1.Effect of Yang-supplementing Moxibustion on Muscular Spasm, Motor Function, and Activities of Daily Living in Post-stroke Hemiplegia

Objective To observe the effect of yang-supplementing moxibustion in improving the muscular spasm, motor function, and activities of daily living (ADL) of patients with post-stroke hemiplegia.Method Totally 120 patients with post-stroke limb spasm due to qi deficiency and blood stagnation were randomized into group A, B, and C, 40 cases in each group. Group C was intervened by conventional treatment, group A was by yang-supplementing moxibustion in addition to the conventional treatment, and group B was by fire therapy in addition to the conventional treatment. The modified Ashworth Scale (MAS), Fugl-Meyer Scale (FMS), and Barthel Index (BI) were observed before and after intervention.Result Respectively after 1-month and 2-month treatment, the MAS, FMS, and BI scores were significantly different from that before intervention in the 3 groups (P<0.01). After 1-month and 2-month treatment, the MAS, FMS, and BI scores in group A were significantly different from that in group B and group C (P<0.05), and the scores in group B were significantly different from that in group C (P<0.05).Conclusion Yang-supplementing moxibustion can effectively improve the post-stroke limb spasm, and can promote the recovery of motor function.

Surface plasmon resonance (SPR) sensor technology,with its features of real-time,fast,no need for labeling,no background interference and non-destructive to samples,etc.,has been widely used in the field of biotechnology,medicine,environmental science and drug detection.This article gives an overview of the recent popular studies and their progress in SPR technology is reviewed,especially giving a detailed overview of the surface modification technique,related hyphenated techniques and application of SPR in clinical examination.Hyphenated techniques is discussed from three aspects of molecular imprinting technique,SPR based immunoassay and nucleic acids SPR biosensor as well.
3.A patient with intractable epilepsy due to ring chromosome 20 syndrome and prenatal diagnosis for his sibling

Objective To investigate the clinical features,karyotype,and the prenatal diagnosis for his sibling of a Chinese patient with rare ring chromosome 20 syndrome induced intractable epilepsy.Methods The clinical data of the patient diagnosed in Peking University People's Hospital were collected.The clinical manifestations,chromosome karyotype were summarized.Results The proband,a boy,started to show intermittent tonic seizures or atypical absence seizures and psychomotor retardation from the age of 11 months.Several anti-epilepsy drugs and globulin had been tried without effect.Common karyotype analysis and epilepsy-related genes analysis revealed no abnormality.However,abnormal karyotype 46,XY,r(20)(p13q13.3) in his peripheral blood lymphocytes was found by high resolution chromosome karyotype analysis with 550 G-banding,and the diagnosis of ring chromosome 20 syndrome,type Ⅱ was confirmed.The mother of the patient underwent amniocentesis at the midterm of the second pregnancy.The cultured amniocytes karyotypes were normal.The second child(a boy) of the family was 1 year old without epilepsy and the psychomotor development was normal.Conclusions Ring chromosome 20 syndrome is a rare human chromosome abnormality.The syndrome is associated with epileptic seizures,behavior disorders and mental retardation.Since karyotype testing is not a routine investigation for the patient with epilepsy,the diagnosis of ring chromosome 20 syndrome is usually delayed or misdiagnosed.The karyotype analysis should be considered for the etiological study of the patients with intractable epilepsy with unknown origin.
4.Quality of Sexual Life in Premenopausal Breast Cancer Patients after Treatment

Objective To explore the change of sexual life quality and its influencing factors of premenopausal breast cancer patients.Methods 94 premenopausal breast cancer patients were included. Changes in Sexual Functioning Questionnaire short-form (CSFQ-14) was used and part of its items was changed. Their sexual life was evaluated before and after treatment by using depth interview and anonymous methods. Results 94 cases received face-to-face communication and study. 88 (93.6%) questionnaires were received, in which 81 (92.1%) questionnaires were effective. Their sexual life quality and sexual satisfaction significantly decreased and there was significant difference in the CSFQ-14 score (P<0.001) before and after treatment. The ratio of bad, worse, and worst sexual life were 39.5%, 29.6%, and 14.8% respectively after treatment. 65.4% of the patients had no sexual activity. The reasons included medical related factors, fear of cancer metastasis and recurrence, physical discomfort, poorer body image, partner sexual attitudes changed, life stress, common psychological problems,
traditional ideas and so on. Conclusion Sexual dysfunction was obvious and sexual life quality significantly decreased in premenopausal breast cancer patients after treatment. Patients' physical and mental damage, the change of body image and spouse's attitude are significant influencing factors. It is necessary to give sexual rehabilitation guidance to patients and their spouses.
5.Rapid detection of diethylstilbestrol using a quartz crystal microbalance with gold nanoparticals amplification.

OBJECTIVETo develop a quartz crystal microbalance (QCM) immunosensor with high sensitivity and selectivity for the rapid detection of diethylstilbestrol.
METHODSDextran was used as reducing agent for preparing gold nanoparticles (AuNPs) with the size of 40 nm. The AuNPs were coupled with anti-DES antibody after amination. A monolayer was generated after immersing the quartz crystal into the solution of 5 mmol/L 11-mercaptoundecanoic acid(MUA) for 16 hours. After the monolayer was activated by 1-ethyl-3-(3-dimethylaminopropry) carbodiimide hydrochloride (EDC·HCl) and N-hydrosuccinimide (NHS), 20 μl of 2.2 mg/ml DES-HS-BSA was dropped onto the surface of crystal to prepare a sensitive membrane which can recognize DES specifically. Then, 50 μl of 1 mol/L ethanolamine (pH 8.5) was used to seal the carboxylic groups to make the sensitive membrane which could identify DES specifically. QCM immunosensor was used as detection platform to optimize the reaction conditions. Under the optimized conditions, 10 μl of 28 μg/ml AuNPs-antibody was mixed with 10 μl of 0.03-2.5 μg/ml DES, and the mixture was added on the sensitive membrane. QCM immunosensor was used to detect the signals and the standard curve was obtained at the same time. The detection limit was calculated based on the standard curve. The specificity was evaluated by testing DES and its analogues with the same concentration.
RESULTSThe optimized concentration for the immobilization of DES-HS-BSA on the surface of QCM was 2.2 mg/ml. The optimized concentration for coupling anti-DES antibody with AuNPs was 7 μg/ml and 15 nmol/L, respectively. The optimized concentration of AuNPs-antibody was 14 μg/ml. The logarithm of DES concentration was proportional to the frequency shift in the range of 0.16-500 ng/ml, Δf=-24.170 lgCDES+69.71, R(2)=0.998. The detection limit of this method was 0.13 ng/ml. DES analogues could not influence the detection of DES obviously, so the sensor had good specificity.
CONCLUSIONThe quartz crystal microbalance immunosensor with gold nanoparticals amplification could detect DES sensitively and rapidly.

6.Transvaginal Contrast-enhanced Sonography in the Differentiation of Benign from Malignant Adnexal Masses

Purpose:The study was designed to investigate the feasibility of contrast-enhanced ultrasound in the differentiation of benign and malignant adnexal masses.Materials and Methods:Sixty-nine consecutive patients with adnexal masses received trans vaginal contrast-enhanced ultrasound.The image and perfusion features were assessed.Results:All of 26 malignant tumors showed detectable contrast enhancement,including 24 cases with a quick,heterogeneous or branching pattern.Among 39 benign lesions,24 were cystic with circle or half-circle enhancement,including 5 cases with intra-cystic septum or papillae slightly enhanced.The other 15 cases were solid,8 of them had slightly dotted enhancement.There are significant difference in enhancement patterns between benign and malignant masses ( P < 0.0001).The 4 cases of borderline tumors showed progressive,heterogeneous enhancement.Conclusion:Contrast-enhanced ultrasound is of value in the differentiation of benign and malignant adnexal masses.
7.Serum vitamin D levels in tuberculosis patients with different blood glucose status

Objective To investigate serum vitamin D levels in tuberculosis (TB) patients with different blood glucose status.Methods Two hundred and forty seven TB patients were recruited from tuberculosis clinics in Jilin province and 80 normal subjects who underwent health check up in Beijing Hospital served as controls.Blood samples were collected,fasting blood glucose (FBG) and serum vitamin D [25 (OH)D] levels were measured.Results FBG results showed that there were 95 patients with normal FBG,69 with pre-diabetes (pre-DM) and 83 with diabetes mellitus (DM).Vitamin D measurement showed that 25(OH) D level in TB patients with normal FBG,pre-DM and DM was 16.1 (10.7,26.2) μg/L,12.9 (9.6,20.1) μg/L and 12.4 (10.4,16.9) μg/L,respectively,(x2 =19.608,P < 0.001) and were much lower than that in the normal controls (20.5 μg/L) (x2 =21.701,P < 0.001).Proportion of TB patients with 25 (OH)D sever deficiency(< 10.0 μg/L)in patients with normal FBG,pre-DM and DM was 20.0% (19/95),31.9% (22/69),and 24.1% (20/83) respectively (x2 =6.376,P < 0.05);proportion of 25 (OH) D deficiency (10.0-19.9 ng/ml) in three groups was 41.1% (39/95),40.6% (28/69),and 57.8 % (48/83),respectively (x2 =15.141,P < 0.05);sufficient 25 (OH) D (≥ 30.0 μg/L) was 14.7% (14/95),7.2% (5/69),and 1.2% (1/83),respectively (x2 =19.118;P <0.05).While the proportion of TB patient with 25 (OH) D insufficiency (20.0-29.9 ng/ml) was 24.2% (23/95),20.3% (14/69),and 16.9% (14/48) respectively (x2 =0.933,P =0.627).In TB patients with normal FBG,risk factors for 25 (OH) D deficiency were smoking (OR =5.619,95% CI:1.293-24.424,P =0.021),cold season (OR =14.402,95%CI:4.070-50.965,P < 0.001) and smear negative TB (OR =6.194,95 % CI:1.873-20.481,P =0.003).Living in rural area (OR =3.429,95% CI:1.040-11.299,P =0.043) was the risk factor for 25 (OH) D deficiency in TB patients with pre-DM and age ≥ 60 years (OR =2.474,95%CI:1.086-5.623,P =0.031) was risk factor for 25 (OH) D deficiency in those with DM.Conclusions Vitamin D level is lower in TB patients than that in normal controls.The diabetic TB patients have the lowest 25 (OH) D level and have highest proportion of vitamin D deficiency and sever deficiency,particularly for elderly patients.
