five rural communities of Pingchao township in Nantong city, during February to April in 2018. The survey covered their trust behavior and attitude scale (PTBAS), and an 8-item Morisky medication adherence scale ( MMAS-8). Both measurement data and enumeration data were subject to descriptive analysis by mean ± SD and proportion respectively. Multiple linear regression was employed to explore the determinants of trust in physicians. Ordered logistic regression analysis was used to explore the relationship between patient-physician trust and medication adherence. Results The mean scoring of patients′ trust and medication adherence was 20. 16 ± 2. 13, 7. 07 ± 1. 54 respectively. 55. 40 percent of the patients had high medication compliance, 28. 53 percent of them had medium compliance and 16. 07 percent had poor compliance. Multiple linear regression analysis showed that the patient′s occupation, course of disease, self-reported health status and mental health status influenced their trust behavior (P < 0. 05). Ordered logistic regression analysis showed that each 1 point increase in patient′s trust in physicians, the medication compliance increased by 11. 0% (OR = 1. 110, P < 0. 001). Medication adherence is also affected by age, employment status, course of illness, and mental health status. Conclusions Diabetes patients′ trust in their family physicians improves their medication adherence.
